The planning Process

Data received from parents and community members via

surveys and family health forums were analyzed to determine

the health needs/priorities of the population

Aimed at creating awareness about healthy lifestyles,

improving health status and behavior change

Selected evidenced-based curricula that are tailored to

individuals with IDD

Implemented 10-Week Nutrition/Wellness Community-

Based programs with reliable pre/post tests to

measure health progress

Determined the effectiveness of the program by

Lessons Learned

7

• It takes the entire ecological

framework to impact change

• Parents and caregiver

involvement is key to

participants’ success

• Multi-tiered interventions

appear to be most effective in

ensuring behavior change

• We are making strides at the

individual, interpersonal, and

organizational levels. We have yet to

gain widespread support at the

community and government levels

Continuing to build community

outreach efforts

• HA/HC initiatives should be used as

a catalyst for gaining continued

support for policy changes in the

ID/DD population
