The Planned Activity Groups provide social and skills based activities for older adults who are socially isolated. There are specific groups for dementia, and also for people who are frail, or have a disability. The program provides its participants with the chance to enjoy social activities, which assist in maintaining independence in the community, whilst at the same time offering carer support. Our centre based groups have a variety of activities to offer such as indoor bowling, cooking, gardening, board games, cards, current affairs and gentle exercises. The group has a number of themes through the year, and whether it is the season, or an event, there is always something going on. These centre based groups also go on occasional outings.

Cost is $10.00 for the day

Transport provided if required. Lunch and beverages also provided.

Do you care for a disabled person or someone who is suffering from an aged related illness? Caring for a disabled or elderly family member can have serious ramifications for the Carer. It can be a thankless 24 hour 7 day a week job. It often leaves a carer feeling exhausted, socially isolated, frustrated and resentful. So you may well ask why do they do it? Carers give many reasons for caring for family members, including, a sense of duty, a feeling of satisfaction, love for the dependent person and feeling that they have no other choice. Attending a Carer Support Group provides participants with peer support. Meeting with people who are living in similar situations can alleviate feelings of isolation and provide a safe and comfortable place to talk about concerns with people who really understand. Support groups assist family carers by giving them information about support services, including respite options, workshops, information about counselling, and referrals for practical help such as Home Care Community Care Packages and the Carers Allowance from Centre Link. The groups encourage family carers to take the time to look after their own health needs. If a family carer is given support and information, they are more likely to feel in control of their living arrangements; less isolated, and acknowledged for the important job that they are doing. There are four Family Carer Support groups in the Frankston Mornington Peninsula. Koori Fitness Program (Frankston) The Koori Fitness Program involves gym-based exercise for Koori participants to improve their fitness and health. This free class is held every Friday (during the school term) from 10am to 11am, involving resistance and aerobic-based exercise, supervised by a qualified gym instructor. Each participant must be medically screened prior to exercise and be over 18 years of age. The class is located at the gymnasium of the Monash Peninsula Activity and Recreation Centre (MPARC), Monash University, Frankston campus. The program is only available for limited numbers.

Koori Kitchen (Frankston) The Frankston Koori Community Kitchen was established several years ago to promote and support Aboriginal and/or Torres Strait Islander health & wellbeing. The Koori Services Team supports participation through a variety of community identified classes, including exercise programs, workshops, cooking classes & cultural excursions. Koori Kitchen operates weekly during school terms at St Paul’s Anglican Church - corner Bay and High Street/s Frankston from 10am – 2pm Lunch is provided and everyone is welcome.

Koori Youth Swimming Program (Frankston) A learn-to-swim program is provided for Koori youth at Jubilee Park Aquatics Centre, Frankston, during the school term. This free program is provided by qualified swimming instructors for Koori youth from 7 months to 18 years of age. Each participant is required to attend an assessment of swimming ability prior to commencing the program. Upon completing assessment, each child will be allocated to a group based on their ability for weekly training throughout the school term. Didgeridoo Workshop – 10 week workshop This workshop is provided by Koori staff to assist participants in creating and playing their own didgeridoo. The workshop occurs on Thursdays from 1pm to 3pm with light refreshments provided. All tools and blank didgeridoos are provided. The workshop also involves consultation with other health professionals for assisting management of health outcomes as well as a musician to assist with playing the didgeridoo. The workshop is only available for limited numbers.

More to Life: An Innovative Chronic Health Conditions Program This 8 week course is a caring and effective group program which focuses on the psychological, emotional, and behavioural strategies, that will improve your life while having a chronic health condition. Suitable for people with arthritis, asthma, diabetes, chronic pain, chronic fatigue and heart, lung or gastric conditions etc. Also suitable for people who have a physical condition, and have confidence, depression or anxiety issues. Cautious with Cannabis This program is part of a State-wide diversion initiative that was originally developed for individuals who have received a “cannabis caution”. However, the program has been opened up to other people wishing to know more about cannabis use and behaviour change. Participation in the program is voluntary and anyone is welcome. The material will be delivered using a mixture of audio visual materials, and individual and group based activities. A certificate of attendance will also be issued to all participants. Where: Frankston When: 10.00am – 12 Noon Starts: : 14th August, 13th November (Other group time if required may be negotiated if urgent) Drink Drive / Drug Drive (Assessment Education Program) A program for any person who is required by law to undergo Assessment Education Programs and/or counselling for drink drive penalties, incurred by alcohol or substance abuse, and who wish to regain their driver’s licence. A six week structured, educational, therapeutic course, developed for drink drivers who have accumulated three or more drink drive offences. Family Drug Help / PenDAP Family Support Group Family Drug Help support groups are run by trained peer based volunteers for friends and family of those with substance misuse. Our groups provide support, education and strategies for family and friends of substance users. Diabetes Education Groups Finding out that you have diabetes can raise many questions. Take an active role in managing your diabetes by following a few simple steps to help you be in control and live well. These interactive groups include a Diabetes Educator, Dietitian, Physiotherapist, local GP and Podiatrist. You will be offered First Steps – an initial 2 ½ hour information session followed by an individual session and then attendance at a 4 week education group is optional. DAFNE (Dose Adjustment for Normal Eating)

