MISSION STATEMENT

John Paolo’s Xtreme Beauty Institute, Goldwell Products Artistry is committed to educating and creating Cosmetologists, Estheticians and Nail Specialists who will be a credit to themselves, their profession and their community. Therefore, we feel it is significant to strive for professionalism and good conduct at our institution. In order to maximize each student’s experience at John Paolo’s Xtreme Beauty Institute, Goldwell Products Artistry we encourage students to learn to the best of his or her capability. John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry is responsive to the changing needs of the community and maintains a caring involvement that is dedicated to personal attention. FACILITIES

John Paolo’s Xtreme Beauty Institute, Goldwell Products Artistry, main-campus (separately licensed) is located near the gateway into Saratoga Springs on Route 50, at 2144 Saratoga Avenue, in the historic Town of Ballston Spa, New York. The Institute’s approx. 5000 sq feet include an esthetics room, and two fully equipped practical laboratories, located on the first floor in a professional salon-like setting. Additional classrooms which may be used for lectures and testing and administrative offices are also located on the first floor. All facilities are fully handicap accessible.

John Paolo’s Xtreme Beauty Institute, Goldwell Products Artistry, the Utica, Central NY branch campus, (separately licensed) is located at 4948 Commercial Drive, New York Mills, New York. The Institute’s approx. 4500 sq feet include a practical laboratory/classroom which is equipped to handle up to thirty students at a given time and is located on the first floor in a professional salon-like setting. Additional classrooms used for esthetics, manicures & pedicures may be used for lectures and testing, and the admissions room and administrative office are also located on the first floor. All facilities are fully handicap accessible. John Paolo’s Xtreme Beauty Institute, Goldwell Products Artistry, the Kingston branch campus (separately licensed) is located at 307 Wall St., Floor 2, in the historic Town of Kingston, New York. The Institute’s approx. 2400 square feet include a practical laboratory, which is equipped to handle up to thirty students at a given time and is located on the first floor in a professional salon like setting. Additional classrooms which may be used for lectures and testing and administrative offices are also located on the first floor. All facilities are fully handicap accessible. CATALOG DISCLAIMER Students should be aware that some information in the catalog may change. It is recommended that students considering enrollment check with The Institute director to determine if there is any change from the information provided in the catalog. Admissions Policy

This Admissions Policy is used to define the enrollment requirements for individuals who wish to enroll at John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry as a regular student. John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry must adhere to the rules and regulation of the U.S. Department of Education, our Accrediting Agency and the New York State Education Department. General Admission Requirements John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry is an equal opportunity employer and follows the same policies in accepting applications from potential students. John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry is open to all students without regard to race, color, religion, age, sex, creed, origin, sexual orientation, disability or marital status. To be eligible for admission, an applicant must be able to read and write English, and the student must meet the following requirements:

Be at least 17 years of age (Age will be verified with Individual’s ID)

Have an initial interview with the Admissions Representative (student, spouse or parent)

Provide a valid driver’s license, State ID with photo or valid passport

Have a verifiable Social Security Number

Be able to provide proof of appropriate educational requirement such as:

1) Copy of High School Diploma, or 2) Copy of High School Transcript showing graduation date 3) Copy of GED certificate 4) Have a recognized equivalent of High School Diploma such as a home-

schooled certificate by the state where the student resided during home school. The student must have completed homeschooling at the secondary level as defined by state law. Due to the passage of Consolidated Appropriations Act of 2012, students who do not have a high school diploma or equivalent and did not complete secondary school in a home-school setting can no longer gain eligibility for Title IV, HEA funds by passing an “ability-to-benefit” test.

Be able to speak, read and write fluently in English (all classes are taught in English)

Provide documentation of eligibility to work in United States.

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry does not accept Ability to Benefit (ATB) students and does not offer a Comprehensive Transition Postsecondary Program. Students with Disabilities John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry complies with the Americans with Disabilities Act of 1990 and is wheelchair accessible. If enrolled under training with a government agency, institution district, and/or other entity, students must meet the admission requirements set out in the training agreement and/or applicable state licensing or certifications requirements. The facility is equipped with ramp access from the parking lot, extra wide hallways and doors, A.D.A. required door handles and a restroom to accommodate disabled students.

If you are interested in attending John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry but are in need of reasonable accommodations, you should schedule an appointment with the Director. At this meeting, we will discuss the nature of the reported disability and its impact on learning. We will also discuss the process of receiving reasonable accommodations at John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry, and the types of accommodations available. Please bring copies of current documentation of a disability to this meeting. Documentation must be provided by a medical expert within the last three years and include:

A diagnosis of the disability;

How the diagnosis was determined (what tests were given and the results); and

A clinical summary, which includes an assessment of how the disability will impact the individual in a college environment and what accommodations are recommended.

Upon completion of the initial meeting, a formal request for the accommodation must be submitted in writing to John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry. The initial meeting, formal request, and response from John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry must take place prior to the pre-enrollment process. Note: In order to be eligible for Title IV, HEA funding, you must be able to benefit with the reasonable accommodations. Financial arrangements: *Students may not be accepted for enrollment if they cannot provide credit worthiness. Please see “Method and Terms of Payment”. The student must make financial arrangements with the Financial Aid Office in regards to FAFSA application, Scholarships and/or Cash payment plan prior to enrolling. Prior to admission the prospective student is given an enrollment agreement form, an interview with John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry official and is given a pre-enrollment questionnaire packet, which they are required to read, understand and sign. The interview will elaborate on course description, the career opportunities and the physical demands on the job and in school, Licensing and State Board requirements. An explanation on attendance and academic requirements will be given to the prospective student and how those requirements can affect the student’s satisfactory performance requirements. The prospective student will be informed that attendance hours may be withheld for non-payment of tuition. A staff member of instructor will give the prospective student a tour of the facilities. Any questions from the prospective student will be answered truthfully, promptly and in sufficient detail to eliminate confusion. Transfer Students Enrollment is available for students wishing to transfer to John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry after they have withdrawn from other schools both in and out of state. John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry does not recruit students already

attending or admitted to another school offering similar programs of study. All requests for exemption or credit for prior education must be submitted prior to the starting of the training. All new students may request exemption from a course(s) based on post-secondary education and training. Transcripts from national or regionally accredited colleges and universities may be submitted for credit. Credit may be given for related courses taken within the last year with an earned grade of 70% or above. Transfer of credit will be limited to enrollment at John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry and an official Transcript must be received prior to starting class. Credit is limited to no more than 1/3 of the total program and certain classes may not be eligible for credit. Note: All hours attempted will count toward the Title IV, HEA funding 150% quantitative requirements not just the hours that were accepted as transfer hours. Reinstatement A prior student requesting to be reinstated as an active student, based on whatever reasons or

circumstances, should do so in writing to the Director. Supportive documentation and/or

information concerning any mitigating circumstances should be noted in the request. The

requesting prior student shall be notified of the Reinstatement Review within five (5) days following

the decision of the Director. Students who are reinstated will return at the same satisfactory

academic progress status as at the time of withdrawal.

Admission Process

1. Tour the facility and attend an Informational Interview. Parent or spouses encouraged to be present.

2. Submit proof of age, diploma and employability 3. Complete Enrollment Agreement

How Eligibility is determined for Title IV, HEA To receive Federal Student Aid, you will need:

1. Qualify to obtain a college or career school education, either by having a high school diploma or General Education Development (GED) certificate, or by completing a high school education in a homeschooling setting approved under state law.

2. Be enrolled or accepted for enrollment as a regular student in an eligible degree or certificate program

3. Be registered with Selective Service, if you are a male (you must register between the ages of 18 and 25)

Men exempted from the requirement to register include:

Males currently in the armed services and on active duty (this exemption does not apply to members of the Reserve and National Guard who are not on active duty);

Males who are not yet 18 at the time that they complete their application (an update is not required during the year even if the student turns 18 after completing the application);

Males born before 1960;

Citizens of the Republic of Palau, the republic of the Marshall Islands, or the Federated States of Micronesia*;

Noncitizens that first entered the U.S. as lawful non-immigrants on a valid visa and remained in the U.S. on the terms of that visa until after they turned 26.

4. Have a valid Social Security number unless you are from the Republic of Marshall Islands, Federated States of Micronesia, or the Republic of Palau.

5. Completed a FAFSA and John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry must have current ISIR to start the initial eligibility process.

6. Sign certifying statements on the FAFSA stating that:

you are not in default on a federal student loan

do not owe a refund on a federal grant

sign the required statement that you will use federal student aid only for educational purposes

7. Maintain satisfactory academic progress (SAP) while you are attending college or a career school

8. Be enrolled at least halftime to receive assistance from the Direct Loan Program 9. The Pell Grant program does not require half time enrollment, but the student

enrollment status does affect the amount of Pell a student may receive. A student may receive Pell for a total of 12 payment periods or 600%. Once the student reached this limit, no further Pell may be received.

