This process is used in response to more eligible applicants than enrollment space to the Registered Nursing Program. The application process is student driven, which means applicants are responsible for supplying all transcripts and additional required supporting documentation, as well as keeping the Registered Nursing Department informed of any changes in applicant name, address, and/or phone number. To be eligible for evaluation for the RN Program Lottery, the Online Academic Summary Form (ASF/Application) must be completed and all transcripts and supporting documents included or it will be considered incomplete and processing will be terminated. All qualified online applications received before the stated deadline will be treated equally regardless of the submission date. Results will be posted online only on the Online Registered Nursing Application, on the listed date provided on the Lottery Timeline. Any correspondence from the Fresno City College Registered Nursing Department will go to applicant’s State Center Community College District Student Email (my.scccd.edu). In order to process the online application, students must check and include the following items. Again, online applications without required documents will result in disqualification. Online applications missing documents for optional criteria will not receive points for that criteria. Please review our Helpful Tips for Required Criteria and Helpful Tips for Optional Criteria. REQUIREMENTS TO APPLY TO THE REGISTERED NURSING PROGRAM: 1. Required: Students must successfully complete the ATI TEAS/TEAS V Examination prior to submitting an online Academic Summary Form to the Registered Nursing Program. Students must achieve a minimum passing score of 62% or higher. No more than two (2) attempts will be allowed at the ATI TEAS/TEAS V Examination. There must be a ninety (90) day lapse between the two ATI TEAS/TEAS V Examination attempts. Results will not be considered for testing done less than 90 days from a previous attempt. The ATI TEAS V Examination will not be accepted after September 30, 2017. For any additional information on the ATI TEAS Examination, please visit www.atitesting.com. 2. Required: Transfer ATI TEAS examination scores to Fresno City College through www.atitesting.com and include a printed copy of ATI TEAS Individual Performance Profile and transcript must be submitted along with the Fresno City College Registered Nursing Program Online Application confirmation email, sent by the Registered Nursing Department, to the Allied Health, PE and Athletics Division office before the application deadline. Online applications without required documents will result in disqualification. Online applications missing documents for optional criteria will not receive points for that criteria. 3. Required: Cumulative GPA of 2.5 or higher. A cumulative GPA calculation will be done using all college coursework documented on all college/university transcripts. Application requirements include the completion of all eight (8) prerequisite requirements and a cumulative GPA of 2.5 or above. “In Progress” courses are not accepted. Grade points and units will be tallied from all college work attempted. Please refer to an Allied Health and Nursing Counselor for any concerns regarding your cumulative GPA. 4. Required: Completion of Science Program Prerequisites with a grade of C or better:

BIO 20-Human Anatomy and BIO 22-Human Physiology, OR BIOL 21A-Human Anatomy & Physiology I and BIOL 21B-Human Anatomy & Physiology II BIO 31-Microbiology and CHEM 3A- Introduction to General Chemistry. 5. Required: Completion of Non-Science Program Prerequisites with a grade of C or better: ENGL 1A-Reading and Comprehension, MATH 103-Intermediate Algebra, PSY 2-General Psychology and SOC 1A-Introduction to Sociology or ANTH 2-Cultural Anthropology. 6. Required: Complete online Fresno City College application. All applicants to the Nursing programs must apply for admission to Fresno City College to obtain a SCCCD Student ID number prior to applying to the Nursing program. Please refer to the Fresno City College website under Getting Started. 7. Required: Complete Online Academic Summary Form (ASF/Application). OPTIONAL CRITERIA: Verified supporting documentation may be awarded points. Students may submit all that apply. Please refer to the Helpful Tips for Optional Criteria on the Fresno City College website. Relevant Health Care Certification 1. Required: Copy of Relevant Healthcare Certification/License. At this time First Aid and Safety/CPR will not be awarded points. Life Experiences or Special Circumstances Points may be awarded for one of the following criteria. 2a. Required: Documentation showing student is receiving college Disability Services. 2b. Required: Proof of eligibility or proof of Financial Aid (BOGG fee waiver, Cal Grant, Pell Grant or other federal grant, or CalWorks). 2c. Required: Submit Life Experiences/Special Circumstances form for First Generation to Attend College Criteria. 2d. Required: Proof of participation or eligibility for Extended Opportunity Programs and Services (EOP&S). 2e. Required: Proof of Veteran Status/Eligible Spouse. Veteran or a spouse of a veteran must submit a copy of the DD214 form. For the required criteria on priority preference for veterans and/or their spouses into the Health Sciences program, the applicants are required to refer to http://www.fresnocitycollege.edu/index.aspx?page=2484. 2f. Required: Required Documentation – Submit Language Proficiency Verification Form Only for applicants using courses from Private or Out-of-State Colleges/Universities: 1. Required: Applicants who wish to use courses completed from a private institution or institution outside of California must complete the course substitution review process before applying to the Registered Nursing Programs or any Allied Health Program. This is a lengthy process and must be done at least 2-3 months prior to application deadline. Course Substitutions must be initiated with a Fresno City

College Counselor/Allied Health and Nursing Counselor. Courses must be evaluated by each department for possible approval. Only for applicants using Foreign Education courses: 1. Required: Students who have completed courses outside of the United States must follow the link to “FCC Students with Foreign Transcripts TO DO LIST”. If applicants wish to use courses completed from a foreign institution this process must be completed before applying to the Registered Nursing Program. This is a lengthy process and must be done at least 3 months prior to application deadline. Courses Substitutions must be initiated with a Fresno City College Counselor/Allied Health and Nursing Counselor. Courses must be evaluated by each department for possible approval.

A “FCC Students with Foreign Transcripts TO DO LIST” can be picked up in Health Science Room HS-1 or downloaded from the Allied Health, Physical Education & Athletics Division web page at http://www.fresnocitycollege.edu/index.aspx?page=1624.

Please follow instruction on both “Foreign Education Transcripts TO DO list” and “Foreign Education Transcript Evaluation Services.” If you have any questions, please meet with an Allied Health and Nursing counselor. Please call (559) 244-2604 for an appointment.

Transcripts and other supporting documentation must be submitted along with the confirmation email, sent by the Registered Nursing Department, to the Allied Health, Physical Education, and Athletics Division office by the application deadline. Online applications without required documents will result in disqualification. Online applications missing documents for optional criteria will not receive points for that criteria. A COMPLETE PACKET MUST CONTAIN THE FOLLOWING: 1. Required: Signed hard copy of Application Confirmation Email sent by Fresno City College Registered Nursing Department. 2. Required: Signed Acknowledgement of Student Responsibilities Form (link in application). 3. Required: Printed copy of ATI TEAS/TEAS V Individual Performance Profile AND TEAS Transcript. Students must electronically submit ATI TEAS/TEAS V Scores to Fresno City College through www.atitesting.com. 4. Required: TWO (2) Official (sealed) college/university transcripts from schools outside of SCCCD. Unofficial

transcripts not accepted at this time. Application must include all college transcripts documenting completion of prerequisite and general education courses with a grade of C or better and any conferred degrees.

5. Required: An unofficial copy of your high school transcripts; or diploma; or GED certificate. 6. Required: If applicable, copy of approved Course Substitution(s) for prerequisite and General Education courses. Courses from private, out of state, or foreign colleges or universities must be evaluated for course substitution, prior to application. Please refer to the Helpful Tips for Required Criteria on the Fresno City College website. 7. Required: If applicable, any supporting documentation for Optional Criteria.
