Bunny Hop Stop BreakfastThe Easter Bunny will be coming for a light breakfast of fresh fruit, doughnuts, cerealand bagels accompanied by coffee, milk or juice. After the bellies are full, all children willhave a chance to take their picture with the Easter Bunny and do a craft. LJPre-Registration is required! Come for this unusual egg hunt where eggs will be scattered aroundthe pool for the kids to find. Make sure to come at the appropriatetime – event starts exactly at the time stated and only lasts a few minutesuntil all eggs are found.

Children will be permitted to swim after all groups have completed thedive until 4:30 p.m. Bring plastic bag or basket to collect your goodies,bags will not be available at the pool; straw or wood baskets cannot beused in the pool. Everyone must pay admission to enter the pool area,regardless of whether they swim or not. Children under the age of 10years must be accompanied by a paying adult/guardian age 18 or olderat all times. No passes are accepted. KS

Funky Peace Glam JamJoin Glitzy Girlz for a funky peace-theme class that includes hair updos with neon braids and colored hair spray, acool make-up application, nail designs and a neat hand glove finish the look. Decorate a craft with stamps, paintsand funky embellishments. The jewelry piece is a chunky bracelet with peace dangles. We then fill the room withneon lighting and dance to great tunes with Freeze Dance, Limbo and more. A surprise “peace offering” for eachgirl is provided. LJ

Celebrate Earth Day’s 42nd Birthday in April with the Bartlett Nature Center’s 12th annualcelebration of this environmental awareness event. Everyone is invited to this family-friendlycelebration where you will see a variety of demonstrations, educational exhibits and hands-onactivities. The world around us is always changing, and so is our Earth Day celebration. One thingthat hasn’t changed – this event is FREE! No reservation required. CB

Rules for Splash Pad1. Parents or guardians must supervise their children at all times.

2. Admission to the facility shall be refused to all persons having anycontagious diseases or infectious conditions such as colds.

3. No alcohol is allowed anywhere in the park.

4. The water from Splash Pad features is not suitable for drinking.

5. Glass containers are prohibited.

6. No running or rough play is permitted.

7. No street shoes are allowed on the Splash Pad.

8. Only bare feet or aqua socks are allowed on the Splash Pad playsurfaces.

9. No baby strollers or carriers are allowed near the play features.

10. No soap, shampoos, conditioners, lotions, or body washes areallowed near the play features.

11. Only clean swim apparel may be worn at the Splash Pad.

12. Children who are not toilet trained must wear tightly fitting plasticpants over a swim diaper.

13. No disposable diapers will be allowed in the Splash Pad area.

14. Use of the Splash Pad facility is prohibited when thunder is heardor lightening is seen, including a 30-minute period after the lastlightening or thunder is detected (please see page 63).

15. No outside food, beverages, or coolers may be brought into theSplash Pad area.

16. One adult (age 16 or older) per three children must be present tosupervise.

17. The Splash Pad will not open if the air temperature is less than 70degrees.

18. Groups of 10 or more wishing to use the Splash Pad must obtain aspecial use permit.

19. Climbing on the equipment and fences is not allowed.

For over 17 years, more than 50,000 youth baseball players have attended one of our White Sox summer camps to focuson essential individual baseball skills. Professional coaches from the White Sox organization have programs designed forchildren from 5-11 years old at every ability level.

Every summer camp participant receives an exclusive U.S. Cellular Field Experience! This opportunity of a lifetime foreach camp participant, along with one guest, will include a parade on the field and a Q&A session with a White Soxpersonality, coach and a current White Sox player! Also, during the camp each player will participate in a special SkillsCompetition. The top 30 qualifiers in each age will compete in the finals on U.S Cellular Field in September. This highly competitive team is for girls already enrolledin the 2012 Girls In-House Softball program. Interestedgirls must attend tryouts which are scheduled for May 12.Tryouts will be held at Tefft Middle School (1100 ShirleyAve., Streamwood) at 10 a.m. Girls will be assessed inpitching, hitting and fielding. Registration will not beavailable until after tryouts have been held, and girls havebeen accepted to the team.

