Annex 4) Four strategic shifts needed in Government plans to accelerate the annual rate of reduction in under-five mortality:

A) Geographic focus – Intensifying our efforts in the countries that have the highest rates of child mortality U5 Mortality x 1000l.v SOWC 2012 in TACRO: Example: Haiti (165), Bolivia (54), Guatemala (32), Suriname (31) Guyana (30), Dominican Republic (27), Nicaragua (27), Trinidad Tobago ( 27), Honduras (24), Paraguay (25), Jamaica (24). ( see the rest of countries to continue to make progress below 20)

B) High-burden populations focus – Intensifying our focus on underserved populations, including rural, low-income and disadvantaged groups.Promote disparities analysis by geographic groups (rural/urban/periurban), by income, by race and ethnic groups, and disable populations.Example: Disparities analysis paper in peer reviewed journals

C) Scaling-up cost-effective solutions to the most important causes of under-five deaths Infectious causes, neonatal deaths (preterm birth complications, intrapartum-related

complications, and sepsis or meningitis and post-neonatal deaths ( pneumonia, diarrhea, and malaria).

Key interventions include: Preventative interventions: zinc supplementation, vitamin A supplementation, improved water sources, water connection in the home, improved sanitation, hand washing with soap, hygienic disposal of children’s faeces, exclusive breastfeeding for the first six months of life, and intermittent preventive treatment for malaria during pregnancyImmunizations: PCV, Hib vaccine and rotavirus vaccine Treatments: Antibiotics for suspected pneumonia treatment; ORS and zinc for diarrhea treatment; and antibiotics for dysenteryHealth service strengthening: Methodologies for diagnosis and situation analysis, building local capacity and local management process, social and community mobilization and strategic alliances. D) Addressing the socio-economic determinants of maternal and child survival: Strengthening the focus on girls’ education, wash, nutrition, child protection, and inclusive economic growth.
