Dear Colleagues,It is with profound gratitude that I

write my final letter to our members. This past year has been full of hard work, learning, networking, and most importantly representing the interests of our members at the state and national level.

At the beginning of my tenure, there was a lot of uncertainty as to the future of healthcare delivery. A new administration was due to be elected as were a multitude of seats in the General Assembly that had the potential to alter the healthcare landscape significantly. Because of the efforts of the VAFP and particularly our legal counsel Hunter Jamerson and Legislative committee led by Dr. Jesus Lizarzaburu, the VAFP was able to advocate for bills that promoted primary care and protect the specialty of Family Medicine. There is no doubt in my mind that these conversations will continue for years to come but the foundation we have laid on our stance to protect Family Medicine has been recognized and will serve us well for years to come.

The next challenge on the horizon will be the topic of Medicaid expansion. The input from our constituency and the advocacy of the VAFP will be instrumental in developing an expansion plan that improves payment parity and encourages Primary Care participation. Rest assured that our members are in good hands with our incoming president Dr. Del Bolin and the VAFP Board of Directors are keenly aware of the challenges surrounding this particular and many other issues.

As for the past year, the VAFP Executive team has had the opportunity to represent the members of the VAFP at the state and national level. We have participated in conferences that

range from leadership development to state advocacy and everything in between. Through the course of these travels, I have had the unique opportunity to meet many of our members and hear your stories and why you chose family medicine. It is moments like these that give me great hope for the future of Family Medicine and I encourage any of our members that are interested in becoming involved to reach out.

The VAFP has also continued with the tradition of spectacular CME programming this past year. The VAFP held our annual meeting at

a new location at the Lansdowne Resort in Northern Virginia last July and the feedback was very positive. Coupled with our familiar location of Wintergreen Resort for our Winter meetings and regularly scheduled KSA/CSA’s, the VAFP had a very successful year of CME opportunities. The CME committee led by Dr. Mitch Miller works hard to schedule programs that are beneficial to the majority of our members and have laid the groundwork for exciting venues in the

years to come, including a return to the Homestead Resort!

With the lead of our Resident/Student Committee, Dr. Emmeline Gasink has helped renew our focus and energy on the future Family Medicine Physicians of Virginia. This has been done through outreach efforts and a renewed focus on student and resident representation at our Board Meetings amongst various other approaches.

I would also be remiss not to acknowledge the daily effort put in by the staff of the VAFP. Led by Mary Lindsay White, we could not ask for a better group of individuals to support our mission and values, so a big thank you to Mary Lindsay White, Cheryl Modesto, Matt Schulte, Kristi Reynolds, and Kristie Duncan!

Through all of the work that the VAFP does for its members and the plethora of interactions I have had over this past year, I am constantly reminded of the great potential we have as Family Physicians and the truly remarkable stories that have led us all to Family Medicine. Collectively I would like to thank all of you for the work that you do to promote the health and wellbeing of the citizens of this great Commonwealth!

I cannot reiterate enough the honor I have had to serve as the 71st President of the Virginia Academy of Family Physicians. I know I also speak for the Board of Directors and every committee member when I say that it is our true honor to serve the amazing Family Physicians of the Commonwealth of Virginia. We are actively recruiting Primary Care physicians for Duke LifePoint who are seriously considering relocating away from crowded cities and a hectic lifestyle to consider a more qualitative lifestyle change with treating 18 -20 patients daily and yet earning a comfortable living.

The practice opportunities offers the freedom of a more stable work life balance and to take better care of the patients providing them the individual service that they deserve.

Duke LifePoint offers a wealth of resources in every aspect of hospital operations such as medical and financial resources. TCRH was one of 750 hospitals awarded an ‘A’ by the Leapfrog Hospital Safety Grades for its efforts in protecting patients from harm and meeting the highest safety standards in the U.S.

TCRH is licensed for 141 private, acute-care hospital. The primary service area is 35,000 and the secondary service area is 6,000. Physicians will be part of well established busy practices consisting of 10 clinics with multi-specialty groups. The expected daily patient volume is 18-20 patients. Office hours are 8AM – 5PM Monday - Friday. The practice is electronically networked.

The primary care physicians would also be part of 10 clinics with multi-specialty groups and employed by Duke LifePoint. There are a total of three practices that are all well-established and currently have seven providers. The practices are all Outpatient practice; no inpatients averaging 18 - 20 patients per day.