All Star teams are required to play in four tournaments,which translates to a minimum 16 games, maximum 30games. Practices will be held once a week during theregular season, and up to three times a week after theregular softball season has ended.

Girls will be given a high quality uniform consisting of ajersey with matching shorts. All girls are required toattend all practices and tournaments unless faced with anemergency, or absence approved by the RecreationManager, John Wilhelm.

Who said softball was a summer sport? The StreamwoodPark District will be offering a fall softball league fromSeptember until October. Games are played at RahlfsWoods and Hoosier Grove. As with the summer league,the fall league depends on parent volunteers in manyaspects of the program, including coaches and concessionoperation.

The 10U, 12U and 14U age divisions will play in anintervillage league on Streamwood Park District fields.Participants may register at any time at the Park Placeregistration office. Registration ends on August 4. Practiceswill start in late August.

The Streamwood Park District is pleased to offer the 6thAnnual Girls Basketball League. This inter-village league isfor grades 3rd-8th. The season will consist of ten gamesbeginning in early January, 2013. Games will be playedonce a week. Practices will be held during the week at theCommunity Center and/or Park Place. Each participant willreceive a uniform consisting of a shirt and shorts.Participants are responsible for their own basketball shoes.

General Notes for Youth Leagues• A candy fundraiser will be required for each child for specified leagues (maximum of 2 children per family), for each season inefforts to keep the costs of the program low. At the time of registration you may opt out of the candyfundraiser (per child with a maximum of 2 children per family) by paying the buyout fee without candy.Any time after the registration deadline, the buyout option is no longer available.

• The fee for candy fundraiser is $60, which must also be paid at the time of registration. Candy will bedistributed with uniforms. It is important to sell all candy which will total $60.

• Each child’s parent or guardian must complete a “Parent Code of Ethics” form at the time of uniform pick-up. No child will receive his or her uniform until the Parent Code of Ethics form has been completed.

• Unsportsmanlike conduct will not be tolerated. Any parent or guardian engaging in improper behavior willbe suspended from the next game. If the unsportsmanlike conduct persists, the next step is expulsion from any remaining gamesof the season.

• All team instruction is the responsibility of the coaches. At no time may any parent or guardian assumethe role of a coach unless first obtaining approval and proper certifications from the Streamwood ParkDistrict Recreation Manager. All family members must remain in the designated spectator area.

• PIF must be filled out and on file with the Streamwood Park District for all community youth leaguesprior to start of session!

The Streamwood Park District soccer program teaches youth soccer skills, while promoting good sportsmanship,physical conditioning and having fun. The Fall season begins in late-August, and runs through early November.Participants may register at the Park Place registration office. We are always looking for volunteer coaches.

U6 and U8: These two divisions are strictly instructional. Games are held at a Streamwood Park District soccer fieldon Saturdays. U8 practices 1 day during the week and U6 practices before games on Saturdays.

U10, U12, U14, and U18:. Both the Girls and Co-ed leagues are part of the Intervillage Soccer program and will requiresome traveling to neighboring towns.

Open registration for fall soccer ends June 23. If registering after the deadline, a late charge of $5 will be added to theregular fee. All individuals who register late are placed on a waiting list and aren’t guaranteed a roster spot.

Golden Lion Academy Martial ArtsGolden Lion Academy Martial Arts is operated through various Park Districts. The focus of the Academy combines thedisciplines of Tae-kwon-do, Hapkido, Judo, and Kung-fu. Through the combination of these Martial Arts a practicalknowledge of self-defense techniques, weapons, free fighting, traditional forms, and discipline is obtained. The Academyis a family organization dedicated to its participants. Students within the Academy are encouraged/challenged to excelinside and outside class in everyday life. Gymnastics safety certified coaches, and their 22,800 square foot facility has state ofthe art equipment. Together Gymkhana and the Streamwood Park District have the goal of providing you with the finestprograms, instruction, equipment and facilities for your child while maintaining a safe, positive, and fun atmosphere.

