Executive Summary ......................................................................................................

interest in the world towards herbal remedies, as the therapeutic efficacy of various Herbal drugs used to treat the lifestyle disorders is insurmountable. India has a rich, old heritage of medical and health knowledge and the AYUSH (Ayurvedha, Yoga & Naturopathy, Unani, Siddha and Homoeopathy) systems are time-tested methods. These systems are sometimes even used as adjuvant to prevent the side effects of modern medicines. The AYUSH systems play a major role in tackling Life style disorders, since they are not only curative but also preventive.

In this context, a workshop was organised in the SPC under the Chairmanship of the Vice Chairman, State Planning Commission. Resource persons serving the domain of AYUSH systems shared their knowledge on the various facets relating to incidence of Lifestyle diseases, the level and magnitude of the problem, reasons for occurrence and contributed vital inputs to combat these diseases.

The world has come a long way in the last 100 years, not just in terms of economic advancements and blurring geographies for trade, but also in the field of science, specifically medicine. As developments in businesses occurred, there was an implicit effect on the pace of lifestyles of the people involved. Although medicine has evolved extensively and has eradicated most of the diseases that were prevalent in the 19th century, the next generation has been plagued with a fresh set of diseases, creatively called “Lifestyle diseases”. These diseases, as the name suggests, are the result of fast-paced lifestyle that necessarily accompanies the developments over the years and have become a major threat to health in the present era.

Inspite of great progress made by modern

scientific system of medicine, the modern drugs have their limitations in use and also result in side effects. Hence, there is a growing

Lifestyle diseases better known as non-communicable diseases (NCDs) and diseases of civilisation, characterise those diseases whose occurrence is primarily based on the daily habits of people and also as a result of an inappropriate relationship of people with their environment. It is a medical condition or disease which is non-infectious and non-transmissible, but is likely to continue progressively unless intervened. According to WHO, Lifestyle diseases (LSDs) may be chronic diseases of long duration and slow progression or they may result in more rapid death. Diet and lifestyle are the major factors found to influence susceptibility to many diseases. The main factors contributing to lifestyle diseases include bad food habits, physical inactivity, wrong body posture, disturbed biological clock and also addictions such as drug abuse, tobacco smoking, alcohol drinking.

The four main types of Lifestyle diseases are Cardiovascular diseases (like heart attacks and stroke), Cancers, Chronic respiratory diseases (such as Chronic obstructed pulmonary disease and Asthma) and diabetes. The other Lifestyle diseases include Hypertension, Obesity, Osteoporosis, Alzheimer’s disease, Arthritis, Atheresclerosis, Chronic Liver disease or Cirrhosis and so on.

AYUSH system is a holistic approach to combat Lifestyle diseases boosting the whole health system in the society. AYUSH systems

of Medicines are well accepted by community particularly with rural areas because these are socially acceptable, comparatively safe and efficacious and easily available which can be prepared locally by the available resources.

Prevalence of Lifestyle Diseases

Lifestyle diseases are now the leading cause of death around the world with developing countries hit hardest, according to a new report from the World Health Organisation (WHO). Non-Communicable diseases are often associated with older age groups, but recent evidence shows that more than 9 million of all deaths attributed to NCDs and occur before the age of 60. Of these "premature" deaths, 90% occurred in low and middle-income countries like India. The WHO has estimated that by 2020, two-thirds of the global burden of diseases will be attributable to chronic non-communicable diseases, most of them strongly associated with life style.

Cardio vascular disorders (CVD) continue to be the major cause of mortality representing about 30% of all deaths worldwide. Lifestyle diseases like hypertension, diabetes mellitus, dyslipidaemia and overweight/obesity are the major risk factors for the development of CVDs. The diet [or lifestyle] of different population might partly determine their rates of cancer and the basis for this hypothesis was strengthened by results of studies showing that people who migrate from one country to another

generally acquire the cancer rates of the new host country, suggesting that environmental [or lifestyle factors] rather than genetic factors are the key determinants of the international variation in cancer rates.

