Module 1: Welcome and Territory Acknowledgement and Overview of Training

Learning Objectives: To acknowledge the Indigenous territory and welcome and introduce the trainers and the attendees. Before the beginning of the training, it is important to acknowledge and honour the Indigenous traditional stewards of the location where the training is taking place. This practice is part of the reconciliation and decolonization work. It is recommended that the trainer research the name of the Indigenous territory and the First Nations people that are on the land. Trainers may choose to invite Indigenous elders to give an opening statement. The segment will also provide an overview of the training agenda and the Open Doors project history and its grant partners. It will share the training objectives of the Open Doors project and encourage the attendees to ask any questions so there is a clear understanding of what will be accomplished in the next 3 days.

Module 2: Group Agreement and Hopes and Fears

Learning Objectives:

Group Agreement: By drafting a group agreement together the objective is to create collaboratively a safe space for the training and to concretely agree to respectful behaviours for the 3 days together. Part of the agreement will involve a discussion that the subject matter can be sensitive and allow for participants to feel comfortable in expressing their views and concerns. Confidentiality, self-care and ways to address disagreements will be covered in the group agreement.

Hopes and Fears: This is a facilitated exercise that allows participants to express their hopes and fears individually and the trainer will collect all the notes and present them to the participants. The trainer will read out loud the fears first and then the hopes. The trainer will acknowledge the honesty expressed and the value in sharing these concerns and stress the commonality of the sentiments. The trainer will discuss how hope motivates us in our work and how fear makes us feel vulnerable however there is a benefit of feeling unsure as it requires reflection and often cooperation with others. The goal is to applaud the participants for their honesty and build a trusting useful relationship with them for the next 3 days.

Module 3: Connections between Violence, Mental Wellness, Substance Use and Social

Context (Video: For her Own Good: Emotional Resiliency after Abuse”)

Learning Objectives: This section demonstrates the social context and location for women’s experiences with violence. Through a facilitated discussion the presenter will explore the intersection of mental wellness, substance use and gender-based violence and the greater context of these experiences. The goal is to demonstrate how each circumstance influences each other and how violence is often the precursor to mental wellness and substance use issues. Through the use of academic research on victimization and its effects, the section discusses how to recognize that many “symptoms” are often coping strategies for external events and need to be viewed with compassion and in the social context.

“For Her Own Good: Emotional Resiliency after Abuse” is a 30 minute video which depicts the stories of 6 survivors who have experienced violence and oppression and their effects. The video is moving and depicts the women’s struggles and their healing. The facilitator should be mindful of the potentially emotional impact of the video and allow time for the participants to ponder the message and get oriented gradually back to the training. The video is followed by a facilitated discussion with the group where the key messages of the video are discussed and recorded on a flip chart. The objective is to ground the training participants in the real lives of women and their competing priorities (safety, housing, income, health) and struggles to access coordinated accessible long term services.

Module 6: Feminist Trauma-Informed Approach and Understanding Trauma

Learning Objectives: This module provides a historical overview of feminist trauma informed practice and how the feminist lens can be incorporated into the daily practices of the shelters and houses. The trainer exercises caution regarding personal histories and on-going experiences of trauma for the participants and includes self-care practices. The objective is to introduce the participants to trauma and the DSM V approach to trauma and how it needs to be adapted to recognize and respect women’s experiences.

Module 7: Overview of PTSD and Supporting Women who Experience Trauma

Learning Objectives: This section provides an overview to the participants of the medical research regarding PTSD and Trauma. It familiarizes the participants with simple and complex PTSD and the physical interaction between trauma and memory. It will introduce the “window of tolerance” analysis for trauma-focused therapy and the effects of co-occurring difficulties. The trainer will lead them in an exercise to map the impact of trauma to reinforce the information. The trainer will present practical information as to the signs and symptoms of trauma and how knowing this information can provide the participants with methods to practice trauma informed and trauma specialized work. The trainer will provide practical tools for the participants to interact with women with an approach that honours collaboration and provides choice to the residents. The trainer will also teach skills for recognizing triggers and warning signs and suggestions for calming, grounding and staying present. Tools for self-care will also be discussed.

Module 8: Vicarious Trauma

Learning Objectives: This module defines vicarious trauma and the impact of vicarious trauma on workers and workplaces. It provides practical tools as to how to recognize vicarious trauma and how to incorporate protective factors in your personal and professional life to reduce the effects of vicarious trauma. The trainer will engage the presenters in a small group activity that will enable them to identify the impact of vicarious trauma collaboratively.

