Report of the East Staffordshire Clinical Commissioning Group

Summary What is the Select Committee being asked to do and why? The Committee is asked to note the work undertaken to date on the Improving Lives Programme and be assured as to how the work programme is being progressed by East Staffordshire Clinical Commissioning Group and Virgin Care as like-minded and trusted partners.

Report 1. Background 1.1 The Improving Lives Programme aims to improve the life experiences and outcomes of people living with long term conditions and those who are frail and older living in East Staffordshire. This is consistent with the Staffordshire Health and Wellbeing Strategy.

1.2 At the Healthy Staffordshire Select Committee meeting on the 5th December 2014, East Staffordshire CCG committed to providing further updates to the Committee at key stages of the work programme. 1.3 Subsequent to this, the CCG sent a letter to the Committee on the 18th February 2015 and shared the outcomes framework. The supporting letter also answered key queries raised by the Committee in December in relation to the scope and the financial envelope of the procurement and provided detailed information in relation to the extent of patient and public engagement undertaken in the lead up to and during the procurement for Improving Lives, as had been requested. 1.4 This report aims to highlight the work undertaken since attendance at the Committee on the 5th December 2014, provide clarity on next steps and overall assurance to the Committee as to how the work is being progressed. 2. Contract Award 2.1 On the 25th March 2015, East Staffordshire CCG announced the award of the Improving Lives Contract to Virgin Care, as our like minded and trusted partner, following detailed work up of the case for change from 2013 and the completion of a robust and legally sound procurement process. 2.2 The process has been clinically led by the Governing Body for the CCG and is supported by all of the CCG’s 19 membership practices. 2.3 The Contract is an outcomes based, seven year contract for a Prime Contractor who will transform, manage and integrate services across acute, community and voluntary sector organisations. This will be through sub-contracting arrangements with existing or new service providers or via direct delivery. Virgin Care may provide some services themselves but they are not able to be monopoly supplier within the system. 2.4 The Contract is a fixed price contract at a value of £270.4M across the seven years. The CCG will be spending no more than they do currently on this Contract for the services in scope. 2.5 The scope of the Contract is non elective (emergency) admissions and anything that prevents or mitigates against a non elective admission. This includes a proportion of activity from within a range of existing contracts including:

Non electives (Attends and Admissions)

Outpatients (First and Follow Ups and Procedures)

Community Based Services

Some Voluntary Sector Services

Out of Hours Service/ 111 2.6 The contract is an NHS Standard Contract and is therefore subject to the same national and local performance measures in relation to quality, safety and access as our existing NHS contracts. The Outcomes Framework is in addition to these measures and performance against these standards and the outcomes framework will be monitored via an agreed governance process outlined within the Contract.

2.7 The Outcomes Framework for the contract, now called ‘The Improving Lives Plan’ includes clinical measures (e.g. blood pressure measurements, non elective admission rates), patient reported measures (e.g. patients and their families feel more supported, confident and safe) and patient experience measures. The plan has been developed based on patient and carer insights including those heard through the Experience Led Commissioning work and other engagement work, and the clinical evidence base. An updated list of all of the patient and stakeholder engagement work that has been undertaken during the work programme is included as an appendix to this report. 2.8 Service commencement will be from 1st April 2016. We are now mobilising the Contract with Virgin Care and have a Joint Mobilisation Board which governs the process and involves senior clinical and managerial representation from both the CCG and Virgin Care. This provides assurance to each organisation’s respective Boards that mobilisation is progressing to plan and that a smooth transition to service commencement will be achieved. Mobilisation is being carried out in partnership, but the CCG can provide assurance that both parties continue to recognise our respective roles as Commissioner and Provider and our individual responsibilities and accountability. 2.9 Services will be NHS funded and continue to be free at the point of need. In accordance with the contract, termination notices have now been served on all services in scope. However, there will be no change to current services between now and the 31st March 2016 and the CCG will discuss the requirement and plans for consultation in relation to plans for any significant change to services. 3. Community Engagement 3.1 The CCG and Virgin Care continue to be committed to patient and public engagement since contract award and into the mobilisation period for the Contract. An updated list of all of the patient and community engagement that has been undertaken since the beginning of the work programme is included as an appendix to this report. 3.2 Four large scale public events have been held since December 2015 to keep members of the public updated about developments of the Improving Lives Programme and have provided interested parties with the opportunity to ask questions of both the CCG and Virgin Care. These have seen attendance rates between 50 – 100 people each time and have been held in both Burton on Trent and Uttoxeter. Further events like these are planned for late October 2015. 3.3 The patient and carer representatives who were on the Programme Board and jointly appointed by the CCG and Healthwatch for the procurement process are continuing to support the mobilisation period. Their role is one of compliance, to ensure that the CCG and Virgin Care are continuing to seek the views of patients and their carers, listening to their experiences and what matters most to them, reflecting these within future plans for the service model and the Improving Lives Plan and reporting back to patients and carers how their insights have helped shape future service delivery. 3.4 Virgin Care are committed to continued patient and carer involvement and engagement. Plans include the establishment of a Citizens’ Panel (name to be confirmed) which include three levels of involvement for members of the public from being informed (eg. via a newsletter), being asked (eg. completion of surveys) and being engaged (sharing

