Personal support for you and your familyDid you know? The Horizon UCD Support Services team is made up of Clinical Nurse Educators, Case Managers who support them, and specialty pharmacies to provide you, your family, and your healthcare provider with help and support. Patients who are prescribed a Horizon UCD medication are assigned a dedicated Clinical Nurse Educator who will provide ongoing assistance.

How does the support team work?

Together, the Horizon UCD Support Services team works to provide you with complete support for your Horizon UCD medication.

The specialty pharmacy will fill and ship your Horizon UCD medication directly to you, and can also help with:

• Refilling your medication

• Shipment of medication supplies, such as caps and syringes

Your dedicated Clinical Nurse Educator provides ongoing one-on-one support. They can assist you in many ways, including:

• Providing help on how to use your Horizon UCD medication

• Reminding you when to refill your Horizon UCD medication

• Helping you with any insurance issues that may arise

• Explaining other financial help options to assist with out-of-pocket expenses for eligible patients

• Assisting with family/life changes that may impact getting your Horizon UCD medication

Four steps to starting your Horizon UCD medication1. Talk to your doctor about your Horizon UCD therapy.

2. Fill out Patient Authorization/HIPAA form. Once the form is submitted, Horizon UCD Support Services can assist you.
