2 QUALITY MANAGEMENT

2.1 GENERAL The Developer has the responsibility for the quality of the Work, including the Work and products of Subcontractors, consultants, Fabricators, suppliers, and vendors.

A. An independent contractor, not owned by the Developer or any of its joint venture members, partners, or affiliated entities

B. A Major Participant in the organization of the Developer C. A direct report to the DB Project Manager

2.3 QUALITY PROGRAMThe Developer shall develop, implement, and maintain a quality program covering all elements of the Project, including, management, administration, design, geotechnical investigations, construction, testing, and environmental monitoring, and compliance. The quality program shall accomplish the following quality functions and objectives:

A. Achieve the goals of the Project listed in Section 1 of the Project Scope.B. Deliver the Project within the Accepted Progress Schedule for the Project.C. Provide a high quality Project, meeting or exceeding the requirements of the Contract.

The quality program shall be documented in a Quality Management Plan (QMP). The QMP shall:

A. Document the processes needed, and their sequence and interaction, for the successful delivery of the Work, for example: design submittals, construction Work, administrative submittals (e.g., payment requests, progress reports, and meeting minutes), and change management.

B. Describe how the Developer will ensure that all disciplines, aspects, and elements of the Work will comply with the requirements of the Contract Documents and that all materials incorporated into the Work will perform satisfactorily for the purpose intended and conform to the Contract requirements.

C. Address all requirements in this Section 2.

The Developer shall engage the Department in the QMP development to facilitate Approval and ensure understanding of requirements. The Department’s participation in the development of the QMP does not waive the Developer’s responsibility for the quality of the Work, nor does it ascribe any responsibility to the Department for the Work. Further, this involvement does not preclude subsequent rejection of the QMP by the Department.

2.4 ALLOCATION OF QUALITY MANAGEMENT RESPONSIBILITIES

2.4.1 Quality Control (QC) and Quality Assurance (QA)The Developer shall:

A. Perform all Quality Control functions, including production necessary to meet Contract requirements for design, construction, administration, etc.; with the exception of Developer QC testing, which shall be carried out by Developer personnel who are not part of production or non-IQF subconsultant personnel who are not part of production. Developer QC inspection may be performed by production personnel.

B. Have the authority to stop any Work if Contract requirements are not being met. C. Identify and recommend measures to prevent quality problems or to improve quality.D. Have no IQF responsibilities.

The IQF shall:

A. Perform all Quality Assurance functions, including verification, inspection, and testing necessary to provide confidence that all Work meets applicable Contract requirements. Work may be verified by qualified individuals who are employees of or retained by manufacturers, vendors, or Suppliers, if Approved in writing by the Department.

B. Identify and recommend measures to prevent quality problems or to improve quality.C. Have the authority to stop any Work if Contract requirements are not being met. D. Have no Developer Quality Control responsibilities.[E.] Review and approve all deliverables prior to delivery to the Department. IQF approval shall

indicate that the IQF believes the deliverable meets Contract requirements and is fit for its intended purpose.

E.[F.] Comply with the Department’s Construction Inspection Manual of Procedures (MOP). The IQF shall perform all construction inspection and documentation in accordance with the requirements of the MOP, at a minimum. The IQF is not required to perform off-Site inspection and testing for the fabrication or production of products from Department pre-approved sources.

F.[G.] Provide a testing laboratory(ies), accredited to AASHTO Materials Reference Laboratory for the testing the laboratory(ies) perform, within 20 miles of the Project Site. If major components

of the Project are constructed off the Project Site, additional testing laboratory facilities may be utilized.

G.[H.] Be considered the “Contractor’s representative” under C&MS 611.H.[I.] Perform the inspection role in compliance with Supplemental Specification 878.

2.4.2 Quality OversightThe Department will perform Quality Oversight (QO) with a focus on obtaining confidence that the requirements of the Contract have been met and on identifying opportunities Opportunities for improvement Improvement (OFIs). QO will occur via collection of objective evidence (e.g., monitoring, auditing, reviews, interviews, inspections, and tests) to assess the Developer’s performance in meeting the Contract requirements. This includes any and all Contract requirements and any and all parties subcontracted to the Developer, including the IQF, Subcontractors, suppliers, and Fabricators. The timing, frequency, and depth of QO will be at the Department’s discretion. Department QO does not relieve the Developer of any responsibility.

The Developer shall:

A. Provide 24 hour notice of, and invite the Department to: each quality checkpoint (QCP), over-the-shoulder review, and interim/final design review.

[B.] Provide access to all facilities, offices and locations on and off-site where Work occurs. To facilitate access to sites where aerial access is necessary, the Developer shall provide a man-lift for exclusive use of the IQF and Department personnel. The man-lift shall be capable of lifting a minimum of two people and shall be capable of providing access to the I-90 Viaduct Structure superstructure from ground level. The man-lift shall be available for use at all times during pier, column, and cap construction, and during I-90 Viaduct Structure structural steel erection. During I-90 Viaduct Structure pier, column, and cap construction, the Developer shall provide access by either providing a man-lift, as described above, or stair tower access at each work location for each substructure pour.

B.[C.] Allow the Department to copy any books or records, or provide copies, as the Department reasonably deems necessary for purposes of verifying compliance with the Contract.

C.[D.] Respond to the Department’s QO results (Nonconformances, Opportunities For Improvement, Corrective Action requests) in a timely manner, in the medium and format desired by the Department.

D.[E.] Build into its Progress Schedule sufficient time for reviews, Approvals, QO, and for the Developer’s subsequent revisions.

E.[F.] Stop Work when directed in order to correct conditions that are unsafe for Project personnel or the general public, or to correct unacceptable design or construction practices.

2.4.3 Independent Assurance: The Department will perform Independent Assurance (IA) focused on verifying that all Project material sampling and testing is carried out by qualified, certified personnel, and using proper equipment,

procedures, and techniques in accordance with Department requirements. IA will occur via the collection of objective evidence, such as split sampling and testing, interviews, witnessing, and review of documentation. The Developer shall cooperate with all IA efforts.

2.5 TIMING OF QUALITY PROGRAM ACTIVITIES

2.5.1 Initial Project ActivitiesExcept for preliminary meetings, development of the Progress Schedule and the development of the QMP, no Work shall begin on the Project until the Department has Approved the QMP. If the Developer chooses to submit an interim Progress Schedule, the Department will allow the submittal of an interim QMP for the same period as the Approved interim Progress Schedule. The associated interim QMP must be Approved by the Department before Work under the interim Progress Schedule can progress. The interim QMP shall include all applicable quality processes and requirements for the Work to be performed in the interim Progress Schedule.