experiences and helping to shape future services). The launch of the Citizens’ Panel is intended in September 2015. In addition to this, there will also be the Improving Lives Forum (name to be confirmed) which will include key providers, stakeholder groups, voluntary sector organisations and be chaired by a Citizens’ Panel member. This will include performance reporting, including delivery of outcomes against the Improving Lives Plan, progress against the system transformation plans, proposals for future ‘Improving Lives’ strategies and plans and regular input from the Citizens’ Panel on fresh ideas and feedback. 3.5 Patients and their carers will also be involved in the further development of the Outcomes Framework, and in particular developing the measures and methods of data collection and testing processes for the patient defined outcomes that patients have told us are important to them but for which we do not currently have validated measures. This work will progress over the next 12 months. 3.6 The CCG and Virgin Care would like to seek the views and advice of the Committee as to whether there are further ways in which we can continue to build upon the patient and carer involvement and engagement work being undertaken during the mobilisation period and into the Contract term. 4. Service Model Development 4.1 Attached to this report is a briefing that provides details of the key elements of the current thinking of Virgin Care in relation to their service model to meet the following key objectives:

Improve the experience that patients and their carers have

Reduce unnecessary admissions to hospital

Integrate services

Embrace different approaches and providers

Engage the public in the shape of the services 4.2 As this is an outcome based contract, the CCG have not specified the service model that Virgin Care will provide. This is to provide Virgin Care with the flexibility to design, in collaboration with service users, staff and provider organisations, a model which will achieve the outcomes within the Improving Lives Plan to improve health and reduce inequalities, reduce pressure in the system and most importantly provide service users with a patient centred service that meets their needs and preferences and help them to be ‘Confident, Supported and Safe’. This is what patients and carers have told us matters to them most. 4.3 Virgin Care have used the insights from the CCG patient engagement work to develop the initial thinking in relation to the Service model, alongside insight that they themselves gathered during the procurement process about what works well and what needs to be improved in the local system. They will continue this dialogue through the mobilisation phase via the mechanisms outlined in Section 3 above. 4.4 Virgin Care established links with key providers during the procurement process and are now continuing conversations with these providers including Burton Hospitals NHS

Foundation Trust, Staffordshire and Stoke on Trent Partnership Trust, Staffordshire Doctors Urgent Care, Age UK, Derby Hospitals NHS Foundation Trust and Primary Care amongst others. 4.5 Virgin Care are keen to develop the service model in collaboration with existing and new providers and will continue these conversations with key providers and patients over the summer and autumn months. 4.6 Meetings with the key union representatives and also open meetings for nursing and therapeutic staff from the main provider Trusts have been held to provide them with the opportunity to ask questions of the CCG and Virgin Care. These were well attended and more dedicated events for staff are planned for the future. 4.7 The CCG recognises its statutory responsibility to continue to engage and consult, if required, on any proposed significant service change. The CCG and Virgin Care will continue an ongoing dialogue with the Healthy Staffordshire Select Committee as plans develop. It is anticipated that we will be able to share further detail on the model and timelines for implementation with the Committee in October 2015. Improving Lives: Public and Patient Engagement From 2012 to 30 June 2015 Since being in shadow form in 2012, East Staffordshire Clinical Commissioning Group (ESCCG) has been gathering the views of patients, carers and stakeholders on the main local health care issues. All of this work has helped the CCG to identify the care of those people with long term health issues and the care of the frail elderly as key priorities and to develop its vision for improving their lives. This programme of work was given the title of Improving Lives in August 2014 to make it easily identifiable and memorable.

20 Nov 2013 Long Term Conditions Stakeholder Event

To gather views on the current provision of services for long term health issues. This event was to test the feasibility of the CCG’s vision for patients with long term conditions for the future. It was part of the Prior Information Notice (PIN) process to establish the feasibility of the vision, outcome based contracting, draft outcomes framework and commissioning using the Prime Provider model.

This included patients, voluntary sector, NHS and private providers and commissioners.

The key learning was that the Prime Provider model was sound and achievable. It was endorsed by patients and carers. Commissioning to an outcome based framework would provide an opportunity to tailor services to patient need and allow and promotes innovation in the service delivery model.

A number of organisations had experience of or expressed an interest in the prime contractor role. The prime contractor role provides an improved experience for patients by integrating pathways of care/support and can drive integration through moving organisations to a shared vision and core value set. Voluntary Sector participants are keen to be an equal partner and an integral part of any future developments. A significant number of credible specific service providers engaged in the PIN processes and are keen to be a part of any future integrated service model.

There is significant overlap between LTC/FE/IC provision, with multiple services supporting the same individual. A population based approach with a simple definition of what is in and out of scope would reduce complexity and would enable a single prime contractor contract, preventing arguments between prime contractors and proactively encourage the delivery of holistic provision to the individual.

27 Nov 2013 Intermediate Care/Frail/Elderly Stakeholder Event

This was an Experience Led Commissioning (ELC) Positive Futures Planning Event which took place in Tamworth. Again, this was to test the feasibility of the vision for intermediate care, outcomes based commissioning, the Prime Provider model and included a draft outcomes framework.

