We have all heard the expression...keep your friends close and your enemies closer. This year, The Tatum Times will be featuring YOUR ENEMY in the hopes that you will be ever mindful and cautious. The light at the end of the tunnel is that with caution, concern, and continuous education this enemy called occlusion will not reek havoc in your practice. The terminology is extensive with words like group function, hydrostatic extrusion, and even the word harmony. It even has fancy acronyms like ICP (intercuspation position), NWSI (non working side interference), CO (Centric Occlusion) and CR (Centric Relation). At the time when Dr. Tatum entered into private practice, the plan was to do full mouth restorative treatment. With this intention, he knew that it was imperative to learn as much about occlusion as possible. The fact that Dr. Tatum had an extensive background in restorative treatment and implantology, helped him to design a root form implant system and a set of guidelines that have the key components for success; Adequate stable bone, compressive loads transmitted to bone, adequate stable gingiva around implants, occlusion built to strengthen and protect from the weaknesses, minimize lateral loading, and most importantly to avoid deflective contacts. Since implants have no proprioception, pain, or warning, the crestal bone micro-fractures when loaded beyond its physiological limits, which results in progressive bone loss. When Dr. Tatum was asked how his perception of occlusion has changed throughout his professional career, he answered, “I have increasingly come to appreciate how vital and imprecise the TMJ really is.” We truly hope that the next series of articles helps each and everyone of you to man- age the occlusal concerns in your practice. So many of you have shown interest in appearing in the Tatum Times, and now is your chance. During the late 70’s and early 80’s many implant papers were published stating that a clinician must expect to lose up to .1 - .3 millimeter of crestal bone per year after loading. For those of you who have attended Dr. Richard Borgner’s seminars, you have heard him say, “this just isn’t true!” He stresses to his students that it is all about managing forces. When Dr. Borgner was asked about this misconception, he stated, “If one insures that the forces transferred from the implant abutment into the supporting bone are within the physiological limits of the bone, than you can expect to have no crestal bone loss long term.” In Dr. Tatum and Dr. Borgner’s practice, they have proven this concept to be true by showing many cases that have been in place for up to 25 years and show virtually no bone loss since placement of the prosthetic components. UPCOMING EVENTS Tatum Surgical is looking forward to seeing you at the 2012 Western/Central District AAID Meeting on June 8-9, 2012 in Vancouver, Canada.
