law the most sweeping healthcareoverhaul in generations.

QUESTION: Now that the bill is law,how do I sign up for my new insurance?

ANSWER: Although there are someprovisions that take effect in 2010— such as bans on lifetime limits andthe denial of coverage to children withpre-existing conditions — the part of thebill that provides new insurance optionsdoes not take effect until 2014.

By then states will have createdinsurance exchanges, possibly online,where people who don’t get insurancefrom an employer or the government willbe able to shop for policies that meetfederal standards.

The policies sold on exchanges will beregulated by state and federal authoritiesand will be subsidized for many low- andmiddle-income Americans.

QUESTION:What’s to stop my insurerfrom raising my rates between now and2014?

ANSWER: Under the new law, statesand the federal government must createa process for reviewing rate increasesstarting with plans issued in 2010.

Although this would not preventinsurers from raising rates in all cases,it is designed to discourage them frommaking unreasonable increases.

Unreasonably high increases couldprompt state authorities to exclude

those plans from theinsurance exchanges

when theyopen in 2014.

QUESTION: I have prescription drugcoverage under Medicare and will behitting the “doughnut hole” pretty soon.What will the bill do to help me pay formy drugs?

ANSWER: Under the Medicare PartD program, seniors pay 25 percent oftheir drug costs up to a yearly limit of$2,830. After that they must pay 100percent of the costs until their out-of-pocket total reaches $4,550, at whichpoint catastrophic coverage kicks in andseniors pay just 5 percent of the cost forthe rest of the year.

The difference between the yearlylimit and the point where catastrophiccoverage kicks in is called the “doughnuthole.”

In 2010, seniors who reach thecoverage gap will be eligible for a $250rebate.

Starting in 2011, the governmentand pharmaceutical makers will beginphasing in subsidies and discountson brand-name and generic drugspurchased in the gap.

By 2020, this program will haveeliminated the gap entirely by covering

75 percent of seniors’ drug costs up tothe catastrophic coverage limit.

QUESTION: I have a great insurancepolicy through my employer and I don’twant it to change. What’s this I hearabout taxing my plan?

ANSWER: Democrats proposedbringing down healthcare costs bytaxing “Cadillac plans”— high-costcoverage plans provided by employers.

Because employer-sponsored healthbenefits are not taxed, employerscurrently have an incentive to providebenefit increases rather than wageincreases.

The “Cadillac tax”— which takeseffect in 2018 — will cap the tax-freestatus of health benefits.

Insurers will pay 40 cents on everydollar they charge above $10,200 forindividual coverage and $27,500 forfamily coverage.

Elite athletes turnto chiropractic for

more than pain reliefTheWinter Olympic Games in Vancouver, British Columbia,

were fun to watch. But there were some painful moments as

well. You probably winced every time a skier lost control while

speeding down the slopes. Or let out a yelp when an ice skater

failed to land a jump and crashed to the ice.

Because you don’t attempt Triple Axels or race down icy

mountains at high speeds, youmight not think you have

anything commonwith the elite athletes who competed in this

year’s Olympic games.

But if you’re seeing a chiropractor for aches and pains from

training for a local marathon or lifting your grandchild, you’re

more alike than you think.

Doctors of chiropractic play an important role in helping

Olympic and professional athletes prepare for competition and

recover from injuries. In fact, their visibility on the U.S. Olympic

Committee’s medical staff has increased steadily over the years.

“We took five chiropractors [to the Olympic Games this year]

and that just shows how far chiropractic has come since the first

chiropractor joined the Olympic medical staff in 1980,”says Dr.

Michael Reed, a doctor of chiropractic who serves as a USOC

medical director. “It has a lot to do with the requests of the

athletes— they recognize the benefits of chiropractic in helping

their bodies perform at peak condition.”

“There is an increasing demand for the expert care of

doctors of chiropractic by athletes of all levels,”confirms

Dr. Rick McMichael,

president of the American

Chiropractic Association.

“But chiropractic’s unique,

drug-free approach can also

helpmembers of the general

public enhance their health

and wellness.

“In addition to the use

of spinal manipulation and

other manual therapies, DCs

offer patients exercise and

dietary recommendations,

ergonomic tips and

rehabilitative procedures— a

full package of conservative,

nonsurgical interventions,”

he adds.

