What is GAMSAT?2The Graduate Medical SchoolAdmissions Test (GAMSAT)GAMSAT is a professionally designed and markedselection test developed by the Australian Councilfor Educational Research (ACER) for medicalschools offering graduate-entry programmes open tograduates of any discipline.

GAMSAT is appropriate for any student whohas completed an undergraduate honours degreeor who will be in the penultimate or final year of study for an honours degree at the time of sittingthe test, or, in the case of applicants to PeninsulaMedical School, who believes he/she has achievedan appropriate level of intellectual maturity andsubject knowledge to meet the demands of the test.GAMSAT is designed to assess the capacity toundertake high level intellectual studies in a medicalor dental course. It is offered once a year only.

GAMSAT evaluates the nature and extent of abilities and skills gained through prior experienceand learning, including the mastery and use of concepts in basic science, as well as the acquisitionof more general skills in problem solving, criticalthinking and writing. If your first degree is ina non-scientific field of study you can still sitGAMSAT and be chosen for admission to the

graduate-entry programmes. A science backgroundis not a prerequisite and academic excellence inthe humanities and social sciences is encouragedand recognised. However, it must be stressed thatsuccess in GAMSAT is unlikely without knowledgeand ability in the biological and physical sciences(see page 5).

Equity and FairnessGAMSAT is developed to rigorous professional andtechnical standards. Test questions are designed anddeveloped by teams of ACER test writers, expertin their subject areas, in close consultation withuniversity faculty specialists and medical schoolrepresentatives. All test questions must pass detailedpanelling, trialling, analysis and final review. Thecontent, style, duration and sequencing of the testsare determined to ensure the testing programme isrelevant, fair, valid and reliable.

GAMSAT test data are subjected to statisticalanalysis to check that each test item has performed asrequired. Test questions in development are carefullyscrutinised in an ongoing attempt to minimise gender,

ethnic or other forms of bias, and to ensure the test isculturally fair.

The test may contain a small number of trialquestions which will not be scored.

The GAMSAT Policy Committee has establishedan Ethics Committee to examine and report on allethical issues relating to the content and conduct of GAMSAT referred to it.

All GAMSAT candidates are asked to sign astatement acknowledging that their test resultsmay be used by relevant authorities for researchpurposes. Candidates are given an assurance thatany use of their registration and test records will betreated with the utmost confidentiality. Candidatenames will be separated from data in all cases.

Structure and ContentThe purpose of GAMSAT is to assess yourability to understand and analyse material, to

think critically about issues and, in the case of theWritten Communication section, to organise andexpress your thoughts in a logical and effectiveway. GAMSAT questions are based on materialdrawn from a variety of sources. They typicallyrequire candidates to read and think about apassage of writing, to interpret graphical displays of information, to use mathematical relationships andto apply reasoning skills to tables of data. Problemsolving is a major focus of the test.

The test is divided into three sections designed toassess performance in the areas of:

I Reasoning in Humanities and Social Sciences

    II Written Communication

III Reasoning in Biological and Physical Sciences

Since problem-based learning techniques arecentral to modern medical curricula, GAMSAT isconstructed with a major focus on the assessmentof problem solving ability across a wide range of subject areas.

Section I Reasoning in Humanities andSocial Sciences

The Reasoning in Humanities and Social

Sciences section tests skills in the interpretationand understanding of ideas in social and culturalcontexts. Different kinds of text will be usedas stimulus, including passages of personal, imaginative, expository and argumentative writing.Although most of the stimulus materials in thissection are in the form of written passages, someunits may present ideas and information in visualand tabular form. The stimulus will deal with a rangeof academic and public issues, with an emphasis onsocio-cultural, personal and interpersonal topics.

Questions in the Reasoning in Humanities and

Social Sciences section demand varying degrees of complex verbal processing and conceptual thinking, logical and plausible reasoning, and objective andsubjective thinking.

Questions that emphasise understanding involvethe recognition of explicit and implicit meaningsthrough close reading of words and phrases andglobal interpretations of text.

Questions that emphasise plausible reasoning involveinterrelating, elaborating and extending conceptsand ideas, and drawing conclusions.

Questions that emphasise critical thinking requirecandidates to make discriminations and judgments inthe realm of plausible reasoning.

Questions in this section are in multiple choiceformat with four alternative answers from whichcandidates choose the most appropriate.

Section II Written CommunicationThe Written Communication section is a test of the ability to produce and develop ideas in writing. It involves two thirty minute writing tasks. Eachtask offers a theme and a number of ideas relatingto that theme. The themes are general rather thanspecific in nature. The first task deals with socio-cultural issues, while the second deals with morepersonal and social issues. In selecting topics for thewriting tasks every effort has been made to minimisefactors which might disadvantage candidates fromnon-English-speaking backgrounds.

Written Communication is assessed on twocriteria. These criteria address the quality of thethinking about a topic and the control of languagedemonstrated in its development. Assessmentfocuses on the way in which ideas are integratedinto a thoughtful response to the task. Control of language (grammatical structure and expression) isan integral component of a good piece of writing. However, it is only assessed insofar as it contributesto the overall effectiveness of the response to the task

and not in isolation. Candidates are not assessed onthe correctness of the ideas or attitudes they display.

