The UERM Hospital Wellness Diagnostic Program

• Contact the Wellness Coordinator for appointment at Room 101, Wellness Assessment Center, Upper Ground Floor, Medical Arts Pavilion.

• For company sponsored Wellness Program, a letter of authorization is required.

• The required fees must be paid directly to the Cashier’s Office prior to the procedure unless previous arrangements have been made through their companies that have credit lines with the UERM Hospital. A copy of the Authorization Letter from the company addressed to the UERMMMCI must be submitted, as applicable.

• All additional diagnostic examinations which are not part of the package will be charged separately at discounted rates. Examinations recommended by the Attending Physician which do not form part of the package are charged to the patient’s account.

• Additional specialist consultant/s will also be charged separately.

• Substitution of procedure is not allowed.

• Refused procedure by the patient is considered paid.

• Cancellation of procedures shall only be allowed if notice was given at least one (1) week prior to the scheduled date. Corresponding payment shall be refunded to the patient, net of bank charges, if payment was made thru debit or credit card.

Prior to the Scheduled Date: 1. Please make sure all procedure-specific preparations indicated in your Itinerary will be followed.

2. Collect stool and midstream urine samples not later than 1 hour before your expected time of arrival at UERM Hospital. 3. Place urine and stool samples in specimen bottles. You may request for specimen bottles from the Wellness Assessment Center. 4. Please bring a Letter of Authorization (LOA) and membership/company ID if accredited by HMOs/companies. 5. Please bring medical history records, if available. Upon Arrival at the Hospital:

1. Go directly to the Wellness Assessment Center at the Medical Arts Pavilion Bldg., Room 101 Ground Floor on your appointment date. The Wellness Assessment Center office opens at 8:00 a.m. 2. Tests will be done consecutively based on your Itinerary. Results will be released 24 hours after the last scheduled procedure. Upon Completion of Procedures:

You will be given a Summary Report containing all your test results. The Wellness Physician initially interprets your test results, but you will be referred to a Specilist Consultant, if necessary.
