Part A – Create Form in Gmail

Go to Gmail

• Go to Gmail at www.gmail.com and “Sign in” using your Gmail account

Enter Google Document

• Click “Document” on the top after enter Gmail

Create Google Form

• You will see this site after clicking “Documents”, click “Create new” on the left hand side

• A slide bar is shown, select “Form”

• A new page is shown, you can now edit your form

Title here will become the name of the file and Title of the Form

Yellow Zone means you are editing this Question

Part B – Type of Question

Type of Question

• There 7 types of Question: – Text– Paragraph text – Multiple choice – Checkboxes– Choose from a list– Scale, Grid

• Used if short answer is required– e.g. Full name of the user

Part C – Editing Google Form

Add Section Header

• Section Header is the Section title of your form

Add items

• Click “Add item”, then choose one of the type of Question

Edit Added item

Moving cursor here, background color of this

question will change

Click this Icon or Double Click will edit this Question

Editing Google Form

Finish Editing this Question, Click “Done”

Enable this will require user to answer this question before submit the form

Delete this Question by clicking this rubbish bin

Duplicate this Question by clicking this icon

Finish editing this Question by clicking this icon

Moving Question

Drag and Drop can move Question 4 to Question 3

Usage of Page break

• Page break can be used when there is Multiple Choice type Question
