Introduction: KAAUH Formulary is a continually updated list of medications and related information, representing the clinical judgment of physicians, pharmacists and other healthcare professionals, used in the treatment and/or diagnosis of disease and promotion of health. It should be evidence based, reflecting international guidelines, and cost effective to serve the health interests of the local population.
The aim of developing a formulary for King Abdullah bin Abdulaziz University Hospital is to serve as a guide for physicians, prescribers, and other health care professionals.
Preface: Drugs are listed in KAAUH Formulary by Pharmacologic-Therapeutic Classification. The American Hospital Formulary Service (AHFS) classification system was used in this formulary. This classification system is one of the most recognized methods for systemic classification of medications. Drugs are listed by generic name. Some brand names are included as a reference to assist in product recognition.
How to use this Formulary: There are two ways to find your drug within the formulary:

For example drugs used to treat a heart condition are listed under the category 2 4 :0 0 C ard io vascular D rug s. If you know what your drug is used for, look for the category name in the table of content, then look under the category name for your drug.

If you are not sure what category to look under, you should look for your drug in the Alphabetical list.

Formulary A formulary is a continually updated list of medications and related information, representing the clinical judgment of physicians, pharmacists, and other experts in the diagnosis, prophylaxis, or treatment of disease and promotion of health. A formulary includes, but is not limited to, a list of medications and medication-associated products or devices, medication-use policies, important ancillary drug information, decision-support tools, and organizational guidelines.
P&T Committee The P&T committee is responsible for managing the formulary system. It is composed of actively practicing physicians, other prescribers, pharmacists, nurses, administrators, quality improvement managers, and other health care professionals and staff who participate in the medication-use process. The P&T committee should be responsible for:
% Overseeing policies and procedures related to all aspects of medication use. % To the medical staff as a whole. % Medication-use evaluation (MUE). % Adverse-drug-event monitoring and reporting. % Medication-error prevention. % Development of clinical care plans and guidelines.

Addition of New Medication to KAAUH Formulary Replacement of existing drugs and superiority over existing drugs are two important areas for Committee consideration. In order to control the growth of the formulary, it is preferred that each addition be balanced by a deletion when practical. In any case, the Committee must be provided with a published scientific journal article (not an advertisement) demonstrating the efficacy of the new drug. Restricted use of a drug is possible if it is felt that a drug should only be used by certain members of the Medical Staff. This would mean that Pharmacy would not honor orders for the drug by non-approved physicians. Use rate of the new drug is essential to insure that reasonable quantities to insure both an adequate supply for regular, sustained use as well as to prevent ordering an excessive quantity that will never be used. Even though a drug is accepted for addition to the Formulary, it will not be ordered until this information is provided in a suitable form. The requesting physician should get the signatures of his Department Head before submitting the completed form to pharmacy and therapeutic coordinator .The requesting physician will be notified of the date and time of the Committee meeting to consider the request and may be invited to attend to lend support.
Evaluating Medications for Inclusion in the Formulary The P&T committee should use a structured, evidence-based process in the evaluation of medications for formulary consideration. The P&T committee should be provided with information that reflects a thorough, accurate, and unbiased review and analysis of the evidence available in the scientific literature. The evaluation process should encourage objective consideration of clinical and care delivery information, facilitate communication, foster positive patient outcomes, and support safe and effective medication ordering, dispensing, administration, and monitoring. Decisions made by the P&T committee should support improved patient care outcomes across the continuum of care.

INSTRUCTIONS FOR COMPLETION OF THE FORM 1. Formulary addition / deletion request form is to be completed by requester when requesting the addition of a medication to the formulary.
2. The form must be filled in legibly in ink, preferably typed, by the physician 3. Replacement of existing drugs and superiority over existing drugs are two important areas for Committee consideration. In order to control the growth of the formulary, it is preferred that each addition be balanced by a deletion when practical. In any case, the Committee must be provided with a published scientific journal article (not an advertisement) demonstrating the efficacy of the new drug. 4. This information must be in some form that can be readily used to arrive at the quantity to be ordered. A statement such as two ampoules per patient visit and ten patients per month is necessary. Even though a drug is accepted for addition to the Formulary, it will not be ordered until this information is provided in a suitable form.
6. The requesting physician should get the signatures of his Department Head before submitting the completed form to pharmacy and therapeutic coordinator . The requesting physician will be notified of the date and time of the Committee meeting to consider the request and may be invited to attend to lend support. 7. Formulary addition / deletion requests should have non-company sponsored literature, and a summary of the literature attached for members review.
8. The requestor will also be notified of the Committees action and will be notified when the medication is actually on hand ready to be used.

