APRIL 2, 2013 The following Mechanical and Electrical Addendum Items shall be made part of the Contract Documents for the referenced project. SPECIFICATIONS 1.01 A copy of the Pre-Bid Meeting Agenda and Sign-In Sheet have been attached for

the Contractor’s reference. 1.02 Reference Specification Section “KDE FORM OF PROPOSAL”: A copy of the

same has been attached herein for the Contractor’s convenience. 1.03 Reference Specification Section “LIST OF UNIT PRICES, PROPOSED

SUBCONTRACTORS, MATERIALS AND MANUFACTURERS”: A copy of the same has been attached herein for the Contractor’s convenience.

1.04 Reference Specification Section “SPECIAL CONDITIONS”:

A. A “REVISED SCHOOL CALENDAR” has been attached herein which updates the last day of school to May 16th, 2013.

B. The Ohio County High School Gymnasium will be continuously unoccupied

throughout the period of June 25th – July 9th, 2013 to comply with the Kentucky High School Athletic Association “Dead Period”. The Owner will subsequently coordinate with the successful bidder to provide the Contractor as much free and unencumbered access to the facility during the period while school is not in session should the “Dead Period” not afford the Contractor enough time to complete the specified scope of work within the Ohio County High School Gymnasium.

C. The Ohio County Middle School and all Elementary School Gymnasiums will

be continuously unoccupied throughout the period of May 16th – July 23rd, 2013.

D. The Contractor shall take all necessary measures required to minimize the interruption to the HVAC system cooling operation associated with the Work at all Facilities which are included in Bid Packages #2 and #3 respectively.

1.05 Reference Specification Section 07540 “THERMOPLASTIC MEMBRANE

ROOFING”: Per Paragraph 3.8 “ROOFING INSTALLER’S WARRANTY”, the Contractor shall assure that the existing roof warranties as denoted below are maintained subsequent to all roof cutting and patching activities.

1.06 Reference Specification Section 15010 “GENERAL PROVISIONS FOR

MECHANICAL”: The Contractor shall assure that all appropriate measures are undertaken during the course of demolition and subsequent construction activities to protect existing facilities and infrastructure to remain upon completion of the Project scope of work. Particular attention shall be paid to interior finished installations. The Contractor shall be responsible to remediate any and all damage to infrastructure to remain as directed by the Owner.

1.07 Reference Specification Section 15110 “GENERAL-DUTY VALVES FOR

MECHANICAL PIPING”: Per Paragraph 3.4.C.4 “GENERAL REQUIREMENTS FOR VALVE APPLICATIONS”, the Contractor shall update section to denote that at Contractor’s Option, grooved-end valves will be allowed for steel condenser-water valve applications NPS 2-½” – 6”.

1.08 Reference Specification Section 15181 “HYDRONIC PIPING”: Per Paragraph 2.9

“CHEMICAL TREATMENT”, The Contractor shall update section to denote that the Contractor shall include within their proposal all labor and materials necessary to maintain the existing chemical treatment paradigm (water quality and chemical and propylene glycol concentrations) at all Facilities which are included in Bid Packages #2 and #3 respectively subsequent to all demolition and installation activities.

1.09 Reference Specification Section 15513 “CONDENSING BOILERS” and the

“CONDENSING BOILER SCHEDULE” on Bid Package #2 Drawing ME.0.1:

A. Basis-Of-Design Boiler shall have cast-iron heat exchanger. All Approved Equal Substitution Boilers shall have Type 316L stainless-steel heat exchangers as specified.

B. Boiler venting shall be double-wall AL29-4C stainless-steel liner with aluminized shell as scheduled.

1.010 Reference Specification Section 16010 “GENERAL PROVISIONS FOR

1.011 Reference Specification Section 16050 “COMMON WORK RESULTS FOR

ELECTRICAL”: All exposed Class 2 and CAT 5 low-voltage control conductors shall be within one-half inch (½”) minimum trade size EMT conduit.

1.012 Reference Specification Section 16145 “LIGHTING CONTROL DEVICES”: Per

Paragraph 2.3.C.4 “BMS COMPATIBILITY”, the Contractor shall assure that the networked lighting controls package specified for the Ohio County High School Gymnasium is provided with the necessary gateway and/or communications protocols to allow for a fully-integrated, hardware I/O integration into the Facility’s BACnet Building Automation System. I/O integration shall allow all networked lighting control system zones to be individually viewed, scheduled and controlled from the BMS Operator’s Interface.