2.5.2 QMP ApprovalThe Developer shall allow 15 Business Days for the Department to Approve or reject initial QMP submittals. For subsequent revisions to the QMP, the Department requires 10 Business Days to Approve or reject the submission. The Developer shall submit each QMP revision to the Department during a submission meeting, in which the Developer shall provide an overview of the QMP revision.

2.5.3 Construction ActivitiesConstruction Work shall not begin until the Design Documents for that portion of the Work are complete, have been signed and sealed by an Ohio-licensed Professional Engineer in conformance with the Ohio Revised Code, have been accepted through the QMP process, and have been Released for Construction in accordance with the Contract.

2.6 QUALITY PROGRAM REQUIREMENTS

2.6.1 Personnel The QMP shall describe the quality management organization of the Developer and shall include the following:

A. An organizational chart showing lines of authority and reporting responsibilities for all Project disciplines.

[B.] The name, position, qualifications, duties, responsibilities, and authorities of each person proposed for a quality management function, including the following key staffing positions:

Each key staffing position The personnel listed in this Section 2.6.1 shall have no other responsibilities on the Project. Key staffing personnel and shall not be replaced without prior written Approval from the Department. Requests to replace key staffing personnel these personnel shall include complete details of the replacement’s qualifications, including resume. The replacement personnel shall have equal or better qualifications than the original.

Key quality staffThe personnel listed in this Section 2.6.1 shall comply with the following:

[A.] Be co-located at the Project office.[B.] Be an employee or Subcontractor of the Developer for Developer positions. Positions listed as

“Independent” shall be an employee or Subcontractor of the IQF. A.[C.] Have sufficient authority and organizational freedom to prevent and resolve quality problems,

and to implement continuous improvement measures.

The Developer and IQF shall comply with the following:

A. Determine the necessary competence and qualifications for all personnel performing Work affecting quality and ensure they are competent on the basis of appropriate education, training, skills, experience, and certifications per Tables 2-1 and 2-2.

B. Provide training to all personnel performing Work affecting quality to ensure they understand the relevance and importance of their activities, the expectations and requirements of their Work, and their specific roles and responsibilities.

C. Provide training, where necessary to achieve necessary competence. D. Maintain records of education, training, certifications, skills and experience.

2.6.1.1 DB Project Manager

The DB Project Manager shall be responsible for the overall control of the production processes and resulting work products.

2.6.1.2 Developer Construction QC Manager

The Developer Construction QC Manager shall be responsible for the control of the construction production processes and resulting work products. The Developer Construction QC Manager shall have no less than eight years of experience in construction Quality Control, including inspection and testing.

The Developer Construction QC Manager shall have a bachelor’s degree in a related technical field and be an ASQ-certified Quality Inspector, Quality Engineer, Manager of Quality, or become certified within six months of NTP. The Developer Construction QC Manager shall also be a licensed Professional Engineer in the State of Ohio or become same by the time of NTP. This position is required for the duration of all construction related activities on the Project.

2.6.1.3 Developer Design QC Manager

The Developer Design QC Manager shall be responsible for the control of the design production processes and resulting work products. The Developer Design QC Manager shall have no less than eight years of experience in design Quality Control, including design reviews and verifications. The Developer Design QC Manager shall be an ASQ-certified Quality Engineer or Manager of Quality, and a licensed Professional Engineer in the State of Ohio or become same by the time of NTP for PE license and within six months of NTP for ASQ certification. This position is required for the duration of all design-related activities on the Project.

2.6.1.4 Developer Highway Lead (Construction)

The Developer Highway Lead shall be responsible for meeting the Contract requirements for all highway elements, including embankments, drainage, pavements, signage, striping, ITS, lighting, and maintenance of traffic. The Developer Highway Lead shall have recent experience working in a similar capacity on major highway projects. This position is required for the duration of all construction-related activities on the Project.

2.6.1.5 Developer Structural Lead (Construction)

The Developer Structural Lead shall be responsible for meeting the Contract requirements for all structural elements on the Project, including bridges, box culverts, walls, and foundations. The Developer Structural Lead shall have recent experience working in a similar capacity on major highway projects with complex structures. This position is required for the duration of all structural construction-related activities on the Project.

2.6.1.6 Developer Documentation Lead

The Developer Documentation Lead shall be responsible for document control of all construction related Developer documents including material tickets, QPL material invoices, TE-24s, material certifications, Developer work plans, DLS reports, Hiperpav data, surface smoothness data, Developer test results, approved JMFs, mass concrete data, WTS reports and daily construction schedules. The Developer Documentation Lead shall be responsible for document gathering, scanning, naming, uploading, and providing access to the IQF. This person shall report to the Developer Construction QC Manager and the position is required for the duration of all construction-related activities on the Project.

2.6.1.7 Independent Quality Manager (IQM)

The IQM shall be responsible for the overall management and implementation of the quality program. The IQM shall have no less than 10 years of experience in quality management, including preparation and implementation of quality plans and procedures for design and construction. The IQM shall be an

American Society for Quality (ASQ) certified Manager of Quality or become certified within six months of NTP. The IQM shall also be a licensed Professional Engineer in the State of Ohio or become same by NTP. This position is required for the duration of the Project.

2.6.1.8 Independent Construction Quality Manager (ICQM)

The ICQM shall be responsible for verification and improvement of the quality of the construction Work. The ICQM shall have no less than eight years of experience in construction quality management, including construction quality planning and procedures. The ICQM shall be an ASQ certified Quality Inspector, Quality Engineer, or Manager of Quality, or become certified within six months of NTP. The ICQM shall also be a licensed Professional Engineer in the State of Ohio or become same by NTP. This position is required for the duration of all construction related activities on the Project.

2.6.1.9 Independent Design Quality Manager (IDQM)

The IDQM shall be responsible for the verification and improvement of the quality of the design Work. The IDQM shall have no less than eight years of experience in design quality management, including design quality planning and procedures. The IDQM shall be an ASQ certified Quality Engineer or Manager of Quality, and a licensed Professional Engineer in the State of Ohio or become same by the time of NTP for PE license and within six months of NTP for ASQ certification. This position is required for the duration of all design related activities on the Project.

2.6.1.10Independent Highway Lead (Design)

The Independent Highway Lead (Design) shall report to the IDQM and be responsible for the review and verification of the quality of all non-structural design Work. The Independent Highway Lead (Design) shall have no less than eight years of experience in the design and review of major highway projects of similar size and complexity. The Independent Highway Lead (Design) shall be a licensed Professional Engineer in the State of Ohio or become same by NTP. This position is required for the duration of all design related activities on the Project.

