A 64 years old Asian female presented with pain on her lower front teeth and wanted to restore her smile without wearing a removable prosthesis.

Her #22, 23, and 24 were extracted that day and study models were taken. Her upper posteriors needed sinus grafts, so we phased her treatment to complete the mandibular arch, graft bilateral sinuses, and then complete the maxillary arch.

The implants were restored 6 months later using the Unipost system and splinted PFM crowns.

Pre-op condition: Fully edentulous maxilla. SA-4 sinuses bilaterally. Division C-w bone in incisor positions. Division A bone in canine and premolar areas.

Surgical report: Bone expansion areas #4, 6, 8, 9, 11 and 13. All sites prepared exclusively with osteotomes.

Implants placed without complication except #9. Revision to procedure involved removal of implant #9 and placement of implant #10 using a combination of rotary and hand instruments.

Straight posts modified outside the mouth and tried in without cement. Hydrocolloid impressions made immediately post op. Jaw relation records made.

Stone model scanned using desktop scanner. PMMA provisional restorations designed using Exocad software.

Restoration milled overnight in PMMA and ready for delivery at 8am. You will notice that I modified the provisional slightly. For example, #10 was designed with a prominence at the gingival 1/3. This was simply ground off with a high speed.

Timeline: Thursday 2pm consult, followed by surgery and impressions. I took my wife to dinner from 6:00-7:30pm and returned to my office from 8-9pm to design restoration. The CAM file was sent to mill at 9pm and went home. Removed PMMA from mill at 8am and finished and polished. PMMA delivered at 8:30am Friday morning.

Patient on liquid diet during integration period. Final restorations will be milled in office, full contour zirconium crown and bridge reconstruction.