BID PACKAGE #1 – GYMNASIUM LIGHTING REPLACEMENT DRAWINGS 1.013 Reference the “ELECTRICAL SYMBOL LEGEND” on Drawing E0.1:

OCCUPANCY SENSOR specification shall be updated as follows:

A. Ohio County High School: Sensor Switch NCM6 (or equal). B. Ohio County Middle School and all Elementary Schools: Sensor Switch CM6

(or equal) 1.014 Reference the “ELECTRICAL SYMBOL LEGEND” on Drawing E0.1: POWER

PACK RELAY CIRCUIT specification shall be updated to a Sensor Switch PP-20-2P (or equal).

1.015 Reference the “LIGHTING FIXTURE SCHEDULE” on Drawing E0.1: Per

“SPECIAL NOTES” Item A, the mounting height of all specified light fixtures shall be flush with the bottom of the trusses. The Contractor shall coordinate the final installation height with the existing gymnasium truss height at each Facility.

1.016 Reference Drawings E1.1B, E1.1C, E1.1D, E1.1E and E1.1F: Occupancy

sensors in the Middle School and Elementary School gymnasiums shall be configured to operate independently by row the length of each gymnasium.

1.017 Reference Drawing E1.1E: The Contractor shall incorporate the scope of work denoted on Sketch XE01 subject to all relevant provisions of the Contract Documents.

BID PACKAGE #2 – HYDRONIC BOILER REPLACEMENT DRAWINGS 1.018 Reference the “HVAC NOTES” on Drawing ME0.1: All exposed LP and Natural

Gas Piping (Interior and Exterior) shall be prepared, primed and painted in a color as selected by the Owner.

1.019 Reference Drawings ME1.1A and ME3.1: The Contractor shall completely

remove all infrastructure associated with the existing heating hot-water system to the limits of construction denoted on Drawing ME1.1A. In particular, the Contractor is advised to remove the two (2) existing heating hot-water expansion tanks which are located within the roof truss space but are not graphically depicted within the “PARTIAL DEMOLITION PLAN” on Drawing ME1.1A.

BID PACKAGE #3 – CLASSROOM HVAC REPLACEMENT DRAWINGS 1.020 Reference the “TYPICAL LOUVER CONNECTION DETAIL” on Drawing ME4.1:

The Contractor shall update the detail to incorporate the scope of work denoted on Sketch XH01 subject to all relevant provisions of the Contract Documents.

PROJECT MANUAL:

§ Invitation for Bids

o Bid Opening at 2:00 p.m. C.D.T. on Tuesday, April 9th, 2013.

§ Bids Opened at the Ohio County Board of Education Board Room.

§ Bids must be accompanied by Signed and Dated Form of Proposal and Notarized 5% Bid Bond.

§ No Bids shall be withdrawn for thirty (30) days.

§ Owner anticipates award of Bid and Notice to Proceed on or near April 18th,

2013. § Form of Proposal

o Project is divided into Three (3) Bid Packages and Proposers are allowed to submit Bids for each Package individually and/or in any combination:

§ Bid Package #1- Gymnasium Lighting Replacement. § Bid Package #2- Hydronic Boiler Replacement. § Bid Package #3- Classroom HVAC Replacement.

o All Bids shall be identified both within the Form of Proposal “Project Title” and on the outside of the sealed envelope as to which Bid Package(s) the Bid represents.

o There is one (1) Alternate which is associated with Bid Package #1 as follows:

§ Provide the amount to furnish, install and integrate T-5 fluorescent lighting in lieu

of LED lighting for Fordsville Elementary, Southern Elementary, Wayland Alexander Elementary and Western Elementary.

o Lists of Unit Prices, Proposed Subcontractors and Materials/Manufacturers shall be

submitted at time of bid.

o Tax-Exempt Bid Break-Out Forms and associated Material Supplier Authorizations and Purchase Orders shall be submitted within seven (7) days of the time of bid.

o Executed contract shall be standard AIA A101-2007 Standard Form of Agreement Between Owner and Contractor with noted KDE Amendments in accordance with all applicable Contract Documents and complying with all relevant Local, State and Federal Codes/Regulations.

o Performance and Payment bonds shall be secured in accordance with AIA A312-1984

Performance Bond and Payment Bond with noted KDE Amendments prior to execution of contract.

o Notarized Affidavit(s) of Liability, Worker’s Compensation and Unemployment Insurance

in accordance with the Contract Documents shall also be provided prior to execution of contract.

§ Prevailing Wage Requirements

o Project is subject to the current Prevailing Wage Rates as per the Prevailing Wage Determination included within the Project Manual.

TIME LIMIT FOR EXECUTION OF CONTRACT DOCUMENTS In the event that a bidder's proposal is accepted by the Owner and such bidder should fail to execute the contract within ten (10) consecutive days from the date of notification of the awarding of the contract, the Owner, at his option, may determine that the awardee has abandoned the contract.