Surprisingly, treating a nonathlete with aches and pains is very

similar to treating an Olympian, says Dr. Ted Forcum of Beaverton,

Ore., president of the ACA’s Sports Council. Forcum served on the

USOCmedical team in 2008.

“(As with Olympic and professional athletes,) it’s important

to ask patients what their goals for treatment are,”Forcum says.

“Is their goal just to be out of pain, or is their goal to function at

a higher level? In most cases, chiropractors treat elite athletes to

help optimize performance, not to treat an injury.”

With the rise in popularity of complementary and alternative

medicine and today’s emphasis on going green, many also

appreciate the option for care that is drug-free. Chiropractic

offers a natural approach to back pain, neck pain, headaches and

themyriad aches and strains that result from leading an active

life.

Even if you aren’t training for a marathon or thinking about

Hearing loss is one of themost common conditions in the

world. Yet there are a number of misperceptions about hearing

loss which leadmany to let it go untreated.

The Better Hearing Institute found that only 4 in 10 people

withmoderate-to-severe hearing loss use hearing aids. Many

waited nearly seven years after they initially learned about their

hearing loss to obtain a hearing aid, and that was after they’d lost

somuch hearing that their quality of life was affected. Hearing

loss can create social and emotional barriers for the individuals

living with it, or the families of those it affects. Research shows

that when left untreated, hearing loss can lead to reduced

earning power, disruptions in family life and can cause a wide

range of other psychological problems.

According to a survey conducted by Hear theWorld,

frustration (46.8 percent), isolation (45.3 percent) and fear (36.8

percent) were the feelings most often associated with untreated

hearing loss.

“What I see inmy practice is that the unnecessary fear and

frustration associated with hearing loss and hearing aids is often

accompanied by a lack of information about ways to prevent or

solutions available to treat the condition,”said Dr. Craig Kasper of

Audio Help Associates of Manhattan. “This is unfortunate given

that hearing loss is a condition that can be treated with great

benefit for the individual, as well as for society.”

Myth: Hearing loss only affects the elderlyFact:Only 35 percent of people with hearing loss are over

age 64. In fact, it affects all age groups.

• The number of Americans with hearing loss has grown

to more than 34million— roughly 11 percent of the U.S.

population.

• In the United States, more than onemillion school-aged

children have hearing problems.

• According to the National Institutes of Health, nearly 26

million Americans have high-frequency hearing loss that may

have been caused by exposure to loud noise at work or in leisure

activities.

Activities that put people at risk for noise-induced hearing

loss include target shooting, hunting, snowmobile riding,

woodworking and other hobbies, playing in a band and

attending rock concerts. Harmful noises at homemay come

from lawnmowers, leaf blowers, and shop tools.

Myth: If I have a hearing loss, my doctor would have toldme.Fact: According to the Better Hearing Institute, only 15

percent of physicians routinely screen for hearing loss during a

physical. And without special training, it may be difficult for your

doctor to realize the extent of your hearing problem.

Even“this won’t hurt a bit”shots tend to

leavemany people squeamish.

Studies have found that up to 10

percent of U.S. adults are so frightened

of needles that they’ll avoid getting any

medical treatment that might require

getting a shot.

But scientists and drug companies are

moving rapidly ahead with several new

technologies that aim to deliver vaccines

and other therapies needle-free.

One of the favored approaches is the

inhalable vaccine, which has been around

as a weakened live vaccine against the flu

— FluMist— for some time.

Researchers at the University of

Colorado at Boulder have developed a

prototype for a new generation of inhaled

vaccines based in part on studies that

were originally done to understand how

people inhale tiny airborne droplets of air

pollution.

Beyond eliminating needles and the

risks of infection associated with them,

the new inhalant system also offers a dry

vaccine that doesn’t require refrigeration

or other special handling. For measles, it

can bemade at a cost of about 26 cents

per dose— about the same as for an

injectable dose.

Another dry-powder system, developed

by the Connecticut-basedMannKind

Corp., has proved effective at delivering an

ultra-fast-acting dose of insulin through

the lungs, rather than by injection, giving

some diabetics a near-natural rush of the

hormone similar to that experienced by

non-diabetics immediately after a meal.

