WELCOME TO MOATFIELD SURGERYMoatfield Surgery has been treating patients in East Grinstead for about 60 years; originally the doctors were based in three separate surgeries. In 1987 the three surgeries, with its six doctors, moved into one purpose built building. Since then, the building has been extended several times, and most recently, additional building work has been completed to accommodate the ever increasing services that we provide. The building is designed to be convenient and welcoming to both patients and staff alike. The waiting areas, consultation rooms and facilities are accessible to people with impaired mobility and the patients’ car park is situated directly in front of the Surgery.

The practice looks after approximately 13,500 people and it is within an area managed by Horsham and Mid Sussex Clinical Commissioning Group. Moatfield Surgery has always been at the forefront of primary care and continues to provide high quality care for patients.The practice holds a contract with NHS England. MINOR IllNESSOur nurses run minor illness clinics on most days throughout the week. Many conditions can be dealt with by the nurse who is qualified to deal with problems such as abdominal pain, bites and stings, cystitis, diarrhoea and vomiting, earache, emergency contraception, eye problems, hay fever, infected wounds, injuries, rashes, sore throats and vaginal discharge, chest infections, back/knee/joint pain, impetigo, headaches, heartburn, rhinitis, sinusitis and other nasal problems, sore throat, tonsillitis and many more.Please tell the receptionist that you would like to make an appointment with the nurse prescriber.

HOW TO SEE A DOCTOR The surgery operates a telephone assessment system. To make an appointment, either telephone 01342 327555 or call at reception during surgery opening hours. On-line booking is available for which patients need to sign up by calling into the surgery. The appointment system combines book-in-advance appointments and a same-day telephone assessment. If you telephone on the day that you wish to be seen, the receptionist will take your details and a member of the clinical team will call you first to assess your medical need. The team has appointments available to book that are not open to receptionists. Please note that we cannot guarantee the doctor of your choice. Should you wish to see a particular doctor, we strongly recommend that you book in advance, but this may mean a wait of up to several weeks.Medically urgent cases will always be seen the same day.If you cannot keep your appointment do let us know so fellow patients benefit from the release of the appointment.Please make separate appointments if more than one member of the family is to be seen.Please make it clear if you are booking a medical examination so that you have a longer appointment at the correct time.The practice conducts regular staff training and as a result, on Friday mornings, the telephones are not open until 9.00am. We have an Appointments Leaflet that explains our appointments system to you. It is available to download from our website and is also available in hard copy from the surgery.

SURGERY HOURSMoatfield Surgery's telephone lines are open between 8.00am and 6.30pm, Monday to Thursday and 9.00am to 6.30pm on Fridays. The phone lines are not open at weekends. The surgery is closed for lunch between 12.45 and 1.30pm.

WHAT TO DO WHEN THE SURGERY IS ClOSEDThe surgery is closed from 6.30pm until 8.00am Monday to Thursday and 6.30pm until 9.00am from Thursday evening until Friday morning (between 8.00am and 9.00am on Friday mornings, we are contactable by telephone for emergencies only). If you require an urgent visit or urgent advice during these times, at weekends or public holidays, please telephone our surgery on 01342 327555. There will be a recorded message giving details of the telephone number you need to contact for the out-of-hours service, operated by NHS 111. If you wish to dial NHS 111 directly for advice, the number is 111. HOME VISITSAll doctors in the practice make home visits during the hours 8.00am - 6.20pm. Arrangements for visiting are made each day according to the availability of doctors and pressure of work in the surgery. Home visits generally involve a lot more of the doctor’s time than a surgery consultation; we would, therefore, ask you to request a home visit only when it is impossible for you to attend the surgery. We are always willing to make arrangements for patients to be seen without waiting if their clinical condition justifies it. Requests for home visits should be made before 10.30am where possible. Always state the patient’s full name and address and give brief details of the illness.

TElEPHONE CONSUlTATIONS RATHERTHAN VISITING THE SURGERY

If you ring up to make an appointment on the same day a member of the clinical team will call you first to assess your medical need. They will then work out with you the best way to help you. This might be advice on the phone, arranging a test or prescription or onward referral or arrange for a doctor or nurse to see you at the surgery today or another day. Please note that these urgent appointments are controlled by the clinical team and no other members of staff have access to these.Sometimes you may just want advice on medication. Please bear in mind that pharmacists spend five years at university, training to degree level in all aspects of medicines, and in such instances it might be more convenient to visit your local pharmacy for this sort of advice, particularly as there is no need to make an appointment.