The primary care physicians will be a referral base for specialists: Anesthesiology, Behavioral Health, Cardiology, Family Practice, ENT, Gastroenterology, General Surgery, Neurology, OB/GYN, Ophthalmolo-gy, Orthopedics, Pain Services, Pathology, Pediatrics, Podiatry, Radiology, and Urology.

The ideal candidate will be board certified or eligible M.D. or D.O.H-1 and J1 Visa candidates will be accepted.Compensation includes a strong financial package:Attractive competitive base salary of income guarantee generally for at

Legal and Legislative UpdateLEGAL AND LEGISLATIVE UPDATE

The 2018 Virginia General Assembly Session proved to be one of the most active in VAFP’s history. Scope of Practice, Medicaid Expansion, Physician Reimbursement, Administrative Burdens, and Legislated Practice of Medicine proved to be the dominating topics this year. Despite major political challenges, I am pleased to report that VAFP enjoyed major legislative successes in 2018. Your Legislative Committee continues to work tirelessly to advance the cause of Family Medicine in the General Assembly. Major initiatives this year included:

Nurse Practitioner Scope - Legislation to create a pathway to independent licensed nurse practitioners, House Bill 793, was signed into law by the governor and is now in the regulatory process. VAFP, in collaboration with the Medical Society of Virginia, the Virginia College of Emergency Physicians, and the Virginia Chapter – American College of Pediatrics, succeeded in substituting the introduced legislation (which would have allowed nurse practitioners to practice independently just 6 months after graduation from a Masters-level program) with legislation, patroned by physician-legislator T. Scott Garrett, M.D., which creates the most rigorous pathway to independent practice in the United States. VAFP is proud to say, patients are protected by the legislation which we worked to pass in the face of deeply concerning alternatives.

Emergency regulations implementing House Bill 793 must be in place by early 2019. Depending on the speed with which the Boards and the Governor act on those regulations, they could be in place sooner or much later. Regulatory deadlines are missed all the time. But it is reasonable to assume that there will be regulations in place by 2019, and that sometime in Q2 of 2019 we will see the first NP applicants for independent practice. We do not yet know how long it will take for the first approvals to go through. But by 2020, VAFP expects independent nurse practitioners to be practicing, and possibly as soon as mid-2019. In 2021, the Department of Health Professions must report back to the General Assembly on the impact of licensing independent nurse practitioners.

Doctors of Medical Science – Legislation, Senate Bill 505, to create a new licensed health profession, Doctors of Medical Science, was continued to 2019 in the face of strong VAFP opposition. This bill, supported primarily by Lincoln Memorial University

which houses a Doctors of Medical Science program, would have licensed advanced-study physician assistants with a scope of practice tantamount to that of a primary care physician. While physician workforce shortages remain, VAFP continues to promote investment into existing physicians and into medical residency programs in order to meet the needs of Virginia’s patients.

Medicaid Expansion - The Governor has signed Medicaid Expansion into law. It took a protracted Special Session in order to break budget impasse over Medicaid eligibility, but a deal between key legislators in the House and Senate emerged at the end of May. Effective January 1, 2018, Virginia’s Medicaid eligibility limits will expand to 138% of the Federal Poverty Line. The Commonwealth’s share will be paid for by a tax on for-profit hospitals, although those hospitals will be partially reimbursed by an increase in Medicaid inpatient reimbursement rates. The enacted Medicaid expansion authorization retains a work requirement provision and a cost-sharing obligation for Medicaid participants. Physicians will have a role in determining when a patient’s medical condition exempts them from these requirements.

VAFP significantly engaged in the Medicaid Expansion debate this year, championing the sustainment of key administrative reforms to the Medicaid program necessary for physician participation. VAFP has also advocated, and continues to actively press, for increases to both in-patient and outpatient physician reimbursement. Ultimately, increased physician participation will be necessary to meet the needs of an additional 400,000 newly insured Virginians.

Credentialing – Sponsored by Delegate Chris Head, VAFP successfully advocated for legislation, House Bill 139, ensuring physicians are retroactively reimbursed for services rendered while third-party payer credentialing applications are pending approval. The Governor signed House Bill 139 into law. The legislation, opposed vehemently by Virginia’s Association of Health Plans, provides that commercial payers must reimburse physicians retroactive to the date of the credentialing application, if their application is approved, regardless of when the application is approved. This change, a major win for VAFP in the Commerce Committees, takes effect July 1, 2018.