In addition, you must meet one of the following:

1. Be a U.S. CITIZEN or U.S. NATIONAL You are a U.S. citizen if you were born in the United States or certain U.S. territories, if you were born abroad to parents who are U.S. citizens, or if you have obtained citizenship status through naturalization. If you were born in American Samoa or Swains Island, then you are a U.S. national.

2. Have a GREEN CARD You are eligible if you have a Form I-551, I-151, or I-551C, also known as a green card, showing you are a U.S. permanent resident

3. Have an ARRIVAL-DEPARTURE RECORD You’re Arrival-Departure Record (I-94) from U.S. Citizenship and Immigration Services must show one of the following:

Refugee

Asylum Granted

Cuban-Haitian Entrant (Status Pending)

Conditional Entrant (valid only of issued before April 1, 1980)

Parolee

4. Have BATTERED IMMIGRANT STATUS You are designated as a “battered immigrant-qualified alien” if you are a victim of abuse by your citizen or permanent resident spouse, or you are the child of a person designated as such under the Violence Against Women Act.

5. Have a T-VISA You are eligible if you have a T-Visa or a parent with a T-1 Visa

Veterans Benefits/Other Funding Sources Selected programs of study at John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry are approved by the Veterans Commission for enrollment of those eligible to receive benefits under Section 3676, Chapters 30 or 32, Title 38. The determination for NYS Division of Veterans Affairs funds are made directly through the VA Commissions. Incarcerated Applicants A Student is considered to be incarcerated if she/he is serving a criminal sentence in a federal, state, or local penitentiary, prison, jail, reformatory, work farm, or similar correctional institution (whether it is operated by the government or a contractor). A student is not considered to be incarcerated if she/he is in a halfway house or home detention or is sentenced to serve only weekends. Our attendance policy specifies that all classed and practical studies are done at The Institute’s physical location; therefore, incarcerated students are not eligible for admissions. Conviction for possession or sale of illegal drugs

A Federal or state drug conviction can disqualify a student for FSA funds. The student self-certifies in applying for aid that she/he is eligible for by using the FAFSA. John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry is not required to confirm this unless there is evidence of conflicting information.

The chart below illustrates the period of ineligibility for FSA funds, depending on whether the conviction was for sale or possession and whether the student had previous offenses. (A conviction for the sale of drugs includes conviction for conspiring to sell drugs)

If a student was convicted of both possessing and selling illegal drugs, and the periods of ineligibility are different the student will be ineligible for the longer period.

A student regains eligibility the day after the period of ineligible ends or when she/he successfully completes a qualified drug rehabilitation program. Further drug conviction will make her/him ineligible again

When a student regains eligibility during the award year, John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry may award Pell and/or Loan for the current year

A qualified drug rehabilitation program must include at least two unannounced drug tests and must satisfy at least one of the following requirements:

1. Be qualified to receive funds directly or indirectly from a federal, state, or local government program

2. Be qualified to receive payment directly or indirectly from a federally or state-licensed insurance company

3. Be administered or recognized by a federally or state-licensed hospital, health clinic or medical doctor

Upon receipt of all required documents and in good order, the prospective student is eligible to enroll in John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry. When all admissions criteria and requirements are met, the prospective student is given the date of the next class. The prospective student is asked to bring in a photo of themself and is informed of the appropriate dress code. The first day of class will include financial aid and academic orientation, in which the students will sign their enrollment contract, and fill out additional paperwork required. Federal Financial Aid To be considered for Federal Financial Aid, a student must complete the Free application for Federal Aid on line. The student and the parent (in the case of a dependent student) may sign the FAFSA on line by using a Pin number. Once the student completes the FAFSA, and it is processed by the government, John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry will receive an ISIR which will contain the Estimated Family Contribution and let the student know if she/he is selected for verification. Verification Each year at least 30% of financial aid recipients are randomly selected for verification by the U.S. Department of Education. If a student is selected for federal verification, they will be asked to complete a Verification Worksheet (provided by the Office of Student Financial Planning) and must provide additional information before financial aid can be disbursed to the student account. This documentation may include but is not limited to official federal tax transcript and W-2 forms (student’s, spouse and/or parents/guardians), proof of untaxed income, housing allowances, etc. Students will be notified in writing of all documents required to fulfill this federal requirement. If after review by the Office of Student Financial Planning, if there are any changes to the Estimated Family Contribution and possible the financial aid available, the student will be notified in writing. CONTACT INFORMATION FOR ASSISTANCE IN OBTAINING INSTITUTIONAL OR FINANCIAL AID INFORMATION Disclosure Requirement: Made available through appropriate publications, mailing, or electronic media. Each institution must make available to prospective and enrolled students information regarding how and where to contact individuals designated to assist enrolled or prospective students in obtaining the institutional or financial aid information required to be disclosed under HEA Sec. 485(a). This information is posted on John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry’s website (JOHNPAOLO.EDU) and can be found in the student catalog. Print copies are available upon request.

GRADUATION REQUIREMENTS

To graduate, a John Paolo’s Xtreme Beauty Institute, Goldwell Products Artistry a

student must complete 1000 hours in the Cosmetology curriculum, 600 hours in the Esthetics

curriculum, or 250 hours in the Nail Specialty curriculum with a minimum overall grade point

average of 70% and must pass both written and practical final examinations. Students must fulfill

PLACEMENT ASSISTANCE

John Paolo’s Xtreme Beauty Institute, Goldwell Products Artistry provides information

on career placement opportunities to students and graduates. An active listing of job

opportunities is maintained on a Placement Bulletin Board and our staff assists in coordinating

positions with the individual needs and talents of our students and graduates. Additionally, staff

is available to review resumes and assist students and graduates in scheduling interviews when

such assistance is necessary. While job placement assistance service may be provided, it is

understood that John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry cannot promise

or guarantee employment to any student or graduate.

STUDENT FILES

John Paolo’s Xtreme Beauty Institute, Goldwell Products Artistry affirms the right of

students and the parent/guardian of dependent minor students to know what records are

maintained about them and the type of information contained in such records. Student access to

their records is limited only by reasonable regulations as to time, place and supervision and John

Paolo’s Xtreme Beauty Institute Goldwell Products Artistry guarantees access to students and/or

the parent or guardian of dependent minor students with reasonable written notice.

With the exception of the parties or agencies permitted by law (including the accrediting

agency, NY State Education Department and the US Department of Education), John Paolo’s

Xtreme Beauty Institute, Goldwell Products Artistry will not release personal or academic

information about a student without the written consent of the student or the parent/guardian of

dependent minor students involved. Students desiring the release of records maintained by John

Paolo’s Xtreme Beauty Institute Goldwell Products Artistry must specify in writing the records to

be released, the reason for such release, and the recipient of the records each time they wish

information to be released. Students should also indicate if they desire a copy of the records to

be released.

Official and student copies of transcripts will be issued at the written request of the

student or the parent/guardian of a dependent minor student provided the student is in good

standing and does not have any financial holds. The initial copy is free of charge, and then a

subsequent fee of $3 per additional transcript.

OPTIONAL FIELD TRIPS

Field trips are an optional resource for students wishing to avail themselves of an occasional out-of-

school experience related to their study program. These field trips may incur a cost to the student (ie.

price of admission, transportation, etc.) Any student wishing to participate must sign a field trip

waiver from John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry office. Clock hours

cannot be awarded during field trips.

N.Y.S.E.D. REFUND POLICY

A. Any monies due the applicant or student shall be refunded within 30 days of official

cancellation or withdrawal. Official cancellation or withdrawal shall occur on the

earlier of the dates that:

1. An applicant is not accepted by John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry. Such applicant shall be entitled to a refund of all monies paid;

2. A student (or the parent/guardian of a student under legal age) cancels their

agreement within three business days of signing the agreement. In this case all

monies collected by John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry will be refunded regardless of whether or not the student has begun

training;

3. A student (or the parent/guardian of a student under legal age) cancels their

agreement after three business days and before instruction begins. In this case

he/she shall be entitled to a refund of all monies paid less:

a. The $100 Non- Refundable Application Fee, plus;

b. The cost of any textbooks, equipment or supplies accepted;

4. A student notifies John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry of

his/her intent to withdraw;

5. A student on an approved leave of absence notifies John Paolo’s Xtreme Beauty Institute

Goldwell Products Artistry in writing that he or she will not be returning. The date of

withdrawal shall be the Student’s last day of physical attendance; or

6. A student is expelled by John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry.

7. A course is canceled by John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry.

In types 2, 3, 4, or 5 official cancellations or withdrawals, the cancellation date will be

determined by the postmark on written notification, or the date said information is delivered to

the Director in person.

Students failing to attend school for14 days and not on an approved leave of absence will

be considered as terminated. Any monies due the student after unofficial withdrawal shall be

refunded within 30 days of a determination that the student has withdrawn.