2.6.1.11Independent Structural Lead (Design)

The Independent Structural Lead (Design) shall report to the IDQM and be responsible for the review and verification of the quality of all structural design Work. The Independent Structural Lead (Design) shall have no less than eight years of experience in the design and review of complex bridges for major highway projects of similar size and complexity. The Independent Structural Lead (Design) shall be a licensed Professional Engineer in the State of Ohio or become same by NTP. This position is required for the duration of all design related activities on the Project.

2.6.1.12Independent Highway Lead (Construction)

The Independent Highway Lead shall be responsible for verification and improvement of the quality of all highway elements including embankments, drainage, pavements, signage, striping, ITS, lighting, Maintenance of Traffic, etc. The Independent Highway Lead shall have a minimum of seven years of experience in performing testing and inspection on highways and shall have a bachelor’s degree in a

2.6.1.13Independent Structural Lead (Construction)

The Independent Structural Lead shall be responsible for verification and improvement of the quality of all structural elements on the Project including bridges, box culverts, walls, foundations, etc. The Independent Structural Lead shall have a minimum of seven years of experience in performing testing and inspection on highway structures and shall be an ASQ certified Quality Inspector, Quality Engineer, or Manager of Quality, or become certified within six months of NTP. The Independent Structural Lead shall also be a licensed Professional Engineer in the State of Ohio or become same by the time of NTP. This position is required for the duration of all structural construction related activities on the Project.

2.6.1.14Foundation Measurement Personnel

The person(s) performing pile driving dynamic measurements shall report directly to the Developer. The person(s) shall have a minimum of five years of experience in monitoring, testing and analyzing the driving of steel pipe piles with dynamic measurements and in performing signal matching analyses. If other foundation capacity validation methods are used or required, such as Osterberg load cell, the person(s) must have a minimum of five years of experience in the method and report directly to the Developer.

2.6.1.15Testing and Inspections Certifications and Accreditations

IQF Ppersonnel performing inspection and material sampling and testing and Developer personnel performing material sampling and testing shall have applicable certifications as listed in Table 2-1 and Table 2-2. The Department will allow Professional Engineer licensure in the State of Ohio in lieu of NICET certification for those personnel certifications listed in Table 2-2.

2.6.2 Administration

2.6.2.1 Management Commitment

Developer management shall provide evidence of its commitment to the development and implementation of the quality program and continually improving its effectiveness by:

A. Communicating to the Developer personnel the importance of meeting Department as well as statutory and regulatory requirements.

B. Establishing a quality policy.C. Ensuring that quality objectives are established.D. Conducting management reviews.E. Ensuring the availability of resources.

Developer management shall ensure that the quality policy:

A. Is appropriate to the purpose of the delivering the Project.B. Includes a commitment to comply with Contract requirements and continually improve the

effectiveness of the quality program.C. Provides a framework for establishing and reviewing quality objectives. The quality objectives

shall be measurable and consistent with the quality policy.D. Is communicated and understood within the Developer team.E. Is reviewed for continuing suitability.

Developer management shall review the quality program, at planned intervals, to ensure its continuing suitability, adequacy and effectiveness. This review shall include assessing opportunities Opportunities for improvement Improvement and the need for changes to the quality program, including the quality policy and quality objectives. Records from management reviews shall be maintained.

The input to management review shall include information on:

A. Results of audits.B. Department feedback.C. Process performance and product conformity.D. Status of Preventive and Corrective Actions.E. Follow-up actions from previous management reviews.F. Changes that could affect the quality program.G. Recommendations for improvement.

The output from management reviews shall include any decisions and actions related to:

A. Improvement of the effectiveness of the quality program and its processes.B. Improvement of product related to Contract requirements.C. Resource needs.

2.6.2.2 Document Control

The QMP shall describe the means by which all documents required by the quality program are:

A. Controlled in accordance with Department’s document management systems and the Construction Inspection MOP.

B. Created and retained in electronic systems compatible with the Department’s IT system.C. Approved for adequacy prior to release.D. Reviewed and updated as necessary and re-approved by the same personnel that performed the

original approval.E. Identified to ensure that changes and revision status are known. F. Available at all points of use, including the IQF and the Department.G. Distributed.H. Prevented from unintended use, if obsolete.I. Organized, indexed and delivered to the Department for Final Acceptance.

The QMP shall describe the means by which records established to provide evidence of conformity to requirements and the effective operation of the quality program are identified, stored, protected, readily retrieved, retained according to the Department’s Record Retention Policy, and disposed of.

The QMP shall describe the procedures for meeting documentation requirements and document control of all Design Documents.

An electronic database of all Developer QC, IQF, QO and IA soil, asphalt, aggregate, concrete and other materials testing shall be provided, maintained, and updated on a daily basis by the IQF. The database shall have the following capabilities:

A. Perform comprehensive statistical data analysis when statistical acceptance or comparison of materials is required by specifications (F & t tests, paired t, PWL, range, etc.).

B. Allow multiple users input from multiple test groups (Developer QC, IQF, QO, IA) and any other Developer- or Department-affiliated members.

C. Track any changes made to the test data by an acceptable security and ownership system.D. Sort by multiple fields such as technician, station, etc.E. Be capable of performing and evaluating statistical analysis on test data when statistical

acceptance or comparison of materials is required by specifications.F. Be compatible with Department construction management software and useable to the

Department after Project completion.

The IQF shall track and document the quantity of material, use of material, and location of materials as per the requirements of Section 106 of the MOP. The IQF shall create and maintain a Project Bill of Materials (PBOM) breakdown for each activity of Work, using estimated quantities of Work items obtained from the Developer. The PBOM shall demonstrate that adequate coverage of approved materials has been achieved for each activity of Work. The PBOM shall be updated based on changes in the field and as necessary to adequately control materials being used in the Work. The IQF shall upload the PBOM and all updates to the Department’s document management system.

Copies of all correspondence between the Developer and other agencies, including the City of Cleveland, regulatory agencies, Railroads, and Utility Owners, shall be provided to the Department via the Department’s document management system at the time of submittal to the applicable agency.

The IQF shall collect, retain, and upload to the Department’s document management systems, within 48 hourstwo Business Days of the activity to which the document pertains, the following, all of which shall be in a format acceptable to the Department:

A. Evidence that all required activities and/or tests have been performed.B. Type, number, and results of all current quality management activities, including reviews,

verifications, inspections, material statistical analysis, tests, audits, Nonconformances, Corrections, Opportunities For Improvement, Corrective Actions, Preventive Actions and monitoring of Work performance and progress.

C. Documentation used and created by the IQF for review of design submittals, including all Design Documents included with interim and final design submittals, review comments, and disposition of comments.

