The pump should only be operated byhealthcare professionals and carers whohave been trained in the functions andindications of the pump. The pump issuitable for the delivery of enteral feedsin hospitals but may also be useddomestically under the supervision of ahealthcare professional.

It is important to check the pump,syringe and any extension lines arecorrected fitted to an enteral feedingdevice only, normally a nasogastric tube,before the pump is operated. It is alsoimportant to check all connectionsbetween syringe, set and tube aresecure. The correct gastric position ofthe feeding tube should also beconfirmed before every feed delivery.

The only syringes which will operate inthis pump are marked “MEDICINA”and are supplied by Medicina Ltd or itsauthorised distributors. All other syringesare not recognised by sensors on thepump and will not function. It is alsoimportant to check the automatic visualsyringe size light on the pump andconfirm that this matches the size of thesyringe used. All syringes may be usedup their maximum volume except thePE60 60 ml syringe which has amaximum volume of 50mls only. Allsyringes are single use only . Thesyringe and set should not be connectedto the patient until the pump is ready, asan incorrectly fitted syringe has thepotential to deliver uncontrolledsiphoned feed to the patient. Syringes,pumps and sets are all latex free andDEHP free.

operation

The pump should only be operatedunder the following environmentalconditions +5°C to +40°C and anatmospheric pressure of 86-106 kPAand a relative humidity of 20%-90%.The pump should also be stored in adry place and care should be taken notto drop or vibrate the pump.

electrical

The pump should only be operatedusing a standard AC power supply 100-240V @ 50-60Hz using the authorisedpower cord. The power cord should beprotected from bending and any heatsource. It should be discarded if itbecomes damaged in any way. Thepump should be unplugged when not inuse. The pump should only be operatedusing the battery for transfers and onlyuntil the low battery alarm sounds. Thebattery should then be recharged.

maintenance

The pump should be cleaned in linewith hospital policies or with a dampcloth or an alcohol type wipe (75%alcohol) No other chemicals should beused to clean the pump. The pumpshould not be repaired or opened byanyone other than authorised MedicinaStaff. This will void the warranty. Alldefective pumps should be returned tomedicina Ltd or to an authorised servicecentre or distributor. The pump shouldbe stored in dry conditions when not inuse.

A important feature of the pump is thatonly “MEDICINA” syringes willoperate in the pump. All other syringesare incompatible. Many current syringepumps can accommodate both enteraland intravenous syringes. This poses aserious risk of hazard to the patientsbecause Intravenous syringes can beaccidentally filled with enteral feeds andconnected to central lines. Thesemisconnections are not possible with theuse of this pump because the lines andsyringes which it uses cannot beconnected to central lines. This offers aunique additional safety feature forenterally fed patients.

The P500 enteral syringe pump hasseveral other useful safety features. Atthe heart of the pump are twomicroprocessors. One to operate thepump and a second to be used asbackup and to supervise all theoperations of the pump. A host ofsensors monitor the delivery, providingcomprehensive visual and auditoryalarms. Although the pump is simple touse, the electronics are at least assophisticated as more expensiveintravenous syringe pumps. Users willfind the P500 pump easy to use and itwill safely and accurately provide thenutritional support needed by theirpatients .

The P500 enteral syringe pump has beendesigned to deliver small volumes of enteralfeeds, accurately and safely, using oral enteralsyringes. The pump is indicated for the deliveryof enteral nutritional support in small volumesto neonates and paediatric patients. The pumpis part of a complete range of equipment whichhas been carefully put together to maximisepatient safety. The non-IV compatible syringes,feeding tubes and accessories provide acomprehensive feeding system.

P500 Basic Controls

Operation

Securing the pump

Select suitable position for the operation of the pump beforeinserting the power cable. The pump should be used in ahorizontal position either on a table or on a drip stand. A poleclamp is provided with the pump. The black plate is screwedinto the underside of the pump with the silver clamp facingthe rear of the pump.

The second screw is used to secure the whole assembly tothe drip stand.

Caution Never mount the pump so high on thedrip stand that is becomes unbalanced or unstable.

Power

When the pump is secured the power cable should beplugged into the mains supply at the rear of the pump.

Press the power button for 3 seconds and the AC Powerstatus light should come on. The battery level status light alsoshows the level of charge in the battery.

