Steps to enter to Japan under the Covid-19 infection

Step 1: While you are at your hometown

1. Submit the Agreement form to University

• Contact the email address below to request the Written Pledge.Email address: sfc-visa@adst.keio.ac.jp

• You will receive this agreement form from the office. Submit this form and the office will issue the Written Pledge if there is no problem with the agreement response.

2. Obtain your Student Visa• Submit the necessary documents with the Written Pledge to the

Japanese Embassy/Consulate in your country. • The necessary documents are listed as below;

・Visa Application form・Passport・COE・Letter of Acceptance・Written Pledge (2 copies)

*Please confirm with the Japanese Embassy/Consulate in your country before you visit there.

3. Monitoring your health condition for the 14 days before entering Japan

• Take your temperature for the 14 days before entering Japan.

• If you show any symptoms of COVID-19 including a fever, respiratory symptoms, or physical weariness, you will cancel your trip to Japan.

4. Book your flight ticket

• Please select the flight arrives in the morning in Japan.

• It takes a while to have the PCR test result. If you arrive in the morning, there will be higher possibility to enter Japan within the day you arrive.

5. Report your arrival information• Once you book your flight, report your arrival

information to university through the following Google form.

• If you are the student dormitory resident under the contract with Keio, please also fill in the following form AND contact the dorm managing company directly to inform your arrival date and move-in date.

• If you made a student dormitory or apartment contract by yourself, make sure to contact the dorm managing company/ real estate agent and inform your arrival date and your confirmed move-in date to the dormitory/apartment.

6. Obtain the Certificate of Testing for COVID-19• You can download the designated format for

the certificate here http://www.mofa.go.jp/files/100078393.docx

• You will undergo tests relating to COVID-19 within 72 hours of your departure from the country at a local medical institution.

• You must use this designate format and also have to receive your doctor’s signature or seal.

• You need to submit this certificate at the airport in Japan.

• From Nov. 1, 2020. it is unnecessary to obtain this certificate if you are from the designated country/area.https://www.mofa.go.jp/ca/fna/page4e_001053.html

7. Enroll in private medical insurance• Please enroll in private medical

insurance (including travel insurance that covers medical expenses during your period of 14 days quarantine at least if you will enroll NHI) .

• Prepare the proof document that you enrolled in the medical insurance.

• If no insurance covers your 14 days quarantine, you won’t be able to enter Japan.

8. Install the App “COCOA” to your smartphone

• Install “COCOA (COVID-19 Contact-Confirming Application) ” to your smartphone.

• Your smartphone will be checked whether the application is installed or not.

9. Install the App “Map” to your smartphone• Install the application of “Map” to your

smartphone.

• You will begin recording and saving location information using the map application function, etc., on the smartphone you are carrying from the time of entering Japan and will continue to do so for 14 days after entering Japan.

• Your smartphone will be checked whether the application is installed or not.

*You have to have ☑ for all of them to be ready to leave your hometown.

Step 2: While you are on the airplane

1. Fill out the Questionnaire• On the airplane, the Questionnaire

will be handed to you.

• Fill out your university information in the column of “Contact Address in Japan”.

• Ask CA if you have questions how to fill it our.

Step 3: What to do right after your arrival in Japan

1. PCR Test• Take the PCR Test at the airport

• It takes about 2 hours to 2 days for the result.

• If you cannot receive the result on the day of arrival, you will stay at the designated hotel by the government.

2. Submit the necessary documents to the quarantine station at the airport

• Written Pledge→you received from the University

• Certificate of Testing for COVID-19→you received from your doctor

• Questionnaire→you received on the airplane

3. Immigration → Luggage pick up• After you receive the PCR test

result, you go to the Immigration.

• After the Immigration, go to the luggage claim to pick up your luggage.

4. Commute from the airport to your accommodation

• You cannot use any public transportation such as train, bus, taxi and so on.

• You will limit your means of transportation to one of the following for the 14 days after entering Japan:-Own car-Vehicle owned by the host company/organization-Rental car-Hired driver service (do not take a regular/airport taxi;

book a private driver service in advance.)

If you don’t know where to arrange a hired driver by yourself, one of the travel agencies in Japan (JTB Corp.) will support you. All of the expenses will be on you.

5. 14 days quarantine• Do not go out from the place you stay for 14 days counting

from the next day of your entry in Japan.

• Do not use the shared space and make sure to reserve your own room, bath, and toilet such as Hotel, weekly apartment.

• Refrain from using restaurants.

6. Monitoring your health condition for the 14 days• You need to monitor your health condition for 14 days.

• If you cannot report your health condition to the Japanese government, you must report it to the University office. (The University will report it to the Japanese government on your behalf. Contact email address: sfc-visa@adst.keio.ac.jp)

• Make sure to bring thermometer to use it for 14 days.

7. Submit the health check form to SFC

• After 15 days of your entry in Japan, you submit the health check form to SFC health center and wait for their response.

• After you receive their permission, you can enter to the University campus.