The event involved a range of stakeholders, including people who use services, family carers, community groups, the voluntary sector; a range of NHS and private sector providers and commissioners

Conclusions were that the model and the approach were feasible. The community’s recommendations were fed back to ESCCG’s GP Steering Group and Governing Body and influenced its decision making when planning its main priorities for

This event for the general public was held at the Pirelli Stadium. As well as updating the public on the work of the CCG over its first year, the CCG launched its procurement to the public, who were asked to comment on the overall vision for improving lives in East Staffordshire. 71 people participated in this event.

14 Aug 2014 Improving Lives Workshop for Peer Support Group Organisers

This workshop gave the CCG the chance to hear from organisers of support groups about the role that peer support groups can play in supporting patients and their carers .

Around 30 representatives of support groups and health-related charities attended.

Feedback will be used to progress peer support networks. The CCG needs to be on the large amount of work already being undertaken by voluntary sector to support people to manage their conditions.

Improving Lives: Update 20 July 2015 Improving Lives is focused on supporting residents of East Staffordshire with long-term health conditions and the frail elderly to:

ohave more control over their own care ohave improved health outcomes oimprove their experience of living with their long-term conditions.

From April 2016, Virgin Care will manage the Improving Lives programme and will be responsible for working with all of the partners locally to arrange for services within the Improving Lives Programme to be delivered.

There are nine months until Virgin Care’s contract formally starts so the services won’t change at all for the moment. The mobilisation phase, i.e. the months leading up to next April, is progressing well.

Virgin Care are actively engaging with local providers of services and will continue to do so over the summer/autumn months to ensure smooth transition.

Virgin Care will also be talking to other local groups interested in supporting the Improving Lives programme. These discussions are at a very early stage, so limited details can be shared at the moment. As soon as there is more clarity and confirmed details these will be shared. Virgin Care are keen to get moving though, and as the CCG’s like-minded and trusted partner, are already speaking with a wide range of organisations and patient groups.

VC will continue to listen to patients and staff, to learn what works well and to incorporate all of this in their plans for the service.

While Primary Care services are not within the scope of this contract, GPs will continue to be engaged over coming months to ensure that they are closely aligned and involved in this programme.

The CCG and Virgin Care will continue the dialogue with local people, charities, voluntary groups and clinicians to make sure that are involved in the development of these services and the improvements deliver better outcomes for patients and staff.

Around 50 members of the public attended the meeting in Uttoxeter on 7th July and over 100 came to the Pirelli Stadium in Burton in May. More public events are being planned for the autumn.

While details of the implementation of the Improving Lives programme are still being worked through, it is possible to outline some of Virgin Care’s priorities, which include:

1. Improve the experience that patients and their carers have 2. Reduce unnecessary admissions to hospital 3. Integrate services 4. Embrace different approaches and providers 5. Engage the public in the shape of the services

Improve the experience that patients and their carers have VC will introduce integration across IT systems into the community teams so patients do not have to repeat their story over and over again.

Devise a robust information and education pathway with expert patients, and support delivery of this through voluntary sector.

Extend Carer’s Club to support carers.

Reduce unnecessary admissions to hospital VC will introduce risk assessment of ‘frequent flyers’ and ensure proactive community care through care coordinators to help people stay well and healthy at home.

There is a need for mental health professionals to play a role (even though it falls outside of the scope of this contract), as more than 25% of people over the age of 65 admitted through A&E have a mental health condition (dementia being the most common).

Greater provision of care in the community, closer to home.

Integrate services There will be roles for specialist GPs focused on specific Long Term Conditions.

Introduce data analytics to identify ‘at risk’ groups and proactively target them with support.

Create single access points to make getting through to the right person easier.

Also creating an integrated health and care record, supporting patients to only have to tell their story once and making sure all of a person’s health professionals have the right information at the right time.

Embrace different approaches and providers Establish care coordinators to support people with their care.

Enable staff with mobile working technology to improve community based care.

Subcontract to voluntary sector organisations and involve them more closely in care pathways.

Engage the public in the shape of the services

VC will be setting up a Citizens’ Panel to ensure the services offer the very best outcomes for patients and that all local residents have the opportunity to be informed, asked or involved in the design of the services.

Patient feedback will be measured in innovative ways. VC are a feedback focused organisation and were one of the first providers to introduce the Net Promoter Score into their services and were the first provider to get more than 1 million pieces of patient feedback. There will be a variety of different feedback methods that suit different groups.

Continue meeting with public in open meetings, to hear views and act upon them.

Virgin Care’s solutions may include:

• offering people access to local support networks where they can meet and socialise, either face to face or online, with others who have similar conditions

• targeted education programmes which provide information and support to enable people to make better lifestyle choices

• tailored telephone helplines led by clinically trained professionals which will offer information and support to people during difficult times

• Supporting more people earlier in the management of their condition

• Where appropriate, using modern techniques such as Telehealth, Skype, Mobile Apps and the Internet

• Helping those people who want to and are able, to self-care through education and skills development.