D. Shop and working drawingsE. Daily occurrence log of construction activities, detailing:

H. Equipment calibration and verification records.I. All reports of inspection and test activities, meeting the content requirements of applicable

Department forms and the following, at a minimum:1. Name and quantity of items inspected/tested2. Inspection/test procedure reference with organization and documented ID3. Specific location of test and specific installation location4. Date conducted5. Name or stamp of inspector/tester6. Observations/comments 7. Specified requirement(s), including upper/lower limits, when applicable8. Results-acceptance or rejection, including actual readings, evaluation of results, and

Corrective ActionsJ. Minutes of all Developer QC and IQF meetings (distribute draft minutes within 48 hours). K. For Regulated Material Work, the data required by Contract requirements and all laws and

regulations. The documents shall be maintained separately for each instance of Regulated Material Work.

L. For Railroad and Utility Work, documentation of the design, construction, rework, progress, and protection during construction, as well as documentation of Utility and Railroad personnel on-site performing Work, including inspection and flagging. The documents shall be maintained separately for each individual Utility and Railroad facility.

M. Any other document not listed above provided by the Developer Documentation Lead.

2.6.2.3 IQF Field Work Photo Documentation

The IQF shall provide comprehensive photo documentation services for pre-construction, on-going construction, and final as-built conditions of the Project. This includes electronic indexing, navigation,

storage, and remote access to the documentation for the duration of the Project. The indexing and navigation system shall incorporate CADD construction drawings, satellite imagery, or other media sources Approved by the Department, to provide a user-friendly, interactive, online interface for all parties. This system shall not be incorporated with the Department’s SharePoint documentation site.

For all photo documentation, indexing and navigation must be organized by both time (date-stamped) and location throughout the duration of the Project. Documentation shall combine indexing and navigation with inspection-grade digital photography designed to capture actual conditions throughout construction and at critical milestones, including QCPs. Documentation shall be accessible online through use of an Internet connection and shall allow for simultaneous multiple-user access. Access shall be secure and accomplished through individual log-in user names and passwords. Online access shall be provided throughout the duration of the Project, until Substantial Completion is achieved.

Pre-construction conditions shall be documented prior to any earth disturbing activities. Photographs shall be taken in focus using the maximum range of depth of field to clearly show the pre-construction conditions. Digital images shall be provided in JPEG or an uncompressed TIFF format, produced by a digital camera with a minimum sensor size of 5.0 megapixels and at an image resolution of not less than 1024 by 768 pixels.

Vantage points and proposed timelines for photographs shall be coordinated with the Department. The IQF shall have a designated representative on Site to coordinate the photo shoots with the Department. The Developer and Department shall be invited to attend any construction progress or team meetings, as necessary to further coordinate the photo documentation effort. The IQF shall provide regular updates regarding the status of the documentation, including photo shoots completed. Updates shall be coordinated with the Developer's Progress Schedule. Photographic records shall be made available to the Department and Developer for various public information presentations, inquiry responses, or documentation reviews. Photographic records shall be uploaded to the online site and accessible within seven Days of being taken.

All online domain/web hosting, security measures and redundant server back-up of the documentation shall be provided by the IQF, including technical support related to using the system.

Final copies (permanent record) of the indexing and navigation system with all digital photographs embedded and active shall be provided in an electronic media format, via either DVD or external hard-drive to the Department. A complete set of the raw digital photographs shall be submitted as well.

2.6.2.4 Internal Audits

The QMP shall describe the means by which the IQF conducts internal audits that are:

A. Proactively scheduled at least twice per year for all processes affecting quality, including those in the QMP, the Traffic Management Plan, the Public Information Plan, the Aesthetics and Enhancement Implementation Plan, and any other management plans.

audited, as well as the results of previous audits. In addition, the Department shall be invited to participate in each internal audit.

E. Defined, including the audit criteria, scope, frequency and methods.F. Objective and impartial.G. Recorded and maintained.H. Published and responded to, such that, that any necessary Corrections, Corrective Actions and

Preventive Actions are taken, without undue delay, to eliminate detected Nonconformities, their causes and to respond to Opportunities For Improvement.

I. Followed-up to verify the effectiveness of actions taken.J. Closed-out and reported to interested parties, including the Department.

2.6.2.5 Nonconforming Work

The Developer shall ensure that Work that does not conform to Contract requirements is identified, clearly described, and controlled to prevent its unintended use or delivery. A documented procedure shall be established to define the controls and related responsibilities and authorities for dealing with Nonconforming Work identified by the Developer, IQF, or the Department.

The Developer shall deal with Nonconforming Work by one or more of the following ways, as applicable:

A. By taking action to eliminate the detected Nonconformity.B. By authorizing its use, release or acceptance as-is under approval, in order, of the Engineer of

Record, the IQM and the Department.C. By taking action to preclude its original intended use or application.D. By taking action appropriate to the effects, or potential effects, of the Nonconformity when

Nonconforming Work is detected after delivery or use has started.

When Nonconforming Work is corrected, it shall be subject to re-verification to demonstrate conformity to the requirements.

Records of the nature of Nonconformities and any subsequent actions taken, including approvals obtained, shall be maintained. This reporting shall be in the form of a Nonconformance report and shall be submitted to the IQM in writing within 24 hours of identification, with a copy sent to the Design-Build Designer, Design-Build Contractor, and the Department.

The IQF shall maintain a log of all Nonconformances and submit it weekly to the Department. Each Nonconformance shall be numbered sequentially, given a brief description and a status, and, if it is not

closed, an expected date for closure. The Department will not grant Final Acceptance for any portion of Work that has an outstanding Nonconformance.

2.6.2.6 Improvement

The Developer shall continually improve the effectiveness of the quality program through the use of the quality policy, quality objectives, audit results, analysis of data, Corrective and Preventive Actions, and management reviews.

The Developer shall take action to eliminate the causes of Nonconformities in order to prevent recurrence. Corrective actions shall be appropriate to the effects of the Nonconformities encountered.

A documented procedure shall be established to define requirements for:

A. Reviewing Nonconformities (including Department complaints).B. Determining the causes of Nonconformities.C. Evaluating the need for action to ensure that Nonconformities do not recur.D. Determining and implementing action needed.E. Records of the results of action taken.F. Reviewing the effectiveness of the Corrective Action taken.

The Developer shall determine action to eliminate the causes of potential Nonconformities in order to prevent their occurrence. Preventive Actions shall be appropriate to the effects of the potential problems.

A documented procedure shall be established to define requirements for:

A. Determining potential Nonconformities and their causes.B. Evaluating the need for action to prevent occurrence of Nonconformities.C. Determining and implementing action needed.D. Records of results of action taken.E. Reviewing the effectiveness of the Preventive Action taken.