Maintenance of Certification – Legislation to prohibit the Board of Medicine, Hospitals, and Health Plans from requiring maintenance of Board certification as a condition of licensing, privileges, or credentialing failed despite strong VAFP support. The American Board of Medical Specialties retained a lobbying team to aggressively oppose House Bill 157. VAFP has met with the American Board of Medical Specialties to discuss Maintenance of Certification and will work again in 2019 to secure legislative relief from this administrative burden and high-stakes certification process.

Step Therapy – VAFP continues to support initiatives to remove administrative barriers to patients accessing the drug regimen prescribed by their physician. Legislation in the Senate advanced further than ever in 2018. VAFP will continue to advance step therapy and other prior authorization reforms in 2019.

CBD and THC-A Oil – The march toward legal medical marijuana in Virginia continued this year. Senate Bill 726, now signed into law, provides that practitioners may issue a written certification for the use of CBD oil or THC-A oil for the treatment of or to alleviate the symptoms of any diagnosed condition or disease determined by the practitioner to benefit from such use. This is an expansive change from current law limiting the treated disease state to intractable epilepsy. As the Board of Pharmacy issues applications for THC Oil Processors, physicians are carefully considering the balance between Virginia’s prescribing laws and federal law.

Lyme Disease – Legislation to extend the sunset provisions on required Lyme Disease Disclosures did not advance in the General Assembly this year. Accordingly, effective July 1, 2018, physicians will no longer be required to provide handout information to patients regarding Lyme Disease testing.

Physician Assistants are essential members of the team-based approach to patient care, and Physician-PA teams add versatility to any clinical setting. Trained in the Medical Model, PAs practice in all medical settings and specialties. They are bridging the gap created by a growing provider shortage and Physician-PA teams are improving access to care. There are eight PA programs in Virginia. Nationwide, more than 6,000 individuals graduate from accredited PA programs in universities and academic health centers each year. PAs are keenly adapted to preventive care with their general medical background.

The AAFP’s Annual Family Medicine Advocacy Summit was held May 21-22 in Washington DC. A few hundred physicians, residents and medical students from all over the United States were in attendance. At this conference, the first day is dedicated to bringing all the members up to date on multiple issues that affect family medicine. Issues like Advanced Payment Models, Health Care Coverage, the Opioid Crisis, and Political Updates were presented and discussed. Afternoon workshops addressed other issues and prepared us for our interactions with legislators and their aides.

I would like to extend my gratitude to those members that attended and supported the AAFP’s efforts to change policy at the national level. I would also like to thank the members of the Virginia delegation for their efforts to attend the Family Medicine Advocacy Summit. The delegation from Virginia included the following physicians.

Please consider attending the Family Medicine Advocacy Summit next year May 20-21, 2019. Better yet, I invite you to reach out to your legislator or legislative aide and get to know them and establish a relationship. By doing so, you can help shape future legislation that serves our patients and our discipline.

Below are my thoughts on ways to become more involved and to interact with legislators.

GETTING INVOLVED

Although it may seem quite a bit intimidating to get involved in activities like this, the truth is that it is not that difficult to stay involved. It all starts with a phone call, or an email to your legislator and/or their aide. The first thing to do is to start a relationship with your legislator. Many times, it is best to reach out to them when they are off session. A meeting in your local district office goes a long way. Even if you do not get to meet with the legislator, establishing a relationship with their legislative aide will serve you well in the long run.

READING YOUR LEGISLATOR

Think about the best medical sales representative that you ever interacted with. Who comes to mind? I would venture to say that that person was not very pushy, was respectful of your time, was very tactful in reminding you of their expertise and what product they were selling, and more importantly, always seemed to be an asset to your needs and not just another sales person.

Just like that medical sales representative that met your needs, on one particular visit, we realized that the issues we were bringing forward from the AAFP to the legislator were not in line with the needs of the particular legislator. We realized it would be pointless to bring a message that would fall on deaf ears. As a result, the strategy was changed to reintroduce our member to the legislative aide and simply ask what medical issues the legislator was working on that we could help with. As you can imagine, that worked a lot better than our previous experiences in which we tried to bring a message that was contrary to their political position.