One week Type 1 diabetes management program. Learn how to adjust your insulin for better blood glucose control and eat what you like, when you like. Where: All Sites – locations are set yearly

Foot Care Education Group Come and join our podiatrists and find out how to keep your feet comfortable using safe, self-management skills. The aim of the group is to encourage participants to retain their independence in looking after their own feet. The session covers topics such as skin and nail care, diabetes, footwear, and first aid. Please bring along any questions you may have to the group. Chronic Heart Failure Rehabilitation Program This program is a 10 week education and exercise program for people with a diagnosis of chronic heart failure. The diagnosis may be new, following an acute episode in hospital, or the need may arise following a recent period of hospitalisation and deconditioning. The exercise is tailored to the individual with continual monitoring from heart failure nurses and a physiotherapist. Prior to acceptance to the program, an individual assessment is required by a heart failure nurse. Where: Frankston Community Health

Pulmonary Rehabilitation Program This 6 week program is for people living with chronic lung conditions, and aims to help: maintain or improve your fitness and muscle strength people feel better and reduce infections and shortness of breath improve breathing, balance, moods, feelings of control, bone density and independence increase your ability to take part in more activities each week clear mucus from lungs understand your medications teach you how best to manage your condition offer social support to people with chronic lung diseases An individual assessment with the course facilitator is provided before commencing the group. Each session runs for 2 hours including 1 hour of safe and monitored exercise and 1 hour of education. There are two sessions per week. You will learn about: medications and puffers managing breathlessness physical exercise

information on diseases healthy eating and swallowing issues managing feelings of being sad or anxious

This seven week program incorporates an exercise component, and education session, to maximize recovery and well-being. Cancer Rehabilitation Program For cancer survivors during treatment or who have completed treatment for cancer in the previous two years; who wish to improve their quality of life by gaining confidence with exercise, managing fatigue, improving nutrition and managing stress. This six week program runs for two hours, twice per week; and combines supervised exercises, education and discussion sessions. A home exercise program is provided for all participants on completion. A Community Kitchen is a place where groups of about 6-8 people come together on a regular basis to meet and cook affordable, healthy meals. The program is for men and women of all ages to learn basic cooking skills in a friendly and relaxed environment. The participants decide what they would like to cook and take the food home or sit down and share a meal together at the end of the session. Cost is $5.00 per session unless otherwise stated.

Community Based Walking Groups There are a number of walking groups across the Peninsula which are supported by Peninsula Health; either with a trained walk leader or a volunteer. The walks are suitable for any age and level of fitness. It is a friendly, social way to keep active and improve your fitness. There are also a number of group walks available, that are provided by the shire councils. LIFE! Plus LIFE! Plus is an 8 week physical activity and lifestyle program designed specifically for people living with or at risk of diabetes and other chronic lifestyle related conditions. The program is designed to help you build the lifestyle skills and confidence you need to get active and stay active. As well as two exercise based sessions a week. The program also involves education based sessions on topics such as healthy eating and the importance of exercise. How to join: Phone Access on 1330 665 781 and ask for an appointment with a Life! Plus Facilitator. Where: Hastings Rosebud Mornington Frankston (offsite at Monash University Gym) Cost: $6.40 per session except for the Frankston session held at Monash University Gym where the fee is $7.00 because it includes the parking fee.

KickStart KickStart is an 8 week exercise program for clients with chronic and complex conditions who are not suitable to attend other groups such as Cardiac Rehab, Pulmonary Rehab, Cancer Rehab and Agestrong etc. It is designed by an Exercise Physiologist who will provide supervision and an exercise program suited to each client to achieve useful and practical goals. It aims to promote regular, independent exercise for clients. Entry to this group is only by recommendation from an Exercise Physiologist. Where: All Community Health sites Cost: $6.40

Agestrong

Agestrong is a strength and balance program designed for older people, especially those who have stopped exercise or who have chronic health conditions. Agestrong is run by a trained exercise leader, in safe and supportive environments. All classes cost under $7.00 per session and are available at 18 venues across the catchment. The class uses resistance and weights to work the major muscle groups of the body. The exercises are adapted to individual needs. The benefits of Agestrong include maintaining and restoring strength, flexibility, balance and increased confidence. You will need to have an assessment with one of our physiotherapists or exercise physiologists prior to joining Agestrong.