2.6.2.7 Scheduling

The Developer shall provide the Department and the IQF with a minimum of a three-week look-ahead schedule, by Thursday of each week, of all planned activities, including design reviews, pre-activity meetings, and fabrication, to allow the Department to schedule its resources. The schedule shall indicate the date and location of each activity. The Developer shall also develop and distribute to appropriate personnel a daily construction schedule of activities for each Work day to facilitate coordination with the IQF and the Department’s forces.

2.6.2.8 Progress Payment Requests

The QMP shall describe the means by which progress payment requests are:

A. Generated in accordance with requirements of the Public-Private Agreement. B. Compared to the Progress Schedule to identify percentage of Work completed.C. Certified by the IQF that any Work listed as 100% complete has been reviewed and verified by

the IQF. D. Accurate and any outstanding issues that would require rework are documented by the IQF

along with a revised percentage of completion based on these issues. E. Backed up by documentation supporting the complete percentages.F. Accompanied by a progress report which includes, at a minimum:

a. A written certification signed by the IQM, IDQM, and the ICQM, indicating that the QMP and all measures and procedures provided therein are functioning as intended and are being fully complied with. This certification will be in addition to the requirements of C&MS 109.09.

b. For the final payment request, a certification from the DB Project Manager and the IQM attesting that all design, construction and materials incorporated in the Project conform to the Contract requirements.

c. A summary of the past and future accomplishments and challenges.d. A summary of internal quality activities.

2.6.2.9 Final Acceptance

At the completion of the Work, or if the Department grants a partial acceptance conforming to C&MS 109.11, the IQM shall jointly conduct a final inspection with the Department and FHWA. Final Acceptance shall conform to C&MS 109.12 and shall include, associated as-built documents, deliverables, certifications, other required documentation, and Developer clean-up requirements. The Department and the IQM will jointly create a punch list of Nonconforming and/or incomplete Work and date of Correction or completion for each. The IQM shall verify that each item has been corrected or completed, provide all final documentation, and perform a final review and inspection with Department representatives to verify that the punch list items have been satisfactorily resolved.

2.6.3 Quality Planning The Developer shall plan, develop, and document in the QMP the processes needed to deliver the Work in accordance with the Contract requirements, including:

A. Geotechnical investigations and testing.B. Field survey verification.C. Control of monitoring and measurement devices, to ensure that tools, gages, instruments, and

other measuring and testing devices used in activities affecting quality are properly maintained, controlled, calibrated, certified, and adjusted at specified periods to maintain accuracy within necessary limits.

D. Design planning, to establish the responsibilities, checks, reviews, timing, procedure or reference standard, and resulting records for all design submittals.

F. Contract deviations, to ensure that general or specific variances from Contract requirements occur only with the Department’s Approval.

G. Validation of computer programs and checking of inputs.H. Cross-discipline reviews, to ensure consistency and prevention of coordination errors.I. Conformance checking, to ensure the right requirements are being utilized.J. Fitness for use reviews, to ensure that Work will meet generally implied expectations.K. Accuracy checking, to ensure requirements are designed right.L. Style checking, to ensure compliance with appearance needs, i.e. CADD, file type, spelling, etc.M. Constructability reviews, to ensure the implementability, accessibility, and maintainability of the

Work.N. Over-the-shoulder reviews.O. Scope checking to verify the completeness of submittals.P. External (Third Party) reviews, to obtain input and buy-in.Q. Design change notices, to ensure that revisions to design are controlled by the Engineer of

Record and communicated clearly to all points of use.R. Field design change requests, to ensure that all and any changes to design occur only under the

direction or approval of the Engineer of Record.S. Construction planning, to establish the responsibilities, inspections, tests, timing, procedure or

reference standard, and resulting records for all incoming, in-process and final products. If inspection, sampling and testing frequencies are not stated in the specifications, the IQF shall develop and provide sample sizes and inspection and testing frequencies in the QMP to meet the requirements of the MOP.

T. Pre-activity construction meetings, to ensure all parties responsible for the quality of the Work have a common understanding of the requirements, the design intent, the applicable Design Documents, procedures, laws, regulations related to the Work. Pre-activity meetings shall be held for all significant activities and repeated whenever there are significant changes in personnel or working conditions, and when there has been a significant lapse of time since the activity was last undertaken.

2.6.4 Design

2.6.4.1 Design Planning

The Developer shall plan and control the design of the Work.

The design planning document (see 2.6.3 D) shall detail the design stages, packages, reviews, verifications, validations, and the responsibilities and authorities. It shall also specify those elements of the Contract Documents, Design Documents, and Construction Documents that require special attention or emphasis, including applicable standards of quality or practice to be met, level of completeness, and/or level of detail required.

The Developer shall determine and ensure compliance with the review, permit and approval requirements of all affected agencies.

The Developer shall organize the interfaces between different groups involved in design in order to ensure effective communication and clear assignment of responsibility.

The Developer shall provide the Department a copy of the design planning document every time it is updated, including for changes in personnel.

2.6.4.2 Design Inputs

The Developer shall review design inputs (reports, regulatory requirements, Contract requirements, etc.) for adequacy prior to use. Incomplete or ambiguous requirements shall be resolved prior to use.

2.6.4.3 Design Outputs

The Developer shall produce design outputs in a form suitable for verification against the design inputs. Any partial design submittal shall be in a form that can be reviewed without having to make assumptions regarding how other portions of the Project might be designed. Any assumptions shall be included with each design submittal.

The Engineer of Record and QC checker shall be clearly identified on all Design Documents.

Design outputs shall be approved by the Developer and IQF prior to release. Reports shall be provided in PDF format. Design plans included in interim and final design submissions and Released for Construction Documents shall be provided in PDF, TIFF, and CADD (MicroStation and Geopak) format conforming to Department standards. Shop and working drawings shall be developed and provided according to the C&MS. All necessary approvals of Design Documents and approved Design Documents shall be provided to the Department prior to the start of the Work detailed by the Design Documents.

The Department and the City of Cleveland manuals contain requirements for design submittals required as part of their design-bid-build processes. All design information required to be submitted as part of the City of Cleveland processes shall be submitted as part of an interim and final design submittals for this Project.

At a minimum, interim design submittals shall be in general conformance to Stage 1 detailed design requirements as per the ODOT Location and Design Manual, Volume 3, Section 1403.7; however, variations may result due to design-build delivery. The timing, packaging and grouping of this information shall be as indicated in the Progress Schedule. The QMP shall include details of what each

design submittal will include and also identify proposed variations from Location and Design Manual submittal content and format.