With rapid economic development and increasing westernisation of lifestyle in the past few decades, prevalence of these diseases has reached alarming proportions among Indians in the recent years. In India (which is already crippled by an ever increasing burden of infectious diseases, poor maternal and perinatal conditions and nutritional deficiencies) 10% of adults suffer from hypertension while the country is home to 25-30 million diabetics. Three out of every 1,000 people suffer a stroke and the number of deaths due to heart attack is also in increasing trend. A report jointly prepared by the World Health Organization (WHO) and the World Economic Forum says

India will incur an accumulated loss of $236.6 billion by 2015 on account of unhealthy lifestyles and faulty diet. According to a survey conducted by the Associated Chamber of Commerce and Industry (ASSOCHAM), 68% of working women in the age group of 21-52 years were found to be afflicted with lifestyle ailments such as obesity, depression, chronic backache, diabetes and hypertension. The study ‘Preventive Healthcare and Corporate Female Workforce’ also conclude that long hours and working under strict deadlines cause up to 75% of working women to suffer from depression or general anxiety disorder, compared to women with lesser levels of psychological demand at work. The study cited scientific evidence that healthy diet and adequate moderate physical activity of atleast 30 minutes for atleast five days a week helped prevent NCDs.

Management of Lifestyle Diseases

A particular lifestyle of person is a cumulative product of his/her physical capacity coordinated with psychological functioning displayed in the form of habits, behaviors, dietary and living pattern based on his own training sought from childhood and also gained from his immediate companions including parents, siblings, peers etc. A healthy lifestyle which includes a proper balanced diet, physical activity and giving due respect to biological clock must be adopted to combat these diseases. In this revolutionised era we cannot stop doing the developmental work, but we can certainly reduce our ailments by incorporating these simple and effective measures to our lives.

Lifestyle medicine is defined as the application of environmental, behavioural, medical and motivational principles to the management of lifestyle-related health problems in a clinical setting. It is an established branch of medicine where we discuss lifestyle's contribution to health in addition to non-pharmacological intervention in the treatment and management of lifestyle diseases such as exercise in diabetes mellitus and weight management in obesity. “Hippocrates” considered as the “Father of lifestyle medicine” used lifestyle modifications such as diet and exercise to treat diseases such as diabetes, what is today called lifestyle medicine. He is often quoted with "Let food be your medicine, and medicine be your food" and "Walking is man's best medicine".

Ayurvedha is the “Science of Life” and Ayurvedha focuses on achieving the objective of promotion of health, prevention and management of disease for a healthy and happy life in the ailing society. Life in Ayurvedha is conceived as the union of body, senses, mind & soul and the living man is conglomeration of three humours (Vata, Pitta & Kapha), seven basic tissues (Rasa, Rakta, Mansa, Meda, Asthi, Majja & Shukra) and the waste products of the body such as faeces, urine and sweat. Thus the total body matrix comprises of the humours, the tissues and the waste products of the body. The growth and decay of this body matrix and its constituents revolve around food which gets processed into humours, tissues and wastes. Ingestion, digestion, absorption, assimilation

and metabolism of food have interplay in health and disease which are significantly affected by psychological mechanisms as well as by bio-fire (Agni).

According to Ayurveda, lifestyle diseases are diseases caused by wrong “Ahara” and “Vihara” (food and regimen). Ayurvedic intervention is targeted towards complete physical, psychological and spiritual well-being, which makes it a wonderful option in treating lifestyle disorders. Ayurveda provides solution in the forms of proper dietary management, lifestyle advises, Panchakarma like detoxification and bio-purification procedures, medicaments and rejuvenation therapies.

The word "Yoga" comes from the Sanskrit word YUJ, which means "to join" or "to yoke". Originated in ancient India, Yoga typically means 'union' between the mind, body and spirit. Unani ........................................................................................................

The Unani system of medicine originated in Greece and Hippocrates is known as the father of this system of medicine. He is said to have freed Medicine from the realm of superstition and magic and gave it the status of Science by declaring that the causes of diseases are not due to supernatural causes or spirits but environmental factors like Air, Food, Water, Seasonal changes and Earthly bodies. The theoretical framework of Unani Medicine is based on the teachings of Hippocrates and other Greek scholars. The Unani System of Medicine has a long and impressive record in India. It was introduced in India by the Arabs and Persians sometime around the eleventh century. Today, India is one of the leading countries, so far as the practice of Unani medicine is concerned. It has the largest number of Unani educational, research and health care institutions.