Module 9: Self-Care

Learning Objectives: Drawing on the information provided to the participants, this module closes the first day of training. The section is a guided discussion of self-care and collective values to build common ground. The section allows the participants time for self-reflection as well as an opportunity to share with the larger group. The trainer will lead a discussion related to professional burn-out that is fostered by a radio interview that addresses self-care, burn out and the social context and underlying inequality that are daily events in this sector. The trainer will lead a discussion related to the social injustice issues raised in the interview and use that framework to inspire the participants in their professional growth. The objective is also to provide them quiet time and space to review their personal self-care practices and to consider the adoption of new practices with new trustworthy colleagues. The module has two self-care exercises – the first one involves individual work regarding self-care practices and the second exercise is the Mind Map exercise which is a group activity. In the Mind Map exercise the trainer engages the participants as a team to name the values that drew them to this work and to determine how these collective ethics can energize and foster sustainability and creativity.

Module 10: Mental Wellness and Violence, DSM-V & Medical Mode and Re-thinking

Learning Objectives: This module starts the second day of training. The trainer welcomes the participants with a belly button writing warm up exercise that is fun and an energizing way to start the day. This module provides clinical and medical mental health information along with new methods for support women with varying levels of mental wellness. It engages the participants to re-think what is normal in their daily lives. The module covers the DSM V and diagnoses and disorders and discusses the pros and cons of DSM diagnoses and labeling. The materials discuss the drawbacks of the medical model and it challenges the audience to consider what behavior is “normal” and “abnormal”. The trainer will engage the participants in a small group brain storming activity as to how mental health diagnoses can be exploited by perpetrators of abuse and how this knowledge can inform the work at the shelters and houses. This module also presents information about prescribed medications and includes an individual exercise where the trainer will ask the participants to self-reflect on the use of medications for women who are diagnosed with a disorder. The participants are given the opportunity to share their reflections if they would like by the trainer. The purpose is to give the participant space and time to consider their own personal values regarding prescribed medications before considering the application of policies at their workplace regarding medications.

Module 11: Starting the Conversation and Safety Planning for Mental Wellness

Learning Objectives: This module trains the participants on how to start the conversation with women who may be reluctant to disclose challenges they have had relating to mental wellness. The trainer will provide practical tools and scripts that will assist the participants in communicating with women in the house and will brainstorm with the group the reasons why women may be reluctant to share this information and how the house can address those issues. The trainer will also address how to respond to disclosure from a woman and how the worker and the program can affirm her autonomy and support her. The trainer will present to the group how the participants and their agencies can safety plan for mental wellness so that they are attuned to symptoms and have a support plan in place to keep women safe and on the road to healing.

Module 12: Supporting Women who Experience Mental Illness Symptoms: Depression, Anxiety and Psychotic Disorders

Learning Objectives: This module provides information about signs and symptoms for depression, anxiety and psychotic disorders and uses on-topic flip charts that address the signs and symptoms for each category during the group discussion. The trainer will instruct the participants as to how to support women experiencing these symptoms along with practical tools and safety plans to use when assisting the woman particularly in a communal living setting. The trainer will present the information to the group and three role-play exercises are built into the module for participants to partner up and practice talking to each other about practical activities and responses for each category. The trainer will also draw on the expertise in the room to add to the list of practical responses. The module covers the use of referrals for services and inquires as to existing protocols and policies from the participants. The use of critical Incident reports and forms will be reviewed as well.

Learning Objectives: This section familiarizes the participants with self-harm thinking and the variety of self-harm methods they may need to address in the shelter or transition house. The trainer will provide the participants with practical methods and supportive responses for women who self-harm understanding the underling context for the behaviour. The participants will learn how to actively listen and be patient and non-judgmental as they support women.

Module 14: Supporting Women with Suicidal Ideation

Learning Objectives: This module will provide information to participants about suicide and how to talk about suicide with a focus on language related to suicide. The section provides tools as to how to start a conversation with residents who are suicidal. The trainer will engage the participants by drawing on their expertise regarding suicide and ask about their relevant protocols and policies. The trainer will instruct the participants as to how to support women experiencing suicidal symptoms along with practical methods and safety plans to use when assisting the woman particularly in a communal living setting. There is a small group activity where participants discuss together how to begin a conversation about suicide with a resident and how to prepare for that conversation. Participants will be able to share their observations with the larger group if they choose to do so. The module covers the use of suicide risk assessment tools and whether the shelters or transition houses use a similar tool and provides community resources for referrals and support services.

Module 15: Substance Use and Violence

Learning Objectives: This module demonstrates the strong link between substance use and trauma and contains information that is helpful for supporting women who use substances. The trainer will instruct the participants on the spectrum of substance use ranging from beneficial use to dependence. The trainer will instruct how abusers can exploit substances and how and why survivors use substances. The trainer also will present information as to the risks of stopping substance use and how women need to be listened to and respected in order to keep them safe. To personalize these experiences, the module includes a small group discussion for participants to discuss together their own coping mechanisms and how healthy and accessible these substances are and how hard they would be to give up? Participants may share their findings if they choose to with the larger group.