2.6.4.4 Developer Design Checking and Reviews

The Developer shall ensure that all Design Documents are checked and back-checked in accordance with the procedures defined in the QMP. All designs shall be reviewed and checked by Professional Engineers licensed in the State of Ohio with experience in the design discipline of the Work being checked, and at least an equal level of qualification and experience as the designer. All structural designs shall be independently checked by qualified Developer QC personnel, including an independent analysis and check of structural calculations. All interim, final, and Released for Construction submittals shall be accompanied by a QC certification from the Developer, signed and sealed by the designer and QC reviewer, indicating the Design Documents have been designed in accordance with the Contract Documents and prepared and checked in accordance with the QMP.

Any and all QC checking or review necessary to produce design submittals in accordance with the Contract requirements shall not be performed by the IQF.

All affected disciplines shall participate in cross-discipline reviews. The Engineer of Record and the appropriate design manager(s) for the discipline(s) involved in the design (e.g., Chief Bridge Engineer, Roadway Design Manager, etc.) shall participate in all design reviews.

2.6.4.5 IQF Design Checking and Reviews

The IDQM shall review all design submittals to verify they are in accordance with the requirements of the Contract and that their development was in accordance with the QMP. All designs shall be reviewed and verified by Professional Engineers licensed in the State of Ohio with experience in the design discipline being reviewed and verified.

2.6.4.6 Over-the-Shoulder Reviews

Over-the-shoulder reviews are informal examinations performed by the IQF and the Department of Design Documents during the design process. Over-the-shoulder reviews will mainly assess whether the requirements and design criteria of the Contract Documents are being followed and whether design QMP processes and procedures are being followed. The Developer shall schedule over-the-shoulder reviews with the IQF and invite the Department during the course of the development of each design package, prior to interim and final submittals. The Developer shall invite affected Utilities, Railroads, and City Departments to each over-the-shoulder review to provide opportunity to comment as requested or as otherwise deemed necessary by the Department. The design planning document (see 2.6.3D) shall define the frequency and timing of over-the-shoulder reviews.

Over-the-shoulder reviews shall be conducted in the Project office of the Department, the Developer or its design engineer and in the presence of the design personnel, with the intent of minimizing disruption of ongoing design Work. The IDQM, design staff, and the Department will jointly determine the materials to be compiled for each review. Formal assembly and submittal of drawings or other

documents will not be required; however the Developer is encouraged to provide informal submittals to facilitate reviews. The over-the-shoulder review may be of progress prints, computer images, draft documents, working calculations, draft specifications or reports, or other Design Documents. If mutually agreed upon for specific review items, the over-the-shoulder review may be facilitated by the transfer of electronic files. The IQF shall document the outcome of each over-the-shoulder review. It shall be the Developer’s responsibility to confirm whether comments made are in conformance with the Contract requirements.

2.6.4.7 Interim and Final Design Submittals

The IQF shall conduct formal interim (40 – 60 percent complete) and final (100 percent complete) reviews of design submittals, in accordance with the processes and procedures defined in the QMP. These formal reviews are required for each Progress Schedule-defined Buildable Unit segment or Developer-defined Buildable Unit segments.

The Developer shall schedule a submission meeting with the IQF to present each design submittal. Other agencies, (i.e., FHWA, City departments, Railroads, and Utilities as applicable to the submission) including the Department, shall be notified in advance of each submission meeting, and shall be accommodated to attend each meeting at its sole discretion. At submission meetings, the Developer shall provide an overview of the submittal, including a summary of all included information. The IQF shall not accept submissions with missing information.

Each formal IQF review shall be a complete review of all Design Documents, to ensure that Contract requirements and design criteria are being met. The IDQM shall not release any design submittal that does not meet Contract requirements or was not developed in accordance with the QMP. The IQF does not have the authority to waive Contract requirements. In addition, the IDQM shall verify that there are no conflicts between the Buildable Unit being reviewed and any previously approved designs. All deviations from these requirements shall be noted and corrected.

Upon IQF approval and verification that a design submittal is in compliance with the Contract, the IQF shall submit Design Documents to all applicable agencies requiring review and upload electronic copies to the Department’s document management system. Submission requirements, beyond those stated in the Contract, to agencies other than the Department shall be determined by and complied with by the Developer.

The Department will not provide formal review of design submittals; however design submittals are subject to Department audit over the course of design development. Department design audits may be limited to scope compliance; however the depth, magnitude, type, and timing of the Department’s audit of design submittals will be at the Department’s sole discretion. As necessary, the Developer shall schedule comment resolution meetings with the Department to resolve comments received as a result of Department audit.

The review time frames of other agencies for each design submittal are as indicated in Table 2-3. Review times shall commence on the next Business Day after submission to the applicable agency. The Developer shall schedule comment resolution meetings with involved agencies, as necessary to address comments received, no sooner than the review times allotted for each agency. The Developer shall not implement deviations to Contract requirements requested by other agencies without the Department’s Approval.

The IDQM shall prepare and distribute minutes of each submission and comment resolution meeting and shall ensure that all review comments are addressed by the Design-Build Designer and verified by the IQF. Final design submittals shall include a written disposition of all comments made during formal interim design submittals.

2.6.4.8 Released for Construction

Following review of final design submittals, the Developer shall resolve all outstanding issues and comments and prepare a full set of Design Documents stamped "Checked and Ready for Review." The Developer shall schedule a submission meeting with the IQF to present the submittal. Other agencies, (i.e., FHWA, City departments, Railroads, and Utilities as applicable to the submission) including the Department, shall be notified in advance of the submission meeting, and shall be accommodated to attend each meeting at its sole discretion. The Developer shall provide an overview of the submittal, including a summary of all included information. The IQF shall not accept submissions with missing information.

The IQF shall verify compliance with all Contract requirements and design procedures, and ensure that all comments from final reviews have been resolved. If upon IQF review it is determined that it is questionable as to whether comments received from the Department or other agencies have been resolved or addressed appropriately, the IDQM may consult with the commenter to resolve such comments.

The IDQM shall sign and stamp on the Design Documents “Released for Construction” after the following requirements have been met:

A. All IQF, Department, and other agency comments and Nonconformances have been resolved.B. Design Documents have been designed in accordance with the Contract requirements and are

consistent with the overall design. C. Design Documents have been checked in accordance with the QMP.

D. All deviations have been Approved by the Department.E. Design Documents have been signed and sealed by the Engineer of Record.

The Developer shall provide detailed quantity estimates for all Work covered by Design Documents, for use by the IQF and the Department to facilitate quality sampling and testing (i.e., the units will be consistent with the units used to determine frequency of sampling and testing). For example, if the number of compaction tests to be taken is based on a specific number of cubic yards of embankment, then the quantity estimate shall also be in cubic yards. Quantities may be provided after Design Documents are Released for Construction; however, quantities shall be provided prior to any construction commencing for the Work covered by the applicable Released for Construction Documents.