Youngsters in the Building BlockPreschool Program will learn through play,as they are taught skills to help developtheir creativity as well as their educationaland emotional well-being by qualified andcaring instructors in a friendlyatmosphere. The program’s curriculum isgeared to make sure that students aretaught the skills they will need to enterKindergarten.

Our Building Block Preschool Program isa year long program running fromSeptember 10, 2012 through May 18,2013. The program follows School DistrictU-46’s holiday schedule. Preschool is stillheld on certain Institute Days; pleasecheck your Parent Manual for moreinformation upon registering.

General Notes for Building Block Preschool•Children must be the appropriate age (3 or 4 years old) before September 1, 2012, and must be toilet trained.

No age exceptions will be made.

• Maximum student to instructor ratio of 10:1,

• Parents are required to bring a birth certificate, or an official document with the child’s birth date, a current schoolphotograph, and proof of residency at the time of registration. Appropriate immunizations are required for children toattend preschool. A Personal Information Form (PIF), including the child’s immunization records, photo and ParentHandbook confirmation letter must be completed and filed with the Park District prior to the child starting class.

• A $100 deposit is required at the time of registration. If the fee is not paid in full, the $100 deposit will be applied to thebalance. A $30 discount will be applied if paid in full. $25 of all deposits is a service fee that is non-refundable. No refunds willbe granted after January 30, 2013.

• If using the Park District payment plan, $100 must be paid at the time of registration. A second payment of 50% of the balanceis due by September 3, 2012. The third payment of 50% of the balance is due by February 3, 2013. All registrations afterFebruary 4, 2013 must be paid in full. Electronic Fund Transfer (EFT) is available for preschool payments.

General Notes: Group Care• Registration for the 2012-2013 school year will begin on June 23. Registration is taken on a first-come, first-servedbasis by transportation routes. At the time of registration, a one time $25 registration fee is required per family. This$25 is nonrefundable. Also, when registering, the first week of each program must be paid for (For example, if youregister before school starts the first three days that school begins in August must be paid in full, depending on thenumber of days you are registering for).

• On the first day your child attends the program, a payment schedule will be given to you. Payments will be taken bi-weeklyor monthly. A Personal Information Form (PIF), including the child’s immunization records, photo and Parent Handbookconfirmation letter must be completed and filed with the Park District one business day prior to the child starting class.

• Part-time enrollment is accepted for a minimum of two days per week. The number of days and which weekdays yourchild will be attending must be noted at the time of registration (5 days, 4 days, 3 days or 2 days).

• The Park District does not hold spots in any of the Group Care Programs, except for a medical leave, which is to bedetermined on a case by case basis. If your child is unable to attend the program for any reason, in order to hold thatspot payment must be continued.

• All registration must occur at least two full days prior to thechild’s first day to allow time to notify the school and change thebus schedule. All Saturday registrations will not be able to startuntil the Wednesday of the following school week.

Kindergarten students (afternoon only, unless in pre-K)-8th GradeThe Streamwood Park District recognizes the community’s need for an after school child care program. In order toaccommodate working parents, the District is committed to providing a quality after school program. Children aretransported from their school via the Park District van to the Community Center. The Afterschool Club runs the entireschool year and parallels School District U-46’s school schedule. The program is held only on school days.

Children participate in a variety of activities supervised by qualified staff. Dailysnacks are provided for the children. Weekly activities include crafts,quiet/homework time, games, sports, and Friday swimming at Park Place. Thestaff is flexible to meet the needs of the children. Parents are responsible forpicking up their children by 6 p.m. daily.

SPD Preschoolers and AM Kindergarten studentsCommitted to providing care for the working parents, we haveexpanded our services to accommodate the younger citizens ofStreamwood. In the tradition of quality care, we now have the Pre-K Kare Klub. Children will be transported, by park district van, fromtheir Streamwood Park District four year old Preschool program or theirKindergarten school (same schools that Afterschool Club services). Once at theCommunity Center, activities will include quiet/nap time, large motor skillactivities, educational games, arts and crafts and free play. At 3:30 p.m. participantswill have the option to be picked up by a parent or remain in the AfterschoolClub Program for an additional fee. The staff is flexible to meet the needs of the

children. Parents are responsible for packing a lunch for the Pre-K Kare Klub. Children remaining in Afterschool Clubwill be provided with a snack.

