HIDROCLOROTIAZIDA: EL USO CONTINUO Y PROLONGADO EN EL TIEMPO PODRÍA AUMENTAR EL
RIESGO DE CÁNCER CUTÁNEO NO MELANOCÍTICO

Los resultados de dos estudios epidemiológicos realizados en Dinamarca indican un posible aumento del riesgo de desarrollo de carcinoma basocelular y espinocelular en pacientes sometidos a tratamientos continuados y prolongados con hidroclorotiazida, que podría explicarse por su acción fototóxica.
Se recomienda:
• Reconsiderar el uso de hidroclorotiazida en pacientes con antecedentes de cáncer de piel no melanocítico.
• Vigilar la presencia de alteraciones cutáneas en pacientes que reciben tratamientos prolongados con hidroclorotiazida e informarles de las precauciones a adoptar en relación con la exposición solar.
La hidroclorotiazida es una tiazida diurética indicada para el tratamiento de hipertensión arterial; edema asociado a insuficiencia cardiaca, renal o hepática; diabetes insípida e hipercalciuria idiopática.
En España, se encuentra comercializada bajo un amplio número de nombres comerciales. Se puede consultar la lista completa de los mismos a través del Centro de Información online de medicamentos (CIMA) de la Agencia Española de Medicamentos y Productos Sanitarios.
El Comité para la Evaluación de Riesgos en Farmacovigilancia europeo (PRAC) ha finalizado recientemente una evaluación de los riesgos de la hidroclorotiazida iniciada tras conocerse los resultados de dos estudios epidemiológicos1,2 llevados a cabo con datos procedentes de bases de datos danesas, que mostraban una asociación entre la administración de este diurético y un incremento de riesgo de cáncer cutáneo no melanocítico (carcinoma basocelular y carcinoma espinocelular). El riesgo observado en los estudios aumentaba con la dosis total de hidroclorotiazida acumulada desde el inicio del tratamiento.
La AEMPS recomienda a los profesionales sanitarios:
-Reconsiderar el uso de hidroclorotiazida en pacientes con antecedentes de cáncer de piel no
melanocítico.
-Solicitar valoración especializada en caso de lesiones cutáneas con sospecha de malignidad.
-Informar a los pacientes en tratamiento con hidroclorotiazida acerca de este posible aumento de
riesgo tras tratamientos prolongados.
-La necesidad de limitar la exposición excesiva al sol y/o rayos ultravioleta, usar fotoprotección
solar adecuada, no otros fármacos fotosensibilizadores.
-La conveniencia de revisar periódicamente la piel y de consultar con un médico en caso de que
aparezcan lesiones cutáneas sospechosas (o de que cambien de aspecto las ya existentes).
Nota informativa de La Agencia Española de Medicamentos y Productos Sanitarios (AEMPS)
emitida el pasado 9 de octubre de 2018; basada en dos estudios epidemiológicos realizados
por el mismo grupo danés; advierte de que el uso continuado de HCTZ puede incrementar la
frecuencia de aparición de cáncer de piel no melanocítico.
La hidroclorotiazida es el diurético más utilizado como antihipertensivo. Aparece asociado a
otros fármacos en numerosos medicamentos para el tratamiento de la hipertensión arterial; es
necesario hacer una llamada a la prudencia respecto de la interpretación de la citada nota
informativa de la AEMPS.
SEHLELHA ante esta información reitera la necesidad de tratar de forma adecuada a los
pacientes hipertensos según las recomendaciones de las guías europeas, recientemente
actualizadas.
También recuerda que en muchos casos es necesario tratar con combinaciones de fármacos,
que con frecuencia incluyen HCTZ.
Mientras no se disponga de más información, SEHLELHA recomienda que los pacientes que
duden sobre la conveniencia de continuar su tratamiento con HCTZ consulten con sus médicos
y mientras tanto no suspendan la toma del fármaco. También recuerda que si en algunos
pacientes concretos fuera necesario suspender el uso de HCTZ; existen suficientes alternativas
farmacológicas para un tratamiento eficaz de la hipertensión arterial.

