    In accordance with Arkansas Telecommunications Providers Rule 9.03(C)(7), enclosed for filing are the Trustees decisions relating to the grant request applications received from various telecommunications providers. Also enclosed are the legislative notification letters required to be provided in accordance with Ark. Code Ann. 23-17-404(e)(8)(D)(ii) and filed with the Commission in accordance with Rule 9.03(C)(2).

    Also enclosed are monthly report for the activity months of April and May 2004, detailing the disposition of each grant request (both initial and true up requests) awarded to each telecommunications provider, the exchange in which the grant beneficiary is located, the name and address of each grant recipient, and the number of additional customers to be served are also enclosed. These reports are filed pursuant to Rule 9.03(C)(7)-(9).

    We are writing to you in our capacity as ihe Trustee of the Arkansas Extension of Facilities Fund. The Rhoads & Sinon Group LLC administers this Fund pursuant to the direction of the Arkansas Public Service Commission effective January 1, 2004.

    The Arkansas General Assembly enacted the Extension of Facilities Fund pursuant to Act 1771 of 2001, which is codified in Ark. Code Ann. $23-17-404(e)(8). The Fund provides grants to extend wireline telecommunications facilities to persons who currently do not have access to wireline telephone service. The Act requires that notice be provided to members of the General Assembly whenever grants are made in their districts.

    We are in receipt of the following grant extension request that have been submitted by the wireline telephone company on behalf of the constituent listed below:
