Personal Medical Info for Belinda Bunde:My medical diagnoses:(1) Neuroaxonal Dystrophy. The brain doesn’t tell the

nerves and muscles what to do. My muscles do whatthey want.

(2) I have a severe case of Raynaud’s disease. My body’stemperature gauge doesn’t work correctly and itcauses tons of pain.

(3) I have scoliosis and two Harrington Rods in my back.(4) My speech is dysarthric and I have a weak diaphram.(5) Temporal Mandibular Joint sysndrome. This affects

my eating and speech.(6) I also have carpal tunnel syndrome.

(7) I have dysphasia which is a swallowing disorder. I knowwhat I can and cannot eat and tend to aspirate onwater. Water must be given through my G-tube.

(8) I am legally blind and have peripheral vision.

I keep all my medical orders in a notebook. Just ask meabout any other information. I know everything.

Medications:

I am on 15 medications taken throughout the day. It isvery important that I get them on time each day. Ialso take 13 PRN medications.

I am very sensitive to pain medications and am allergicto some of these. (See pharmacist contact info.)

My mother is my medical Power of Attorney.I have a Living Will. I am an organ donor.“Quality over quantity” is important to me and my lifeand future. Example: I don’t want to be on arespirator. My family and my doctors know that.When I’m riding on the van, I need to have a chest strapand a waist safety belt on for my safety.

Medical Equipment:• Hospital Bed with a remote control;• AFO’s used daily over two pairs of socks;• Night Call Light plugged in at night;• Breathing Machine: PRN at this time; morning and night

and PRN sometimes. Depends on the season.

• Sometimes I need it four times a day. I know when toput the medicine in. Staff need to assist me.

• E-Z Stand Lift: for use to get out of my wheelchair tothe bathroom and in and out of bed.

I need community transportation systems available to mefor my use.“Discussion/Justification”Current action: I want to find alternative transportation from communityso that I can join meetings where I’ve been invited to participate (Region VDD Council; NE Visually Impaired (VIPS) – Saunders County chapter) andnew social/professional opportunities available to me.Future Dream: Getting my own wheelchair accessible van! (I wrote a grantand almost got a van! Will look into reapplying again when I get basics.)

“What needs to be done”Contact Handi-van for their calendar of scheduled travel and cost.Current Action status: Have received calendar; cost is $2 round trip inSaunders County; $4 round trip to Lincoln, Omaha, Fremont.Learn about actual use: need one-person staff or is someone available atthe destination point to assist until returns to Wahoo by van? I want internet and e-mail access from my home computer.“Discussion/Justification”Current action: I have my own computer and have already received e-mailfrom a new contact/colleague but this goes through a 3rd party and I can’taccess/reply on my own. JAWS would help my independence but I need toget it set up. With internet, I might find cheaper ways to print my T-shirts for my business and stay in touch with my business partners easier.Future Dream: Getting an updated system and ideally, a laptop with atleast Windows ’98 so that I can use for PowerPoint presentations to tellpeople about people with disabilities and my Essential Lifestyle NewsletterPlan (ELNP).

“What needs to be done.”First, see if I can get JAWS installed and then receive my training to be able touse computer and especially e-mail and internet; Get best price on internet/cost;Learn “touch typing” skills and practice typing on my keyboard (voice technologywould help so that I hear each letter I type in as I learn) = find good software =check with Shane Buresh and Service Coordination; maybe Dixie Travarten, Omaha.

I need an easier way to use my computer and use it on myown when I want.“Discussion/Justification”It is hard for me to see my keyboard and what is on my computer monitorbecause of my legal blindness and my peripheral vision. I depend on othersand this is inconvenient for me and limits my access, learning, and time.

“What needs to be done.”Contact the Nebraska Visually Impaired Program (VIPS) w/DeLoris Yochum.• Look into the “JAWS” software program;• Look into magnification screen to read mail independently;• Look into stick-on large keyboard text that I can see better.

“Current status”NE VIPS representative contacted by DeLoris Yochum. Shane Buresh contacted B;B signed authorization for release of info. Further meeting to be set up. (ServiceCoordinator supported B in scheduling during the day.) B also wants to attend atleast the Saunders County VIPS chapter meetings in Wahoo. Next one: May 7th.
