Post falls, Weight loss, pressure ulcers!• “Physician “support! we aren’t the acute!”• “Survey issues; “strict liability”• “Trying to implement a new system in an already

packed day/week is impossible”• Documentation errors, omissions, self serving

comments….

OBSTACLES IDENTIFIED IN THE FIELD

• I have given that in-service – it doesn’t work• It won’t work because we don’t have the staff• We don’t have enough licensed nurses; “we would

rather be at the bedside”.• The documentation process is too cumbersome to

complete

survey outcomes.
