Victim at his Fault, there are four nos of people and they have alloted to install lamp fitting work in

entarance area of thair shed, C 1 257 phase 1 naroda gidc. 11 KV Gidc 7 and Bhagavati feeders are passing through near thair entarance area of shed at distance of 13 feet. During installation of lamp fitting they have

used 20 feet long ms ladder. Victim and other 3 person are handling that ladder and comes in contact with 11

KV GIDC 7 hence transient current experience by them. Victim at his fault due to lose of control to handling of ladder. Its leads to one Fatal human

accident to Mr. Sanjay Kanubhai Shah and Non Fatal human accident haapen to other three. 1) MAKAVANA

touched to the detoriated pole’s inside iron part & leakage of current caused the fatal accident when it

On Dated 7/5/2016 at 15:45 a Non Fatal electrical accident was occurred to shri Maheshbhai Somabhai

Barot at Collector's new bungalow garden.The message was not received on same day but I Knew

about accident in Divya Bhaskar news paper Next day on dated 8/5/2016.I have personally visited site &

found that a 11kv polytechnic feeder is passing from collecor bungalow garden.On this pole a 4.5 feet angle

is fixed.The 11kv line jumper was passing near this angle very closed to angle.The distance was only 6 to

7 inch.A daily wages worker goes to near pole for watering the plant in garden.The land near this shackle pole was wet meanwhile a bird sit on the jumper hence

the jumper may touch the angle or angle came in induction zone .The daily wages worker having without

footwear walking on wet land near RSJ pole & got electric shock.He got injury on left leg hill.It was minor

injury.The eye witness the watchman of collector bungalow has seen the worker crying.he came near to

worker & admitted him to hospital.No any transient tripping was noted at that time in S/S.

As per Police Station report on dated 16.5.16 DE of visnagar city sub-division visited the site on dated

16.5.16. During investigation it was came to know that victim has erected iron pipe for marriage mandap in front of house of Vinaji Chhanaji Thakor and then for

fitting of cloth roof he climbed on Vinaji Chhanaji Thakor's iron roof shed & immediately shouted and fallen on the ground. Hence he was shifted to civil

hospital where doctor declared him dead. As per site inspection and visit there is no any evidence for

electric shock found. Also in police inquest panchnama & PM report there is no any evidence & spot of electric shopck found on the body of victim though cause of accident due to electric shock was

The deputy engineer of Patan Rural sub division has visited the accident site & found that at villge Badipur

near the RCC road a nacked galvanise telephone pole is standing nearer to 3-ph 4-Wire LT line PSC Pole. A one other extra iron wire is fitted between telephone

pole & PSC pole & PSC pole earthwire. The earth wire was fitted to stay clamp pair and grounded with rigid PVC pipe. 2 nos. of two core PVC Srevice wire was

laid for 1-ph connection with hanged jumper. The tape wrapped with the joint of jumper was opended &

touched with the shackle. Hence the leackage current has made leakage path up to telephone wire through

GI wire iron wire & Galvanise telephone pole to ground. On dated 11.07.2016 the the buffalo of shree

nor Shri Babubhai Revabhai Parmar . Hence total investigation was not carried out for leakage path. For this SDO Mansa has written letter to police department

A light fixture having two horizontal support is fitted/wounded with iron wire with LT pole. Both upper

and lower horizontal support of light fixture are wounded with iron wire with the LT pole. The upper

horizontal support of light fixture was tightly in contact with LT pole’s earth wire. The wire of light fixture is having punctured insulation which was open and it

was touching the upper horizontal light fixture support which in turn wounded with earth wire of LT pole. Thus

the leakage current passed through this punctured insulated wire to the earth wire of LT pole and

On dated 7.8.16, victim shri Keyurkumar Manharlala Raval was came to the premises of Thakor Kalaji Jagaji for repairing of main switch and fan wiring.

Due to faulty (short) Service Line (Twin Core) in which phase & neutral wire was short due to damaged insulation, the returned leakage current flow from faulty service wire to LT pole and then LT pole to

Transform centre via Neutral conductor which was earthed at transformer centre & due to wet land in the nearby area which was full of water because of heavy

rainfall the said leakage current flow in stay wire of service pole via wet land & the said stay set was

connected with the fencing which was made by third party for protection of his plot, the leakage current

flows in said fencing as well.Victim whose house was on the other side of plot

around which the fancing was done,has made arrangement on the back side of her house i.e. in

varanda, a folding door in open situation, the hook of the same rests on that private fencing. While opening that folding door & to keep the door open she tried to rest that hook on that fencing & she got electric shock.

The illigal construction of house of Mahendrabhai Becharbhai chenva was going on directly under 11KV Kunj(AG) feeder emanating from 132KV IDAR S/S.The victim was working on iron ladder of cement concrete mixture machin's bucket. The The mixture machine's lift bucket structure was erected by mistake very near

to 11 KV kunj(Ag ) feeder by the labour, which was only at a distance of 1.5 feet from live 11KV "R" phase

conductor.While working on iron ladder of mixture machine , the victim's head accidently came in contact

with live 11 KV Phase & victim fall down on ground ,which result in imediate death of victim .The burnt

marks found on forehead & leg of victim .There is no mistake/ foult of UGVCL. The victim him self & house owner Shri Mahendrabhai Becharbhai chenva both are

responcible for fatal accident occured to the victim .There was three phase power on 11 KV Kunj AG

feeder at the time of accident.There was slive of PVC pipe of aproximate 20 feet found around the all three

On dated 10.8.2016 the time about 2.00 AM there was heavy rainfall and cyclonic wind pressure at village vallavata. A tree branch fallen on live jgy Lt(3-phase 4 wire) line of 11kv dolpur feeder and among these the three phase conductors wires are short

circuited between each other and broke these three conductors, the broken conductors are snapped and

fallen on the ground and a buffallow came in the contact with one conductor and the broken lt line was

live and due to this live conductor the buffallow got shocked and died on the spot.The tree is about

horizontlly 8 feet away from this Lt line

mahadevpura(m) at about 3 AM due to climing on transformar center situated on 11 KV bhavsor urban

feeder. Also i visited site and it has come to know from victim's brother and other villagers that he was

suffering diesses of T.B. and also victim was mentally not fit. Immediately victim was admitted to civil hospital ahmedabad for further treatment but during treatment

he got dead on dated 9.3.2017

The victim has tied GI wire between two iron shade of elctric room and labour living room to dry the wet cloth, labour living room iron shade was supported with iron pipe and the elctric fan was hanged to the iron pipe by

use of GI wire with the hook of the fan and power supply to the fan was given by flexible copper

wire(Phase and neutral) from fuse of the elctric board,the elctric fan was internally short circuited and leakage current passed through iron pipe to entire iron

shade so the current also passed through GI wire which was tied between two iron shade of elctric room and labour living room meanwhile the victim had put his wet clothes on that GI wire for drying purpose at

that time he came in contact with elctrocuted GI wire in which leakage current was passing at that time victim

got electrocuted,
