What is a Federally Qualified Health Canter (FQHC)?
The East Liberty Family Health Care Center became a Federally Qualified Health Care Center in2000. The federal government provides the Health Centers Initiative to significantly increaseaccess ti primary health care services in 1,200 communities for more than 15 millionpeople. Our service area is a federally-designaated Medically Underserved Area.
As are all FQHCCs, we are a community-based organization that provides comprehensive primarycare and preventive care to persons of all ages, regardless of their ability to pay. We operate udera consumer Board of Directors governance structure. We bring care to underserved,underinsured and uninsured persons.
Our current annual budget is $6,5 million. Our support as a FQHCC from Section 330 of thePublic Health Services Act is $1.274 million, which is 19% of our budget. We are grateful forevery dollar that comes to us from this source. It means, however, that we must receive frompatient fees and from contributions the remaining 81% of our budget.

Our Core Values are:
ast year, we served 106 men and women with 2,679 visits. Our service area has a higherincidence of elderly persons (16.3%) than does Allegheny County and the nation (12.4%). Ourprimary service area is zip code 15206 which includes East Liberty, Garfield and Lincoln-Lemington.
“This program is still far less expensive than most other forms of care. It’s so valuablebecause it helps older persons who are frail and elderly to age at home with dignity”, saidRenee Teichman, homecare coordinator.
Perhaps because it is not funded by government or insurance driven bureaucracies, it isrecognized for being one of the few programs for the frail elderly that starts by asking themwhat they really need instead of which entitlement category they fit. Our nurses providecomprehensive whole person care. As needed, our doctors visit.
In addition, our nurses and medical assistants help with:
Transportation to ELFHCC and to other health care specialists as well as to hospitalsfor testing.
Delivery of medicines and other health-related supplies.
Being a patient advocate in coordinating homebound care,including direct patient care, with doctors and other staffat ELFHCC as well as other health-related agencies.
Social work
Helping patients connect with MedicarePart B, the waiver program, theDepartment of Aging, and othergovernment and private programsand services

Education of their families and caregivers

We serve those who are frail, elderly and homebound!
I love the season of Lent. The forty days is an exciting periodof preparation through prayer, study and self-denial. I think it’sgood for all us to focus on the life, the teachings, the suffering ofJesus every day. But especially at this time of year, we rejoice aswe remember that He is the hope of the world.
Many of the patients that we see at the Center believe thathope is lost. They often are lonely and alone. Many are poor andvulnerable. Some express anger, fear or disappointment. We aregrateful that God has sent them to the Center.
We desire to be the listening ear, the loving heart, the caringhands and willing feet that serve them. The Bible reminds us that“the needy will not always be forgotten”. At the Center, they areour priority! They are our reason for being. We are God-centeredand because of His love, we are patient-centered.
We strive to give them outstanding comprehensive whole person primary and preventive care.Within that definition, we care very much about their spiritual lives. Through our example, we praythat they will see Jesus Christ. We trust that they will know how much we love them and desire tomeet their needs.
Thank you to all of you who help us in meeting those needs. Your prayers, your volunteerservice and your financial support give us encouragement and enable us to continue. In recentyears, we have faced tough economic challenges.
But you have been faithful. And God has been faithful. He is our Hope and our Strength!We pray that you and your loved ones will have wonderful Holy Week and Easter

The Homebound Elderly Care Team includes Dr. Diane Balestrino,Dr. Todd Wahrenberger, Dr. David Hall, and Dr. Mark Meyer;Renee Teichman, R.N., Carol D’Amico, R.N. and Deb Keck, R.N.;and medical assistants Tujwina Gonzalez and George Rivers.
A Message of Hope
Last year, our care and help in the home prevented 51hospitalizations. “In those situations, we saved the healthcaresystem thousands of dollars at each occurrence. The comfortand stability of those seniors who were able to stay in theirown home rather than being uprooted to a hospital ornursing home is our paramount concern”, said Teichman.
Last year, nine of the patients died. Teichman explained,“That’s understandable. We’re dealing with some very frailindividuals. We stay with every homebound patient as longas he or she wants our care, is homebound, and as long asshe or he is alive. We are very grateful to the foundations and individuals who help make thisprogram possible. It has been a life saver for many older people.”
