SCHOOL NURSE-Medications
GMS has one full time nurse and one part-time nurse. The clinic is located on the first floor beside the athletic office.
If your child needs to take prescription or over-the-counter medications at school, written parent permission is needed. Please complete form 5330-F1. This is available in the health clinic or at:
http://www.goshenschools.org/parents-students/health-services/
Medications must be sent in their original container and will be given according to the label.
SCHOOL NURSE Illness and School Physicals
Students who become ill at school must report to the school nurse. Generally we will treat as needed and the student will return to class. If a student is too ill to remain in school, a parent will be contacted. Please make sure contact numbers are up to date.
Students must go through the health clinic. They can not call or text you on their own.
School Physicals (green form) need to be completed for 6th grade entrance. These are different than the yellow athletic form. Please return to the school nurse by August 2013.
IMMUNIZATIONS
New guidelines for immunizations started in 2010. The vaccines added for grades 6-12 are:– Tdap (tetanus, diphtheria and
acel.pertussis)– MCV4 Menactra (meningococcal)– 2nd Varicella (chickenpox) (or date of disease)
