Update on Pertussis in Ontario Region

Cases in Ontario

The purpose of this memo is to inform health care providers working in First Nations communities

across Ontario that there have recently been several confirmed cases of Pertussis in Ontario

Region. Pertussis is a highly contagious disease that can have devastating effects, especially in our

vulnerable infant population. It is important that we strive to stay up to date on our immunization

programs. Our immunization program in combination with client education is our best defense

against the spread of this disease. Anyone with cough persisting for 2 weeks or longer along or with

paroxysmal cough of any duration, cough with inspiratory "whoop" or cough ending in vomiting or

gagging, or associated with apnea should be further assessed for Pertussis.

Background of Pertussis

Pertussis (whooping cough) is caused by the bacterium Bordetella pertussi. It can affect anyone of

any age though its’ greatest severity is amongst infants who are too young to have completed their

immunization series. It is highly communicable and most often is transmitted from infected

adolescents and adults to infants. Its’ transmission route is respiratory through contact with infected

respiratory droplets. It has a usual incubation period of 9-10 days but can range from 6-20 days.

Clients are most contagious during the first two weeks after the onset of a cough. They are no longer

considered infectious once they are 3 weeks post the onset of a cough or after 5 days of appropriate

antibiotic treatment. Until a confirmed case is either 3 weeks post onset of cough or 5 days into

appropriate antibiotic treatment, they should be advised not to be in contact with any infants or

pregnant women in their third trimester.

The disease has three stages:

catarrhal stage (1-2 weeks): runny nose, sneezing, low-grade fever, and a mild cough

Page 11: Week of November 20-26

Page 2of 3

paroxysmal stage(1-8 weeks): bursts of rapid coughing, ending with an inspiratory whoop and

sometimes post-tussive vomiting

convalescent stage (several months): the slow recovery stage

Confirmed Cases:

Pertussis is a reportable disease. Confirmed cases,(either laboratory confirmed or clinically

compatible symptoms with an epidemiological link) must be reported to the Zone CD Nurse

Immediately by telephone.

Routine Publically Funded Pertussis Vaccine Programs

Vaccine Components Vaccine Trade Name Ages dose is given

DTAP-IPV-Hib Pediacel 2mths, 4mths, 6mths,18mths

Tdap-IPV Adacel-Polio, Boostrix Polio 4 years old

DTaP-IPV Quadracel 5-6 years old requiring catch-up

who did not receive at least 3

previous doses of (DTAP-IPV

component)

Tdap Adacel, Boostrix 14-16years, one dose after the age

of 18 years in lieu of routine Td.

Please refer to Ontario’s Publically Funded Immunization Schedule –March 2015 for more details on routine,

catch-up and high risk schedules.

Additional Notes on Adult Tdap Vaccine

Adults aged 18-64 years of age, who have not previously received a dose of acellular pertussis on or

after their 18 birthday are recommended to receive one single lifetime adult booster of Tdap.

This lifetime dose of Tdap will replace one of the Td booster doses given every 10 years. A client does

not have to wait for ten years to pass following their previous Td booster to be eligible for the Tdap

dose. If Td was given in error where Tdap could have been given, wait 4 weeks and then offer Tdap.

Who Should We Prioritize In Vaccinating

Infants who are missing their 2mth, 4mth or 6mth old immunizations for Pertussis.

All other infants and young children

Parents, grandparents or caregivers of newborns, infants and young children

Other adult household contacts of newborns, infants and young children

Pertussis Vaccination in Pregnancy

All pregnant women following 26 weeks of pregnancy who have not received a dose of a pertussis containing

vaccine in adulthood should be encouraged to receive Tdap vaccination.

Recommended Actions for Health Care Providers

Health care providers are encouraged to offer immunization against Pertussis to all those eligible

under Ontario’s Publically Funded Immunization Schedule-October 2015.

Anyone with a cough persisting for 2 or more weeks or paroxysmal cough of any duration, cough with

inspiratory "whoop" or cough ending in vomiting or gagging, or associated with apnea should be

further assessed for Pertussis.

Report any confirmed cases of Pertussis to the Zone CD Nurse via telephone immediately.