GENERAL INFORMATIONThe Streamwood Park District is proud to be a member of Northwest Special Recreation Association(NWSRA). Throughout the northwest suburbs, NWSRA is the leader in providing outstandingrecreational opportunities for children and adults with disabilities. NWSRA’s team of full-timeTherapeutic Recreation professionals and trained part-time staff offer a multitude of year-roundprogram offerings including sports, camping, social programs and much more. Having fun, being

included, making friends, learning new things and going new places are just a few of the benefits of NWSRA programs! Inclusionassistance is provided by NWSRA to those individuals who wish to participate in their home park district program. When registering,the parent of an individual with a disability should inform the park district (and NWSRA, if desired) of any accommodation needed inorder for the individual to successfully participate in the program. The park district staff will then contact NWSRA for assistance withthe accommodation. Special Leisure Services Foundation, the fundraising arm of NWSRA, partners with public and privatecorporations, foundations and individuals to support and promote outstanding recreational opportunities for children and adults withdisabilities. Throughout the year, the organization hosts a variety of fundraising events that help support accessible transportation,general programs, inclusion opportunities, the Kevin T. If youwalk your pet in or around a park, please be sure to pickup after it. Keeping your parks clean will enhance thebeauty of the park. We appreciate your cooperation.

Control of PetsPets, including dogs, cats, and other domesticatedanimals kept as pets, may be brought into any park, butonly when leashed in accordance with Village ofStreamwood ordinance(s), licensed as necessary, andcontrolled by the person bringing such pet into thepark. It is expressly prohibited for any pet, even thoughon a leash, to enter or be on a park playground.Defecation by any pet while on Park District propertyshall be immediately and completely removed by theperson in control of the pet. No pets shall be allowed inany Park District facility or building unless brought in fora Park District program or event involving such pet. Insuch case, the pet must be leashed, licensed andcontrolled as aforesaid. The provisions of this sectionshall not apply to service animals in the performance oftheir duties. All Streamwood Park District ordinances areenforced by Streamwood Police Department.

Smoking PolicyIn compliance with state and federal laws, theStreamwood Park District has set the following policy:No cigarette, pipe, or cigar smoking will be allowed inany of the Park District facilities. The policy includesPark District activities as well as private rentals. Yourcooperation is appreciated.

Child Car Seat RequirementAll parents and guardians of children participating in aPark District trip or program where they are beingtransported by a Park District vehicle, must provide achild restraint system for any child 7 years old or youngeror sign a waiver giving permission for the child to betransported without such system. All participants mustwear seat belts. For trips using a school bus, there areno restraints for adult passengers or children.

Photo DisclaimerRegistrants and participants permit the taking of photosand videos of themselves and their children during ParkDistrict activities for publication and use as the ParkDistrict deems necessary.

Emergency ShelterIn case of a disaster situation as identified by theEmergency Services and Disaster agency, Park Place willbe used as an emergency shelter. If this occurs, it maybe necessary for some programs to be postponed orcancelled.

Lightning Prediction SystemThe Streamwood Park District has installed threelightning prediction systems. One located on the roofof Park Place Family Recreation Center, the second atHoosier Grove Park, and the third at Rahlfs Woods Park.These systems gather atmospheric information toaccurately predict when conditions are conducive for alightning strike. When conditions are “right,” the warningwill sound (a single fifteen (15) second horn blast)followed by an orange strobe light located outside onthe roof at all locations and inside of the building at ParkPlace. At this point, participants in outdoor activitiesneed to cease all activity and seek shelter immediately.When the hazardous weather has passed, the “all clear”will sound (a series of three-5 second horn blasts), andthe orange strobe will turn off. Only at this point shouldoutdoor activities resume.