The Humoral theory, which is the fundamental of Unani Medicine, supposes the presence of four humors viz. Dam (blood), Balgham (phlegm- white fluids of the body), Safra (yellow bile) and Sauda (black matter). The therapeutics of this system include Ilaj bit tadbeer (Regimenal therapy), Ilaj bil ghiza (Dietotherapy), Ilaj bid dawa (Pharmacotherapy) and Jarahat (Surgery). The six essential factors for maintenance of health as prescribed under this system are Air, Food and drinks, Bodily movement & repose, Psychic movement & repose, Sleep & wakefulness and Excretion & retention.

He mentioned about some of the contributory factors for lifestyle disorders such as sedentary lifestyle, little or no exercise, longer hours spent in front of computers or TV screens, over supply of fast food with less or no nutritional values etc.

He made a detailed presentation about the causes, preventive measures, regimen, health promoting tips and curative herbs for each of the common lifestyle disorders. He explained about the three main components of Unani line of treatment to chronic lifestyle illnesses (i) Activities leading to the reduction in the increase of the abnormal or excess humour and reversing the accumulation of toxins, associated with the illness (ii) Encouragement of the concoction and elimination of the abnormal humour (iii) Strengthening of the organs system affected by the disorder by application of an appropriate lifestyle management programme and the use of Ilaj bit tadbeer (Regimenal therapies), cupping, leeching, Turkish bath, physical exercises, massage etc.

Lifestyle medicine which was earlier restricted to the adult or ageing population is becoming increasingly more important with the increase of obesity, metabolic syndrome and type-2 diabetes mellitus in children and young adults. The Contributory factors like sedentary lifestyle, little or no exercise, longer hours spent in front of computers or

TV screens, over supply of fast food with little nutritional values are the main reasons for this 21st century phenomenon. Relatively simple lifestyle changes that include eating more whole foods and less processed foods and staying physically active on a daily basis, losing weight and learning to manage stress better can help patients to avoid medications for hypertension, chronic inflammatory conditions and to reduce rise of strokes and heart attacks.

For the prevention of lifestyle disorders, Unani aims at managing the Asbabe Sitte Zarooriya (six essential health governing factors) to minimize the accumulation of toxins, especially when they arise from a poor, inadequate diet, high stress, less physical and mental activity, inadequate sleep and excessive work load.

Food

Eating a diet rich in micronutrients with enough healthy proteins provide our cells with the building blocks to make plenty of neurotransmitters including dopamine, serotonin, and nor-epinephrine, which helps us to keep our body balanced and healthy.

Exercise

• Physical activity is another keycomponent of health lifestyle.

• Moving our bodies regularly has allkinds of benefits from lowering the rise of D.M and Heart disease to improving the mood and enhancing libido.

• Exercise has been shown to be aseffective as anti-depressants for mild depression and anxiety, with even long lasting effects.

• Adailyformofmovementorexerciseis a healthy habit that supports bone and muscle health, lower stress levels, improves cellular oxygenation and improves over all circulation.

Sleep

• Disrupted sleeping pattern preventnatural healing and balancing of many systems in the body. During a healthy sleep cycle, our body releases hormones to accelerate healing, e.g. melatonin and growth hormone. Inflammation in the body is reduced, detoxification takes place and damaged tissue is repaired while new cells are being formed. Recent studies have shown that insufficient sleep increases the risk of several common cancers, including breast cancer.

Stress

• Chronic stress stimulates the release ofcortisol (stress hormone) in the body. Over a longer period of time, cortisol suppresses the immune system with the possible development of immune related problems. Chronic stress also slows down metabolism and increases fat storage in the body with the typical belly fat that is hard to shed.

• In lifestyle medicine, patients areeducated, supported and empowered to incorporate stress management techniques that stimulates the relaxation response in the body. This can include deep breathing, visualization, mind fullness and meditation. Lifestyle changes are an integral part of an integrative approach to health and well being and should always be part of the prescription for good health.

Siddha Medicine ('rpj;j kUj;Jtk;" or 'jkpo; kUj;Jtk;" in Tamil) is one of the oldest systems of medicine in India. Contemporary Tamil literature holds that the system of Siddha medicine originated in Southern India in the state of Tamil Nadu and is reported to have surfaced more than 10,000 years ago. Siddha literature is in Tamil and it is practised largely in Tamil speaking part of India and abroad. Siddha system has a rich cultural heritage and health traditions.

