Eastern Shore Rural Health System, Inc.’s governing Board of Directors continues to serve as the voice of our community, representing the needs of a diverse population. It has been my pleasure to work with this passionate and committed group of directors in leading ESRHS to carry out its mission of enhancing the quality of life for people on the Eastern Shore. The diligence and activism of the board members has been recognized through their work in grassroots advocacy – sharing the goals, values, and needs of this worthwhile organization with local, state, and national legislators.

I am pleased to welcome dynamic new members to the Board. The experience and skill sets of our members enhances the board’s functionality. Not only do we have Shore representation from both counties, but we are also blessed with a variety of professional backgrounds that continues to enrich and engage us in our work. We are consistently recruiting new board members, so you are encouraged to reach out if you have an interest in joining!

One of the exciting elements of serving on a working board is the opportunity to connect with others from all over the Shore, with the ultimate goal of leaving our community a better place than we found it. ESRHS collaborates extensively with numerous area partners – Community Services Board, Accomack County Public Schools, and the Eastern Shore Health District – to name just a few. Each of our directors serves on a committee, and many enjoy serving on multiple committees. These committees range from finance and personnel, to advocacy, strategic planning and development, to compliance, and more. There are incredible opportunities for training and networking throughout the year – and these opportunities continue to yield meaningful results.

In closing, I am grateful to the directors for their dedication and commitment to fundraising for the new Eastville Community Health Center – we proudly celebrated the milestone of breaking ground on the new facility this past August! Now that construction is well underway, I look forward to witnessing our dream become a reality.

We use the slogan, “Built by the community, for the community” to honor the efforts and generosity of our friends and neighbors throughout the Eastville Capital Campaign. As president of the board of directors, I sincerely thank you for your unrivaled support.

By The Numbers . . . A look at last fiscal year

While community health centers and the people who work in them may differ, there is one characteristic we share in common – and that is the amazing, compassionate, quality care provided to all patients. Over our 43 years, we have grown to serve as the healthcare provider of choice for 31,391 people. We continue to be here for those who have nowhere to go for healthcare, and for those who could go anywhere and choose ESRHS. From our awesome outreach to our amazing clinical team of medical and dental providers and nurses, to our astounding educators and business employees, we have one mission and that is to provide comprehensive, accessible, affordable care in a caring, professional and safe environment.

Our accomplishments are numerous this past year. Many thanks go out to our donors and employees who have made it possible for the groundbreaking of the new Eastville Community Health Center. Continued accolades go out to our employees for their focus on quality care yielding not only improved patient outcomes, but also grant awards allowing us to invest in our infrastructure. Congratulations to all who participated in our telecommunications and emergency preparedness drills held in our centers and those who supported our efforts in assuring our facilities are ready for community emergencies; ESRHS is your community health center, ready to respond to your community’s needs.

We continue to be overwhelmed with gratitude for the community support toward “raising the roof” of the new Eastville Community Health Center. Whether you volunteered on a committee for the capital campaign, purchased a silent auction item, golfed in the rain at the 2018 Eastern Shore Open or donated to ESRHS, the new Eastville Center will indeed be built by the community, for the community!

Not only are we building a state-of-the-art facility that will offer medical, dental, and mental health services as well as an on-site pharmacy, we continue to expand our dental program and outreach services, collaborate with numerous organizations on the Shore, and actively engage with local, state, and national legislators to raise awareness of the Community Health Center movement.

I am incredibly proud of the accomplishments of such dedicated staff and board of directors, and recognize that being the Shore’s “best kept secret” has remained a persistent challenge; but we have been making significant strides! From local presentations to cohosting the Healthcare Forum for Governor Northam, being featured in Virginia Health Care Foundation’s and Eastern Shore Healthy Communities’ Annual Report, to hosting visits for our commonwealth leaders, we remain steadfast in raising awareness about ESRHS. Please join us in this effort.

Dr. Gabriella Richardson

Dr. Gabriella Richardson was named Employee of the Year for 2018. This award recognizes an employee, nominated by employees and selected by the Board of Directors, who performs exceptionally and excels in all areas of his or her job responsibilities. Dr. Richardson, who joined ESRHS in 2016, is a pediatrician at Onley Community Health Center.

Her fellow staff members cited Dr. Richardson’s positive personality and her ability to both recognize areas of need before they become issues, and find innovative solutions to challenges. She also is recognized as a trustworthy, hard working person and great team player who gives great feedback and explanations.