It is important to remember that even though the systemmay not sound when weather conditions arethreatening, you should exercise good judgment.Proceed to stop all outdoor activities and move to a safelocation until the dangerous conditions have passed.Horns from the Park Place location can be heard atDolphin Park baseball fields. These warning systems willbe active during the months of April through October.Further, the District will be checking the systemsmanually, on the first and third Tuesdays of every month(April-October) at 10:30 a.m. Please contactSuperintendent of Parks and Planning, Keith Gorczyca,at (630) 483-3161 for additional information.

Behavior PolicyAppropriate behavior is expected from all participants andobservers in programs and in facilities to protect theenjoyment and safety of all patrons. Behaviors that will notbe accepted include, but are not limited to, the following:

*Endangering the health and safety of other participants,patrons or staff.

• Disrupting a program or creating a disturbance at a facility.

• Continuous refusal to follow program and/or facility rulesand guidelines.

• Use of verbal harassment, profanity, vulgarity, obscenity orracial slurs.

• Blatant disrespect of staff and program or facility rules andguidelines.

• Damage, vandalism, or theft of private or Park Districtfacilities, equipment or supplies.

• Possession or use of illegal substances or medication,including the use of alcohol or smoking where prohibited.

• Inappropriate or offensive dress, including, but not limitedto dress which is too revealing and clothing whichdisplays obscene or offensive words or pictures.

• Possession or threat of a weapon.

• Gang activity including display or possession of symbols,hand signals, soliciting membership, intimidating orthreatening any individual, wearing or displaying colors oritems of dress, etc.

• Inappropriate display of affection, including but notlimited to prolonged kissing and touching of any privatearea of the body.

The following outlines steps that may be used with patronswho are not behaving appropriately. Membership or programfees will not be refunded for any person removed from aprogram or facility due to violation of this behavior policy.

Some programs or facilities may have stricter or more detailedbehavior policies.

Program ParticipantsAll program participants are expected to behaveappropriately and follow the rules of the program and facility.A participant will receive written notice for inappropriatebehavior, and in the case of a minor, a copy of the notice willbe provided to such minor’s parent/guardian. If a participantreceives three notices of inappropriate behavior, or if theparticipant is involved in one severe incident, as determinedby District staff, the participant may no longer participate inthe program until a meeting is scheduled and held. Theparticipant, a parent/guardian if the participant is a minor,the program instructor, the program supervisor and any otherpersons whom the District considers necessary for aresolution of the inappropriate behavior, shall be present atsuch meeting. At the meeting, the question of continuedparticipation in the program will be considered and,primarily for minor participants, a Behavior Contract may beprepared as a condition of continued participation. Such aBehavior Contract will be personalized to the circumstancesand the participant, and will include specific expectationsand consequences for failure to meet such expectations,including possible suspension and/or termination fromprogram participation. Inappropriate behavior by adults mayresult in more rapid progress toward suspension ortermination from program participation.

FacilitiesAll facility participants are expected to follow the rules ofthe facilities. Specialty areas of the facilities (example: fitnesscenter and pools) have posted signs stating special rulesspecific to that area. Disregard or abuse of facility rules mayresult in removal by staff or police from the facility for adesignated length of time or permanently.

ObserversObservers of programs are also expected to behaveappropriately. Parents, friends and any observers are held tothe same standards of behavior as participants. Ifinappropriate behavior of a program observer is reported toor observed by a staff person, the observer will be asked toleave by the staff or police, if necessary.

AppealsAny participant who is suspended or terminated fromprogram participation shall have the right to appeal to theExecutive Director within fourteen days of any notice ofsuspension or termination. The Executive Director shallthereupon schedule a meeting concerning such appeal, andthe appellant shall be entitled to present his or her versionof the facts at such meeting. Within five days after suchmeetings, the Executive Director shall notify the appellantin writing that the suspension or termination has beenaffirmed, modified, or reversed. The Executive Director shallnotify the Board of Park Commissioners of the meeting andthe decision.

It is a vision of the Streamwood Park District to providesafe and wholesome programs in well maintained facilitiesfor our residents and guests. Rules must be followed andpolicies such as this must be established to protect thisvision for all involved. Our goal is not to remove patronsfrom programs or facilities, but to work together withparticipants and/or parents/guardians to the best of ourability to correct or deal with a problem. The StreamwoodPark District asks for your cooperation in behavingappropriately and also teaching your children theimportance of appropriate behavior and following rules.

