    WHAT IS A WORK PERMIT ?

    A %ork &ermit is a written do ument authorizing a person or a group to perform spe ifitasks as identified on the %ork &ermit and sub'e t to spe ifi onditions and pre autions.

    (efore most work in any #A!)* fa ility an be started the workmen must obtainadvan e approval in the form of a work permit.

    *nly one work permit shall be issued for ea h 'ob.

    %ork performed on a single e+uipment involving more than one work group, ea h groupshall get separate work permit.

    -a h employee or ontra tor who is +ualified will be issued a wallet size authorizationard signed by his anager or )ontra t Administrator. This ard will list the employeeor ontra t employees +ualifi ations and shall be arried on by the person while at work.The authorization ard is valid for a two year period.

    AUTHORI,ED ISSUER)is an individual who has su essfully ompleted the re+uired training, authorized by hismanager to issue work permits, and hold a valid work permit authorization ard.

    WHAT IS THE DURATION OF WORK PERMITS?

    5 &ermits are valid for only one shift 7C or /2 hours shift1 but may be e4tended upto a total time of 2; hours if endorsed by an authorized issuer and a eptor.

    5 &ermits for onstru tion type a tivities in non0operation areas an be issued for

    periods up to one week.5 %ork &ermits may be e4tended up to a total time period of /2< hours during the shutdown"turnaround for a spe ifi 'ob and spe ified e+uipment or group of e+uipment su h as welding ma hines, s affolding, et .

    HOW TO ENDORSE WORK PERMIT AT SHIFT CHANGE?

    At $h(%t 1ha34e) the e'#+!"ee 1!'(34 !3 . t" $ha++ .! the %!++! (34)5 Review all a tive work permits in his area and endorse the ontrol room opy of

    the permit.5 %ithin one hours of shift hange, visit the 'ob site and endorse the hard opy.

    To issue a work permit for a 'ob in your unit and the 'ob will involve e+uipment inanother area, you must he k with the authorized representative in the other area andhave him endorse the permit.
