Healthy Merits Well-Being Program We understand every client is unique. That’s why our Healthy Merits program provides the flexibility you and your employees need for success. Our program helps your employees and their spouses find inspiration to create successful, lasting behavior change. This translates to a healthier bottom line for you. Healthy Merits is designed around the tools and resources you and your employees want most: an online progress dashboard, device integration, mobile-friendly delivery and engaging, easy participation programs. By focusing on total population health and providing the support of the nation’s second largest Third Party Administrator (TPA), Meritain Health, we’ve crafted a truly unique wellness experience. An online questionnaire guides participants to the areas they should focus on to improve their well-being. Tracking. Track challenge progress via text message, mobile app, web portal, or wearable device (including fitness apps). Rewards. Get your team going with employer-funded premium differentials, raffles, fitness gear, gift cards, and more. Biometrics (optional). Offer biometric screenings at your office, local labs, and/or a physician’s office. Health Coaching (optional). Pair your participants with health experts to help them achieve their wellness goals.

Sometimes simply offering a program isn’t enough. That’s why we work hand-in-hand with you to create exciting program components, get the program off the ground and ensure you and your employees are getting the most out of the experience.

Create Launch EvolveHealthy Merits clients are paired

with a population health management consultant whose main focus is on

wellness. We discuss your goals, your population and create a program and

incentive design that puts success within reach for all participants.

Kick off the program with momentum. Hold in-office events. Send announcements. Encourage

employees to start with the Health Assessment and help them choose personal challenges. We provide all the communications and guidance

you need to help you create a culture of well-being.

As your participants get going, your consultant makes suggestions on how

you can evolve your program. We take a look at engagement, claims

impact and health improvements and share your successes with you. We put on our creative hats and make

recommendations that are a perfect fit for your culture.