2.6.4.9 As-Built Submittals

As-Built Documents shall be produced as the Project progresses and each Buildable Unit is completed. As-Built Documents shall be prepared in conformance with the Location and Design Manual, Volume 3, Section 1200 (Plan Preparation) and be submitted in both hardcopy and electronic (PDF, TIFF, and CADD) format, including MicroStation and Geopak files, conforming to Department CADD standards.

The Developer shall prepare formal As-Built submittals for each Buildable Unit that incorporate all updates to the Released for Construction Plans Documents including any design changes, actual field as-built changes, actual survey info; and up-to-date copies of all other Design Documents including Project reports, calculations, design files, etc. The Developer shall also provide a set of As-Built drawings for each Buildable Unit that clearly identify all changes made to the Released for Construction PlansDocuments. This shall include as-constructed information on all Utilities relocated (public or private) as part of the Project. As-built Documents shall be reviewed and verified by the IQF for completeness, accuracy, and Contract compliance prior to submission to the Department.

2.6.4.10Design Changes

All design changes shall be identified, received, tracked, reviewed, responded to, approved by authorized personnel, and distributed prior to their implementation in accordance with a defined procedure. All design changes shall follow the same process for checking and review as the original design.

In all cases, the IDQM shall certify in writing that the design change:

A. Has been designed in accordance with Contract requirements.B. Has been checked in accordance with the QMP. C. Is consistent with other elements of the original design.[D.] Has been reviewed by the IQF and the Department. D.[E.] Is fully compatible with the design of related elements.

2.6.5 Construction

2.6.5.1 Construction Planning

The Developer shall plan and implement the monitoring, measurement, analysis and improvement processes needed to:

A. demonstrate conformity to Contract requirements,B. ensure conformity of the quality program, andC. continually improve the effectiveness of the quality program.

The QMP shall specify procedures to address all elements that affect the quality in the production, placement and finishing of all construction items, including roadway surfacing, paving, structural concrete, lighting, pavement markings, etc. These elements include the Developer management and process control personnel, testing equipment and laboratory facilities, testing frequencies, aggregate production, stockpile management, proportioning, mixing and processing, transporting, placing, spreading, depth or thickness, finishing, compaction, joints and mix designs.

The construction planning document (see 2.6.3 S) shall detail the construction stages, products, methods, inspections, reviews, sampling, tests, verifications, validations and the responsibilities and authorities. It shall also specify those elements of the Contract Documents, Design Documents and Construction Documents that require special attention or emphasis, including applicable standards of quality or practice to be met, level of completeness, and/or level of detail required.

The Developer shall organize the interfaces between different groups involved in construction in order to ensure effective communication and clear assignment of responsibility.

The Developer shall provide the Department a copy of the construction planning document every time it is updated, including for changes in personnel.

No construction Work may proceed without Design Documents released for the purposes of construction, for the applicable Work.

2.6.5.2 Purchased Products

The Developer shall ensure that purchased product conforms to specified purchase requirements. The Developer shall evaluate and select suppliers based on their ability to supply product in accordance with Project requirements. Criteria for selection, evaluation and re-evaluation shall be established. Records of the results of evaluations and any necessary actions arising from the evaluation shall be maintained.

Purchasing information shall describe the product to be purchased, including, where appropriate,

A. Requirements for approval and certification of product, procedures, processes and equipment, including requirements associated with the Developer’s option to use the Department’s TE-24 program.

B. Requirements for qualification of personnel, and C. Quality program requirements.

The Developer understands that purchased products being certified under the Department’s TE-24 program are subject to Department staffing levels and availability for shop inspection acceptance prior to product delivery to the Site. The Developer shall coordinate and schedule the Work to account for any necessary time for the Department to complete the off-site inspections under the TE-24 program, if the Developer chooses to use this option for certification.

The Developer shall ensure the adequacy of specified purchase requirements prior to its communication to the supplier.

The Developer shall establish and implement the inspection or other activities necessary for ensuring that purchased product meets specified requirements. Where the Developer intends to perform verification at the supplier's premises, the Developer shall state the intended verification arrangements and method of product release in the purchasing information.

2.6.5.3 Material Identification

The QMP shall include procedure(s) to identify and control materials, equipment and elements of the Work to prevent the use of incorrect or defective materials and equipment. These procedures shall ensure that identification of the item is maintained by appropriate means, on records traceable to the item, as necessary, throughout fabrication, erection, installation and use of the item. The product status with respect to monitoring and measurement requirements shall be known throughout product realization (development, manufacturing, and delivery). Material identification shall include a direct relationship to the Department’s construction management system materials identification and tracking processes.

2.6.5.4 Monitoring and Measurement

The Developer shall apply suitable methods for monitoring and, where applicable, measurement of the construction processes. These methods shall demonstrate the ability of the processes to meet requirements. When requirements are not met, Correction and Corrective Action shall be taken, as appropriate.

The Developer shall monitor and measure the characteristics of the product to verify that requirements have been met. This shall be carried out at appropriate stages of the product realization process in accordance with the construction plan (see 2.6.3 S). Evidence of conformity with the requirements shall be maintained. Records shall indicate the person(s) authorizing release of product to the IQF.

The IQF shall verify compliance by:

A. Establishing checklists of all applicable requirements to be verified at quality checkpoints (QCPs) before Work is covered or put into use.

B. Implementing QCPs for all Work [C.] Documenting, and making readily available, the objective evidence used to verify compliance

with specified requirements. Work shall not proceed past a QCP without 100 percent% compliance and evidence of samevisual inspection compliance, with documentation following as described herein.

At a minimum, QCPs shall be established at the stages of construction listed below:

A. Environmental Controls:1. At the completion of each property’s hazardous material or regulated waste mitigation

Work, prior to general demolition Work.2. Before any construction occurs and after implementation of Best Management Practices

(BMPs) and environmental controls.3. Prior to crossing any stream, dewatering, diverting watercourses or building cofferdams.

B. Earthwork: 1. After all clearing, grubbing, and excavation to check subgrade or embankment

foundations.2. Per C&MS for lift requirements and at intervals of embankment construction of 20,000

cubic yards or 10 vertical feet, whichever occurs first (applicable to all embankments, including walls).

3. After completion of mechanically stabilized earth (MSE) wall panel placement every ten vertical feet.

C. Structures:1. Before placement of the leveling pad of a MSE wall or other type of retaining wall (to

confirm subgrade materials).2. At completion of bridge embankment settlement and before start of bridge foundation

pile driving.3. At approval of pile driving submittals (including design calculations, wave analysis,

hammer and dynamic load testing).4. After completion of pile driving at each structure support (pile group) (including pile-

driving results and records).5. Before concrete placement of any substructure element, including piling and drilled

shaft in-filling.6. After girder and diaphragm placement.7. Before concrete placement of deck, approach slabs, diaphragms and parapets.8. Before installation of post-tensioning cables into structural segments.9. At the completion of the first mass concrete pour.

