A gastrostomy is a stoma tract which

A gastrostomy is a stoma tract which provides
direct access into the stomach, allowing food
and water to be given, whilst bypassing the
mouth and throat.
The procedure is necessary when a
patient requires long-term nutritional
support of more than 30 days.
Gastrostomies can be created surgically
or using a percutaneous technique. Initial
kits are used to create the tract and fit
the first feeding device then replacement
devices can be used when the first tract
has become established.
A gastrostomy will benefit patients who
have difficulties feeding. These can be
due to neurological or gastrointestinal
disorders or a problem with swallowing.
As gastrostomies are concealed under
clothing they do not hinder a patient’s
activities of daily living.

A gastrostomy tube is a feeding tube which
allows direct access to the stomach after the
initial tract has been established.
They are secured using an internal
balloon and an external skin disc,
allowing them to be fitted/replaced on an
outpatient basis or in the community.
The Medicina gastrostomy tubes are
available in 12/14/16/18/20/22/24FR
sizes.
They are all made from silicone and are
provided in kit form. The kit ensures that
all necessary elements are at hand when
required, ensuring speedy and effective
placement.

“ “A gastrostomy adapter is attached to
each G-tube to prevent the tube from
becoming loose which can cause
leakage. The adapter also allows the
G-tube to be compliant with enteral
feeding devices (NPSA 19).
Medicina also provide a number of
extension sets to allow the patient to be
fed using a pump or via a bolus.
The maximum balloon volume for the
12FR, 14FR and 16FR tubes is 5ml and
for all the others is 20ml.
The volume of the balloon
should be checked weekly to
ensure the gastrostomy tube is
secure at all times.

required to allow the tube to be inserted
efficiently and comfortably.

A radiologically inserted gastrostomy
is often placed in patients where
access for an endoscope is difficult,
for example head and neck cancer or
oesophageal problems.
A radiologically inserted gastrostomy
(RIG) is a gastrostomy tube that is
inserted using x-rays and other
radiological imaging techniques.
The patient will usually swallow some
Barium Sulphate prior to the procedure
allowing the insertion site to be easily
found by the radiologist.
During the procedure air may be inserted
into the stomach using a nasogastric
tube to inflate the stomach and help
place the tube accurately and safely.
Medicina also provide a T-fastener kit
(see accessories) that is used to anchor
the anterior stomach wall to the
abdominal wall. This technique is often
used to ensure that the tract is secure
and the tube cannot be accidentally
pulled out into the peritoneum. Individual
technicians use different techniques to
place the T-fasteners.

The Medicina jejunostomy tubes are
available in both 7 and 9FR sizes and can
be accessed directly with the Medicina
range of oral/enteral syringes.

The Medicina jejunostomy kits are used to
surgically place a tube through the abdomen
and into the jejunum (small bowel) .
The tube can be used for the critically ill,
early post-operative patients and patients
with intestinal disease or injury.
The Medicina jejunostomy kits are
available in 7 and 9FR sizes.
Jejunostomies are normally created in
the operating theatre using an open
technique.
1. Firstly a tunnel is formed in the Jejunal
wall using an Introducer needle and the
tube is passed through the needle into
the jejunum.
2. When the needle is removed the tube
is fixed to the jejunal wall by means of a
purse string suture. The jejunum is then
sutured to the abdominal wall.
3. A second Introducer needle is used to
tunnel along the abdominal wall in the
opposite direction to the tunnel in the
jejunum, to create a safety loop. The
tube exits the second Introducer at the
skin site.
4. A skin disc then anchors the tube
externally.
A jejunostomy tube can be replaced
using a J-type guidewire. The new tube
is threaded over the wire which is then
removed.

dressings help stop leakage and

are provided in three sizes to fit all
sizes of tube.

extension setsMedicina have four different
extension sets to aid the process of
feeding down a gastrostomy tube.
One set allows the tube to be
connected to a pump. This set also
has a Y-site should any medications
need to be administered. The bolus
feeding set is for use when the rate
of delivery needs to be controlled. It
comes with a 60ml syringe attached
and also has a Y-site for medication.
We also have a paediatric (shorter)
version of this set that doesn’t have a
plunger or a Y-site.
clampA separate clamp is available for all sizes
of gastrostomy tubes. The clamp is used
to close off the tube before or after
feeding.
adapterThe gastrostomy adapter allows the
gastrostomy tube to be fitted to an
extension set. This adapter comes pre-
fitted to all Medicina gastrostomy tubes.
guidewireA separate J- guidewire is available should
the patient have a gastrostomy tract that is
not straight, or to help replace a
jejunostomy tube.
gastrostomy tidyThe gastrostomy tidy is a device
designed for patients who are body-
conscious or who pull on their
gastrostomy tube. The device allows the
tube to be kept tidy and secure in a
specially made pouch. The pouch comes
in child and adult sizes, and can be
adjusted to fit.