RETIRADA DEL MERCADO DE ALGUNOS LOTES DE MEDICAMENTOS QUE CONTIENEN VALSARTÁN

La Agencia Española de Medicamentos y Productos Sanitarios informa que se ha detectado la aparición de una impureza probablemente carcinogénica (según la clasificación de la IARC de la OMS)(1) en determinados medicamentos que contienen valsartán. La AEMPS ha ordenado en aplicación del principio de precaución la retirada de los medicamentos.
Para facilitar la gestión de esta alerta el Ministerio de Sanidad, Consumo y Bienestar Social ha firmado un protocolo con el Consejo General de Colegios de Farmacéuticos y los 17 laboratorios afectados.(3)
La Agencia Española de Medicamentos y Productos Sanitarios (en adelante, AEMPS) informa acerca de la detección de N-Nitrosodimetilamina
(NDMA)φ en el principio activo valsartán fabricado por Zhejing Huahai Pharmaceutical Co. (Ltd (BS 1) Channan Site, RC-317016 (China). Dicha impureza se ha generado como consecuencia de un cambio en el proceso de fabricación de valsartán autorizado por el EDQM (siglas en ingles de European Directorate for the Quality of Medicines & HealthCare, Dirección Europea de la Calidad de los Medicamentos) del Consejo de Europa.
Esta alerta afecta a múltiples países a nivel europeo y mundial. En el caso de España afecta a varias presentaciones de medicamentos que incluyen valsartán producido por ese fabricante y que ha sido distribuido en numerosos países. En España, tras recibir la correspondiente investigación se ha procedido a ordenar la retirada de los medicamentos afectados (ver anexo).

RETIRADA DE LOTES DE IRBESARTÁN

La Agencia Española de Medicamentos y Productos Sanitarios informa de la retirada de lotes de medicamentos que contienen como principio activo irbesartán.
Las recomendaciones para la sustitución de los medicamentos son también de aplicación en este caso, pudiendo acudir los pacientes a la farmacia con el envase afectado para su reemplazo.5
En el marco de los controles adicionales puestos en marcha para todos los medicamentos del grupo denominado de los “sartanes”1, se ha detectado la presencia de N-nitrosaminas2 en determinados lotes del principio activo irbesartán fabricado por Zhejiang Huahai Pharmaceutical Co. (China), utilizados en la fabricación de medicamentos comercializados en la Unión Europea.
Por ello, en España, la Agencia Española de Medicamentos y Productos Sanitarios (AEMPS) ha ordenado la retirada del mercado de lotes de medicamentos de dos titulares de autorización de comercialización, que se relacionan en el anexo 1 de esta nota informativa.
La información sobre los lotes retirados se puede consultar además en la sección de Alertas de medicamentos de uso humano3 de la web de la AEMPS.
Las recomendaciones para la sustitución de los medicamentos acordadas en julio del año pasado4, son de aplicación para este nuevo principio activo afectado, pudiendo acudir los pacientes a la farmacia con el envase afectado para su reemplazo (ver protocolo de gestión del Ministerio de Sanidad, Consumo Bienestar Social para la retirada de lotes de irbesartán)5.

La Agencia Europea de Medicamentos (EMA) y las autoridades nacionales siguen trabajando en la investigación de los hechos y se informará de las novedades al respecto.

Olmesartán es un antagonista del sistema renina angiotensina (ARAII) que ha demostrado ser eficaz en disminuir la
presión arterial (PA) pero no se ha establecido su eficacia en la disminución de la morbimortalidad cardiovascular, a
diferencia de otros ARA II.
Desde el 2 de enero de 2017 Olmesartán se ha excluido de la financiación en Francia debido a:
- La falta de pruebas de la eficacia sobre los eventos cardiovasculares (infarto de miocardio y accidente
cerebrovascular) o la mortalidad.
- Al riesgo de enteropatía, muy rara pero grave, específicamente relacionada con su uso, que no se ha observado con
otros ARA II.
Las autoridades sanitarias francesas (HAS) recomiendan valorar a los pacientes y cambiar a otro tratamiento
antihipertensivo por el incremento del riesgo de enteropatía similar a esprue, reacción adversa muy rara (menos de
1/10.000), pero grave. De acuerdo con los datos de farmacovigilancia no aparece con otros ARA II ni con IECA, por lo
que no se puede concluir que sea un efecto de clase común a todos los ARA II.
La enteropatía por olmesartán cursa con diarrea crónica, con pérdida de peso importante, vómitos, deshidratación y,
a veces, con insuficiencia renal funcional e hipopotasemia, que puede conducir a una hospitalización prolongada.
Puede aparecer meses o años después del inicio del tratamiento, lo que dificulta su diagnóstico. Los síntomas
desaparecen al suspender el tratamiento y recurren si se vuelve a tomar olmesartán.

Background: Previous studies have shown reduced cardiovascular (CV) risk with
increasing high-density lipoprotein cholesterol (HDL-C) levels. However, at elevated
HDL-C levels (>60mg/dl), its atheroprotective functions, such as cholesterol
efflux and anti-oxidant capacity, may be impaired. The association between
high levels of HDL-C and adverse outcomes remains unclear.
Purpose: To study the relationship between elevated HDL-C levels (>60mg/dl)
and adverse CV outcomes in an at-risk population.
Methods: Participants included 5,965 individuals (mean age 63.3―12.4 years,
35% female, 23% African American) Individuals were also stratified by HDL-C categories
Conclusion: Elevated HDL-C levels are paradoxically associated with an increased risk of adverse CV events in
an at-risk population, suggesting dysfunctional HDL and impaired atheroprotection.
