The enrollment form must be submitted to MassMutual. The MassMutual Participant Information Center and Retirement Access give you two easy ways to enroll in the retirement program. All you need to do is complete this worksheet and then call or log in to enroll. You must complete the entire enrollment process either by phone or by Internet for your enrollment to become effective. A reference number will be provided to you, once you complete the enrollment process. The PPD Plan uses monthly enrollment eligibility therefore if you enroll in the plan by the 15th of the month, your enrollment will be effective on the first paycheck of the following month. Please note, during the time you enroll in the plan, your status will remain as "Eligible" and your account online will read as pending enrollment. Once your first contribution is posted, your account will become "Active" and you will then be able to view your balances and transactions online. Worksheet Instructions

Once you have called Participant Information Center or have accessed the Retirement Access website as indicated on the preceding page, follow the instructions in this document.

To enroll, you must:

• Change your contribution percentage from zero, and

• Select your investment options.

1 Identify Yourself

Enter Your SSN:

Enter Your Pin # (MMDDYYYY) of birth:

Choose a New PIN or Internet password (This information is confidential and should not be shared with anyone else for security reasons)

(Note: Separate Phone PIN and Internet Password are required for maximum security)

2 Choose your Contribution AmountEnter the percentage (%) of your payroll that you want to contribute to the Plan. Election percentage must be a whole number.

Pre-tax: _______________________% (between 1% and 50%*)

After-tax Roth: _______________________% (between 1% and 50%)

* New hires, and eligible employees who earned less than $120,000 in 2017, may contribute from 1% to 50% of pay (in whole percentage increments) not to exceed the Federal limit of $18,500 for calendar year 2018.

Eligible employees who earned more than $120,000 in 2017 are considered to be highly-compensated employees and are limited to a 13% maximum annual contribution for calendar year 2018.

* For employees who are age 50 or older in 2018, you can contribute up to 50% of pay, up to $24,500 (the IRS maximum of $18,500 plus the 2018 Catch-up contribution amount of $6,000). Highly-compensated employees who are age 50 or older in 2018, can also contribute up to $24,500, however, if you are limited to the 13% maximum contribution rate, please complete a Catch-up Contribution Form.

3 Choose your Investment ElectionsYou will be prompted to enter your investment choices. Use the worksheet below as your guide, and follow the prompts to enter your desired contribution percentages in whole numbers for each investment option. If you do not wish to select a particular investment option, enter “0”%. Please note your percentage needs to add up to 100% (you may choose to contribute on both a pre-tax and after-tax Roth basis).

• Continue this process until you have made all of your investment elections.