Module 16: Understanding Substance Use

Learning Objectives: This module explains how substance dependency develops and reinforces this information by watching the “Rat Park” video and then having a group discussion of the findings and conclusions from the video. The trainer will engage the participants in a group discussion of what are substances and how are they regulated. The discussion will discuss both legal and illegal substances and how these categories play a role in the general perception of the user. The module also provides specific information as to types of substances including depressants, stimulants, hallucinogens and cannabis. The goal is to provide the participants a context as to why substance dependency develops in order to develop effective supports for women on the road to healing and self-sufficiency. Participants will view the video Safe For All and learn about some of the harm reduction practices that may be put in place in their own agency and how that will support the Open Doors Inclusive Services Model.

Module 17: Supporting Women who Use Substances and Safety Planning for Substance Use

Learning Objectives: This module teaches the participants how to support women who use substances and provides information regarding signs and symptoms of intoxication, withdrawal and overdose. The trainer will engage the participants about their existing policies and how they can be amended if necessary to accommodate women who use substances. The module includes a small group discussion and brainstorm session where participants discuss with each other their use of substances and how it impacts their safety and whether they practice harm reduction. This exercise personalizes these issues to increase empathy for the women residents who may be using substances. The module also covers safety planning for substance use and community resources and harm reduction resources. Note, this module is the last section of the second day of the training.

Module 18: Record Management and Case Notes

Learning Objectives: This module provides an overview of record management practices and information privacy practices when supporting women in an inclusive service model. The trainer will discuss privacy principles and policies and present sample policies, consent and release forms. Topics covered include maintaining the security of records, retention and disposal of records, limits to confidentiality and circumstances when disclosure is allowed. The trainer will provide practical guidance on how workers at transition houses and shelters can produce case notes that are useful for supporting women in an inclusive service model. The module discusses the benefits and risks of recordkeeping and challenges the participants to consider and review their current case note practices. The module also discusses how information sharing needs to be practiced in a feminist trauma informed way when supporting women in an inclusive service model. The trainer provides the participants practical methods to balance the need for confidentiality and limit the re-victimization of the women.

Module 19: Supporting Mothers

Learning Objectives: This module provides practical guidance on how workers at transition houses and shelters can support mothers experiencing trauma, mental wellness and substance use. The trainer will invite the participants to think about their own beliefs about mothers who use substance and to decouple the assumption between substance use and parenting capacity. The module discusses the importance of creating a supportive and non-judgemental environment to encourage women to seek health care and promote the health of the future baby.

Module 20: Revisiting House Rules, Eligibility and Intake

Learning Objectives: This module provides an example to illustrate how house rules may be developed from systemic oppression that reinforces the dominant culture’s values and beliefs. The trainer leads participants to examine eligibility and suitability criteria for women experiencing trauma, mental wellness and substance use and what information is necessary to determine if the communal setting is a good fit. The trainer discusses intake forms and the intake process for these women and how the process can be adapted to meet all parties’ needs. The trainer will lead the group in a small group exercise regarding house rules and guideline to evaluate whether these rules reflect the foundational principles of the inclusive service model. The trainer will also discuss critical incident reporting and engage the participants to consider whether their current practices are consistent with this model.

Module 21: Debriefing Practices

Learning Objectives: This module provides practical tools as to how staff can support each other in the critical work that is done at shelters and houses. The trainer will engage the participants to recognize the reality and challenges for staff related to limited resources and understaffing along with the constant demand for services. Incorporating an inclusive service model into these challenging circumstances may seem insurmountable to some participants but the trainer will explore how this can be an opportunity for growth and creative problem solving. This module demonstrates the importance of incorporating debriefing practices into shelters and transition houses that are practicing an inclusive service model. The trainer will provide practical tools as to how to debrief with a co-worker involving 4 steps: venting, personal context and reframing, social context and action. The trainer will also review with the participants their current debriefing practices at the shelters and houses.

Module 22: Applying Foundational Principles and Revisiting Policies

Learning Objectives: This module reviews the foundational principles of the Open Doors Inclusive Service Model and provides the participants an opportunity to review their policies in order to consider how to implement an inclusive service practice. Participants will have time to work in small groups to review and discuss their local policies and practices and determine what needs to be revised to open doors for women.

Module 23: Sharing Ideas and Going Forward

Learning Objectives: This module provides the trainer a closing opportunity to help the participants synthesize the content they have learned, to reflect on the training and to consider next steps. The trainer will revisit the hopes and fears observations from day one and measure any and all changes made in the context of the participants’ hopes and fears going forward. The training guide also provides other closing activities to consider such as the exit ticket, the review game and 3-2-1. Finally, the trainer should leave sufficient time for the participants to fill out the Open Doors evaluation form and to place the form in the envelopes at their tables with the last participant sealing the envelope.

Learning Objectives: This module provides participants with the time to report back on their planning sessions and their visions on the implementation of the Open Doors Inclusive Service Model. The trainer will discuss with the participants the next steps for bringing back this model to their communities and agencies.