Resident Fees: A “resident” is defined as anyone livingwith in the geographic boundaries of the StreamwoodPark District. Residents of the Streamwood Park Districthave priority when registering for most programs.

Nonresident Fees: Nonresidents are charged 50% morethan the resident fee listed, not to exceed $25 for mostclasses. This does not apply for leagues, trips,memberships or other programs as deemed necessary.

Business Rate: All people that work at a business withinthe Streamwood Park District boundaries may receiveresident rates on all programs, services, andmemberships (proper identification required along withcurrent pay stub, program participants will be randomlysorted in the nonresident lottery).

Refund PolicyTo request a refund on a program, a Refund RequestForm must be completed and submitted at theRegistration Counter at either center. Administrativeapproval must be received before a refund will beissued.

1. A full refund will be given when the Park Districtcancels a trip/program or a refund is requested as aresult of being placed on a waiting list.

2. A refund minus a service charge will be given whena refund request is received a minimum of four daysbefore the start of the registered program.

3. A prorated refund with a service charge may begranted when a refund request is received with less thanfour days before the start or after the start of theregistered program and the reason is approved.Documentation may be requested with your refundrequest in order to be approved.

The prorated refund will be based on the date therequest was received, not the last attended class. Arefund will not be approved if the session is more thanhalf over. No refunds will be issued for individuallymissed or forgotten classes.

4. No refund requests will be granted that are made morethan two (2) business days after a one day program orspecial event.

5. Refunds for trips will not be given unless one of thefollowing occurs:

A. Park District cancels the trip. B. A replacement is found to take the trip.

6. Deposits for certain programs and trips are identifiedas nonrefundable and therefore the refund policy doesnot apply.

7. The cost of purchased individual supplies orequipment such as t-shirts, uniforms, trophies, costumes,etc. will be deducted from your refund. Noreimbursements will be given for required equipmentor supply purchases made by the participant or parent.

8. Please allow 3-4 weeks for your refund to beprocessed. If originally paid by check or cash, a checkwill be mailed to you. If paid by VISA, Master Card, orDiscover Card, we must credit the card with whichpayment was made.

8. Any programs with special policies void thepreceding.

Service Charge PolicyAll refunds issued which are not a result of a Park Districtcancellation will be assessed a service charge. Theservice charge for programs with fees of $10 or morewill be $5; programs with fees less than $10 will becharged 50% of the program fee.

Class CancellationsEvery effort is made to avoid canceling classes.However, a minimum number of participants must beregistered. Notification of class cancellation may bemade with as little as 24-hour notice. In cases where aprogram is canceled, you will be notified prior to thestart of the program and a full refund will be issued.

Transfer Policy1. All transfer requests will be honored if there areopenings in the class requesting to be transferred to andthe participant is the appropriate age for the program.

2. If additional payment is due, this must be made at thetime of the transfer request. For refund see #7 underRefund Policy.

Program ScholarshipsThe Park District recognizes that there are people in the

community who are often unable to participate inrecreation programs due to economic hardships. If anyresident individual or family would like to apply for ascholarship, pick up an application packet at Park PlaceRegistration Office or call the Registration Counter, (630)483-3025, or Debbie Karner, Superintendent ofRecreation at (630) 483-3012 to have one mailed. Allrequests will remain confidential.

Important Information regarding Waiver and ReleaseThe Streamwood Park District is committed toconducting its recreation programs and activities in asafe manner and holds the safety of participants in highregard. The Streamwood Park District continually strivesto reduce such risks and insists that all participants andparents/guardians of minors registering forprograms/activities must recognize that there is aninherent risk of injury when choosing to participate.

You are solely responsible for determining if you or yourminor child/ward are physically fit and/or skilled for theactivities contemplated. It is always advisable, especiallyif the participant is pregnant, disabled in any way orrecently suffered an illness, injury, or impairment, toconsult a physician before undertaking any physicalactivity.