The pump will then self test all the circuits and all lights willflash once before the pump is ready to use. The pump isturned off by pressing the power key for 5 seconds.

Syringe installation

Caution Only “MEDICINA” single use syringesmay be used in this pump.

A suitable syringe for the volume to be delivered is selectedand connected to an extension set. All connections should bechecked. Both are then primed aseptically either by using thesyringe or the bolus fast rate button. To insert the syringe into the pump the syringe clamp is raisedand rotated. The syringe barrel flanges are inserted into thegrooves on the syringe bed and the plunger flanges areinserted into the syringe drive mechanism. The drivemechanism should be adjusted so it is tight on the syringeplunger. Fitting adjustments can be made by pressing themechanism release button on the side. When the syringe iscorrectly fitted the syringe clamp can be rotated back intoposition. It is important to check that the automatic syringesize indicator light matches the syringe size.

Caution the 60ml syringe PE60 has a maximumvolume of 50mls and cannot be fitted into the pumpwith a higher volume than 50mls.

Set Delivery Parameters The rate of delivery is set with the rate adjuster wheel. The adjustments can only be made when the stop button hasbeen depressed. Rates are locked during delivery. Deliveryrates start at 0.5mls/Hour up to 100mls/Hour in 0.5mlincrements for all sizes of syringes.

Clear Accumulated volumeThe volume delivered during 24hrs which may be severalfeeds is recorded by the pump. This can be displayed bypressing the select button until the Emls lights up. The maindisplay shows the total volume given that day. At the beginningof a new day the accumulated 24 hr volume should be clearedby pressing the select button to display the ∑mls volume lightthen the clear button . The display should read 000.0.

Clear Preset volume

The pump can be set to limit the volume of a particular feed byselecting preset volume which lights up. The adjuster wheel can dialin the maximum volume for that feed. When this amount has beendelivered the pump will alarm. At the beginning of a set of feedsthis should read 0.0 and can be cleared by using the adjuster wheelto make it zero. (Zero indicates that no Preset Volume is set).

Start Delivery

Caution it is important to check the pump syringeand any extension line are correctly fitted to an enteralfeeding device only, normally a nasogastric tube, beforethe pump is operated. It is also important to check allconnections between syringes, sets and tubes aresecure. The correct gastric position of the feeding tubeshould also be confirmed before every feed delivery.

The delivery is started by pressing the start button andstopped by pressing the stop button. [illustrations 5 &6]. Thedelivery operation light will flash from right to left dependingon the speed of delivery. The rate is locked during deliverybut the accumulated volume and preset volume can bechecked by selecting those functions. At the end of the feedthe pump will alarm ‘over’, press stop and note volumedelivered in that episode of feeding and or the accumulatedvolume over 24hrs.

Summary

The following auditory andvisual alarms are displayed bythe pump. All can be silencedby pressing the stop button.

Alarms

BATTERYWhen the battery is about to become exhausted the low battery alarm will sound.Delivery should be stopped and the battery should be recharged at the mains. Thebattery life at average rate of delivery is approximately 4 hours. The battery is fullyrecharged in approximately 12 hours. The battery indicator light also illustrates thestate of charge with all three bars indicating a fully charged battery.

AC powerWhen this light is on the pump is connected to the mains power supply.

OVERThis alarm sounds and the light comes on at the end of a delivery. This means thatthe delivery is complete. The same alarm and light will also sound and light upwhen a preset maximum volume is reached. Delivered volumes should be notedand new feeds initiated or the pump should be switched off until the next feed.

OCCLThis alarm sounds and lights up if the line becomes occluded. The occlusion shouldbe identified and removed then the feed can recommence. The occlusionpressures are fixed for the average range of enteral feeds.

OCCL & OVERIf these two alarms sound and light up at the same time then the syringe is notfitted correctly into the pump or has become disengaged.

The syringe position or type should be checkedand the pump restarted.

Test intervals will bein line with local hospital policies.

Caution If the AC light does not illuminate with the AC power on thefuse in the plug should be replaced. If it still does not illuminate the pumpfuse may need replaced. The pump should be returned to Medicina Ltd orits authorised distributor.

Caution Pump batteries should be replaced after 5 years by returningthe pump to to Medicina Ltd or its authorised distributor.