REPEAT PRESCRIPTIONS If you need regular treatment your doctor may authorise the issue of repeat prescriptions. This means that your repeat prescriptions will be put onto our computer. When you receive your prescription you will also receive a tear-off portion giving details of your repeat medication. Please allow two working days to collect your prescription from the reception desk and three working days if you would like it to be sent to a pharmacy. Please DO NOT leave your prescription request until you have run out of tablets. There is a chemist in the building and there is a collection service available for the other chemists in the town. Please enquire at reception about this service. ‘Pharmacy 2U’ also offer an electronic prescription ordering and delivery service. Please ask for a form at the reception desk.Repeat prescriptions may be requested:

2. By post, enclosing a stamped addressed envelope if the prescription is to be returned.

3. By submitting your request personally at the reception desk.

4. By fax on 01342 316240.

5. By email which is [email protected] order to avoid mistakes and ensure your safety, we do not receive telephone requests for repeat prescriptions, unless you are housebound. Due to a high volume in the first few hours of the morning relating to appointments, home visit requests and emergencies, we ask you to raise prescription queries with our team after 11.00am.

'FIT' CERTIFICATES You do not require a doctor’s Fit Note for any illness lasting six days or less. Your employer may, however, require you to complete a self-certification form. For any illness lasting longer than six days you will need to see the doctor for him/her to issue a Fit Certificate (which replaced the sick certificate in April 2010).The main difference between the old sick note and the Fit Note is now the doctor can advise the patient that he/she may be fit for work provided certain criteria are observed – whereas previously the patient might have been bound to an inappropriate time off work.

NEW REGISTRATION Patients are registered with the practice and not an individual doctor within the practice. However, you do have the right to express a preference as to which doctor you would like to see. To register as a patient please present in person with photo identification and a passport if you were born overseas, and proof of address. This must be in the form of a recently dated utility bill, lease document etc. We do not normally accept bank statements or credit card statements as proof of address.You must be ordinarily resident within our catchment area, an outline of which can found at the back of this booklet.Our registration pack contains a health questionnaire, which is confidential. Please complete as much as you can. This information enables our doctors to briefly assess your general health and familiarise themselves with your medical history.Once we have received your registration documentation, please allow five working days for the processing stage to be completed before making an appointment or requesting any medication. It may take several weeks for your paper record to reach the surgery.

CHANGE OF NAME AND ADDRESSPlease let us know as soon as possible of any change of name or address. Unless you do this the Primary Care Support Service may remove your name from our list of patients without letting you know.

GENERAl PRACTICE REGISTRARS, FOUNDATION DOCTORS AND MEDICAl STUDENTS

The practice is an approved training practice and undertakes the teaching and training of doctors. Our service benefits greatly from attached Registrars, who are fully trained doctors wishing to specialise in family medicine. Foundation doctors are fully qualified doctors who are rotating around various specialties to increase their experience. Some may choose to go on to do further training to become a GP. Medical students also join us during their training to become a doctor. All are closely supervised and assessed throughout their time with us. They will have longer appointment times.

COMPUTERISATION At times you may be asked to complete a patient questionnaire so that we can update our patient records. Accurate record keeping is essential and is to everyone’s advantage. It holds the key to many benefits, particularly in preventative medicine. The practice is registered under the Data Protection Act and patients can be assured that confidentiality will remain our prime concern.For routine audit purposes, representatives of the Primary Care Support Service will be required to look at patients’ medical records. The auditors may not be medically qualified but are all bound by their contract of employment to respect confidentiality. However, if you do not wish your notes to be used in this way, please inform us at the front desk and your notes will be annotated accordingly.Insurance companies and solicitors will ask for access to your medical records in certain circumstances, eg mortgage applications and accidents. We will only release this information if we have your signed authority to do so.Under the Access to Health Records Act you are entitled to view your health records. Please check the fee we charge for this with the receptionist.

