LAf tH:) Office of Marine and Aviations Operatfofls SUBJECT: PERIODIC PHYSICAL EXAMINATION REQUIREMENTS FOR NOAA CORPS OFFICERS On behalf of the National Oceanic and Atmospheric Administration's Commissioned Corps (NOAA Corps), I would like to extend our deepest appreciation to all of the military treatment facilities and medical providers that have previously and are currently providing medical services to the active duty men and women of the NOAA Corps. As time has evolved, most military treatment facilities have changed how they provide periodic health care and screening to active duty service members. The currcnt trend has moved toward a preventive health assessment screening history with a visit to a health care provider only if a problem becomes evident on the history. The present problem faced by our NOAA Corps officers is, as a uniformed service, we still require the complete history and physical examination by a licensed physician, nurse practitioner, or physician's assistant with specific labs/x-rays both for the periodic physicals for active duty and for diving. The specific requirements are noted below. The periodic history and physical examination must include: A. Completed form DO 2807-1 (Report of Medical History) with examiner signature B. Completed form DO 2808 (Report of Medical Examination) with signature 1. Vision - near and distant vision (corrected and uncorrected), visual fields, tonometry (required annually starting at age 40), depth perception 2. Height, weight, blood pressure 3. SUBJECT: PERIODIC PHYSICAL EXAMINATION REQUIREMENTS FOR NOAA CORPS OFFICERS

On behalf of the National Oceanic and Atmospheric Administration's Commissioned Corps (NOAA Corps), I would like to extend our deepest appreciation to all of the military treatment facilities and medical providers that have previously and are currently providing medical services to the active duty men and women of the NOAA Corps.

As time has evolved, most military treatment facilities have changed how they provide periodic health care and screening to active duty service members. The currcnt trend has moved toward a preventive health assessment screening history with a visit to a health care provider only if a problem becomes evident on the history. 1. Vision - near and distant vision (corrected and uncorrected), visual fields, tonometry (required annually starting at age 40), depth perception

2. Height, weight, blood pressure 3. Complete blood count (CBC) 4. RPR (Serologic test for Syphilis) 5. Lipid panel to include total cholesterol, HDL, LDL, triglycerides, VLDL

(done only every 5 years for those over age 35; done for those less than age 35 if they use tobacco, have diabetes mellitus, have elevated blood pressure, family history of elevated lipids, or family history of heart disease before age 50 in males and before age 60 in females)

6. EKG if clinically indicated, resting pulse < 40 or> I00, required armually starting at age 40

7. Urinalysis by dipstick to include specific gravity, glucose, protein, blood, leukocyte esterase, nitrite

8. Chest x-ray only if clinically indicated (mandatory for diving and aviation) 9. PAP/pelvic exam and breast exam every 2 years (except for those who

have had a hysterectomy) 10. Screening mammography for females at age 40 and annually II. Audiogram - 500, 1000, 2000, 3000, 4000, 6000 hertz 12. Tuberculin skin test or Quantiferon test required armually (unless

contraindicated) for all shipboard /aviation personnel or those officers embarking on a NOAA vessel for greater than 24 hours. All others are required to have this once every 5 years

13. Fecal Occult Blood Test (FOBT's), required annually starting at age 40 14. Spirometry 15. OSHA Respirator Questionnaire 16. Other tests as deemed necessary

C. Completed DD 2813 (Active DutylReserve Forces Dental Examination I. Type II dental exam

Though the needs of our officers are outside of the trend toward the preventive health assessment approach, it is our request that these specific needs above be honored to the fullest extent possible at the time of the allotted appointment or during subsequent ones.
