Ac antiphospholipides, lupiques,beta2 GPI,hyperhomocystéinémie, hyperlipémie,anémie de Biermer,lp(a), gpe A,,ATIII,Prot C,S,dysfibrinogénémie,facteurs VIII,XI,XIII,IX, VLeiden,MTHFR,II 20210A, ACE, IL8,groupe sanguin A, syndrome néphrotique
Infections chroniques,entéropathies inflammatoires,Behcet, lupus ,Buerger,cancers,syndromes myéloprolifératifs,traumatismes,brûlures graves,voyages, immobilisation,plâtres, paralysie membre inférieur, insuffisance cardiaque, IVC,varices, Insuffisance respiratoire, bronchospasme chez BPCO,CPC, syndrome apnée sommeil, Infarctus, artérite décompensée, moignon d’amputation, cathéters, athérosclérose.
Obstétrique, chirurgies, antécédent MTE, Age, obésité,saisons,compressions veineuses, malformations veineuses.

• Incidence cumulée de 55 % sans prévention 5% décès précoces EPparésie ou paralysie membre inférieur
9 essais Héparines Réduction risque entre 56 et 82%

• Incidence de 25% de MTEV sans ttt
• Héparines, anti agrégants, thrombolytiques…..la tvp n’est plus un problème ?
Réanimation médicale
• Plusieurs facteurs de risque…
• 30% de TVP lors d’echo-Doppler systématiques toutes les 72 h

Optimisation de l’interrogatoire dans l’évaluation du risque thrombo-embolique veineux
Projet OPTIMEV
Problématique
• L’estimation du risque de MTEV est indispensable– Diagnostic, traitement, démarche étiologique
• Elle repose sur le recueil des facteurs de risque par l’interrogatoire.
• Les facteurs de risque sont bien identifiés mais– les études sont anciennes,– hospitalières,– concernent des formes cliniques devenues moins
fréquentes
L’interrogatoire est au centre de l’évaluation du risque mais …
• Tous les ATCD de MTEV n’ont pas la même valeur :– Surale asymptomatique / TVP proximale– mais aucun score ne les différencient …
• Le nombre d’ATCDs, les circonstances et le rythme de survenue ont ils une importance ?
L’interrogatoire est au centre de l’évaluation du risque mais
• Les patients très âgés ont ils des FDR spécifiques ?
• Comment définir la déambulation hémodynamiquement efficace ?
• Le traitement préventif modifie-t-il le poids des FDR ?– PTH réglée sous prévention optimale une
situation à haut risque ????
