Employee Benefits Plan Monthly Cost Calculator

The Basic Plan for single coverage consists of Life Insurance, Accidental Death &Dismemberment coverage, and Extended Health Care coverage. For family

coverage, the basic plan also includes Dependent Life Insurance.

1. Circle the amount of Life Insurance coverage you wish to apply for. This is also the amount ofyour Group’s Accidental Death and Dismemberment coverage. Your selection is applicable for allemployees in the Group.

2. Enter the province of residence you and your employees reside in: _____________________________

3. Using the rate calculator page, complete the monthly cost calculator by following these simple steps:

(a) Refer to the employee’s enrolment form to determine either single or family coverage, and the age band that each employee falls under.

(b) Determine the appropriate rate grouping for each employee, based on your answer to questions 1 and 2.

(c) Place the number of employees and corresponding rate in Chart A for single coverage.For family coverage, do the same in Chart B.

(d) Calculate the total.

4. Dental Coverage is optional. If you choose this selection, all employees must participate in theDental Plan.

(a) Write the number of employees for both single and family coverage and the corresponding rate in Chart C.

(b) Calculate the total. This is your monthly premium.

5. Place the totals from Chart A, B & C into Chart D. Calculate the total.

For a Long-Term Disability* quote, please fill out the Request for LTD form and submit with your application.

If LTD coverage is selected, the cost for this coverage will be added to the total monthly cost.

Please enclose a cheque for the 1st month premium with your application.

* Long-Term Disability may not be available in all plans.
