• Each state has its own licensure requirements and application process. It is very important to know the requirements in the state where you wish to become licensed prior to applying to an educational program to make sure it will meet the educational requirement.

THE LIMITED LICENSE ALLOWS YOU TO PERFORM ALL THE DUTIES OF A DENTIST BUT ONLY IN A SPECIFICALLY NAMED PRISON, HOSPITAL, SCHOOL, OR PUBLIC CLINIC UNDER THE SUPERVISION OF A REGISTERED DENTIST.

PRACTICE IN A PRIVATE OFFICE IS NOT PERMITTED.

Limited License Requirements (1) :

An application including CORI request form.

Secured employment.

A non-refundable licensing fee of $90.00.

Proof of the diploma in dentistry with a certified translation.

Certificate in CPR/AED or current certificate in BLS.

Limited License Requirements (2) :

TOEFL or IELTS certificate for international dentist.

A physician’s statement attesting to the health of the applicant.

Certified letters of standing from all jurisdictions.

A practice history, if applicable.

A report from the National Practitioner Data Bank (NPDB) Self-query.

A statement disclosing any disciplinary action against the applicant.

Proof satisfactory of good moral character.

Successful completion of the Massachusetts Dental Ethics and Jurisprudence Examination.

An affidavit, signed and witnessed by a Notary Public.