HEAlTH PROMOTION

We encourage all of our patients to share the responsibility for their health, both in preventing disease and in treating existing disease. Prevention really is better than cure. Many of the more serious diseases can be prevented by a healthy lifestyle and without the need for drugs. Please feel free to discuss with us both your general health and lifestyle and to ask for advice. We keep a large supply of free literature on a variety of health topics. Always adhere to the medicine regime as prescribed by your doctor and seek advice before making any changes.

SMOkING CESSATION

Smoking is the single largest preventable cause of ill health in this country. It is a major cause of cancer, heart attacks, angina and chest diseases.Do you want to stop smoking?Have you got some will power?Are you available for regular monitoring and support?If so, book into one of the Stop Smoking Sessions:Individual help and counselling by practice nurses can be arranged on most days but must be booked in advance.Advice and information is available on nicotine replacement therapy and Zyban. Please ask a receptionist for details of when we hold our Stop Smoking Clinics.

NON-NHS SERVICES AND FEES

The practice provides several services that are not contracted for and paid for by the NHS - such as private sick notes, insurance claim forms, fitness to travel letters etc. Because of this, the practice charges a fee for these services. The charges are in line with British Medical Association recommendations. Patients are advised that these charges are payable in advance of any requests being actioned.

SUPPORT STAFF

PRACTICE NURSESPractice nurses are available in the treatment room every morning and some evenings. You may make an appointment to see the nurse if you have a problem that does not require a doctor, such as dressings, injections before going abroad, blood pressure checks, cuts and ear syringing, or information about diet, alcohol or smoking.

THE COMMUNITY MIDWIFEPlease contact the community midwife after you have seen a GP (who must refer you to the midwife) as soon as your pregnancy is confirmed. She is available weekday mornings between 8.30 and 9.30am on 01444 441881 Ext 8414 .

COMMUNITY NURSESDistrict nurses are attached to the practice and visit patients who are confined to their home and require nursing care. Patients can be referred by the surgery by self-referral or by the hospital if nursing care is needed on discharge. They can be contacted by telephoning ONECALL on 0845 092 0414.

HEAlTH VISITORSThe health visitors can give advice on health care, particularly to expectant mothers and small children. If you have a problem affecting your health they can put you in touch with sources of help. Doctor and practice nurse - currently Wednesday afternoon with Dr Clarke and Monday afternoon and Friday morning with our asthma nurse, Laura Ireland. The clinics might change at some point in the future so please check with the receptionist.DIABETIC ClINICDoctor and practice nurse - Fridays. Patients are asked to have their yearly blood test two weeks prior to appointment. An external specialist diabetic nurse sees patients at Moatfield Surgery to support them in taking control of their diabetes care.CERVICAl SMEAR ClINICCervical screening reduces the risk of developing cervical cancer; it is not a test for diagnosing cervical cancer. It is a test to check the health of the cervix, which is the lower part of the womb (often called the neck of the womb).We offer the test to all women aged 25-64. This entails women being invited every 36 months in the age group 25-49 and every 60 months for women aged 50-64. Appointments should be made when the patient is mid-cycle of her menstrual period.MATERNITY CAREMidwife's antenatal clinic - Tuesdays and Thursdays.ANTICOAGUlANT ClINIC (INR ClINIC)The practice nurses are overseen by Dr Pasan. Patients on anticoagulant therapy are monitored regularly and dosages are changed accordingly using a finger-prick method of blood sampling and the results entered into a computer software decision support database. The patient has the convenience of a local service and a rapid assessment of INR and future dosage regime all within a 10-minute appointment. Patients must bring their yellow record book and let the clinic nurse know if there is any change in their medication.MINOR SURGERYThe doctors offer a minor surgery service for many procedures, including in-growing toenails, removal of warts, verrucae and other small lumps and bumps. Please make an appointment with your doctor to discuss this.TRAVEl ClINICConsult the practice nurse at least eight weeks before you are due to travel for advice on vaccines.Please make an appointment at reception. There is a charge for some vaccines and vaccination certificates. DO I NEED TO BE PROTECTED AGAINST PNEUMOCOCCAl INFECTION?Everybody aged 65 and over should now be immunised to help protect them against pneumococcal infection which can cause diseases such as pneumonia, septicaemia (blood poisoning) and meningitis.Please phone Moatfield Surgery during September to make an appointment if the above applies to you. This is a one-off vaccine and there is no need for repeat doses.