Consolidated Call Center Opens

New Software Detects Gaps in CareEastern Shore Rural Health will soon be implementing software that will allow electronic point

of care identification of care gaps. The software, named Mediquire, will allow our care teams to identify and address issues not currently being taken care of in house during the patient visit or refer the patient to the appropriate specialist when needed. Examples of care gaps are patients who have not completed preventive care testing such as colorectal cancer screening, cervical cancer screening and breast cancer screening, necessary immunizations and chronic care gaps such as diabetes control, blood pressure control and appropriate treatment for heart and vascular disease. This tool will help ESRHS clinical support staff and providers address all of the patient’s needs.

Non-Health Care Needs Assessed In addition to new software to identify care gaps, patients are asked about factors in their lives,

known as social determinants of health, that can impact their health such as if they have a safe place to live and enough food to eat, and if they are depressed. A new support services brochure is available outlining services Eastern Shore Rural Health provides that can link a patient to help outside of the health care arena. This is done in recognition that health is determined in part by access to social and economic opportunities; the resources and support available in our homes, neighborhoods, and communities; and other conditions in which we live. Differences in living conditions explain in part why some Americans are healthier than others and why Americans more generally are not as healthy as they could be. This needs assessment is part of our Patient Centered Medical Home approach to both support your health needs and goals, and create a proactive plan for your care, after considering all factors that impact your health.

Eastern Shore Rural Health consolidated all of its call centers into one facility starting Aug. 6, 2018.The consolidated center was formed to more quickly and efficiently handle patient calls. Patients still dial the phone number for their center, but the call now goes to a centralized call center.

The center started with a staff of five business assistants who worked previously at different centers and one manager, Carrie Wilson, who was formerly the center manager at Bayview Community Health Center. A triage nurse and two more business assistants have since joined the call center to handle the volume of calls and accommodate patients in a timely manner. The center has increased the percentage of answered calls – before a patient hangs up - from around 50% to 85% to 93%. Call center staff are also able to make an appointment for you at all ESRHS facilities – you no longer have to call each center separately. Two dental business assistants may be added in the near future.

Use Patient Portal to Reach Us!With our patient portal you can ask your care provider questions that aren’t urgent, request

medication refills, and check lab results. You can also review and print your list of medications and review notes from your visit with a provider. Groundbreaking for new Eastville Community Health Center

held August 15A groundbreaking ceremony was held for the new Eastville Community Health Center on Aug. 15. Governor

Ralph Northam addressed attendees at the special event. The new medical and dental center replaces the aging Bayview and Franktown Community Health Centers and will open in late 2019.

The new facility will have more than 24,000 square feet compared to the 13,700 square feet that the current Bayview and Franktown have combined. By consolidating the two centers, ESRHS will be able to offer a state-of-the-art facility that will provide potential expanded hours to include 10-hour days and Saturday hours, medical digital x-ray, expanded lab services and hours, potential telemedicine and additional providers and staff. Medical digital x-ray is currently not at either facility. Additionally, the current Franktown has dental services, but Bayview does not, so the new center will allow residents in the southern part of Northampton County increased access to dental care.

A Request for HelpESRHS is a 501(c)(3) non-profit Federally Qualified Health Center. As part of the nation’s health care safety net,

we serve both insured and uninsured patients. The annual federal grant we receive can only be used to cover the medical and dental expenses of safety net patients who are underinsured or uninsured and meet federal poverty guidelines. The funds may not be used for construction. ESRHS plans to finance the project through a combination of federal and state grants, bank loans, and this capital campaign. As of press time, $10,000,506 has been raised to fund the new center.

Quality Performace RecognizedEastern Shore Rural Health was awarded another quality award by the Health Resources and

Services Administration (HRSA) this year. The top 30 percent of performers on HRSA clinical quality measures across the country were rewarded for superior overall clinical performance. ESRHS received $150,858 in quality money. HRSA is a federal agency that provides funding to ESRHS and other community health centers across the nation to provide care to uninsured and underinsured patients who pay a minimal fee based on family size and income.

ESRHS New Chief Nursing Officer Dr. Sandra (Sandie) Hearn, PNP joined Eastern Shore Rural Health March 19 as CNO. Dr. Hearn hails from Boston and recently retired from the U.S. Navy af-ter 30 years of service. She received her bachelor’s of science in nursing from Northeastern University, her master’s degree as a Pediatric Nurse Practitioner (PNP) from Catholic University of America, and her doctorate in nursing prac-tice from Old Dominion University in May 2018. She has extensive clinical and administrative expertise including serving as the CNO at the Naval Medical Center in Portsmouth with a 225-bed facility, nine outpatient clinics and 1000 nursing staff that she supervised upon her retirement. As CNO she supervises all ESRHS nursing and support staff. Dr. Sandra Hearn

Sentara Healthcare/Optima Health has awarded Eastern Shore Rural Health System, Inc. a $250,000 grant in support of the new Eastville Community Health Center. The grant is administered by the Sentara Foundation of Hampton Roads.