In the event that a program is canceled or John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry is closed any student who has enrolled but not yet started training shall receive

a full refund of all monies paid. Any student currently enrolled in a program slated for

cancellation shall be allowed to complete their training before the program and/or John Paolo’s

Xtreme Beauty Institute Goldwell Products Artistry is closed, receive a pro-rata refund for

transfer to another school based on hours accepted by the receiving school, participate in a

Teach –Out Agreement or the student may receive a full refund of tuition.

for education purposes only. Therefore, if a student withdraws before completing the program, a

portion of the funds received may have to be returned. John Paolo’s Xtreme Beauty Institute

Goldwell Products Artistry will calculate the amount of tuition to be returned to the Title IV,

HEA Federal fund programs according to the policies listed below:

Return of Title IV, HEA Policy This policy applies to students who withdraw officially or unofficially, fail to return from

a leave of absence or are dismissed from enrollment. It is separate and distinct from John

Paolo’s Xtreme Beauty Institute Goldwell Products Artistry’s Refund Policy.

The calculated amount of the Return of Title IV, HEA (R2T4) funds that are required to

be returned for the students affected by this policy, are determined according to the following

definitions and procedures as prescribed by regulations.

The amount of Title IV, HEA aid earned is based on the amount of time a student spent in

academic attendance, and the total aid received; it has no relationship to the student’s incurred

institutional charges. Because these requirements deal only with Title IV, HEA funds, the order

of return of unearned funds does not include funds from sources other than the Title IV, HEA

programs.

Title IV, HEA funds are awarded to the student under the assumption that he/she will

attend school for the entire period for which the aid is awarded. When a student withdraws,

he/she may no longer be eligible for the full amount of Title IV, HEA funds that were originally

scheduled to be received. Therefore, the amount of Federal funds earned must be determined. If

the amount disbursed is greater than the amount earned, unearned funds must be returned.

The Payment Period for Clock Hour Schools such as John Paolo’s Xtreme Beauty

Institute is one-half of the academic year or program length (whichever is less).

The Date of Determination is the date that John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry determines is the withdrawal date. For schools that are required to take

attendance, the date of determination is no longer than 14 days after the Last Date of Attendance.

The Date of Determination starts the clock for timely refunds of Title IV funds, within 45 days

after the “Date of Determination”.

The Withdrawal Date for schools required to take attendance is the Last Date of

Attendance (LDA).

The Institution has 45 days from the date the institution determines that the student

withdrew to return all unearned funds for which it is responsible. John Paolo’s Xtreme Beauty

Institute Goldwell Products Artistry is required to notify the student if they owe a repayment via

written notice. John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry must advise the

student or parent that they have 14 calendar days from the date John Paolo’s Xtreme Beauty

Institute Goldwell Products Artistry sent the notification to accept a post withdrawal

disbursement. If a response is not received from the student or parent within the allowed time

frame or the student declines the funds, John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry will return any earned funds that John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry is holding to the Title IV, HEA programs. Post-withdrawal disbursements will

occur within 90 days of the date that the student withdrew.

Withdrawal Policy

“Official” Voluntary Withdrawal

A student is considered to be “Officially” withdrawn on the date the student notifies the Financial

Aid Director or School Director in writing of their intent to withdraw. The date of determination

for return and refund purposes will be the earliest of the following for official withdrawals:

1. Date student provided official notification of intent to withdraw, in writing.

2. The date the student began the withdrawal from John Paolo’s Xtreme Beauty

Institute Goldwell Products Artistry’s records.

Upon receipt of the withdrawal information John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry will complete the following:

1. Determine the student’s last date of attendance as of the last recorded date of

academic attendance on John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry’s attendance record, and

2. Perform two calculations:

a. The student’s ledger card and attendance record are reviewed to determine

the calculation of Return of Title IV, HEA funds the student has earned, and if

any, the amount of Title IV, HEA funds for which John Paolo’s Xtreme

Beauty Institute Goldwell Products Artistry is responsible. Returns made to

the Federal Funds Account are calculated using the Department’s Return of

Title IV, HEA Funds Worksheets, scheduled attendance and are based upon

the payment period.

b. Calculate John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry’s refund requirement (see School Refund Policy)

3. The student’s grade record will be updated to reflect his/her final grade.

4. John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry will return the

amount for any unearned portion of Title IV, HEA funds for which John Paolo’s

Xtreme Beauty Institute Goldwell Products Artistry is responsible within 45 days of

the date the official notice was provided.

5. If applicable, John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry

will provide the student with a letter explaining the Title IV, HEA requirements, to

include,

a. The amount of Title IV, HEA assistance the student has earned. This

amount is based upon the length of time the student was enrolled in the

program based on scheduled attendance and the amount of funds the student

received.

b. Any returns that will be made to the Federal program on the student’s

behalf as a result of exiting the program. If a student’s scheduled attendance

is more that 60% of the payment period, he/she is considered to have earned

100% of the Federal funds received for the payment period. In this case, no

funds need to be returned to the Federal funds.

6. Calculate John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry’s

refund requirement (See School Refund Policy)

7. John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry’s Controller will

return to the Federal fund programs any unearned portion of Title IV funds for which

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry is responsible

within 45 days of the date the withdrawal determination was made, and record on the

student’s ledger.

8. If applicable, John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry

will provide the student with a refund letter explaining Title IV requirements:

a. The amount of Title IV aid the student has earned based upon the length of

time the student was enrolled and scheduled to attend in the program and the

amount of aid the student received.

b. Advise the student in writing of the amount of unearned Title IV, HEA aid

and tuition and fees that he/she must return, if applicable.

9. Supply the student with final student ledger card showing outstanding balance due

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry and available

methods of repayment.

10. A copy of the completed worksheet, check, letter, and final ledger card will be

kept in the student’s file.

Withdraw Before 60%

The institution must perform a R2T4 to determine the amount of earned aid through the

60% point in each payment period. The institution will use the Department of Education’s

prorate schedule to determine the amount of the R2T4 funds the student has earned at the time of

withdrawal.

Withdraw After 60%

After the 60% point in the payment period, a student has earned 100% of the Title IV,

HEA funds he/she was scheduled to receive during this period. The institution must still perform

R2T4 to determine the amount of aid that the student has earned.

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry measures progress in

clock hours, and uses the payment period for the period of calculation.

The Calculation Formula

Determine the amount of Title IV, HEA aid that was disbursed plus Title IV, HEA aid

that could have been disbursed.

Calculate the percentage of Title IV, HEA aid earned:

a. Divide the number of clock hours scheduled to be completed (from the first day of

class until the last date of attendance) in the payment period as of the last date of

attendance in the payment period by the total clock hours in the payment period.

(rounded to one significant digit to the right of the decimal point, ex. .4493=44.9%)

b. If this percentage is greater than 60%, the student earns 100%.

c. If this percentage is less than or equal to 60%, proceeds with calculation.

Percentage earned from (multiplied by) Total aid disbursed, or could have been

disbursed = AMOUNT STUDENT EARNED.

Subtract the Title IV aid earned from the total disbursed = AMOUNT TO BE

RETURNED

100% minus percent earned = UNEARNED PERCENT

Unearned percent (multiplied by) Total Institutional Charges for the period =

AMOUNT DUE FROM JOHN PAOLO’S XTREME BEAUTY INSTITUTE

GOLDWELL PRODUCTS ARTISTRY.

If the percent of Title IV aid disbursed is greater than the percent unearned (multiplied

by) institutional charges for the period, the amount disbursed will be used in place of

the percent unearned.

If the percent unearned (multiplied by) institutional charges for the period are less

than the amount due from John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry, the student must return or repay one-half of the remaining unearned Federal

Pell Grant.

Student is not required to return the overpayment if this amount is equal to or less

than 50% of the total grant assistance that was disbursed/could have been disbursed.

This student is also not required to return an overpayment if the amount is $50 or less.

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry will issue a grant

overpayment notice to the student within 30 days from the date John Paolo’s Xtreme

Beauty Institute Goldwell Products Artistry’s determination that student withdrew,

giving the student 45 days to either:

1. Repay the overpayment in full to John Paolo’s Xtreme Beauty Institute, Or

2. Sign a repayment agreement with the U.S. Department of Education.

Order of Return

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry is authorized to return

any excess funds after applying them to current outstanding Cost of Attendance (COA) charges.

office upon student request.

In accordance with Federal regulations, when Title IV, HEA financial aid is involved, the

calculated amount of the R2T4 Funds is allocated in the following order:

Unsubsidized Direct Stafford loans (other than PLUS loans)

Subsidized Direct Stafford loans

Direct PLUS loans

Federal Pell Grants for which a Return is required

Federal Supplemental Education Opportunity Grant

Iraq and Afghanistan Service Grant for which a Return is required

The Student

Earned Aid

Title IV, HEA aid is earned in a prorated manner on a per diem basis (clock hours) up to

the 60% point in the payment period. Title IV, HEA aid is viewed as 100% earned after that

point in time. A copy of the worksheet used for this calculation can be requested from the

financial aid director.

