Nursing UpdateMagnet
In 2002 SJHC became a Magnethospital. SJHC, along with SEHC werethe first hospitals in Ohio to achieve thisprestigious award. Magnet statusrepresents excellence in nursing care basedon evidence based practice, nursinginnovation, strong, participative nursingleadership, and nursing research. Only372 hospitals across the country haveachieved Magnet status. In 2006, SJHCwas re-designated and currently we arein the process of seeking a secondre-designation. The nursing document thatsupports and provides evidence of meetingall of the Magnet standards was submittedon October 1st. The document is quiteextensive and is quite an accomplishment.We are awaiting word that our document“passed” and we can expect surveyors tovisit us in late December or January 2011.
MEWSOn November 1, nursing implemented
the Modified Early Warning System(MEWS). This scoring system identifiesadult patients at the earliest signs ofdeterioration so interventions can beimplemented. The scoring system is basedon heart rate, systolic BP, respiratory rate,temperature, and level of consciousness.The MEWS score is calculated beforepatients are transferred from the ED orIntensive Care, upon admission to thenursing units and every eight hours.(Exceptions include DNR, hospice, L & D,and pediatric patients.) You will be notifiedwhen the patient has a higher MEWSscore.
E-PrescribingAvailable at HMHPPharmacies
The St. Elizabeth and St. Josephemployee and ambulatory pharmacy staffencourage physicians to send patientprescriptions via the e-prescribing system.We are enrolled with Surescripts and can beidentified in the system as follows: SEHCEmployee Pharmacy, SEHC AmbulatoryPharmacy and SJHC AmbulatoryPharmacy. Over the past 100 years of HMHP, theMedical Staff Offices of all three hospitalshave accepted physician staff credentialinginformation (State Licenses, CME, etc.) viaa manual process. Once a physicianprovides the Medical Staff Office(s) withtheir credentialing application orre-application information, the staffextracts the information and subsequentlyplaces it into our database called Cactus.As you can imagine, this is a timeconsuming and labor intensive process. Forthis reason, we have been exploring ways ofimproving the credentialing andre-credentialing process via an on-lineapplication platform which is an add-onapplication to the Cactus software. Thison-line credentialing and re-credentialingprocess is configured for our medical staffand allied health professionals. Theapplication is called AppCentral, and it isanalogous to the on-line application thatthe State Medical Board of Ohio uses for asimilar purpose.
AppCentral helps streamline theapplication process so that HMHP and itsmedical staff/allied health professionals canbe more productive with their time.
Physicians access a user-friendly webbrowser from any location (office, home,etc.) to submit their applications,documents, and copies of licensure(s).Common information is entered one timeto populate multiple documents, which inturn provides time savings for the applicantand the medical affairs staff. In addition,the re-appointment applications arepre-populated for quick on-line review andsubmission. The on-line application allowsfor drop-down pick lists, medical schoollook-ups, and automatic reminders.
AppCentral appears to be a completeon-line application solution that will allowour medical staff and allied healthprofessionals an easier way to submitdocuments accurately through the web.This will result in reduced turnaround timeand will speed up the approval process.We are planning to roll out AppCentral atall three HMHP facilities in the firstquarter of 2011, so please be attentive toany mailings from your respective hospital.

St. Elizabeth Acute RehabilitationUnit is a 30-bed unit with the goal to havepatients regain as much independence aspossible. Acceptable diagnoses to beadmitted to the unit are: stroke,amputation, orthopedic, brain injury,burns, spinal cord injury, pain, multipletrauma, neurological disorders andarthritis. An Interdisciplinary Teamconsisting of physician, rehabilitationnurse, physical therapist, occupationaltherapist, speech therapist, social servicespecialist, case management, psychologist,recreational therapist and dietary specialistcollaborate to organize a plan of carespecific to each patient.
The team, headed by the physician,meets weekly to discuss the goals, progress,current treatment and discharge plan foreach patient. The patient’s family isscheduled for family education sessions andactually participates in therapy so that theyunderstand the patient’s therapy programand challenges.
Two requirements for admission to theunit are that the medical condition of thepatient is reasonable and necessary forrehabilitation services and that the patienttolerates 3 hours of therapy at least 5 daysper week. Patients actually receive therapy7 days per week.