Requesting a Non-Formulary Medication for a specific patient Non-formulary medications shall not be dispensed by the pharmacy department unless the item is not readily available in the community and only obtained for in-house patients if a similar item or combination of items is not presently on the formulary. Non-formulary medications may only be requested by the attending physician, utilizing the "Request for non- formulary medication for a specific patinets, form shall be reviewed by Drug Information pharmacist and approved by the Director of pharmacy. !

REQUEST FOR NON-FORMULARY MEDICATION FOR A SPECIFIC PATIENT

INSTRUCTIONS AND GENERAL INFORMATION
1. Non-Formulary request is to be completed by Physician when requesting a specific medication for a specific. (This is not a request for addition to the formulary)
2. Non-Formulary request must include; complete review of patient history, scientific justification, submission of the treatment protocol, supporting literature, and completed SFDA justification form for non-registered items.
3. Only ORIGINAL Non-Formulary Request will be accepted and processed for drug evaluation.
4. The form must be filled out LEGIBLY by the requesting prescriber (one patient per form).
5. The Director of Pharmacy / Drug Information Pharmacist will evaluate the request and provide
the medication which will take maximum 10 working days from the date of receiving the request.
6. For urgent request, the drug will be available within 24 hours or less according to the availability of the drug.
7. For urgent requests consider, if the drug is a life-threatening agent or a Patient Owned Medication (POM) that is out of stock and the patient cannot discontinue the medication suddenly, or other serious condition.
8. For Patient Own Medication, the prescriber will request it as Non-Formulary Request which will
be provided within 5 days, if it is applicable.
9. Consultants are reminded to request special medications only as a last resort when there is no
alternative medication in the formulary.
10. The Medications requested through this form will be dispensed only to those patients for whom they have been requested. (Medications WILL NOT be dispensed to other patients even when being treated by the requesting physician).
11. Requesting Physician will be notified when the medication is available.
12. For unclaimed out-patient, the expiry date for the request is one month from its date of approval.

Look-Alike and Sound-Alike Medications A list of Look-Alike, Sound-Alike Medications is identified, maintained and reviewed annually.Tallman lettering labels must be used for all Look-Alike, Sound-Alike Medications to ensure medication safety. All Look-Alike, Sound-Alike Medications should be stored in an individual containers with orange labels and separated from one another. All staff involved in the medication process shall be educated on look-alike sound-alike medications.
Concentrated Electrolytes KAAUH must maintain patient safety measures to reduce the risk of errors related to the medication use process, in regard to concentrated electrolytes. As concentrated electrolytes are High Alert Medications, the safeguards outlined in the APP High Alert Medication Guidelines are applicable (e.g.. Independent Double Checking, Medication Segregation, etc).

Drug Recall Drug Recall: is the process by which medication declared contaminated, mislabeled or dangerous by the manufacturers, national, international drug control body, is retrieved from various storage and dispensing areas of the hospital both inpatient and ambulatory care areas. There are three classes in which recalled are placed relative to its potential harm. The recalled classes are as follows:
C lass I: This recall is for dangerous or defective products that could cause serious health problems or death. This class is the most serious and requires immediate action. You should remove the product(s) from service. C lass I I : This recall class is for products that might cause a temporary health problem, or pose only a slight threat of a serious nature. You should identify and take corrective action regarding the product(s).
C lass I I I : This recall class is for product(s) that are unlikely to cause any adverse health reaction, but that are in violation of SFDA or equivalent regulatory agency regulations. This class requires identifying the product(s) and recognizing its potential harm
When a recall or discontinuation notice is received from FDA, SFDA, Manufacturer or any equivalent regulatory agency, the drug(s) in question must immediately be removed from all storage areas to ensure that the drug(s) is/are not available for use. Prescriptions must be checked to determine if any prescription has been dispensed with the lot number in question. The medical staff are notified through the Pharmaceutical Care department that a recall notice has been received for a particular drug.

The AHFS Pharmacologic-Therapeutic Classification Was developed and is maintained by the American Society of Health-System Pharmacists (ASHP). ASHP is the national professional association that represents pharmacists who practice in inpatient, outpatient, home-care, and long-term-care settings. ASHP has a long history of fostering evidence-based medication use as well as patient medication safety. The following table lists the AHFS Classification.