Post Withdrawal

If you did not receive all the funds that you have earned, you may be due a post-

withdrawal disbursement. John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry may

use a portion or all of your post-withdrawal disbursement for tuition and fees (as contracted with

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry). For all other school charges,

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry needs your permission to use

the post-withdrawal disbursement. If you do not give permission, you will be offered the funds.

However, it may be in your best interest to allow John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry to keep the funds to reduce your debt at school.

The post-withdrawal disbursement must be applied to outstanding institutional charges

before being paid directly to the student.

Institution Responsibilities

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry’s responsibilities in regards to

Title IV, HEA funds follow:

Providing students information in this policy.

Identifying students who are affected by this policy and completing the return of Title IV

funds calculation for those students;

Returning any Title IV, HEA funds due to the correct Title IV programs.

The institution is not always required to return all of the excess funds; there are situations

once the R2T4 calculations have been completed in which the student must return unearned

aid.

Overpayment of Title IV, HEA Funds

Any amount of unearned grant funds that a student must return is called an overpayment.

The amount of grant overpayment you must repay is half of the grant funds you received. You

must make arrangements with John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry

or the Department of Education to return the amount of unearned grant funds.

Student Responsibilities in regards to return of Title IV, HEA funds

Returning to the Title IV, HEA programs any funds that were dispersed to the student in

which the student was determined to be ineligible for via the R2T4 calculation.

Any notification of withdraw should be in writing and addressed to the appropriate

institutional official.

A student may rescind his or her notification of intent to withdraw. Submissions of intent

to rescind a withdraw notice must be filed in writing.

Either of these notifications, to withdraw or rescind to withdraw must be made to the

official records/Application personal at your school.

Refunds vs. Return to Title IV

The requirements for the Title IV, HEA program funds when you withdraw are separate

from any refund policy that John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry

may have to return you due to a cash credit balance. Therefore, you may still owe John Paolo’s

Xtreme Beauty Institute Goldwell Products Artistry to cover unpaid institutional charges. John

Paolo’s Xtreme Beauty Institute Goldwell Products Artistry may also charge you for any Title IV,

HEA program funds that they were required to return on your behalf.

Chapters 30 or 32, Title 38. The determination for NYS Division of Veterans Affairs funds are

made directly through the VA Commissions.

Students and parents can make inquiries into additional financial aid other than federal

Pell Grants, Subsidized Loans and Unsubsidized Loans. If you receive a scholarship from a

community group, business or other organization, please forward a copy of the award notice you

received. The award must be accounted as part of your financial aid package, and we need you to

keep us informed. Obtaining scholarships can require a bit of research, but there are tools to help

you with that research.

SCHOOL POLICIES AND REGULATIONS CONCERNING STUDENT CONDUCT

AND CONDITIONS FOR SUSPENSION or EXPULSION

1. Students are expected to adhere to school attendance policies.

2. Students must be in uniform at all times.

3. Students must conduct themselves in a business-like manner while working on the clinic

floor.

4. Students are expected to clean up after themselves at all times.

5. Each student is assigned a duty that is her/his responsibility and must be performed each

day at the designated times.

6. Students are to eat in the lunch room only.

7. No smoking is permitted on school property.

8. Each student is responsible for her/his own kit and tools. All equipment must be

available every day for class. If lost or stolen, it must be replaced immediately. Everyone

is expected to maintain the kit and tools as originally issued.

9. Students are required to show consideration for the property of others. NO

BORROWING!

10. Any unprofessional behavior or conduct in the classroom or clinic toward staff or

customers will not be tolerated.

11. A student can be suspended or terminated for late payments or non-payment at the

discretion of the Director.

12. Cell phone use is PROHIBITED during school hours. All cell phones must be

turned off or set on vibrate. If you must answer an urgent call, quietly exit the

classroom and take the call in an area where others won’t be disturbed. Outgoing

calls should be made during lunch or break only. Incoming calls should be limited to

urgent calls only.

Non Compliance with the above regulations constitutes grounds for suspension and/or

expulsion. If a student does not obey the rules of John Paolo’s Xtreme Beauty Institute

Goldwell Products Artistry, he/she will be warned and a record will be kept in the

student’s file. If the Director feels the infraction is intolerable the student may be

dismissed from school.

COPYRIGHTED MATERIALS;

The institution prohibits copyright infringement. Disciplinary action will be taken against

any student who distributes materials, including peer-to-peer file sharing and the prohibited use

of the institution’s information technology system for those activities. Any student involved in

such an act will be reported to the proper authorities and charges will be pressed.

SCHOOL CLOSURE POLICY

If a school voluntarily closes permanently and ceases to offer instruction after students have

enrolled, and instruction has begun, John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry must make arrangements for students.

The policy requires that:

Full refund of tuition is made

A list of all students who were enrolled at the time of school closure including the amount of

each full refund is submitted to NACCAS.

CAMPUS SECURITY POLICY

The personal safety and security of each student, instructor and staff member is both an

individual and community concern. While John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry strives to provide a learning environment free of undue risks to persons and

property, it is each person’s responsibility to pay attention to his or her surroundings, to avoid

potentially hazardous situations, and to exercise his or her own best judgment to maintain

personal safety and well-being. In addition, each member of the community must call attention

to conditions or situations which threaten the safety and security of others by contacting

appropriate campus and/or community authorities.

Policies and Procedures

1. Crimes on campus can be reported by completing an Incident Report which is available in

the Administrative office. Reports should be submitted to the Director who will review the

report, confer with other staff members on responsive actions and when appropriate, information

describing the incident or unsafe condition will be posted on a bulletin board dedicated to

campus security.

2. All campus facilities are open to members of the campus community, their invited guests

and clinical clients. All visitors are required to register with the administrative office. All

campus buildings are locked and unlocked at regular, specified times and are not accessible when

locked except by authorized persons.

3. In an emergency, students may contact police by dialing 911 from any school phone.

Students are encouraged to report crimes to the police and to utilize other services of the police

department.

4. Students, employees and campus visitors are subject to all local, state and federal laws, in

addition to campus regulations. Law enforcement officers may come on campus at any time.

John Paul’s Institute has the responsibility, under the Campus Security Act, to advise appropriate

authorities of violations of civil or criminal law committed by anyone on campus when a request

is made by those authorities for specific information or when there is danger to life and/or

property.

5. John Paolo’s Xtreme Beauty Institute, Goldwell Products Artistry subscribes to and

conforms to the Drug free Workplace Act and the Drug free Schools and Campuses Act. In

accordance with those Acts, notification is sent to all members of the community detailing

campus policy about substance abuse. The use of any drugs within the campus community must

be within the limits of federal and state laws. John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry reminds students of their responsibility to know and to obey those federal and

state laws and statutes prohibiting the use of illegal drugs. While John Paolo’s Xtreme Beauty

Institute Goldwell Products Artistry is not responsible for the enforcement of federal and state

drug laws, those agencies charged with this legal responsibility have the authority to carry out

their duties on campus. Individuals who illegally possess, use or supply prohibited drugs within

the campus community do so knowing that they risk action by the appropriate civil authorities.

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry policy specifically states that:

o The manufacture, sale, distribution, possession or use of controlled substances by

students or employees is prohibited.

o The Institution is opposed to the illegal or irresponsible use of alcohol and expects

its students and employees to obey John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry Regulations and the local, state and federal laws concerning the

possession, use or distribution of alcoholic substances.

o The Institution prohibits sexual assault committed by a student, faculty member or

staff member against any student, faculty member, staff member or visitor on campus

property or in the setting of any program sponsored by John Paolo’s Xtreme Beauty

Institute Goldwell Products Artistry.

6. Sanctions for violation of this policy may include (but are not limited to) counseling,

chemical dependency referral, probation, suspension or expulsion.

Annual Security Report

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry distributes an annual security

report by October 1 of each year – including crime statistics and campus policies and procedures

regarding security matters – to enrolled students and current employees. The institution ensures

that prospective students and prospective employees are informed of the availability of the report

and given an opportunity to obtain it from the main administrative office. The statistics portion

of the report is also submitted to ED, as required.

o The Director of Campus Safety compiles, maintains, distributes and the CEO/President

submits the crime statistics to ED

Timely Warning

In addition to the required annual campus security report, schools are required to provide a timely

warning to the campus community of any occurrences of the following crimes that are reported to

campus security authorities or local police and are considered to represent a serious or continuing

threat to students and employees. These crimes are: criminal homicide, forcible and non-

forcible sex offenses, robbery, aggravated assault, burglary, motor vehicle theft and arson, crimes

that show evidence of prejudice, violations of liquor and drug law., illegal weapon possession

and persons referred for campus disciplinary action for liquor, drug and weapons law violations.

