Evaluation
It is UAMS policy that all new employees complete a New Employee Medical Screening within the first
31 days of employment.
KIDS FIRST Employees:
Your departmental nurses will provide this for you. You do not need to receive your TB skin test or the
NEMS here.
NEMS
You will be receiving an appointment card tomorrow which will have your appointment time and location
for your New Employee Medical Screening.
Any questions may be directed to:

• You received a TB skin test on 09/23/2013 this must be read by Employee student health services on 09/25 or 09/26 at the location listed below.
• Your new employee screening appointment is on _10/04 at 0800 at location listed below.
• A. SEHS Main clinic is located in the basement of the Family Medical building at 521 Jack Stephens Dr. PH: 501-686-6565. Office hours: 08:00-4:00.
• B. SEHS Central clinic is located on the ground floor RM G-605 of the central hospital building.
• PH: 501-686-8810. Office Hours are 7:00 am- 330 pm.

• Please bring any immunization records you have with you to the New Employee Screening
• If you have any questions please feel free to contact either office at the numbers listed above.
NEMS No shows
• If you can NOT keep your NEMS appointment for any reason please call the number listed on your card.
• If you do not show up to your scheduled appointment your supervisor will be notified.
TB Skin TestAll new employees are required to do a 2- Step
Tuberculin Skin Test (TST.)Your skin test cannot be read before 12pm on Wednesday or it can be read
all day Thursday.
If you have had a skin test in the last 12 months, your documentation will be allowed
to count as your first TST.
The TST placed during orientation will be counted as your second TST.
UAMS policy requires proof of 2 MMR’s, and a Flu shot annually
for all employee’s.
If you work in a patient care area you are also required to have
proof of 3 Hepatitis B’s or titers and a TDAP/TD.
Please bring any immunization records to your appointment.
Immunizations
• If you are unable to provide proof of your required immunizations UAMS will provide these at your New Employee Health screening.
• Notify your supervisor.
• Contact the Company Nurse at (855) 3391893
• Complete the online incident and injury report and it will be forwarded to SEHS.
On the Job Injury
• For Blood or Body Fluid exposures, come
to SEHS clinic (686-6565).
• For any injury that requires a physician
assessment, please go to the Family
Medical Center (686-6560)
• Convenient option for UAMS employees and their family’s primary care provider
• FMC has an employee clinic for acute problems and an extended hours until 7:00pm
