The UIP is following with significant attention the evolution of COVID-19 virus outbreak. Still having in mind the UIP mission of "promoting joint meetings or international congresses", the first concern of course remains to guaranteeing safety of all UIP members and of the population in general. While a significant panic can be generated by the dissemination of medical fake-news, UIP suggests to all meetings organizers the most detailed analysis of the current situation, together with promotion of proper evidence-based information. On March 5, as per the originally set timeline, the Engagement Working Group (EWG) activity was
reported to the UIP Executive Committee (EC), finding not only approval but also appreciation for the
productive attitude of all the EWG members. Seventeen projects are fully ready to be presented and
approved by the EC. Many of them will develop synergy with already existing UIP committees, so
following one of the same UIP objectives: developing teamwork among societies and colleagues.
With the same aim, a UIP “Project Leader” role has been introduced as an opportunity for all vein-
lymphatic experts who want to contribute to the activities of the UIP, according to the following
regulation:
“The International Union of Phlebology (UIP) recognizes the role of “Project Leader” to UIP individual
members and/or members of a UIP Society who propose a valuable project for Phlebology advancement.
The same project and the “Project Leader” status must be approved by the EC majority. The “Project Leader”
status can not be assigned to members of already existing committees of the UIP”.
Therefore, everybody willing to give his/her valuable contribution to phlebology advancement is
welcome inside the UIP. Indeed, UIP starts with “U”.

6th of March 2020.
Last month’s newsletter suggested different ways to par-
ticipate in the World Lymphedema Day and contribute to
the cause.

During March, one event under the Auspices
of the UIP took place.
From the 4th to 6th of March the Egyptian
Vascular Summit was held in Alexandria,
Egypt. The event consisted of three days of
interactive discussion on Vascular medicine
with a special focus on Diabetic Foot,
Peripheral Arterial Disease, EVAR & TEVAR
and Venous Disorders. There was also a
special Nursing Program.
The program covered Challenging Cases &
Clinical Scenarios, Guidelines, Controversies &
Updates and Round Table Discussion.
Everything went as planned and it was a
complete success.

DO YOU WANT YOUR EVENT TO BE UNDER THE
AUSPICES OF THE UIP?
The UIP will give permission to use the official logo for courses, symposia, or meetings that receive the
auspices of the UIP.
The organizers of events are required to send a request for the auspices to the President of UIP with a copy
to the General Secretary. The request should be in writing and accompanied by the Scientific Program.
Meeting organizers may request recognition from and granting of auspices by the UIP under the following
conditions:
1. The meeting is organized or endorsed by one of the following: UIP-affiliated societies in Tier 2/3 countries, UIP-affiliated societies in Tier 1 countries, Non-UIP-affiliated, not-for-profit societies / organizations, For-profit, non-industry societies / organizations
2. A program is submitted to the Scientific Committee and judged to have sufficient educational or scientific merit on veins and lymphatics to warrant the granting of UIP auspices.
3. The conclusions of the Scientific Committee are approved by the President, General Secretary, and Vice President responsible for the region.

Specialisation in Germany is regulated by the Medical Chambers.
Phlebology is not a primary specialization, but is organized as an additional titulation.
This means: after finishing the specialization, the doctor can apply for a further
training to become a Phlebologist. During this training period the doctor has to be
employed in a center with the allowance of educating phlebologists (this allowance is
decided by the medical chambers).
There are two options: Specializations, that have already been trained about vascular diseases in their
curriculum , like (vascular) surgeons, internists / angiologists and dermatologists need to perform 12 months of
additional training time, other specializations (usually General Practitioners) will have to cover 18 months.
There is a list of skills that have to be performed a minimal number of times under teaching situation in the
diagnostic and the treatment aspect, including a proper documentation, which might be requested at the end
of the training:
This training can be performed in a hospital or in an office, provided they have the full education allowance. At
the end of the training period an oral examination in the medical chamber is performed, where the tutorial
certificates are controlled.
The titulation “Phlebology” endorses the doctor with the right to bill phlebological procedures. The option to
get reimbursement from the NHS when treating patients with surgery is only given, if the primary specialization
is a surgical one.
In addition to this pathway there is a certificate to ensure permanent postgraduate formation in phlebology in
times of rapidly changes in knowledge and techniques, this is renewed every three years provided a number of
phlebology congresses and hands-on workshops have been visited and 30 medical chamber education points
have been certified during the three years.
This way of formation based on specialization first and then the additional title of Phlebology is also visible in
the distribution of the 1750 members in the German Society of Phlebology, which is as follows: 15,2 % of
General Practitioners, 7,8% Trainees for Phlebology, 30,7% Surgeons, 27,1% Dermatologists, 8,8% Vascular
Surgeons, 0,3% Gynecologists, 7,4% Internal Medicine, 0,5% Orthopedics, 2,2% Others (e.g. technicians,
Physiotherapists)

PHLEBOLOGY SPECIALISATION IN GERMANY
Diagnostics: 40 Plethysmographic investigations of superficial and deep veins (PPG), 100 CW Doppler Investigation of arteries at the ankle, 100 duplex ultrasound of superficial and deep veins, 40 ABI (ankle brachial index)
Therapy: conservative treatment of lymphoedema including bandaging techniques, compression therapy and indication for manual lymphatic drainage, conservative treatment of varicose veins, ulcer treatment, as well as 50 interventions of the superficial venous system, e.g. phlebectomy, endovenous ablation, high-ligation and stripping, 20 sclerotherapies of varicose veins.