ATTENDANCE POLICY

Regular attendance at John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry

is an integral part of maintaining satisfactory progress during the student’s course. Satisfactory

progress in attendance and academic work is a requirement for all students enrolled in this

school.

Students are required to maintain prompt and regular attendance in all classes and are

responsible for any and all assignments presented in their absence. Good attendance is directly

related to achievement and success. A complete record will be kept of student attendance.

Attendance will be recorded by instructional hour. At the end of every quarter, each student’s

final attendance status is recorded on the student’s transcript.

Daily attendance is expected of each student. Absences may occur, and make-up

opportunities are available each week to assist in keeping up with scheduled progress.

1. Students are expected to attend all classes and to report on time.

2. Roll call will be taken at:

9:00am 7:00am (Kingston Cosmetology)

12:30pm 12:00pm (Kingston Cosmetology)

5:00pm

If a student is not set up and ready for class and in proper uniform by each hour, as stated

above, that student will be considered late.

3. If a student misses roll call, the only way that student may enter class is to be signed in by

the teacher in the classroom and then at the reception desk with his/her time sheet. At

that time, the student must be in uniform and set up for class.

4. When a student is late for class, he/she will be considered absent for those hours or

portion thereof to the nearest quarter hour and he/she will be given credit from the nearest

available quarter hour time slot.

Example: Arrive at 9:40am - Credit from 9:45am

Arrive at 10:05am - Credit from 10:00am

5. A student’s last day of attendance must be a complete regularly scheduled day or evening.

No graduation will take place until 3:30pm (days or Saturdays) and 10:00pm for the

evening class.

6. A period of 30 minutes is allowed for lunch each day.

7. No student is allowed to work on a customer or classmate unless otherwise instructed by

the teacher.

8. Each student must complete 1000 hours of training in school for cosmetology. Each

Cosmetology student is provided with 84 hours, 50 hours for Esthetics and 21 hours for

Nail Specialty of allowed absence at no charge. Any absence in excess of the allowed

hours, and not covered by an excuse, will be charged to the student at a rate of $13.50 per

hour for Cosmetology and Esthetics or $12.00 per hour for Nail Specialty after their

expected graduation date.

9. An acceptable excuse for absence from school must be in written statement from a

physician, or death in the immediate family. This statement must be turned into the office

before the last day of the month that the absence occurred.* If a student’s illness is due to

an infectious, contagious disease or condition that is harmful to others, please report this

to the Director so that information may be passed to other students, such as warning

signs. Jury duty and/or other court ordered appearance are also an acceptable excuse for

absence when written verification is submitted.

Excuses for absence apply to the student’s illness, that of the student’s dependent

children, or a student caring for a parent or parent of a spouse.

Excuses not turned into the office before the end of the month of the absence will not be

accepted at any other time.

* Example: Absence occurred on October 15th - October 19th - excuse turned in

November 5th will not be accepted. Excuse turned in October 30th will be accepted.

10. When school is closed due to weather, holidays, or staff meetings, student will not be

charged with an absence or be given credit for that day or evening.

Leave Of Absence Policy

Students may request a Leave of Absence for medical, military or some unusual

circumstance up to one hundred eighty (180) calendar days. Students must demonstrate a

reasonable expectation to return to class and may be required to provide documentation.

Students must fill out a “Leave of Absence Request Form ” in advance unless unforeseen

circumstances prevent the student from doing so. Requests must be in writing and signed by the

student and must include the reason for the leave. A leave may be granted to a student who did

not provide the request prior to the LOA due to unforeseen circumstances. The reason for the

leave will be documented and the official request completed when possible

. If a leave of absence is approved, the student must return to school on the scheduled

return date or contact the Director to request an extension. In the event that a student does not

return on their scheduled return date, the student will be withdrawn and the withdrawal date for

the purpose of calculating a refund will always be the student’s last day of physical attendance.

At the Director’s discretion, students may be granted multiple leaves of absence as long

as the total number of days for all leaves does not exceed 180 days within a 12 month period.

This 12 month period begins on the first day of the student’s initial LOA.

A student who is granted a LOA that meets these criteria is not considered to have

withdrawn, and no refund calculation is required at that time. No monetary charges or

accumulated absences may be assessed to the student during a leave of absence. When a student

returns from an approved leave of absence, the student shall be placed in the instructional

program at the point commensurate with the skill level retained by the student at the time of

his/her return in accordance with the following:

For leaves of absence less than 30 calendar days, John Paolo’s Xtreme Beauty Institute

Goldwell Products Artistry shall assess the student’s retention level either through a counseling

session with the student or by employing a written or oral evaluation instrument designed to

measure a student’s level of retention in the instructional program; or

For leaves of absence 30 calendar days or longer, John Paolo’s Xtreme Beauty Institute

Goldwell Products Artistry shall assess a student’s retention level either through a written or oral

evaluation instrument designed to measure a student’s level of retention in each course in the

curriculum.

An approved leave of absence will extend students contract period and maximum time

frame by the same number of days taken in the leave of absence. Changes to the contract period

will be noted on a signed and dated contract addendum.

Make Up Time

To avoid additional absence charges, John Paolo’s Xtreme Beauty Institute, Goldwell

Products Artistry, has adopted the following make up policy:

1. Students are permitted to make up hours of absence before they reach their scheduled

graduation date at no charge. To make up hours completed after the scheduled graduation

date, the student will incur a charge of $13.50 per hour for Cosmetology and/or Esthetics

or $12.00 per hour for Nail Specialty.

2. Students can attend no more than eight (8) hours per day or evening and can have no

more than 40 hours per week

3. There must be room in the make up class for the student. The inclusion of the make up

student cannot exceed room capacities or student to teacher ratios.

4. Make up hours must be made up in the content area that was missed.

Saturday Sessions

A signup sheet is made available for those students who wish to attend makeup classes on

Saturday. Saturday hours are 9:00am – 2:30pm. Students must attend a minimum of 3 hours.

SATISFACTORY ACADEMIC PROGRESS POLICY

The Satisfactory Academic Progress Policy applies to all students enrolled in an approved

program, whether receiving Federal Title IV, HEA funds, partial funding assistance, or self-pay.

Federal regulations require all schools participating in state and federal financial aid, Title

IV, HEA programs to monitor Satisfactory Academic Progress (SAP). Students must meet

formal standards that measure their satisfactory academic progress toward graduation. The

policy is provided to all students prior to the first class session and is consistently applied to all

students. Evaluations are maintained in the student file.

New SAP definitions went into effect on July 1, 2011. John Paolo’s Xtreme Beauty

Institute Goldwell Products Artistry developed policies to determine the academic standards that

students are expected to meet and built a means and schedule of measuring the achievement of

those Qualitative and Quantitative (Pace) standards.

SAP standards are established in accordance with State Licensing, Accreditation and

Federal regulations. All students must maintain Satisfactory Academic Progress according to the

following standards in order to continue enrollment. Satisfactory Academic Progress is

measured at the end of each payment period and will be checked prior to disbursement of aid.

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry’s SAP policy for Title

IV, HEA students is the same as John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry’s standards for students enrolled in the same educational programs who are not receiving

Title IV, HEA funding. The Director of financial aid reviews the Title IV, HEA SAP policy to

ensure it meets all federal requirements and the Executive Director notifies the financial aid

office if John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry changes its academic

policies.

SAP evaluation periods are determined by program. Students will receive written

notification of their SAP status within three days of the end of the evaluation period. Copies of

all signed SAP evaluations will become part of the student’s permanent record. Students who are

not making SAP will be notified in writing. Failure to make SAP will impact eligibility for Title

IV, HEA financial aid.

The standards set for by John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry

as minimum requirements for maintaining satisfactory progress are:

(1) Qualitative - For all students to maintain a passing grade average of 70% or better in

theory and practical work throughout the course.

(2) Quantitative (Pace) - For each student to proceed through the program at a pace

leading to completion of the course within the maximum time frame of 150% of the published

length of the course. At the time of each Satisfactory Progress Evaluation the student must be

maintaining 67% cumulative attendance.

Qualitative Measure of Satisfactory Academic Progress (SAP)

The qualitative element used to determine satisfactory academic progress is a reasonable

system of grades as determined by assigned theory study. Theory is evaluated after each unit of

study. Students must maintain a cumulative theory grade average of 70% at the end of each

evaluation period.