GETTING RESULTS

You have to remember that legislators, just like doctors pursue their office because of some sense of duty. Yes, one can argue that many of them can appear to forget what brought them there in the first place, but at the end of the day they started with good intentions. Many times, they vote in ways that we don’t understand because they do not understand an issue like we do. Especially when it comes to medical care and the delivery and practice of medicine, it is us who have the expertise. Therefore, all we need to do is make ourselves available. My advice to those who are new to advocating is to become an asset to your legislator.

VAFP Leaders Attend ACLF and NCCL

Ten family physicians represented the VAFP at the Annual Chapter Leadership Forum (ACLF) and the National Conference of Constituency Leaders (NCCL) April

26-28, 2018 in Kansas City, MO. VAFP President Rupen Amin, MD, MBA, VAFP President-Elect Del Bolin, MD, PhD, VAFP First Vice President Emmeline Gasink, MD,

VAFP Director Jerome Provenzano, MD, VAFP Past President and AAFP Director Sterling Ransone, MD attended ACLF and Drs. Timothy Yu, Denee Moore, Emily Young, Robin Anderson and Susan Osborne attended NCCL.

ACLF is the AAFP’s leadership development program for chapter-elected leaders, aspiring chapter leaders, and chapter staff. Among other roles, ACLF functions as an orientation for emerging leaders who serve on chapter boards, as well as professional development for new and seasoned chapter staff. Drawing more than 200 attendees each year, ACLF also features targeted breakout sessions on chapter governance, advocacy, and communication.

NCCL is the AAFP’s leadership and policy development event for underrepresented constituencies. NCCL serves as a platform for different perspectives and concerns of AAFP members to help bring about change. The five constituencies with representation include: Women, Minorities, New physicians (in the first seven years of practice following residency), International Medical Graduates (IMG), from schools outside the U.S., Canada, and Puerto Rico and gay, lesbian, bisexual, and transgender (GLBT) physicians or physician allies. At NCCL, physicians develop skills to advocate for issues that are relevant to specific constituencies, practices, the specialty and patients.

The Virginia Academy of Family Physicians (VAFP) is accepting nominations for the 2018-2019 elected Resident and Student Director positions on the VAFP Board of Directors. The VAFP Board of Directors is the governing body of the VAFP and is comprised of physicians across the Commonwealth from many different practice types and geographic regions. The elected Resident and Student Directors are voting members of the Board, funded to attend required meetings, and serve a one-year term beginning in November 2018.

DUTIES/RESPONSIBILITIES

• Be a member of the VAFP.

• Review meeting agenda and supporting materials prior to each Board meeting.

• Attend the AAFP National Conference of Family Medicine Residents and Medical Students scheduled July 25-27, 2019 in Kansas City, Missouri and serve as the Virginia Delegate.

“CHOOSE VIRGINIA” SCHOLARSHIPS

The Virginia Academy of Family Physicians (VAFP) Foundation is delighted to offer a medical student scholarship and a resident scholarship in 2018 for assistance with education loan repayment. Many medical students and family medicine residents in Virginia have expressed an interest in staying in Virginia after their graduation to complete a family medicine residency or to practice in a medically underserved area. Over the past two years the VAFP Foundation has received donations to support this effort to keep our own.

This year the Foundation will offer one graduating Virginia medical student a $1,500 scholarship. The successful candidate will meet the following criteria:

• Graduating Virginia medical student in good academic standing.

• Entering a Virginia family medicine residency program. A letter from the program director is required.

• Will have completed the PGY-1 year successfully and be entering into the PGY-2 year at that program. A letter from the program director is required in order to receive the scholarship check for loan repayment at that time.

• Must submit a 250-500 word letter explaining why he/she wants to practice family medicine in Virginia as a resident and as a family physician.

In addition, we are offering one $2,000 scholarship for a graduating PGY-3 Virginia family medicine resident who wishes to practice in an underserved area of Virginia. The requirements for this scholarship are as follows:

• Graduating as a PGY-3 Virginia family medicine resident

• Have an up-to-date Virginia medical license.• Practicing family medicine in an underserved

medical area after graduation as designated by United States Human Resource Service Administration (HRSA) and/or Virginia Department of Health. A letter verifying this is required.

