2. After confirming the accuracy of your basic information, click on the "Select a Category" drop down menu. Start

with Section II.A: Courses Taught. This section will automatically populate with any CU Denver courses that you

are listed as the instructor for. Double check the accuracy of these course listings. (See screenshot on following

page).

3. If you have taught classes outside of UC Denver, click "Add Activity" on the bottom right hand side of the course

list. NOTE: Fields marked with a red asterisk are required and you will not be able to save your entry unless all

asterisked fields are completed.

4. After selecting a category from the drop down menu, all previously entered activities will be listed below the

category drop down. To edit or delete a previous entry, click the corresponding word to the right of the

description. A window showing the details of your entry will appear below the listing of activities, and the

activity which is being edited will be highlighted in the list above.

5. To add a new entry, click Add Entry which is on the bottom, right corner of the screen. For more information

about each of the fields, click the '?'. The notes section of each form has a 4,000 character limit, so please

provide as much detail as you can about each activity. There are no spell-check applications, so it may be best

to type your note in a Word document, then simply copy and paste into the form.