FAMIlY PlANNING SERVICES* Confidential advice and a full range of family planning services are available at Moatfield.* All doctors are qualified in family planning. Nurse Practitioners are also qualified in family

planning other than in coils.* You can see any doctor you wish, who may not be the doctor you are registered with.* Emergency contraception (also known as the 'morning after pill') may be taken up to three days after unprotected sex. The earlier it is used the more effective it is. If needed, then do make an appointment within this time. If there are no routine appointments then tell the receptionist that this is an emergency. Pharmacies provide a walk-in service at your convenience.

SElF TREATMENT OF COMMON IllNESSES AND ACCIDENTSMany common aches and pains can be simply treated at home without the need to consult a doctor.BACk PAINBack pain causes 13 million working days to be lost in Britain each year. The spine supports the whole weight of the upper body so it is understandable that it sometimes goes wrong. Because of the complex nature of the spine it is advisable to consult your doctor if back pain persists for more than a few days, or if the pain has been caused by abuse ie lifting excessively heavy weights etc. BURNSApply large quantities of cold water to the affected area as soon as possible and maintain this until the pain subsides. This may take as long as 15 minutes! If the skin is unbroken but blistered, apply a loose, dry dressing.COlDSEven in this day and age there is still no magic cure for the common cold. Go to bed, take plenty of drinks. If you have a headache or are feverish, take aspirin or paracetamol. Do not bother to take antibiotics as these will have no effect! DIARRHOEAIn adults, diarrhoea is usually caused by a viral infection and is therefore unable to be treated directly. The symptoms can usually be eased by drinking plenty of fluids.Consult your doctor if the symptoms persist for more than a few days.

Diarrhoea in very young children and babies needs careful attention. Most babies have loose bowel action during their first six months due to their predominantly liquid diet. Sudden bouts of unusually watery diarrhoea should be treated by taking the baby off solids and increasing fluids. If the symptoms persist for more than 24 hours, or are accompanied by vomiting, a rash or weakness, consult your doctor.SPRAINSTreat with a cold compress, containing ice if possible, for 15 to 30 minutes to reduce the swelling. Then firmly apply a crepe bandage and give the sprain plenty of rest until all discomfort has subsided. Further strain will inevitably lead to further swelling and a longer recovery period.NOSEBlEEDSSit in a chair, lean forward with your mouth open, and pinch your nose just below the bone for approximately 10 minutes, by which time the bleeding should have stopped. Avoid hot drinks or hot food for 24 hours. If bleeding persists go to casualty.MINOR CUTS AND GRAzESWash the wound thoroughly with water and a little soap. To stop bleeding apply a clean handkerchief or dressing firmly to the wound for about five minutes. Cover with a clean dry dressing.SUNBURNCalamine lotion will relieve the irritation whilst paracetamol will also help.Children are particularly susceptible to sunburn and great care should be taken to avoid over-exposure to the harmful effects of the sun.INSECT BITES AND STINGSAntihistamine tablets can be obtained from the chemist without prescription and will usually relieve most symptoms.Note: bee stings should be scraped away rather than ‘plucked’ in order to avoid squeezing the contents of the venom sac into the wound.HEAD lICEThese creatures, contrary to popular belief, prefer clean hair and are, therefore, not a sign of poor personal hygiene. Contact health visitors or school nurses for advice on treatment.GERMAN MEASlES (RUBEllA)The rash appears during the first day and usually covers the body, arms and legs in small pink patches about 2-4mm across and doesn’t itch. No other symptoms are usually present apart from occasional aching joints. It is infectious from two days before the rash appears, until the rash disappears in about four or five days from that date. The only danger is to unborn babies and, therefore, it is important that all contacts are informed in order that anyone who may be pregnant can contact their doctor. Immunisation can prevent this disease.MEASlESThe rash is blotchy and red and appears on the face and body around the fourth day of illness. It is at its most infectious from two or three days before the rash appears until eight or ten days after that date. Immunisation can prevent this disease.
