Urinary incontinence (UI) is the involuntary loss of urine. It happens to men and women of all ages. Urinary incontinence affects an estimated 25 million people in the United States and 200 million people worldwide. It is estimated that 50 percent of older women and 15 percent of older men suffer from this problem. UI is as prevalent as diabetes and hypertension. Yet, unfortunately, it is a silent condition. Most of the time patients are too embarrassed to tell their doctor or (you may have guessed it as the title states) they feel it is a normal part of aging. Patients often think there is nothing they can do about it and so they suffer in silence. Fortunately, this is not true! There are different types of UI which require different treatments. Some patients may also have a combination of more than one type, known as mixed incontinence. The three main types of Urinary Incontinence are as follows: u Stress incontinence is leakage of urine that occurs during physical “stress” activities such as exercising, sneezing or coughing. These activities can increase pressure within the bladder and weaken the urethral pressure. v Urge incontinence is when one gets a strong urge to urinate and is unable to hold it till one reaches the restroom. w Overflow incontinence is when the bladder overfills and it cannot hold any more and the urine “spills over.” If you experience bladder leakage – you should be confident and talk to your doctor about it to learn what treatments are available. Treatments can vary depending on the type of problem. Often changing voiding and dietary habits may improve the problem. Sometimes medication or physical therapy may be recommended. Other times, highly successful, minimally invasive surgical procedures can provide long lasting treatment. So you are aware, here are some interesting statistics gathered from the National Association for Continence (NAFC): u More than half of all residents in nursing homes suffer from incontinence. v Stress incontinence affects approximately 15 million U.S. adult women. w An estimated 10-25 percent of patients will still experience bladder control issues after undergoing radical prostatectomy surgery or prostate cancer surgery. x Urinary incontinence is a common risk factor for falling, and in people ages 65 and older, falls are the leading cause of injury and/or death. y Women wait an average of 6.5 years before seeking professional help for their incontinence problems. The pathway we all need to follow when we are nearing the Medicare age of 65 is not always a

smooth one. The Medicare Maze requires us to make a number of decisions, some of which we may not be able to reverse in the future. Be careful in selecting your Plan as you, and only you, will need to be satisfi ed with the Plan you select for many years. Ask questions from many sources, don’t rely on just one source.

Let’s discuss the various options in navigating this maze.

Medicare eligibility is based upon your age of 65 and/or certain health conditions, and has four main parts, A, B, C, and D, each covering specifi c medical services. Let’s briefl y discuss the “Alphabet Soup” called Medicare. Original Medicare (OM), Part “A” (Hospital Insurance) helps cover hospital inpatient, in addition to some skilled nursing facilities, hospice, and home health care. If you have worked and paid taxes for at least 40 quarters, Part “A” premium is no cost to you. In 2017, Part “A” had a deductible of $1,260, plus additional amounts for long hospital stays. OM Part “B” (Medical Insurance) helps cover doctor visits, outpatient and home health care, durable medical equipment and some preventable services. It has a 2017 Annual Deductible of $147 and a monthly premium of $104.90. Part “C,” known as Medicare Advantage (MA), started in 1997, is administered by federally approved private insurers and includes all benefi ts and services covered under Parts “A” and “B.” Most MA plans include coverage for

prescription drugs. Part “D” (Prescription Drug Plans) helps cover the costs of only prescription drugs.

To cover medical costs not fully covered by OM, you may need to purchase a Medigap Policy, which helps cover most of these “gaps.” To purchase a Medigap policy, you must be covered by OM. There are 10 approved standardized Medigap policies. These

policies, approved by the federal government, are issued by insurance companies. These companies charge premiums for the policies that cover these gaps. The 10 standardized plans provide the same 10 levels of coverages, no matter which insurance sells you a policy. Medigap policies do not include coverages for long term care, vision care, dental care, hearing aids, eyeglasses, or private-duty nursing.

Also, OM does not cover prescription drugs. For prescription drug coverage, a Part “D” plan would need to be purchased from an

insurer marketing Part “D” coverages. Part “D” plans are approved by the federal government and marketed by various insurance companies.

What is a plant-based diet?It is typically defi ned as a “whole-foods, low-fat, plant-based diet” that includes vegetables, fruits, whole grains, legumes, seeds and nuts (in small amounts) with the total absence of meat and dairy.1 Foods included in the plant-based diet are nutrient dense and low in calories which allows for no restrictions on portion sizes of vegetables, fruits, whole grains and legumes. However, it does restrict nuts, seeds and avocados to small portions, defi ning the diet at low-fat. Omega-3 fatty acid rich foods such as ground fl ax seed and chia seeds are unlimited. Overall, plant-based diet guidelines recommend consuming 10% of calories from fat, 15-20% of calories from protein, and 70-75% from carbohydrates.2

How is this different than a vegan diet?The vegan diet excludes all animal products such as meat, seafood, and dairy with no specifi c guidelines on avoiding or limiting processed foods with excess sugar or fat (for example Oreo’s are vegan).

Why should we consider plant-based eating?Heart disease is the number one killer in America. Take a look at other countries around the world with low rates of cardiovascular disease, they are consuming mostly plant-based diets. A study from the Lifestyle Heart Trial showed that 82% of the patients diagnosed with heart disease who followed a plant-based diet had some level of regression of atherosclerosis.2

If it is proven to reduce risk of heart disease, what about diabetes? In a study comparing a low-fat vegan diet with the American Diabetes Association diet guidelines, 43% of the participants following the vegan diet reduced their diabetes medications compared to only 26% of the ADA participants. The vegan diet participants also saw a greater decrease in their HbA1C (an average measure of blood glucose over a 3 month period).3

How do I take steps forward plant-based eating?Maybe you are not ready to jump into total plant-based fanfare, but you would like to incorporate more plant

based meals into your diet. First, think about the meatless meals you are already making, like a lentil soup or vegetarian lasagna and make adjustments to include whole grains.

If you plan to go full speed ahead with plant based eating one health care system recommends the following:

• Start your meal with a soup or salad

• Fill half of your plate with legumes/whole grains/starchy vegetables + half with non-starchy vegetables

• Finish the meal with a piece of fresh fruit1

The Bottom Line:Increasing plant foods and substituting plant protein for animal protein is associated with improved cardiovascular risk factors and benefi cial to your overall health.

As always, talk to a medical professional before making changes to your diet if you have a chronic disease and/or medical condition. If eating a strict plant-based diet, you may need to supplement vitamin B12, calcium, vitamin D and iron as these are predominantly found in animal products.