Students must make up failed or missed tests and incomplete assignments. Practical skills

performances are counted toward course completion only when considered satisfactory or better

and are therefore, not a component of satisfactory progress. If performance does not meet

satisfactory academic requirements, it is not counted and the performance must be repeated until

a satisfactory level of performance is achieved. John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry used the following grading scale:

A=90-100 Excellent B=80-89 Good C=70-79 Passing D=69 or below FAILING #11

Students who fail to meet the Qualitative standards are placed first on Financial Aid

Warning; if no improvement over the next payment period, the student will be place on academic

suspension, with a loss of Title IV, HEA funds and they may appeal the decision. Please review

the appeal and probation requirements stated in this policy for guidance on this process. The

Director of Financial Aid monitors qualitative progress.

Quantitative (Pace) Measure of Satisfactory Academic Progress (SAP)

Students are required to complete his/her educational program in no longer than 150% of

the published length of the program as measured in clock hours as determined by the program in

order to considered making Satisfactory Academic Progress.

Evaluations are conducted at set intervals depending on the student’s program and

additionally at the end of each payment period to determine if the student has met the minimum

requirements. Evaluations are based on the cumulative attendance percentage as of the last day

of the evaluation period. The attendance percentage is determined by dividing the total hours

accrued by the total number of hours scheduled. An evaluation of the cumulative attendance

since the beginning of the course will indicate that, given the same attendance rate, the student

will graduate within the maximum time frame allowed.

Clock hours that have elapsed during a leave of absence will extend the student’s contract

period and maximum time frame by the same number of clock hours and days taken in the leave

of absence and will not be included in the student’s cumulative attendance percentage

calculation.

Cosmetology

For example: A full time student (30 hours per week) could complete their program in a

minimum timeframe of 34 weeks and a maximum timeframe of 51 weeks. For determining

satisfactory academic progress all Cosmetology students are evaluated at 450 and 900 actual

hours to determine that they are maintaining satisfactory progress and are therefore eligible for

Title IV funds if applicable.

Esthetics

For example: A full time student (30 hours per week) could complete their program in a

minimum timeframe of 20 weeks and a maximum timeframe of 30 weeks. For determining

satisfactory academic progress all Esthetics students are evaluated at 300 actual hours to

determine that they are maintaining satisfactory progress and are therefore eligible for Title IV

funds if applicable.

Nail Specialty

For example: A full time student (25 hours per week) could complete their program in a

minimum timeframe of 10 weeks and a maximum timeframe of 15 weeks. For determining

satisfactory academic progress all Nail Specialty students are evaluated at 100 and 200 actual

hours.

Academic Year Definition

900 clock hours and 26 weeks of instruction for Title IV, HEA funding.

A student who does achieve the minimum standards is no longer eligible for Title IV,

HEA Program Funds, if applicable, unless the student is on warning or has a prevailed upon

appeal of the determination that has resulted in the status of probation.

Financial Aid Warning

A student on Financial Aid Warning may continue to receive assistance under the Title

IV, HEA programs for one payment period only. At the end of the payment period if the student

has met the minimum grade and attendance requirement the student is considered to be meeting

Satisfactory Academic Progress.

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry evaluates Satisfactory

Academic Progress at the end of each payment period and checks the following:

1. Grade point average (GPA) and the student must maintain a minimum of a 70% GPA.

2. Attendance

3. Clock hours, the student must meet half the weeks of instruction for that programs

payment period to be eligible for the next payment.

4. Weeks of Instruction, the student must meet half the weeks of instruction for that

programs payment period to be eligible for the next payment.

If a student falls below a 70% GPA, or if the student is not completing the required

amount of clock hours to keep Pace with the requirements for graduation within the 150% time

frame, it will result in the student being placed on Financial Aid Warning for one payment

period. A student who is put on a Financial Aid Warning can continue to receive Title IV, HEA

funding for the next payment period after they receive the warning status.

If the student is not meeting Satisfactory Academic Progress at the end of the Financial

Aid Warning Period, the student will be placed on Academic Development Status, with loss

of Title IV, HEA funding and will be required to meet specific criteria of an improvement plan

to assist them in regaining SAP and Title IV, HEA eligibility. During this period the students

will not be eligible to receive Title IV, HEA funds but he/she may continue on a cash pay basis

with an approved payment plan. Arrangements for payment must be approved within 10 school

days of notification of development status.

If the student is making SAP at the end of the Financial Aid Warning, they shall be

returned to normal SAP status with no loss of Title IV eligibility.

Appeal Process

If the student does not make SAP at the end of the Financial Aid Warning period, they

lose their Title IV, HEA financial aid eligibility and will be placed on Academic Development

Status, with loss of Title IV, HEA funding, with the right to appeal. The student may have the

opportunity to have their Title IV, HEA financial aid eligibility reinstated by appealing the

Academic Develoment Status, with loss of Title IV, HEA funding decision and be placed on

Financial Aid Probation if the appeal is granted.

The student has five (5) days to institute an appeal. The appeal must be given to the

Executive Director who will meet with the Financial Aid Director to make a decision on the

appeal.

The student must describe in writing any unusual circumstances that they believe

deserves special consideration. The basis on which a student may file an appeal: death of a

relative, injury, or illness of the student or other special circumstance. The student must provide

supporting documents. The student must provide information as to why they did not make SAP

and what has changed that will allow them to make SAP by the next evaluation point.

Once the Executive Director receives the appeal, they will evaluate the appeal and

provide a decision within ten (10) business days. The student will receive written notification of

the outcome of their appeal and that the decision is final. A copy of the decision will become part

of their permanent record.

Financial Aid Probation

If Financial Aid Probation Status is approved, the student will regain Title IV, HEA

eligibility for the next payment period only. The student must be making SAP at the end of that

payment period to regain Title IV, HEA funding for the next payment period.

When a student is place on Financial Aid Probation status, he or she will be required to

do the following:

1. Agree to a written academic plan that specifies how the student will regain SAP. The

plan may include but is not limited to mandatory tutoring, scheduled advisement sessions,

extra course assignments, repeating a course for which the student received a failing grade,

and/or repeating a course from which the student withdrew.

2. Sign the academic plan (a copy will be kept in the student’s file)

A student on Financial Aid Probation because of a successful appeal is eligible for Title

IV, HEA funds for ONE payment period only. Students who regain SAP at the end of the next

payment period will have regained full eligibility for Title IV, HEA funding. The student on

financial aid warning, probation or Title IV, HEA funding suspension for a payment period may

not receive Title IV, HEA funding for the subsequent period unless the student makes SAP.

Reinstatement of Title IV, HEA Aid

Reinstatement of aid is limited to the period under evaluation. Students making SAP by

the conclusion of the Warning or Probation period will be removed from the warning/probation

status and regain eligibility for Title IV, HEA funding.

Transfer Students

Enrollment is available for students wishing to transfer to John Paolo’s Xteme Beauty

Institute Goldwell Products Artistry after they have withdrawn from other schools both in and

out of state. John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry does not recruit

students already attending or admitted to another school offering similar programs of study. All

requests for exemption or credit for prior education must be submitted prior to the start of

training. Credit may be given for related courses taken within the last 12 months with an earned

grade of 70% or above. An official transcript must be provided prior to starting class. Credit is

limited to no more than 1/3 of the total program hours and certain classes may not be eligible for

credit.

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry will count all transfer

hours that apply towards the student’s current program in determining SAP. All transfer hours

will be counted both as hours attempted and hours completed for the purpose of determining

when the allowable maximum time-frame has been exhausted. SAP evaluation periods are based

on actual contracted hours at John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry.

Leaves of Absence and Interruptions, Withdrawals

If student enrollment is temporarily interrupted for a Leave of Absence (LOA) (not to

exceed 180 days) and documentation to support the required leave of absence is submitted to the

Director (i.e. Doctor’s excuse, etc.), the student will return at the same satisfactory academic

progress status held prior to the leave of absence. Clock hours that have elapsed during a leave

of absence will extend the student’s contract period and maximum time frame by the same

number of clock hours and days taken in the leave of absence and will not be included in the

student’s cumulative attendance percentage calculation.

Reinstatement

A prior student requesting to be reinstated as an active student, based on whatever reasons

or circumstances, should do so in writing to the Director. Supportive documentation and/or

information concerning any mitigating circumstances should be noted in the request. The

requesting prior student shall be notified of the Reinstatement Review within five (5) days

following the decision of the Director. Students that are reinstated will return at the same

satisfactory academic progress status as at the time of withdrawal.

Reinstatement before 180 Days

Students who withdraw prior to completion of the course and wish to re-enroll within six

months (180 days) of the original official withdrawal date will return at the same satisfactory

academic progress status as at the time of withdrawal.

Student who have been terminated or withdrew from school may re-enroll (if determined

eligible) within 180 days and will not incur additional charges, however these students will be

responsible to pay any remaining balance from the previous enrollment that cannot be covered

with reinstated federal funds.

When a student returns from a temporary withdrawal or approved leave of absence, the

student shall be placed in the instructional program at the point commensurate with the skill level

retained by the student at the time of his/her return in accordance with the following:

student’s retention level either through a counseling session with the student or by employing a

instructional program.

