The Community Foundation is interested in learning about the results of your capacity building efforts, the lessons you learned, and how those lessons will be applied in the future. We see this grant evaluation as a joint management tool to obtain feedback to improve programs and stimulate good planning. 2. Capacity Building Initiatives and OutcomesReferring to your initial grant proposal, please list each initiative, measureable goals and the outcomes for each.

As stewards of charitable donations, the Foundation must account for the monies disbursed through the granting process. Therefore, this grant evaluation is due within one year of the grant funds being requested. If the evaluation is not received by the due date you will not be eligible to receive grants from the Community Foundation during the next grant cycle.