Another Colorado enterprise, a

company called Pharmajet, has developed

another no-needle technology that

actually uses spring-loaded pressure to

inject vaccine through the skin. A small

plastic cylinder full of vaccine does not

look like a needle, although there’s still a

tiny sting from the serum going through

the skin.

The U.S. Food and Drug Administration

approved the system in 2009 in time to

be used to deliver seasonal-flu shots in

Colorado and New Jersey the fall.

Several groups of researchers and

companies are also working toward

vaccine-patch delivery systems that would

carry active ingredients through the skin

using small particles.

One group, at the University of

Michigan, has designed a protein than can

carry vaccine through cell membranes into

the interior of cells, particularly immune

cells just under the skin. Tests onmice

have achieved activation of the immune

system equal to that brought about by

conventional vaccines.

Scientists at Queensland University

in Australia have been working on a

“nanopatch”vaccine that utilizes tiny

projections— to small to feel— that

nonetheless penetrate the skin deeply

enough to activate those same immune

cells. They’re still testing withmice, but

say one advantage of the system is that it

can achieve the same level of immunity

as shots, using about 100 times less actual

vaccine.

Most people recognize the link

between what they eat and their physical

health. But many don’t know that there

is a link between what they eat and their

mood. Literally, says Elizabeth Somer,

M.A.,R.D., what you eat or don’t eat for

breakfast can have an effect on your

happiness quotient by afternoon.

The effects also are cumulative: eat the

right foods for months, years, decades

and you will be that much happier and

mentally sharp in the years to come. In

short, follow these tips that come from,

“Eat YourWay to Happiness,” the latest

book by Somer, and she promises you will

say, “I never knew I could feel this good!”

Feel Good Tip #1: Eat breakfastPeople who eat breakfast have more

energy, a more sustained good mood,

they perform better at school and at

work, and they sleep better at night.

They also are less prone to food cravings,

have an easier time losing weight and

maintaining the weight loss. But, Somer

is not talking doughnuts and coffee. You

must follow the 1,2,3 rule. The breakfast

must have:

1) a whole grain to provide needed

high-quality carbs for the brain during the

morning hours

2) a little protein to keep you satiated

and maintain even blood sugar levels

throughout the morning

3) one, preferably two, colorful fruits

and vegetables.

Somer’s favorite is as a bowl of

whole-grain cereal topped with berries

and low-fat milk, and served with sliced

watermelon. Or, a morning smoothie

made with watermelon, lemon yogurt,

and dash of ground ginger, served with

whole-grain raisin bread.

Feel Good Tip #2:Keep lunch light and low-fat

Not only will a heavy lunch leave you

groggy, but eat too much fat midday

and it turns on a brain chemical, called

galanin. According to research from

Rockefeller University, the more fat we

eat, the more galanin we produce. People

who eat a fatty lunch are likely to eat

more calories later in the day compared

to someone who eats a low-fat lunch. You

definitely need some fat, like the healthy

fats in nuts and olive oil and the omega-3

fats, but don’t go overboard.

A light, low-fat meal helps you stay

alert through the afternoon hours, boosts

energy, and fills you up without filling

you out. An example would be a turkey

breast sandwich on whole wheat piled

high with spinach leaves and served with

a glass of low-fat milk and a fruit salad or

watermelon, orange slices and pineapple.

Feel Good Tip #3: Include super mood foodsSomer says that it is a style of eating,

not just a few foods, that will stack the

deck in favor of feeling great. The 10

secrets of happy people discussed in her

book include habits, such as focusing

on“real foods”not processed ones,

cutting back on the quick fixes, and

keeping meals light. If you are following

those guidelines, then adding super

mood foods gives you an even greater

nutritional bang for your buck.

Super mood foods are ones loaded

with vitamins, minerals, fiber, and

antioxidant-rich phytonutrients, while

being moderate in calories.

A perfect example is watermelon,

which is higher in lycopene than are

tomatoes, and is an excellent source

of vitamins A, B6, and C. Better yet,

watermelon is 92 percent water, which

means it is a natural hydrator, and it

contains two amino acids, citrulline and

arginine that maintain blood vessels in

the body.