EDUCATION: ONLINE MODULES
THE ACP/UIP EDUCATION MODULES HAVE A FACELIFT.
Earlier this year, the ACP/UIP Education modules received a much needed update and refresh. The
modules are now much more easy to access and module instructors have been invited to present their
modules as a series of webinars.
The training schedule has also had an update, with new learning outcomes for each module in line with
changes to the curriculum of the Australasian College of Phlebology.

For many years Phlebology has been considered the younger sister of
Angiology, a fact that in the last 15 years underwent a historical
change from several points of view.
The advent of Duplex Scan allows us now see in depth what we have
in venous vessels, what is the pathology and we can solve the prob-
lem under direct vision.
This fact that is considered transcendental within the Phlebology has
determined that many pathologies that were previously treated
blindly or by palpation today are treated using the viewfinder of the
duplex scan - ¨the eyes of the Phlebologist¨)
Simultaneously, Angiology in its subspecialties such as hemodynam-
ics, has advanced to areas we had never imagined could be achieved.
Performing this type of invasive studies, which are increasingly ad-
vanced, it is possible to treat smaller vessels open to blood flow, and
repair arterial flows in territories which we supposed impossible sev-
eral years ago.
The results of arterial stents are very good, either centrally in the coronary or peripheral, in smaller and distal
vessels. These advances in arterial diseases were achieved in Phlebology applying hemodynamics to deep ve-
nous pathologies, with very encouraging results in venous stents, with similar outcomes in arterial permeabili-
ties. The rates of primary and secondary permeability at 5 years according to the Kaplan-Meier survival analysis
were 57.3%, 77.2% and 80.9%, respectively.
Finally, Mabud et al retrospectively analyzed 406 patients to assess the safety and permeability of venous stents
of the lower extremities in the treatment of DVT.
If we stop to think that other pathologies, Phlebology progressed systematically, the artery-vein synergism,
which occurs in Angiodysplasies, does not occur in the other pathologies that are well differentiated.
Referring to the Angiodysplasias, in the Klippel Trenaunay Syndrome , the marginal vein began to gain great
importance, and in recent years it became ineffective in its value, in maintaining a leg with good venous circula-
tion.
I cannot omit to mention the new methods to perform thrombectomies, which are the latest in recent times.
I personally believe that much remains to be done, and that Phlebology will continue bringing news. Changes
that will be reflected in the coming years.

The latest generation percutaneous mechanical thrombectomy devices, including FlowTriever (Inari Med-
ical), ClotTriever (Inari Medical), Indigo (Penumbra, Inc.) and Jeti (Walk Vascular, LLC), have demonstrated
favorable safety profiles and may be less invasive than other thrombectomy devices.
I cannot fail to mention the advances in sclerotherapy with the introduction of two Foam sclerotherapy
techniques, the Tessari technique (Italy) and the Cabrera (Spain) technique, both with a surprising result,
in the treatment of varicose veins and complex angiodysplasias.
The use of Lasers yag 1064 for transdermal pathologies, such as telangiectasias, has been a fundamental
change, to accompany sclerotherapy.
And last but not least I must mention the advances in the different techniques of recent years for the
treatment of superficial venous insufficiency, as the Endoluminal Laser, the radio frequency, the Asval
technique, the Glue (Cyanoacrylate), and many other methods of obstruction of the superficial venous
system.
In conclusion, Angiology with hemodynamics and endovascular surgery has had great advances and un-
precedented improvements in morbidity and mortality.
Phlebology is not far behind and I think that in recent years it has advanced more rapidly, being a solid
specialty, which not only treats varicose veins but multiple diseases as the mentioned above.
I personally believe that much remains to be done, and that Phlebology will continue bringing news.
Changes that will be reflected in the coming years.

PAST, PRESENT AND FUTURE
I stanbul, Turkey’s most populated and most significant city in
cultural and economic terms, is one of the oldest cities in the
world. Archaeological findings that have surfaced recently
indicate that the first settlement in the city dates back to at
least 8500 years.
As host to many different cultures and civilizations as well as to
people of various races, religions, cultures and languages
throughout the ages, Istanbul has always preserved its
cosmopolitan feel. Istanbul is as much a world capital now as it
was in the past. A clear example of that is the multicultural
representation fort the XIX World Congress of the International
Union of Phlebology, to be held on 2023 in this beautiful city.

together and have the opportunity to discuss and exchange
knowledge about aspect of Phlebology related matters and to