Reinstatement after 180 Days

Students who have been terminated or withdrew from school and re-enroll (if determined

eligible), after more than 180 days will pay a $100 Application fee and will be charged for

contracted hours at the current tuition rate. The returning student’s Enrollment Agreement will

reflect the hours to be completed with a pro-rated tuition.

Course Incompletes, Repetitions and Non-Credit Remedial Courses

Course incompletes, repetitions and non-credit remedial courses do not apply to this

institution’s form of instruction. John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry does not issue incompletes and does not offer non-credit remedial courses. Students do

not repeat courses. All hours attended are considered attempted. It is not possible for students to

withdraw from individual subjects. Students withdrawing from John Paolo’s Xtreme Beauty

Institute Goldwell Products Artistry will receive credit for all work successfully completed up to

the point of withdrawal. Transfer clock hours received will not be included in the calculation of

the student’s GPA; however these hours will be counted toward quantitative requirements.

Official Withdrawal From School

In the event that circumstances beyond the student’s control make it necessary for

withdrawal the student should provide written notification of their intent to withdraw. The

Director will then complete a Withdrawal Notification Record using the student’s last date of

attendance as their withdrawal date.

Unofficial Withdrawal From School

In the event that the student withdraws without officially notifying John Paolo’s Xtreme

Beauty Institute Goldwell Products Artistry, the Director will contact the student in writing after

5 days of non-attendance to advise them that they will be withdrawn as of their last date of

attendance after 14 days of non-attendance.

STUDENT EVALUATIONS

All students do not progress at the same rate. The staff at John Paolo’s Xtreme Beauty

Institute Goldwell Products Artistry, assesses each student individually and expects that each

will show a measurable rate of progress at the end of each level. Students who receive less than

a 70% on written tests will receive remedial help and retake the test. The retake score will be the

only score recorded

Student evaluation is done on an individual basis daily. Progress records for both written

and practical grades are maintained and reported to the student throughout each unit, written

reports are provided on a regularly scheduled basis. Failure to maintain a 70% (“C”) average or

better may result in repeating the unit. A unit may be repeated only once. Students may make up

exams and receive additional training in weak areas at no extra charge.

COMPLAINT PROCEDURES

Internal Complaint Procedure

If a student has a complaint or problem with another student, he/she should advise their

Instructor who will talk to the other student. If the situation is not resolved, the student may then

make an appointment to meet with John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry Director. The Director will meet with each party individually to assess the matter and

will then advise both of a plan to resolve the problem. Notes will be taken and a report filed in

all parties’ permanent files. If this action fails to resolve the problem, an appointment will be

made for all relevant parties to meet with the Director and/or Executive Director. The problem

must be resolved here. If a student is found responsible, the perpetrator may be placed on

Probation and an additional meeting held within thirty (30) days to review the status of the

situation. If the problem persists, the troubled student may be dismissed from John Paolo’s

Xtreme Beauty Institute Goldwell Products Artistry.

If a student has a complaint or problem with an instructor, he/she should bring this matter

to the attention of the Director. The Director will meet with the Instructor and if necessary, a

meeting with all involved parties will be scheduled in an attempt to reach a resolution. Again, a

written report will be filed in all parties’ permanent files.

What You Should Know about Licensed Private Schools and Registered Business Schools in New York State

What is the purpose of this pamphlet?

All prospective and enrolled students in a non-degree granting proprietary school are required to receive this

pamphlet. This pamphlet provides an overview of students' rights with regard to filing a complaint against a school

and accessing the tuition reimbursement fund if they are a victim of certain violations by John Paolo’s Xtreme

Beauty Institute Goldwell Products Artistry.

Trade schools which are licensed by the New York State Education Department and business schools which are

registered by this Department are required to meet very specific standards under the Education Law and

Commissioner's Regulations. These standards are designed to help insure the educational appropriateness of the

programs which schools offer. It is important for you to realize that the New York State Education Department's

Bureau of Proprietary School Supervision closely monitors and regulates all non-degree granting proprietary

schools. John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry are required to have their teachers meet

standards in order to be licensed by the Department. Schools are also required to have their curriculum approved by

the New York State Education Department every three years, thereby helping to insure that all curriculum offered in

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry are educationally sound.

In addition, staff members of the Bureau of Proprietary School Supervision are often in John Paolo’s Xtreme Beauty

What can a student or employee complain about?

advertising, standards and methods of instruction, equipment, facilities, qualifications of teaching and management

personnel, enrollment agreement, methods of collecting tuition and other charges, school license or Application,

school and student records, and private school agents.

How can a complaint be filed by a student or employee?

The steps you must take to file a complaint with the New York State Education Department are:

1. Write to the New York State Education Department at 116 West 32nd Street, 5th Floor, New York, New York

10001, or telephone the Department at (212) 643-4760, requesting an interview for the purpose of filing a written

complaint. Bring all relevant documents with you to the interview, including an enrollment agreement, financial aid

application, transcripts, etc. An investigator from the Department will meet with you and go through your complaint

in detail.

2. If you cannot come for an interview, send a letter or call the office to request a complaint form. You must

complete and sign this form and mail it to the office. Please include with it copies of all relevant documents. You

should keep the originals. You must file a complaint within two years after the alleged illegal conduct took

place. The Bureau cannot investigate any complaint made more than two years after the date of the occurrence.

3. The investigator will attempt to resolve the complaint as quickly as possible and may contact you in the future

with follow-up questions. You should provide all information requested as quickly as possible; delay may affect the

investigation of your complaint. When appropriate, the investigator will try to negotiate with John Paolo’s Xtreme

Beauty Institute Goldwell Products Artistry informally. If the Department determines that violations of law have

been committed and John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry fails to take satisfactory and

appropriate action then the Department may proceed with formal disciplinary charges.

What is the Tuition Reimbursement Fund?

The Tuition Reimbursement Fund is designed to protect the financial interest of students attending proprietary

schools. If a school closes while you are in attendance, prior to the completion of your educational program, then you

may be eligible for a refund of all tuition expenses which you have paid. If you drop out of school prior to

completion and you file a complaint against The Institute with the State Education Department, you may be eligible

to receive a tuition refund if the State Education Department is able to provide factual support that your complaint is

valid and to determine that there was a violation of Education Law or the Commissioner's Regulations as specified in

Section 126.17 of the Commissioner's Regulations. To file a claim to the Tuition Reimbursement Fund, you must

first file a complaint with the State Education Department at the address included in this pamphlet. The staff of the

State Education Department will assist you in the preparation of a tuition reimbursement form (a sample of this form

should have been provided to you upon enrollment).

What is the tuition refund and cancellation policy?

All schools must have a tuition refund and cancellation policy for each program included in the catalog and in the

student enrollment agreement.

refund and cancellation before you sign the enrollment agreement. If you do not understand it, or are confused by

What should students know about "private school agents?"

Private School Agents are employed by schools for the purpose of recruiting or enrolling students in John Paolo’s

Xtreme Beauty Institute Goldwell Products Artistry; they are not school counselors. Private school agents cannot

require a student to pay a placement or referral fee. Each school agent must be licensed by the New York State

Education Department, must have an Agent identification card and must be a salaried employee of John Paolo’s

Xtreme Beauty Institute Goldwell Products Artistry. School agents who cannot show an Agent Identification Card

are breaking the law if they try to interest students in enrolling in a particular school or group of schools. The

name(s) of the agent(s) who enrolled a student must appear on that student's enrollment agreement. Therefore, you

should write down the name of the agent who talked to you. Each student will be required to confirm the name(s) of

the agent(s) when signing the enrollment agreement. A full refund shall be made to any student recruited by an

unlicensed private school agent or even by a licensed agent if there is evidence that the agent made fraudulent or

What should students know about "grants and guaranteed student loans"?

A grant is awarded to a student based on income eligibility, and it does not need to be repaid (for example, New

York State Tuition Assistance Program (TAP) grants or Pell grants provided by the federal government).

Guaranteed student loans are low interest loans provided under the Federal Guaranteed Student Loan Program. The

decision to apply for such a loan is yours--John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry cannot

require that you apply for a loan. You should understand that if you pay school tuition with money loaned to you

from a lender you are responsible for repaying the loan in full, with interest, in accordance with the terms of the loan

agreement. A failure to repay the loan can hurt your credit rating and result in legal action against you. Even if you

fail to complete your educational program, you are still responsible for repaying all of the money loaned to you.