• Must submit a 250-500-word letter explaining why he/she wants to practice in an underserved area of Virginia.

• Practicing at the underserved site for one year. Documentation is necessary to receive the scholarship check for loan repayment after one year of practice in an underserved area.

M. Norman Oliver, MD, MA has been appointed State Health Commissioner by Governor Ralph Northam. Since April, he has served as acting State Health Commissioner and prior to that position, he served as the Deputy Commissioner for Population Health at the Virginia Department of Health. In his role as the Deputy Commissioner for Population Health, Dr. Oliver worked with others in the health department, other state agencies, and healthcare systems across the state to improve the health and well-being of all citizens of the Commonwealth. He remains committed to a cross-agency and multi-sector approach to implementing population health initiatives.

Improving population health requires addressing the health-related social needs of all Virginians, including the Commonwealth’s most vulnerable communities. Dr. Oliver has a long record of accomplishment, studying health inequities, particularly as it affects racial and ethnic minorities, has been the focus of his research. Most

recently, his research interests lie in the area of improving our understanding of the role of racial discrimination, bias, and prejudice in establishing and maintaining these health inequities and the understanding of the interplay between race and socioeconomic position in these disparities. Improving our understanding of racial and ethnic health disparities and developing effective interventions to reduce or eliminate them are major goals of the health department.

Before accepting the Deputy Commissioner position, Dr. Oliver was the Walter M. Seward Professor and Chair of the Department of Family Medicine at the University of Virginia

School of Medicine. As Chair, Dr. Oliver helped lead the transformation of the Department’s clinic sites into patient-centered practices focused on population health. While serving as Department Chair at the University of Virginia School of Medicine, Dr. Oliver also served as an Ex Officio Member on the VAFP Board of Directors.

Dr. Vaughn, of Suffolk, joined the Board in 2018. He has more than 25 years of experience as a family physician and leader within Sentara Medical Group. Currently, Dr. Vaughn juggles patient care responsibilities while serving as the Associate Medical Director for Sentara Medical Group and Chair of Sentara Quality Care Network. A past president of the Virginia Academy of Family Physicians, he has held other leadership positions including medical director of Sentara Specialty

Rehabilitation and Medical Staff President of Sentara Obici Hospital. Dr. Vaughn received his Doctor of Medicine degree from Eastern Virginia Medical School and a B.A. in chemistry from the University of Virginia. He completed his Family Medicine residency at VCU-Riverside.

VAFP Member S. Hughes Melton, MD, MBA, FAAFP, FABAM Named Commissioner of the Virginia Department of Behavioral Health and Developmental Services

Dr. Melton is a leader, teacher and healer appointed April 2018 by Governor Ralph Northam to lead DBHDS and transform Virginia’s system for people with addiction, mental illness and developmental disability. The transformed system will provide a consistent level of high quality, accessible services to all Virginians through increased integration with primary care, efficiency of operations and accountability to stakeholders. Dr. Melton and his nearly 7,000 team members lead an agency that stewards $1.06 billion to operate fourteen state hospitals, collaborate with forty locally-run community services boards and license over 800 private providers. As stated by Dr. Melton, “I am thankful for the team I joined here as well as the crucial role primary care plays in the care of our citizens with mental illness. My thanks for all they do each and every day to take care of their patients.”

Prior to his role as Commissioner of DBHDS, Dr. Melton served as the Chief Deputy Commissioner of Public Health and Preparedness at the Virginia Department of Health (VDH). He has been active on the state and national levels serving on the boards of the Virginia Academy of Family Physicians and the Medical Society of Virginia and as President of the AAFP Foundation. In 2011, the American Academy of Family Physicians selected Dr. Melton to be the Family Physician of the Year for his innovative work in the primary care treatment of addiction and chronic

pain, and for his leadership in recruiting healthcare professionals to Russell County, what was once considered a medically underserved county.

Dr. Solomon has been selected to receive a 2018 AAFP Foundation Family Medicine Leads (FML) Emerging Leader Institute Scholarship. The year-long Family Medicine Leads (FML) Emerging Leader Institute focuses on ensuring the future of the Family

Medicine specialty by increasing the number of future Family Medicine leaders and provides training for this important role. Dr. Solomon is a second year resident at the Carilion Clinic-Virginia Tech Carilion School of Medicine Family Medicine Residency.