CRITERIA FOR THE ASSESSMENT OF WRITTENCOMMUNICATIONAssessors consider the following issues:

Thought and Content(the quality of what is said)

what is made of and developed from the taskthe kinds of thought and feeling offered inresponse to the task

Organisation and Expression(the quality of the structure developedand the language used)

the shape and form of the piecethe effectiveness and fluency of the language

Each piece of writing will be assessed by threeindependent raters.

Section III Reasoning in Biological andPhysical Sciences

In addition to testing problem solving within ascientific context, this section examines the recalland understanding of basic science concepts. Theseskills include the ability to identify knowledge in new

contexts, to translate knowledge from one symbolicform to another, to estimate measurements and torecognise limits in accuracy, to formulate hypotheses,to extrapolate and interpolate, to formulategeneralisations in the light of given relationships,to deduce consequences from models, to discoverrelationships, to analyse data, to make comparisons, and to follow a line of reasoning. Stimulus materialis presented in a variety of formats including verbal, mathematical, graphical and visual. Section III ismade up of questions in three discipline areas in thefollowing proportions:

    Chemistry 40%

    Biology 40%

    Physics 20%

Questions from the three discipline areas areintegrated throughout the section and are in multiplechoice format.

Questions in this section do not specifically requirethe use of a calculator. However, you may, if youwish, take a non-programmable calculator of yourown to the test centre for use in Section III only.

The level of subject knowledge required for theReasoning in Biological and Physical Sciencessection of the test generally corresponds to the firstyear of a university course in biology and chemistry.

    7/256 GAMSATUK

The test focuses primarily on problem solving andon the use of knowledge that will be presumed atthe beginning of the medical course: that is, a basicunderstanding of biology and chemistry (includingorganic chemistry) at first year university level.

A student who has mastered the concepts coveredin A Level physics, and in the first year of universitylevel chemistry and biology, should have adequateknowledge to answer questions in this section.

Successful GEM students will graduate withBachelor of Medicine, Bachelor of Surgery (BM BS)from the University of Nottingham.

There are two phases to the course: An 18-month foundation programme structured

around problem-based learning (PBL). Thefoundation programme is based at the purposebuilt Medical School on the Royal DerbyHospital site.

30 months of clinical training in healthcarefacilities across the mid-Trent region youll joinstudents from the undergraduate course for thisphase. For further information follow the link to:http://www.nottingham.ac.uk/gem/prospective/graduateentrymedicine/index.aspx

Problem Based Learning (PBL)The PBL Course is divided into a number of clinicalmodules reflecting the core medical specialitiesand systems of the body: Structure, function anddefence; Respiratory; Cardiovascular; Limbs andback; Alimentary; Endocrinology; Urogenital;Neuroscience and Integrative. Modules are thendivided into 6-7 patient case studies introduced inPBL on a weekly basis.

    Progress Assessment

Formative assessments occur throughout the PBL

Course and are taken as a progress indicator. Inaddition to this throughout the foundation course, students are required to produce a Portfolio of Personal and Professional Development (PPD)including drug reviews, patient case-studies from GPvisits, ethics coursework and reflective assignments.Summative assessments including written papers,on-line multiple choice & clinical reasoning exams, and clinical skills assessment occur at the end of year 1 and at the end of the 18 month foundationprogramme.

    The Clinical Course

Ward and clinical teaching is supported throughlectures, tutorials and e-education packages.Progress is also supported by logbooks andeducational portfolios.

The distinct phases of clinical training include:

    1. Clinical Phase I Clinical Practice Course

This provides core knowledge in common/

important conditions within medicine andsurgery. Teaching primarily occurs in twoforms, either in small groups by specialists or viaattachments to clinical firms. There are also acourse in therapeutics and a community followup project.

    2. Clinical Phase II

    Year 3 placements comprise two 20 week integrated rotations in Obstetrics andGynaecology; Child Health; Health Care of the Elderly and Psychiatry; Ophthalmology;

Otorhinolaryngology and Dermatology plus a 4week Special Study Module.

    3. Clinical Phase III

The Advanced Clinical Experience Courseinvolves rotation through four 9 week attachments: Medicine; Surgery; MusculoskeletalDisorders and Disability; General Practice andSpecial Study Module. This is followed by a9-week elective and, finally, a 4-week Preparationfor Foundation course that includes shadowingthe junior doctors you will tae over from at thestart of August.

Approximately 40 Special Study Modules areavailable during Clinical Phases II and III, providingstudents with the opportunity to delve deeply into aparticular area of clinical interest.

For our MBBS5 and MBBS4 courses we haverevised the selection process for those looking tostart in 2010 and beyond. This exciting approach toselection, involving a series of short interviews andsimple tasks rather than one single long interview, offers applicants a far greater opportunity to activelydemonstrate the qualities needed to be a gooddoctor (as laid out in the General Medical CouncilsTomorrows Doctors). This revised approach toselection was piloted with this years student intake

and was well received by applicants to both coursesas well as by the staff involved in the selectionprocess.

Building on our own research and on that of Medical Schools across the globe, it is clearthat selection through Multi Mini Interviewsis not only more reliable but ensures fairnessfor applicants. By giving the opportunityto actively demonstrate skills and abilitieswe believe that this process will fairly andobjectively evaluate those qualities deemedto select those best suited to a career in

medicine. Dr Muriel Shannon,MBB5 Admissions Tutor and Dr Aileen OBrien,MBBS4 Admissions Tutor
