Part A – Create Form in Gmail

Go to Gmail

• Go to Gmail at www.gmail.com and “Sign in” using your Gmail account

Enter Google Document

• Click “Document” on the top after enter Gmail

• You will see this site after clicking “Documents”, click “Create new” on the left hand side

• A new page is shown, you can now edit your form

Title here will become the name of the file and Title of the Form

• There 7 types of Question: – Text– Paragraph text – Multiple choice – Checkboxes– Choose from a list– Scale, Grid

• Click “Add item”, then choose one of the type of Question
