  • Building a Google Form for Classroom Assessment

    Getting Started (using the computer)

    • Go to drive.google.com and sign in to your Google account

    • In the top left corner of the screen select the blue “NEW” button

    • At the bottom of the drop down menu select “More”

    • Select the purple Google Forms icon

    • *If you click the arrow to the right of the Google Forms icon you can select from pre-made templates

    Google Form Basics (using the computer)

    • To give your form a title - click the words “Untitled form” and replace the text.

    ADDING A QUESTION S

    1. Adds a question o The default question is a

    multiple choice question

    2. Adds a section title and instructions

    3. Adds a photo from your computer or from a Google Images search

    4. Adds a video by either URL or by a YouTube search bar

  • CUSTOMIZING RESPONSES S

    1. Toggle this switch to accept or not allow responses to your form

    2. Click the Google Sheets icon to send responses to a spreadsheet saved to

    your Google Drive

    o Responses can also be downloaded as Excel files

    3. Click the options button to delete responses if reusing a Google Form

    the next year

    HAVING GOOGLE GRADE IT FOR YOU S

    • Click the gear shaped icon to get into settings

    • Under settings � quizzes toggle the slider to “make this a quiz”

    o Scroll down for check boxes to customize what the students, or “respondents” can

    see when they finish their quiz

    • Under settings � general you can choose to limit students to only one response per quiz

    CREATING AN ANSWER KEY S

    • Once your settings are saved, go back to the “questions” portion of your form

    • At the bottom, click on “ANSWER KEY” to program correct answers, points values,

    and make questions required