It is your right to select a lender for a guaranteed student loan. John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry cannot require you to apply to a particular lender or lending institution. However, John Paolo’s

Xtreme Beauty Institute Goldwell Products Artistry can recommend a lender, but if it does, John Paolo’s Xtreme

Beauty Institute Goldwell Products Artistry must also provide you with a statement about your right and ability to

obtain a loan from another lender and the insurance premiums charged on these loans. Read and understand all the

UNFULFILLED FINANCIAL OBLIGATIONS

All enrolled students at John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry must be mindful of their financial obligation to pay for their time of attendance, books,

kit and any additional miscellaneous expenses accrued while in contract with John Paolo’s

Xtreme Beauty Institute Goldwell Products Artistry as stated in their signed Enrollment

Agreement. If a student is in arrears to John Paolo’s Xtreme Beauty Institute Goldwell Products

Artistry, the student will be referred to the Executive Director, who has the right and ability to

suspend or terminate said student.

LEGAL FEES

All legal fees incurred by John Paolo’s Xtreme Beauty Institute, Goldwell Products

Artistry for the collection of any unpaid debt will be reimbursed to John Paolo’s Xtreme Beauty

Institute Goldwell Products Artistry by the student. The student understands such liability.

CENTER AND CLINIC RULES

For insurance reasons, no student or customer is allowed to bring their own supplies such

as tint, shampoos, hair spray, etc. Only products supplied by John Paolo’s Xtreme Beauty

Institute Goldwell Products Artistry will be used.

NO STUDENT IS ALLOWED TO REFUSE A CUSTOMER

1. Customers are to be properly greeted.

2. Each customer should be given two full and complete shampoos, unless otherwise

directed by your instructor.

3. No conditioner is to be given for any service without prior purchase.

4. Clinic services are to be discussed thoroughly with customer, student and instructor

before beginning.

5. Each step of a procedure is to be checked before continuing on to the next step in order to

receive credit for the operation.

A. Hair Cut

Guidelines are to be checked in addition to each section and final check of

cut. Any hair dropped on the floor must be swept up immediately after the

cut is finished to avoid a safety hazard.

B. Color Application

Four sections, application of color, holiday checking, strands and final

check.

C. Cold Waving

Nine sections, winding, application of lotion, test curls, rinsing, blotting,

and neutralizer will all be checked.

D. Sets

Setting pattern, setting lotion use, brushing, final style and spraying will be

observed. No basic settings.

6. Only desk operator will give out supplies from behind the desk. No one else is permitted

to be at the desk, unless authorized by the instructor.

REMEMBER: Any utensils or materials obtained from the center are to be cleaned and

returned to the center before the student is excused.

7. All hair is to be swept up and disposed of in waste can before going on to next procedure.

8. All cold wave rods are to be checked before permanent wave lotion is applied.

9. Only half a bottle of lotion will be given out initially for a perm. More may be issued if

necessary.

10. All neutralizer is to be used.

11. Procedures must be followed correctly in order to have instructor sign for operation.

12. Absolutely no sliding of end papers is allowed.

13. Up to 40ml of color will be issued for each color service. If additional supply is needed

the client will pay an additional charge.

14. All equipment will be sanitized for each customer.

15. When clinic instructor is busy on the floor, please wait your turn.

16. All customers must remain seated for final check by instructor and for signing for

operation.

17. Each customer is to be treated with respect and be pleased that he/she came in.

18. Hair cuts are to be done before permanent waving unless otherwise instructed.

19. All operations and breakdown sheets are to be signed the same day.

20. All color must be mixed at desk with a teacher observing.

21. Record cards must be filled out and handed in at the end of signing.

22. The instructor will determine if sufficient instructional time exists for any processing

treatment. If there is insufficient time the instructor will inform the customer.

23. To participate in clinic, student must be dressed in proper uniform and have a neat

appearance.

24. The instructor’s word on the clinic floor is final in any situation.

25. Students must carry a professional approach at all times.

26. All temporary rinses are to be applied at the station.

27. Each station and surrounding work area must be kept neat and clean.

28. All supplies must be cleaned and returned to the desk.

29. Color work will be done in specified area only.

30. It is the student’s responsibility to maintain a complete original kit, supplies and

equipment. If any equipment is lost or broken, it must be replaced by the student. Kit

inspections will be conducted at periodic intervals.

The above listed rules apply to every student in this school. Clinic practice is a privilege

and must be earned and not abused. Clinic is an opportunity to learn what you have been taught

in the classroom on live models. This instruction is based on the fact that the instructor believes

you can handle the work and he/she will help guide you through each step. No student is

expected to perform work without correction and guidance. Therefore, the above rules must be

observed and anyone not cooperating with these rules will not participate.

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry, is grateful for every

customer that comes in for service. We will maintain a high standard of clinical rules,

professionalism and cleanliness in order to satisfy our customers. Remember our customers are

our future in Cosmetology, Esthetics or Nail Specialty.

Rules of Cleanliness

Stations will be kept clean and neat at all times. Materials are to be set on the station for

one customer at a time. All other materials are to be placed in the compartment at the station or

in your locker. Valuables, including purses, are to be locked in a locker. JOHN PAOLO’S

XTREME BEAUTY INSTITUTE GOLDWELL PRODUCTS ARTISTRY WILL NOT BE

HELD RESPONSIBLE FOR PROPERTY LEFT UNATTENDED. Kits will be returned to

lockers and should be locked.

It is the responsibility of the student to have their work station clean as well as the

compartment under their station.

No smoking, beverages, candy, food, etc., are allowed in the station area for either

students, customers or instructors. Chewing gum is not allowed.

Each student should take pride in John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry and pick up after themselves in the lunch room, work areas, etc. A clean and

neat school will keep our customers coming in. Customers give you the training that is needed

for this course.

Dress Code- General Guidelines for Student Dress and Grooming

Appropriate student dress and grooming are important factors in the safe and orderly

operation of John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry. Each student’s

appearance should reflect a positive image of John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry and contribute to a distraction-free learning environment. All students are

required to adhere to John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry grooming

and dress code. Exceptions will be permitted for bona fide religious reasons.

Specific Guidelines/Restrictions

Pants – must be fitted at the waist or upper hip and must not reveal underclothing. No denim or

sweat pants permitted.

Shorts and Skirts -must be fitted at the waist or upper hip, must not reveal underclothing, and

must be mid-thigh in length or longer. No Denim skirts or “Spandex” type

material permitted.

Tops, Shirts, Blouses - must not reveal underclothing, midsection, torso, back, chest, breasts or

cleavage. Tee shirts must not contain any offensive pictures, symbols or sayings,

including but not limited to:

*Demeaning, violent, racial or sexual statements

*Clothing that advertises or promotes tobacco, alcohol or other drugs

No Halter tops or “spaghetti strap” tank tops are permitted.

Shoes -must not detract from or interfere with the learning environment or present a safety or

health hazard. Clean tennis shoes or closed-toe shoes are preferred.

No Beach type sandals or flip flops are permitted.

Head Coverings -No type of head covering is to be worn with the exception of those worn for

bona fide religious purposes.

Underclothing -Students must wear appropriate underclothing, pajamas are not permitted.

Hair/Makeup -Hair must be well groomed, neat and clean at all times. If hair is long, it must be

worn up or back from the face (not hanging loose). Make-up can be worn,

but not applied at the station.

Uniform Smock-Students must wear their uniform smock or apron at all times. A good general policy

regarding the dress code is: if there is:

ANY QUESTION about an item being inappropriate, DO NOT WEAR IT.

Dress and Grooming Violations

Students who come to school in violation of John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry dress code will have the option of correcting the violation or being sent home for

the remainder of the day. Students who are new to John Paolo’s Xtreme Beauty Institute Goldwell

Products Artistry, or students with extenuating circumstances, will be expected to be in compliance

after a reasonable “grace period” to be determined by the Director. There may be times when

students should be dressed more formally. Some instances include: attendance at special events with

an external constituent or audience, when an external constituent or audience is scheduled to visit

John Paolo’s Xtreme Beauty Institute Goldwell Products Artistry.

Program Description: Students will acquire expertise in the field of cosmetology through

classroom instruction, observation and practice on both mannequins

and live models.

Program Goal: Upon completion of 1000 hours students will be prepared to take the

New York State licensing exam in Cosmetology. Possible career opportunities

Program Description: Students will acquire expertise in the field of Nail Specialty through classroom

instruction, observation and practice on both mannequin hands and live models.

Program Goal: Upon completion of 250 hours students will be prepared to take the New York State licensing

exam in Nail Specialty. Possible career opportunities include but are not limited to: Nail Technician,

Manicurist/Pedicurist, Salon Manager, Sales Representative.

Grading System: Student evaluation is done on an individual basis daily. Progress records for both written

and practical grades are maintained and reported to the student monthly. Failure to maintain an average

of 70% or better may result in repeating the unit. A unit may be repeated only once. Students may make up

exams and receive additional training in weak areas at no additional charge.

A=90-100 Excellent B=80-89 Good C=70-79 Passing D=69 or below (FAILING)

INSTRUCTIONAL METHODS

Students are instructed in theory which includes lectures, demonstrations, discussion, workbook and

testing. Further instruction is given in practical participation on the clinic floor with students working on both

mannequins and live models.
