YES NO Goals and objectives for this internship experience: A GPSI intern serving with the Serve Illinois Commission on Volunteerism and Community Service (Serve Illinois) will receive a broad range of government experiences. Goals include an understanding of how state government is organized and services delivered. There will be a strong emphasis on developing a collaborative environment with federal government programming to assist in strengthening the statewide network of volunteer support organizations. The intern will gain an understanding of the AmeriCorps program and other National Service grant programs. He/She will learn to provide effective technical assistance to programs involved in volunteer management and will support and promote volunteerism throughout the state of Illinois. Experiential learning outcomes for the intern based on the goals and objectives: The GPSI serving with the Serve Illinois Commission on Volunteerism and Community Service (Serve Illinois) within the Illinois Department of Public Health (IDPH) will assist with a wide range of duties associated with promoting volunteerism and national service in Illinois. Duties include but are not limited to: • Develop promotional materials and mailings related to national days of service, including 9/11 Day of Service and Remembrance, Martin Luther King Jr. Day of Service, and AmeriCorps Week. Coordinate mailings to individuals, elected officials, and community-based organizations across the state. Develop programs and activities for participation by Commissioners, elected officials, and other Department employees. • Develop and distribute monthly communications to programs and assist in the development of the Commission’s statewide newsletter, Serve Illinois, issued quarterly. • Track and monitor financial and progress reports for Commission programs. Send letters to programs that are not in compliance and follow-up with programs to ensure a plan is in place to ensure compliance. • Coordinate with training contractors to present at conferences and assist in the planning and preparation of training sessions and conferences. • Assist with volunteer recognition programs such as the Governor's Volunteer Service Awards • Develop materials and correspondence to improve awareness of and communication between national service

Participate in preparation activities for technical hearings conducted on some classes of permits. Obtain an understanding of the State's oil and gas regulatory process including routine inspections, records review, spill response and cleanup, the issuance of violations, and the administrative review process, including enforcement hearings. Learn how Department's oil and gas records are kept and reviewed and how to search for information within those records. Gain an understanding of the legislative process. Create tangible work product such as position papers, revised forms, research projects, procedures and other documents that can be added to the Interns portfolio. YES x NO Goals and objectives for this internship experience: The Center for Rural Health works with Illinois rural and urban underserved communities and health care providers to increase access to health care, improve the quality of the health care provided and address health disparities for Illinois residents. The intern will assist with the administration of the State Loan Repayment Program, a federal/state partnership to enable health care professionals to pay down their educational loans while working in a health professional shortage area in Illinois. The intern may also assist with data collection and analysis and the monitoring of the placement of foreign medical graduates in Illinois who are in the J-1 Visa Waiver program. Experiential learning outcomes for the intern based on the goals and objectives: The intern will be exposed to health care providers and sites such as hospitals and health systems and learn how to communicate with each of them to reach the program’s goals. Accordingly, will this internship require the intern to drive an agency motor pool vehicle to fulfill their internship experience?

YES NO Goals and objectives for this internship experience: Program Remote Monitoring System Displays with Visual Basic and Wonderware eDNA software. Create, maintain and update software for displaying IEMA remote monitoring systems (RMS) data using eDNA applications. Program Visual Basic code for analysis of RMS data. Assist in maintenance of the Reactor Data Link (RDL) and eDNA Web databases. Perform QA/QC testing of programmed displays, monitoring points, and event manager. Experiential learning outcomes for the intern based on the goals and objectives: The intern will gain proficiency at utilizing AVEVA's eDNA applications for creating graphical displays and managing data point catalogs and databases. The intern will learn how to use IEMA graphics display software for displaying remote monitoring systems data. The intern will gain proficiency in programming languages such as Visual Basic. The intern will gain proficiency in using Microsoft Excel software and SQL Management Studio. The intern will become familiar with nuclear power plant and health physics terminology.