The term Siddha means achievements and Siddhars mean saintly persons who achieved results in medicine. “Siddhargal" or “Siddhars” were the premier scientists of ancient days. They were spiritual adepts who possessed the Ashta siddhis or the eight supernatural powers. The origin of Siddha system of medicine is attributed to the great Sage Agathiyar or Agasthya, considered as guru of all Siddhars. He is believed to be the founding father of Siddha Medicine and the Siddha system is believed to have been handed over to him by Lord Muruga.

The Siddha System is largely therapeutic in nature. According to this system the human body is the replica of the universe and so are the food and drugs irrespective of their origin. The system has developed a rich and unique treasure of drug knowledge in which use of metals and minerals is very much advocated. Siddha medicine is claimed to revitalise and

rejuvenate dysfunctional organs that cause the disease and to maintain the ratio of Dosha, Kayakarpam (special combination of medicine and life style) and Muppu (the universal Salt) which are the specialities of Siddha system of medicine. Panchabootham (Earth, Water, Fire, Air & Space), Mukkuttram (Vatha, Pitha, kabha) and Arusuvaigal (Sweet, Bitter, Sour, Astringent, Salt & Pungent) are the basic principles of Siddha philosophy.

The word ‘Homoeopathy’ is derived from two Greek words, “Homois” meaning “similar” and “pathos” meaning “suffering”. Homoeopathy is a method of treatment for curing the patient by medicines that possess the power of producing similar symptoms in a healthy human being simulating the natural disease, which it can cure in the diseased person. It is based on the natural law of healing- "Similia Similibus Curantur” which means "let likes be treated by likes”.

Homoeopathy was given a scientific basis by Dr. Samuel Hahnemann (1755-1843), a German Physician in the early 19th century. It treats the patients not only through holistic approach but also considers individualistic characteristics of the person. It was introduced in India when some German missionaries and physicians started distributing homoeopathic medicines amongst local inhabitants. The origin of Homoeopathy in Tamil Nadu was at Pudukkottai district.

Homoeopathy aims at enhancing the innate resistance power of the individuals by stimulating this inherent power by suitable nature friendly medicines. Homoeopathy medicines act on immune system of the human body, thereby stimulating production of antibodies through Immune response genes by producing artificial antigen like protein substance. These drugs basically perform the role of immunomodulators. According to the WHO, homoeopathy is the second

largest system of medicine in the world and is experiencing and annual growth of around 20–25%.

Homoeopathy was more popular in treating infectious epidemic diseases that raged throughout America and Europe during the 1800s. Homoeopathy is very useful in treatment of psychosomatic disorders, autoimmune diseases, geriatric and pediatric disorders, ailments during pregnancy, obstinate skin diseases, life style disorders and allergies, etc. It also has a positive role in improving the quality of life in incurable chronic diseases like Cancer, HIV/AIDS, terminally ill patients and incapacitating diseases like Rheumatoid Arthritis etc.

Role of Homoeopathy in management of lifestyle disorders

The multi-factorial causations for lifestyle disorders include changes in lifestyle, abundance of food, lack of physical activity, smoking, stress, ageing etc.

The effects of stress can be removed by homoeopathic Medicines like Ignatia, Nat-mur, staphisagria, Nux-vomica, colocynthis, opium, hyoscyamus, stramonium, pulsatilla and by counselling.

Type II Diabetes Mellitus

Type II Diabetes is no longer an adult disease. Until recently, it almost occurred only in people above 45 years of age. In early detected cases it can be completely cured with constitutional miasmatic treatment. Insulin resistance can be prevented by insulin resistant drug Insulinum. Medicines are also available to decrease Diabetic induced Atherosclerosis. Oral Hypoglycemic herbal products available in homoeopathy which do not have any side effect or after effect are Cephalandra indica and Syzygium Jambolanum, Gymnema sylvestra, Jonosia Asoka, Grindilia. The scope

of homeopathy in skin complications due to diabetes mellitus is also very good.

Obesity

Obesity is defined as an abnormal growth of the adipose tissue due to an enlargement of fat cell size (hypertrophic obesity) or an increase in fat cell numbers (hyperplasic obesity) or combination of both. Genetic factors and Environmental factors are the major causes for Obesity. The major aetiological factors for obesity are PCOD, Hypothyroidism, Hormonal imbalance in females and other life style factors.

