Person Specification – Community Navigator

Essential Criteria

Experience

A minimum of 2 years active and successful experience of working with older people within a health/ social care or information and advice setting

Experience of person-centred care planning

Demonstrable experience of working to contract specifications.

Demonstrable experience of working with and supporting volunteers.

Demonstrable experience of constructive partnership working with voluntary, statutory and community organisations

Skills and knowledge

Knowledge and understanding of issues which can impact on the health and wellbeing of older people.

Excellent interpersonal and communication skills (including the ability to write clear reports and communicate sensitively with people at all levels).

Demonstrable experience of gaining knowledge of local services

Ability to work on own initiative and under pressure.

Ability to work co-operatively and effectively as part of a team.

Competency in use of Microsoft Office applications (Word, Excel, Outlook) and databases.

Ability to systematically record data efficiently.

Excellent demonstrable administrative and organisational skills

Understanding of the principles of equality and diversity.

Full driving licence, access to a vehicle and willingness to travel throughout East Sussex (for which mileage will be paid).

Willingness to participate in fundraising activities.

Desirable Criteria

Knowledge of local statutory and voluntary services in East Sussex

Knowledge and understanding of policy and practice in Adult Social Care and Health, including personal budgets

Geographical knowledge of Lewes, Wealden and Havens areas.