• ALOS is 14 days• Patient satisfaction is 92.9%

HMHP Ambulatory Go LiveOn Monday, Nov. 1, 2010, CarePATH
went live in 29 Humility of Mary HealthPartners (HMHP) physician practices at 15different ambulatory sites in Youngstown,Warren, and Boardman. There was a fullystaffed command center at the HMHPFinance Building in downtownYoungstown as well as onsite support staffat each location. The support staffincluded individuals from Epic, CatholicHealth Partners (CHP) CarePATH andPower Users from our region, as well asother CHP regions. As with theimplementation of any major system, therewere glitches that came up; however, thesewere identified and handled quickly. Thecommand center closed at the end ofbusiness Friday, Nov. 12. Ongoing supportwill be available through the CHP HelpDesk and our local clinical and IT supportsystems. Work to improve and optimizethe system will be an ongoing process.
Affiliated Physician OfferingHMHP is continuing to work on
offering affiliated physicians theopportunity to purchase CarePATHambulatory for their own offices. HMHPwill subsidize the system to the maximalextent allowable by law. The advantage tothe affiliated physicians is that theirpatients will be truly “one patient, onerecord.” Anything that happens in theiroffice will be available to the health careteam caring for their patient in the hospitaland vice versa.
Site assessments will be available forphysicians interested in determining if theCarePATH ambulatory EHR would fittheir office operations. The assessment isfree and enables members of theCHP/HMHP affiliated physicianCarePATH team to understand theequipment and infrastructure needs of thepractice. Following the assessment, atentative contract is developed andprovided to the physician for review and to

decide if they want to proceed withimplementation. Site assessments areexpected to begin in mid to late Dec. 2010.It is anticipated that the first affiliatedphysician practice installations will start inthe first quarter of 2011.

Inpatient Roll-outsCHP went live with the CarePATH
inpatient product on Feb. 21 at MercyFairfield in Cincinnati. On Nov. 6, MercyAnderson and Mercy Clermont hospitals inCincinnati went live. Jewish Hospital inCincinnati will be the fourth CHP hospitalto Go Live with the inpatient system inmid June 2011.
The HMHP CarePATH steeringcommittee is continuing to work onpreparations for Go Live of CarePATH atall three HMHP hospitals. Our officialinpatient kickoff will be in Jan. 2011 andour tentative Go Live date for all threeHMHP hospitals is in Aug. 2011. Following the kickoff, there will benumerous opportunities for the staff andphysicians to learn more about theCarePATH inpatient system.

With the assistance of TinaCreighton and HMHP Public Relations,the department has developed a websitethat will be an accessible site not only forthe community to view, but also for staff,personnel, and future students andresidents. The website will containinformation about surgical procedures andtechniques, the environment and dynamicsof the operating room, the surgical sectionsand faculty, and our surgical facilities. Weare proud of our heritage and history inYoungstown, Ohio, and include the detailsof our historical beginnings of the surgerydepartment and its evolution from 1911 topresent day operations. Surgical faculty willbe highlighted to recognize our governingbody, national association and committeeleadership, and department news andevents that are noteworthy and exceptional.

Physician Licensure RenewalForm Redesigned
On October 4, 2010, the MedicalBoard launched a redesign of the on-linephysician renewal application form. Inaddition to the standard questions, the newform includes a series of mandatorydemographic questions to provide dataupon which policy makers can makedecisions based on a more accurateillustration of the Ohio physicianworkforce than has ever before beenavailable.
The demographic questions ask thenumber of hours per week a physicianspends in direct patient care and otheractivities; the clinical setting of suchactivities; the county and zip code ofthe location(s) the physician provides themost patient care; whether the physician isa solo practitioner, part of a group practice,or employed by a clinical facility orhospital; whether languages other thanspoken English are available at the primarypractice location; and board certificationstatus. All of the demographic questions on
the on-line renewal form can be answeredby choosing the correct response from adrop down box. Answers must be providedto the questions in order to continue therenewal process. The average time tocomplete the on-line renewal remains 15minutes or less. Licensees are responsiblefor assuring that the information providedto the Medical Board is complete andaccurate.
License renewal notices are sent by theMedical Board six months before thelicense expiration date. Licensees with lastnames beginning with C-D will be the firstgroup to use the redesigned renewal form.