The Vice-Chairman, SPC asked the Commissioner of Indian Medicine and Homoeopathy, as to what they expect from State Planning Commission to strengthen Indian Medicine department. She informed that, funds under State Balanced Growth Fund could be utilized and the various wings functioning under Indian Medicine department may send proposals for health interventions like Anaemia, Lifestyle diseases etc. to SPC through the concerned District Collector or the Departmental head. She suggested the Commissioner of Indian Medicine to submit proposals on Anaemia Control Programme for the districts of Ariyalur and Perambalur.

She also informed about Tamil Nadu Innovation Initiatives (TANII), another resource for implementing novel schemes which is available in SPC and suggested IM&H to submit proposal on IEC programmes for alternative medicines under this fund. Further, she stated that the SPC will fund Government Departments/Universities/Institutions for the conduct of research Studies and will assist in conducting Workshops under various sectors on the current issues that needs greater intervention.

The V-C observed that, various lifestyle disorders have been imported from the West and she expressed her concern on the increase in lifestyle disorders even in rural areas and enquired about the kinds of regimen to address

them through AYUSH. She suggested that awareness on addressing Non-communicable diseases through AYUSH may also be brought out in the form of advertisements as being done by Tamil Nadu Health System Project. Simple messages/ solutions on nutritious diets and good practices under Indian Medicines may be displayed in small capsules, easily understood even by laymen. She mentioned that, SPC is engaged in the promotion of Millet consumption and also suggested that AYUSH could propagate Millet consumption as one of the parameters to lead a better life. Also she emphasized on popularizing the recipes of millets and herbals through Schools, Noon meal centres and Indian Medicine hospitals.

She concluded that the Roof garden scheme of Horticulture Department may be linked with AYUSH for promoting medicinal plants and people may be encouraged to grow valuable tree species like Curry leaf, Pomegranate etc. even in large pots.

The Commissioner of Indian System of Medicine stated that, Indian Medicine is all about cultural ethnicity, which is more preventive in nature. She mentioned that, the basic principles of Ayurvedha, Yoga & Naturopathy, Unani, Siddha and Homoeopathy are all the same. In Indian systems of Medicine, prescriptions are made on ethnic behaviour like meat soup in Unani, Kanji in Ayurvedha, Kashayam in Siddha etc. Further, she stated

that, the Indian systems of Medicine are ancient systems and we do not have a modern scientific research and documentation. However, she mentioned that the Hon’ble Chief Minister has sanctioned Rs. 20.00 crore for Pharamacological study of Indian Medicines. Further, she cited the example of Nilavembu Kudineer, which is a proven anti-viral herbal drug, certified by the Government and is being prescribed by all the practitioners.

She stated that our body is the best medicine to recover and that sleep is our good recovery machine. Western medicines are not standardized for our Indian ethnicity. We should follow the dietary system of our ancestors like consumption of millets, cereals, vegetables etc., since our gene would have adaptability on these by nature. We are rediscovering the facts about traditional diet only after realising the hardships in the management of life style disorders. She observed that the practice of kitchen garden has decreased and suggested for promotion of kitchen/roof garden.

She also mentioned that the causes for lifestyle disorders are due to lack of physical activities, viz. Cycling, Walking etc. Further, she emphasized on herbal foods, regular practice of consuming all the 6 tastes, especially the bitter taste to be a part of our daily diet to lead a healthy life. She informed that the Department has taken up a trial study on “Control of Diabetes through Yoga” which

would be completed within a month’s time. The intermediary results of the study show that 60% improvement is realised in the diabetic patients through practice of Yoga.

She highlighted the fact that, nowadays, Breast and Cervical Cancers are more commonly reported and its occurrence is mainly due to increased prevalence of stress among women. Also, impatience among people irrespective of age has become an increasing trend which has its greater impact on health. Hence, she suggested that there is a need for the conduct of research studies viz., “RoleofBehaviouralchanges–Acauseforlifestyle disorders ” and “Stress - A cause for Cancer in Women” .

In regard to the V-C’s advises on promoting millet consumption through AYUSH, the Commissioner of Indian system of Medicine and Homoeopathy assured to take up a programme to promote millets along with vegetable diet in a mission mode. Agreeing to the suggestion of V-C for organising workshops/ Seminars in association with SPC, she replied that NRHM funds are awaited by the Department and if it is not made possible, she would come up to State Planning Commission. As regards submitting of proposals on Anaemia Control Programme for the districts of Ariyalur and Perambalur, she stated that she would take up the programme in 5 blocks under SBGF.