Warning of RiskRecreational activities/programs are intended tochallenge and engage the physical, mental and emotionalresources of each participant. Despite careful andproper preparation, instruction, medical advice,conditioning and equipment, there is still a risk ofserious injury when participating in any recreationalactivity/program. Understandably, not all hazards anddangers can be foreseen. Depending on the particularactivity, participants must understand that certain risks,dangers and injuries due to inclement weather, slipping,falling, poor skill level or conditioning, carelessness,horseplay, unsportsmanlike conduct, premises defects,inadequate or defective equipment, inadequatesupervision, instruction or officiating, and all othercircumstances inherent to indoor and outdoorrecreational activities/programs exist. In this regard, itmust be recognized that it is impossible for theStreamwood Park District to guarantee absolute safety.

REGISTRATION INFORMATION

REGISTER EARLY - CLASSES AND PROGRAMS FILL QUICKLY!

IMPORTANT : Please read as our Procedures were recently updated to serve you better!Registration for all of our programs and classes is on a first come, first served basis.

Proof of residency is required (state IDs or driver’s license) every time you register for a program. Participants will becharged nonresident fees without appropriate proof of residency. MasterCard, VISA, and Discover Card, as well as cash,personal checks and money orders are acceptable forms of payment. Post dated checks will not be accepted.

For your convenience we have five easy ways for you to register: Walk-in, drop-off, fax, mail-in, and phone.

Walk-in:Walk-in registration will be taken on a first come,first served basis. Simply fill out the registration formcompletely, bring in proof of residency, along withpayment. The Clerk will confirm the status of yourregistration and it will be processed that same business day.

Mail-in: Complete your program registration form, alongwith a copy of your proof of residency and payment, andmail to the Streamwood Park District Registration Office,550 S. Park Blvd., Streamwood, IL 60107-2000. This methodof registration is best with check or money order paymentmethods; if there is a waiver on file, the Clerk will contactyou to complete payment information if by credit card. Donot send credit card information through the mail. Oncethe application has been processed, the registration clerkwill contact you by phone to confirm your registrationstatus.

Drop-off: Complete your program registration form, alongwith a copy of your proof of residency and payment. Ifpaying by credit card you will be contacted for your creditcard information the next business day. The drop box islocated inside Park Place Family Recreation Center at thefront counter for your convenience. You will be notified ofclass status by phone.

Fax: Complete your program registration form and fax withproof of residency to (630) 483-0562. You will becontacted on the next business day for your credit cardinformation. Do not record any credit card numbers on thefaxed form!

Phone: A “Waiver on File” form, along with proof ofresidency, must be on file at Registration prior to using thisregistration option. Once on file, you can simply call theRegistration office and process registration using a creditcard. If a Clerk is not available when you call, please leaveonly your phone number, name and class number desired;a Clerk will return the call to retrieve credit cardinformation; do not leave credit card information in yourmessage. (Note: due to additional forms that are needed for certainclasses/programs, phone registration has some exclusions. Please callthe office for more information.)

For Your Protection: At times when we are hand-enteringcredit cards, not swiping the actual card through themachine, the credit card company requires additionalinformation to process your purchase. We may need tocontact you to verify or receive additional information.Please understand that this procedure is for your

protection and security.

Lightning is a severe hazard that must beviewed seriously! Park users should stop andseek shelter any time they believe lightningthreatens them, even if a signal has not NOTsounded!

Park users will be warned by the Thor-GuardLightning Prediction System, sounding ONEprolonged blast of the horn, signaling a StrongRisk for Lightning in the area. The strobesignals a Continuos Lightning Alert condition.All should seek immediate shelter!

Three short blasts signal it is safe to return tothe area.

Vandalism Hotline Public playgrounds have become the targets of arsonists and when a loss occurs, an entire playground worth tens of thousands of dollars can be lost to thechildren of the community within a matter of minutes. Other park amenities such as shelters have also been damaged or destroyed. Park staff also spendshours removing graffiti on park structures and playgrounds. As much as staff and local law enforcement officers try to prevent these incidents, sometimes localresidents and park users are the best source of information about who may be involved in these acts.