Each surgical section is individualizedwith information about the latestprocedures and technology, along with thenames and biographies of section members.Each section will have a list of theprocedures performed by their surgeons.

TheDepartment of Surgery follows the missionand guidelines of HMHP in thedistribution of funds for furthering theeducation of our surgical residents as wellas attending surgeons and to enhance ourresearch efforts in the biomedical,community health and behavioral sciences.

Nursing UpdateMagnet
In 2002 SJHC became a Magnethospital. SJHC, along with SEHC werethe first hospitals in Ohio to achieve thisprestigious award. Magnet statusrepresents excellence in nursing care basedon evidence based practice, nursinginnovation, strong, participative nursingleadership, and nursing research. Only372 hospitals across the country haveachieved Magnet status. In 2006, SJHCwas re-designated and currently we arein the process of seeking a secondre-designation. The nursing document thatsupports and provides evidence of meetingall of the Magnet standards was submittedon October 1st. The document is quiteextensive and is quite an accomplishment.We are awaiting word that our document“passed” and we can expect surveyors tovisit us in late December or January 2011.
MEWSOn November 1, nursing implemented
the Modified Early Warning System(MEWS). This scoring system identifiesadult patients at the earliest signs ofdeterioration so interventions can beimplemented. The scoring system is basedon heart rate, systolic BP, respiratory rate,temperature, and level of consciousness.The MEWS score is calculated beforepatients are transferred from the ED orIntensive Care, upon admission to thenursing units and every eight hours.(Exceptions include DNR, hospice, L & D,and pediatric patients.) You will be notifiedwhen the patient has a higher MEWSscore.
E-PrescribingAvailable at HMHPPharmacies
The St. Elizabeth and St. Josephemployee and ambulatory pharmacy staffencourage physicians to send patientprescriptions via the e-prescribing system.We are enrolled with Surescripts and can beidentified in the system as follows: SEHCEmployee Pharmacy, SEHC AmbulatoryPharmacy and SJHC AmbulatoryPharmacy. Over the past 100 years of HMHP, theMedical Staff Offices of all three hospitalshave accepted physician staff credentialinginformation (State Licenses, CME, etc.) viaa manual process. Once a physicianprovides the Medical Staff Office(s) withtheir credentialing application orre-application information, the staffextracts the information and subsequentlyplaces it into our database called Cactus.As you can imagine, this is a timeconsuming and labor intensive process. Forthis reason, we have been exploring ways ofimproving the credentialing andre-credentialing process via an on-lineapplication platform which is an add-onapplication to the Cactus software. Thison-line credentialing and re-credentialingprocess is configured for our medical staffand allied health professionals. Theapplication is called AppCentral, and it isanalogous to the on-line application thatthe State Medical Board of Ohio uses for asimilar purpose.
AppCentral helps streamline theapplication process so that HMHP and itsmedical staff/allied health professionals canbe more productive with their time.
Physicians access a user-friendly webbrowser from any location (office, home,etc.) to submit their applications,documents, and copies of licensure(s).Common information is entered one timeto populate multiple documents, which inturn provides time savings for the applicantand the medical affairs staff. In addition,the re-appointment applications arepre-populated for quick on-line review andsubmission. The on-line application allowsfor drop-down pick lists, medical schoollook-ups, and automatic reminders.
AppCentral appears to be a completeon-line application solution that will allowour medical staff and allied healthprofessionals an easier way to submitdocuments accurately through the web.This will result in reduced turnaround timeand will speed up the approval process.We are planning to roll out AppCentral atall three HMHP facilities in the firstquarter of 2011, so please be attentive toany mailings from your respective hospital.
