more than 15,000 patients• 82% type 2 diabetes patients had reduced glucose levels within a year of

their first consultation• Focus on patients with long-term conditions• Sees 35 patients and carries out 10-20 phone consultations in 3 days a

week, significantly reducing GP workload.• “It’s rare you will get people querying why they have to see the pharmacist

nowadays. Patients know they can see their GP, one of the nurses or a pharmacist - they know they will be directed to the right clinician to meet their needs.”

• Providers will receive confirmation of access to SharePoint• From Friday 11 September to Thursday 17 September