D. Surfacing, Paving, and Concrete:

1. Before placement of each course above subgrade on any permanent roadways components (Granular Borrow, UTBC, lean base, LSE, CSE, etc).

2. Before placement of each lift of asphalt concrete or Portland cement concrete paving on any permanent roadway components.

E. Field Painting of Structural Steel:1. Follow QCP requirements as set forth under C&MS 514.

F. Traffic Devices, Maintenance of Traffic: 1. Prior to opening to traffic.2. At the end of each month, review Work Zone Traffic Control documentation for

compliance and action.

The QMP shall define the process for accomplishing QCPs. At a minimum, the following process shall be followed:

Performance of a visual inspection of the Work completed up to the QCP. Attendees to include Developer QC and IQF personnel, with notice provided to the Department. Items to be inspected shall be signed off by Developer QC and IQF personnel on the QCP checklist to identify compliance. The Developer Construction QC Manager shall sign-off on the QCP checklist certifying that all visual checks have been performed, items checked are in compliance with Contract requirements, and the documentation to verify compliance is available. Work may proceed upon Developer Construction QC Manager sign-off on the QCP checklist, at the Developer’s sole risk.

Within five Business Days of the visual inspection, Developer QC shall submit to the IQF all documentation required to verify compliance of the Work for the QCP.

Within seven Business Days of the visual inspection, the IQF shall review the documentation and verify compliance of the Work for the QCP. The ICQM shall sign-off on the QCP checklist upon verification.

Within three Business Days of ICQM sign-off of the QCP checklist, the Developer shall receive notice and sign-off from the Department on the QCP checklist that the QCP has been Accepted. The Department will not sign off on the QCP checklist until all previous Developer and IQF QCP sign-offs have been provided. The QCP will be closed upon the Department’s sign off on the QCP checklist.

2.6.5.5 Dispute Resolution

The QMP shall propose a procedure to resolve disputes that may arise between Developer QC, IQF or QO sampling and testing. Procedures shall be in compliance with the Department’s dispute resolution processes and non-specification materials policies. If the dispute resolution system involves a "dispute resolution laboratory," the name of a recommended laboratory shall be included. The dispute resolution laboratory shall not be a participant of either the Developer or the Department’s project team and must be an accredited AASHTO Materials Reference Laboratory. The Developer shall be responsible for all costs of the dispute resolution laboratory. Dispute resolution processes shall only evaluate if a result is

or is not accurate. Dispute resolution processes that initiate additional testing without showing initial testing incorrect will not be acceptable.

2.6.5.6 Materials Testing

Developer Quality Control shall sample and test materials in accordance with the requirements (location, frequency, lot sizes, test methods, etc.) specified in the C&MS, Supplemental Specifications, MOP, Sampling and Testing Manual, and Contract Documents. If frequencies and intensities are not defined, Developer Quality Control shall perform testing at frequencies and intensities deemed necessary by the Developer to assure proper process control verification. Developer QC testing shall include:

A. QC testing for all QC/QA specifications, including C&MS 455 and Supplemental Specifications 840 and 878. C&MS 455 shall be required for items 305, 451, 452, 511, 524, 526, and 622.

B. QC testing necessary to assure proper process control for non-QC/QA specifications

The IQF shall sample and test materials in accordance with the requirements (location, frequency, lot sizes, test methods, etc.) specified in the C&MS, Supplemental Specifications, MOP, Sampling and Testing Manual and Contract Documents. The IQF shall maintain an ongoing log of the number of tests required, number of tests performed and quantity of material tested for all material test types. IQF testing shall include:

A. QA testing for all QC/QA specifications, including C&MS 455 and Supplemental Specifications 840 and 878 (i.e., the IQF shall perform the QA role for all specifications that call for Department QA). C&MS 455 shall be required for items 305, 451, 452, 511, 524, 526, and 622.

B. Testing for all non-QC/QA specifications in accordance with the MOP.C. Asphalt core sampling and testing, as required by the Contract Documents.D. Testing of all intermediate strength concrete breaks for loading and/or curing purposes. Results

shall be reported prior to curing, removal, or loading.E. Additional concrete strength testing such as coring, as required by the Contract Documents.

IQF and QO tests shall be performed side-by-side, at the Department’s discretion in order to establish a confidence level between the results of IQF and QO tests. The IQF shall increase the frequency of testing when necessary to assure or improve quality, e.g. sliver fills. Results of the IQF’s testing will be compared with the Department QO testing. If the results of the QO sampling and testing validate the results and quality of the material, the material will be accepted.

2.6.5.7 Property Handling

The QMP shall specify procedures to control the handling, storage, shipping, cleaning and preservation of materials and equipment to prevent damage or deterioration. The Developer shall be responsible for the security and preservation of all equipment, materials and Work.

The Developer shall exercise care with Department property while it is under the Developer’s control or being used by the Developer. The Developer shall identify, verify, protect and safeguard Department property provided for use or incorporation into the Project.

2.6.5.8 Laboratories

Accreditation shall also be obtained for AASHTO and ASTM test methods that are modified or referenced by Ohio test methods. Laboratories shall be AMRL accredited before starting Work. A copy of AMRL accreditation certificate(s) shall be submitted to the Department upon receipt by the testing laboratory.

2.6.6 Environmental Monitoring and Compliance The Developer shall describe in the QMP the methods, processes and procedures to provide for the effective implementation and documentation of the environmental protection, training, compliance and monitoring program.

2.6.7 Maintenance of Public Facilities The Developer shall specify in the QMP procedures to be followed in the maintenance of roadways on the Project and off-site haul roads for controlling noise, dust and debris associated with the hauling operations. Such procedures shall provide the public with safe and effective alternative routes, as well as safe and convenient access to residences and businesses affected by construction activities.

2.6.8 ApprovalsThe Construction and Materials Specifications (C&MS), Supplemental Specifications, MOP, and City of Cleveland Specifications contain multiple references to “approvals by the Engineer” or similar language. The Department waives submittal requirements of the stated specifications, however such approvals and submittals shall be the responsibility of the IQF to provide review and approval for. The IQF does not have the authority to waive specifications or any other Contract requirements.

2.7 DeliverablesUnless otherwise indicated, all deliverables shall be submitted in both electronic format and hardcopy format. Acceptable electronic formats include Microsoft Word, Microsoft Excel, or Adobe Acrobat (.PDF) files, unless otherwise indicated. At a minimum, the Developer shall submit the following to the Department:
